The Journal of clinical and aesthetic dermatology2013eng
Myiasis is a tropical infection most often caused by Dermatotobia hominis, also known as the botfly. It is rarely seen in the United States. The infection has a slow evolution and often presents with painful lesions that mimic furunculosis, boils, and infected cysts. The mechanism of infection is based on the deposition of botfly eggs onto an arthropod, which acts as a vector of transmission. The arthropod infects various hosts and botfly eggs are concomitantly deposited on the host. The Dermatobia eggs transform into their larval form and penetrate the skin through adjacent adnexal structures

A 26-year-old woman presented with a 2-day history of fever peaking to 39 degrees C and cold shivers that developed after a 2-weeks trip to Guatemala and Belize. Prior to the fever the patient had felt symptoms of a common cold and general malaise. Moreover, she complained of generalised myalgia and nausea. She had taken paludrine as a prophylactic against malaria. Borrelia spirochaetes, the pathogen of relapsing fever, were detected in a thick blood smear preparation. On the basis of the anamnesis, geography and specific exposure, the patient had a form of relapsing fever that is transmitted

Leishmaniasis is a protozoal infection with an increased risk of transmission to those serving in the U.S. Military due to theaters of operation in endemic regions. There has, in recent decades, been robust experience with old-world leishmaniasis in the Military Health System (MHS); however, new-world leishmaniasis, which may result in mucosal leishmaniasis, has been less studied. A total of 88 patients from 2012 to 2022 with diagnosis codes for "mucocutaneous leishmaniasis" or "leishmaniasis, unspecified" were identified in the Military Data Repository and reviewed. Within this cohort, upon m

In an apparently routine case, what appeared to be infected sebaceous cysts of the scalp turned out to harbour botfly larvae imported from a tropical country. Although the parasite Dermatobia hominis is not indigenous to the United States, cutaneous myiasis caused by the fly is fairly common in Central and South America, and several cases have been reported in people returning or emigrating from these regions. The authors therefore suggest that infection resulting from D. hominis invasion be considered in persons who present with furuncular lesions and give a history of travel to endemic areas

In the developing world, there exists a scarcity of services and training in otolaryngology, audiology, and speech therapy, which is reflected by the gap between health care delivery in high-income countries and low-income countries. We surveyed, by questionnaire, the countries of Central America, except for Belize, because of the lack of otolaryngology services, on the following issues: availability of otolaryngology, audiology, and speech therapy services and equipment; otolaryngologist, audiologist, and speech therapist training; and availability of services in rural areas. Surveys were dis

Cutaneous infestation by the human botfly, Dermatobia hominis, results in furuncular myiasis. This condition is endemic to the forested areas of Mexico, Central and South America. However, because of widespread travel, furuncular myiasis has become more common in North America. Misdiagnosis and mismanagement can occur owing to limited awareness of the condition outside endemic areas. To our knowledge, there is only a single report of botfly myiasis in the recent emergency medicine literature, which is surprising since the emergency department is likely to be the place many patients with this c

Existing systemic treatments for New World cutaneous leishmaniasis (CL) caused by Leishmania (vianna) braziliensis are unsatisfactory. Liposomal amphotericin B has been used extensively for the treatment of visceral leishmaniasis, but in few cases of CL, and an appropriate regimen for CL has not been described. We successfully treated a patient with multiple L. braziliensis CL lesions acquired in Belize. Liposomal amphotericin B (AmBisome) was given to our patient as an inpatient for seven daily doses of 3 mg kg(-1) day(-1) and then as an outpatient at 3 mg kg(-1) twice weekly for a further th

A case of cutaneous leishmaniasis in a traveller from Belize, Central America is reported. Leishmaniasis presents rarely in Australia and delays in diagnosis and treatment often occur. A high index of suspicion in a patient who has returned from an endemic region is required. Subsequent confirmation of a diagnosis of cutaneous leishmaniasis is best achieved by demonstration of the organism on skin biopsy, aspiration or smear. The histology is variable and depends on geographic, parasite species and host factors. Speciation of New World disease as either Leishmania braziliensis or Leishmania me

