Canadian portrait of ergonomics in bariatric surgery.

BACKGROUND: Proper ergonomics is crucial for surgeons in the operating room to reduce the risk of musculoskeletal injuries (MSI). Surgery performed on patients with high body mass indexes (BMI) is associated with an increase in non-neutral postures and a higher risk of developing MSIs in laparoscopic models (Sers et al., in Appl Ergon 97:103501). The aim of this study is to establish the prevalence and identify the risk factors of MSIs among bariatric surgeons in Canada. METHODS: This cross-sectional study is based on a researcher-administered questionnaire conducted using a videoconference p

Metabolic and bariatric surgery in vegetarians and vegans.

BACKGROUND: Vegetarianism is constantly increasing worldwide. However, the role of metabolic and bariatric surgery (MBS) in vegetarians/vegans is unclear as there is very limited data on this topic. The aim of this study was to evaluate MBS outcomes in vegetarians or vegans. METHODS: Retrospective analysis of a prospectively maintained database of a single-bariatric surgeon was carried out. All patients with a vegetarian or vegan lifestyle undergoing MBS were included. RESULTS: Eleven patients were included; none were lost to follow-up. Ten patients were women, the mean age and Body Mass Ind

Research progress on bariatric surgery for hyperuricemia.

Hyperuricemia is closely linked to obesity. As lifestyles and dietary patterns evolve, the prevalence of hyperuricemia has been on the rise. Bariatric surgery, an efficacious intervention for morbid obesity and its associated metabolic disorders, not only manages the weight of patients with severe obesity but also exerts beneficial therapeutic effects on hyperuricemia and gout. Moreover, it demonstrates substantial efficacy against other obesity-related metabolic conditions. However, the dramatic fluctuations in serum uric acid levels and acute gouty attacks in the immediate postoperative peri

Rhabdomyolysis as a rare complication of bariatric surgery.

Rhabdomyolysis after bariatric surgery is a quite rare occurrence with low recognition. Due to the breakdown of striated muscle fibers, creatine kinase and myoglobin are released into systemic circulation with variable effects on renal filtering functions. Herein, it was aimed to present a patient who de- veloped rhabdomyolysis following revision bariatric surgery. This 34-year-old male patient was admitted for bariatric surgery. He had had a gastric band surgery approximately six years ago, with regain of weight starting one year after surgery gradually reaching the previous weight level. Con

Bariatric surgery: a potential cure for asthma?

Asthma incidence and severity are increased in obese populations. Systematic reviews have shown benefit from weight-loss interventions on asthma outcomes, but the role of bariatric surgery is still unclear. In this review, cohorts of obese asthmatic patients undergoing bariatric surgery were examined regarding different asthma outcomes. The available data on patients who were followed up showed improvements in asthma control, exacerbation risk, asthma-related hospitalisation, medication use and airway hyperresponsiveness, with some patients not requiring further treatment for asthma. Follow-up

Considerations for bariatric surgery in patients with cirrhosis.

With the ever increasing global obesity pandemic, clinical burden from obesity related complications are anticipated in parallel. Bariatric surgery, a treatment approved for weight loss in morbidly obese patients, has reported to be associated with good outcomes, such as reversal of type two diabetes mellitus and reducing all-cause mortality on a long term basis. However, complications from bariatric surgery have similarly been reported. In particular, with the onslaught of non-alcoholic fatty liver disease (NAFLD) epidemic, in associated with obesity and metabolic syndrome, there is increasin

Outcome of bariatric surgery in older patients.

BACKGROUND: People are living longer than they were expected to 2 decades ago. Increased life expectancy and reduced mortality encompasses a simultaneous increase in the number of older adults with obesity that entails an increase of co-morbidities, such as diabetes, hypertension, cancer, and many other diseases. The aim of our study was to compare the outcomes of bariatric surgery in patients age ≥65 in comparison with younger patients. METHODS: This retrospective study compares bariatric surgeries performed in a private institution between the years 2013 and 2015. The study included 9044 pa

Bariatric surgery and gene expression in the gut.

PURPOSE OF REVIEW: The current review provides an overview of recent literature on new findings related to bariatric surgery and gut gene expression. RECENT FINDINGS: Bariatric surgery modulates the expression of intestinal genes. Experimental and clinical investigations have demonstrated the association of gut rearrangement with changes in intestinal expression of genes related to glucose metabolism. Recent data suggest that bariatric surgery also affects expression of genes belonging to other pathways, including nutrient transporters and metabolism of vitamin B12, decreasing pathway-encodin

Bariatric surgery as a treatment for metabolic syndrome.

