Death from Barotrauma Due to Compressed Air: A Medico-legal Analysis.

Lesions of the digestive tract due to barotrauma resulting from compressed air application are not common, are rarely lethal, and largely affect the sigmoid and descending colon. Moreover, their pathogenic mechanism is a topic of discussion because these lesions have multiple characteristics. Here, the authors describe an autoptic case of death from lesions of the ascending and transverse segments, with perforations and bleeding suffusions as well as ischemic areas covered the colonic wall that was extremely thinned, congested, and hemorrhagic, with considerable flattening leading to disappear

Prevalence of Barotrauma in Recreational Scuba Divers After Repetitive Saltwater Dives.

INTRODUCTION: The aim was to investigate the prevalence of middle ear barotrauma (MEB) and to establish risk factors of MEB after repeated saltwater dives. METHODS: In this prospective observational cohort study 28 divers were examined over 6 consecutive days of diving in the Red Sea, Egypt. Participants underwent an otoscopic examination before the first dive, between each dive and after the last dive. In addition, they performed Valsalva maneuver (VM) and were questioned about dive-related complaints. Otoscopic findings were classified according to TEED classification for MEB (0 = normal ot

Middle ear barotrauma causing transient facial nerve paralysis after scuba diving.

Middle ear barotrauma is a well known entity with typical injury occurring when diving or ascending in a commercial jetliner. Patients often present with symptoms of acute onset otalgia, hearing loss and sometimes haemotympanum (with or without tympanic membrane perforation). On rare occasions, facial nerve paralysis can occur when the tympanic segment of the facial nerve is dehiscent within the middle ear. We present a case of spontaneously resolving facial nerve palsy associated with middle ear barotrauma following a brief, shallow dive. Prompt and astute diagnosis leads to proper management

Widespread subcutaneous emphysema and barotrauma resulting from high pressure gas injection.

Widespread subcutaneous emphysema is an unusual emergency presentation. We present a case of accidental high pressure insufflation, the pathophysiology and subsequent medical management in the acute setting. Such presentations are rare but dramatic and can have important life-threatening consequences that require immediate treatment.

A novel technique of otic barotrauma management using modified intravenous cannulae.

This article provides the first detailed description and systematic evaluation of the management of otic barotrauma using modified intravenous cannulae. A 24-gauge IC cannula was modified as a tool for tympanostomy tube placement and middle ear ventilation. The medical records of 271 ears of 156 adult patients (median age 49 years) who underwent this procedure were reviewed retrospectively. Hundred and ninty-one tubes were placed for otalgia because of hyperbaric oxygen therapy, 58 tubes were inserted for air travel prophylaxis and 22 tubes were placed for management of otic barotrauma post-fl

Paranasal sinus barotrauma in sports self-contained underwater breathing apparatus divers.

OBJECTIVE: TO evaluate potential pre-dive parameters in relation to paranasal sinus barotrauma in sports self-contained underwater breathing apparatus divers. STUDY DESIGN: The pre-dive and follow-up records of 46 healthy sports self-contained underwater breathing apparatus divers were analysed. METHODS: In the routine pre-dive examination of diving candidates with no symptoms of rhino-sinus disease, rhinoscopy, sinus radiography and nasal endoscopy indicated no signs of rhinitis and/or sinusitis. Any signs of paranasal sinus barotrauma were noted, and several parameters were evaluated in re

Reverse sinus barotrauma with periorbital emphysema as an occupational hazard - a case report.

Sinus barotrauma is the second most common diving injury among divers. It results from an imbalance between the pressure within the sinuses and the ambient pressure. It may occur not only as "barotrauma of descent" but also as "barotrauma of ascent/reverse barotrauma," which is less common. Here, we describe a professional diver who had worked at a fish farm for nearly 3 years, diving with SCUBA equipment, and developed reverse sinus barotrauma with periorbital emphysema. This diving injury is preventable. To help prevent it, divers must have adequate education and knowledge of diving physics,

[A case of pulmonary barotrauma complicated with cerebral arterial air embolism in a diver].

Pulmonary barotrauma is a kind of disease caused by the injury of lung tissue or blood vessel when the gas pressure of lung is too high or too lower than the external pressure of the body, which causes the air to enter the blood vessel and adjacent tissue. It could be happened in the escape of the divers with the light diving equipment or the sailors from submarine. Generally, the decompression chamber was used to treating the disease, and the minimum air pressure of 0.5 MPa recompression therapeutic schedule was used to selecting. In November 2019, a patient with pulmonary barotrauma combined

The assessment and management of inner ear barotrauma in divers and recommendations for returning to diving.

Inner ear barotrauma (IEBt) constitutes a spectrum of pressure-related pathology in the inner ear, with antecedent middle ear barotrauma (MEBt) common. IEBt includes perilymph fistula, intralabyrinthine membrane tear, inner ear haemorrhage and other rarer pathologies. Following a literature search, the pathophysiology, diagnosis, and treatment of IEBt in divers and best-practice recommendations for returning to diving were reviewed. Sixty-nine papers/texts were identified and 54 accessed. Twenty-five case series (majority surgical) provided guidance on diagnostic pathways; nine solely reported

The role of functional endoscopic sinus surgery in the management of recurrent sinus barotrauma.

Sinus barotrauma occurs when an individual is unable to equilibrate the pressure within his sinuses with atmospheric pressure. Aviators affected by recurrent sinus barotrauma are unfit to fly until the underlying cause is established and treated. While most cases result from intranasal pathology, a significant number are the result of sinus pathology or anatomical abnormalities. This latter group have been difficult to manage in the past but the advent of computerized tomography and the Hopkin's rod endoscope have allowed them to be operated on with precision and safety. Complications of this

Air embolism due to pulmonary barotrauma in a patient undergoing cesarean section.

Air embolism may occur following criminal abortion, vaginal douching, powder insufflation as treatment for vaginal infections, and orogenital sex. The patient reported in this work deteriorated following pulmonary barotrauma. Diagnosis was made 16 h after the appearance of neurological signs. She was transferred immediately to the hyperbaric unit. The speed and completeness of recovery are directly related to the prompt diagnosis and commencement of therapy. Failure is more likely related to delay.

Severe frontal sinus barotrauma in an airline passenger: a case report and review.

The problems of sinus barotrauma in air travelers have received little attention in the literature of emergency medicine. Presented here is a report of severe (Grade III) frontal sinus barotrauma in an airline passenger. The pathophysiology and treatment of frontal sinus barotrauma are reviewed. Emergency physicians should strongly consider a diagnosis of sinus barotrauma in patients who present with symptoms of sinusitis and a history of recent flight.

Pseudoephedrine prophylaxis does not prevent middle ear barotrauma in hyperbaric oxygen therapy.

A common complication of hyperbaric oxygen (HBO₂) treatment is middle ear barotrauma (MEB), which can lead to pain, treatment abandonment, or delay in treatment. Studies have shown that pseudoephedrine decreases MEB for pressure changes in SCUBA divers and airplane travelers. We conducted a randomized, double-blind, placebo-controlled trial to determine if pseudoephedrine effectively decreases MEB rates in patients receiving their first HBO₂ treatment. There was no statistically significant difference between the pseudoephedrine and placebo groups concerning ear pain ratings, tympanic membrane