Obesity is the pandemic of the 21st century. Obesity comorbidities, including hypertension, dyslipidaemia and glucose intolerance define metabolic syndrome, which increases mortality risk and decreases the quality of life. Compared with lifestyles (diet and physical activity) and pharmacological interventions, bariatric surgery is by far the most effective treatment for obesity and its comorbidities. This minimally invasive surgical treatment is based on an increase of satiety (by hormonal regulation and decreasing stomach volume) or a decrease in nutrient retention (gastric and/or intestinal

Is bariatric surgery safe in the elderly population?

BACKGROUND: Bariatric surgery has proven to be the most effective treatment for morbid obesity in all age groups and is considered superior to medical treatment. The aim of our study was to report the outcomes of bariatric surgery in patients over 65 years of age at our institution. METHODS: A retrospective review of a prospectively collected database was conducted of all patients > 65 years who underwent a bariatric procedure between 2005 and 2015 at our institution. We compared this group to a control group of patients < 65 years of age who were operated on during the last 5 years, from 201

Treatment of Adult Obesity with Bariatric Surgery.

In 2013, approximately 179,000 bariatric surgery procedures were performed in the United States, including the laparoscopic sleeve gastrectomy (42.1%), Roux-en-Y gastric bypass (34.2%), and laparoscopic adjustable gastric banding (14.0%). Choice of procedure depends on the medical conditions of the patient, patient preference, and expertise of the surgeon. On average, weight loss of 60% to 70% of excess body weight is achieved in the short term, and up to 50% at 10 years. Remission of type 2 diabetes mellitus occurs in 60% to 80% of patients two years after surgery and persists in about 30% of

Micronutrient deficiencies in the pre-bariatric surgery.

INTRODUCTION: Data already demonstrated the increased prevalence of obesity in various segments of the population. In this context, bariatric surgery is accepted nowadays as the most effective tool in the treatment and control of morbid obesity. Several studies have evaluated the nutritional status after bariatric surgery, especially mixed by detecting a reduction in food intake of protein, vitamins and minerals. However, other studies have investigated the presence of nutritional deficiencies prior to surgery, which may be aggravated by the surgical procedure, resulting in serious postoperati

Bariatric surgery for morbid obesity in craniopharyngioma.

OBJECTIVE: To retrospectively analyse the effectiveness of bariatric surgery for hypothalamic obesity in patients with craniopharyngioma (CP). PATIENTS: Patients who developed morbid obesity after surgery for CP and who underwent laparoscopic gastric banding (LAGB), laparoscopic sleeve gastrectomy or gastric bypass were included (n = 9). Patients with common obesity who underwent bariatric surgery served as controls (LAGB n = 40, sleeve gastrectomy n = 49 and gastric bypass n = 54). RESULTS: CP was diagnosed during childhood or adolescence [median (range) 10 (1-21) years] and age at bariatri

[Management of the post-bariatric surgery patient].

Worldwide the obesity epidemic is becoming one of the leading causes of mortality and morbidity. The rates of bariatric surgery procedures are sharply increasing. Today it is the only treatment option for a substantial and durable long-term weight loss. However, bariatric surgery is not a guarantee of successful weight loss and maintenance. For all patients undergoing bariatric surgery, education and clinical management to prevent and detect nutritional deficiencies are recommended. Particularly for patients undergoing malabsorptive procedures, the management of potential nutritional deficienc

[Abnormalities of bone metabolism in bariatric surgery].

Obesity is an ever-increasing disease in our environment, and a major risk factor for the development of other chronic diseases that increase morbidity and mortality. Bariatric surgery is an effective treatment for obesity, since it not only allows a significant and sustained loss of weight, but also an important reduction of its co-morbidities. However, this treatment is not free of complications, since along with weight loss, frequent nutritional and metabolic deficiencies have been described. These complications are particularly frequent when mixed surgical procedures with a restrictive and

Risk of biliary complications in bariatric surgery.

Gallstones are commonly observed after rapid weight loss, particularly after bariatric surgery. Preventive measures of gallstone formation and potential related complications are still debated. This study aimed to propose a standardized strategy according to the results of the literature. Thus, preventive measures should be determined according to patient status (evaluated clinically and by routine ultrasound) and the type of bariatric surgery. Cholecystectomy should be performed in patients with symptomatic gallstones irrespective of the planned operation, or for asymptomatic gallstones durin

[The obstructive sleep apnea in bariatric surgery].

INTRODUCTION: Obesity is a well known risk factor for obstructive sleep apnea (OSA). Medical therapy is not effective for morbid obesity. Bariatric surgery is therefore a reasonable option for weight reduction for patients with clinically severe obesity. PATIENTS AND METHODS: 283 obese patients were operated on from 1999 until 2005 in our Institution and they were examined with a history, physical examination and the Epworth Sleepiness Scale (ESS). Obese patients with a ESS score > or = than 10 were evaluated with a Polysomnography (PSG). RESULT: 61 patients (21.5%) resulted with a ESS > or