Studies examining the prevalence of zoonotic agents in the Caribbean are very limited. The objective of this study was to examine the seroprevalence of seven zoonotic agents among individuals residing on 10 English-speaking Caribbean countries. Sera from healthy, pregnant women were collected from Antigua-Barbuda, Belize, Bermuda, Dominica, Grenada, Jamaica, Montserrat, St. Kitts-Nevis, St. Lucia, and St. Vincent-Grenadines and tested for the presence of IgG antibodies to dengue virus, hepatitis E virus, hantaviruses, leptospiral agents, spotted fever group rickettsiae (SFGR), typhus group ric

Leishmaniasis is an infectious disease caused by Leishmania protozoa, transmitted by the bite of phlebotomine sandflies. It causes a spectrum of clinical syndromes, of which the most common are cutaneous and visceral leishmaniasis. Clinical presentation is highly variable and is dependent on multiple factors, such as Leishmania species and patient characteristics (including immune competence). The relationship between these variables is poorly understood, and there is no single, evidence-based treatment for the disease. Currently management focuses on identification of the species, but this re

Studies to determine the persistence of released, sterile screwworms, Cochliomyia hominivorax (Coquerel), were conducted in Belize, Central America, in 1987-1988. A total of nine releases were made, each consisting of 4,000 females marked with a fluorescent dust. Previous similar studies with baited fly traps indicated that flies died or dispersed from the release site within 3-4 d. For this study, flies were recaptured at sentinel animals. Recapture rates varied greatly between releases, a result that was not correlated with any weather parameter. The pattern of recapture indicated a sharp dr

As tropical countries become common travelers' destinations, more and more returning travelers are expected to present with cutaneous lesions secondary to myiasis. The skin lesion starts as a small red papule and gradually enlarges to become a furuncle. Familiarity with the characteristic clinical presentation and proper management would avoid an unnecessary diagnostic work-up and therapeutic intervention, including surgery and the use of antibiotics.

In order to elucidate the mechanisms for the elimination of Leishmania, the histological evolution of the lesions of cutaneous leishmaniasis was studied in 118 biopsies from four geographical areas known to be associated with different species or sub-species of Leishmania. Basically there were 3 types of response: A, parasites were eliminated within intact macrophages which later evolved as epithelioid cells; B, they were eliminated as a result of the lysis of the macrophages either individually or in small clusters, but the process was incomplete; C, there was necrosis proceeding to completio

Rotavirus is the leading cause of acute diarrhea in children younger than five years of age around the world. Severe dehydration and mortality rates are higher in developing countries, especially those from Latin America, Africa, and Asia. The vaccine has been introduced in the national immunization programs of more than half of Latin American countries, and impact data from some of these nations has been already published. The two rotavirus vaccines, the 2-dose monovalent (RV-1) and the 3-dose pentavalent (RV-5) vaccine, have been available in the market to all Central American countries. Rot

Two hundred thirty-three British troops were tested before and 2 months after a 6-month period in Belize. No erythematous indurated reactions developed on the initial tests; four subjects developed such reactions on the second test. Three of these subjects had skin ulcers at the time of the second test; the fourth developed an ulcer 2 weeks later. Leishmania braziliensis sp were isolated from all of these ulcers. Two subjects who showed no reaction on either of their tests developed skin ulcers within 1 month of the second test. Leishmania braziliensis sp were isolated from these ulcers. The l

Venezuelan equine encephalomyelitis (VEE) epizoodemics were reported at 6-10-year intervals in northern South America beginning in the 1920s. In 1937, epizootic VEE virus was isolated from infected horse brain and shown as distinct from the North American equine encephalomyelitis viruses. Subsequently, epizootic and sylvatic strains were isolated in distinct ecosystems; isolates were characterized serologically as epizootic subtype I, variants A/B and C; or sylvatic (enzootic) subtype I, variants D, E, and F, and subtypes II, III, and IV. In 1969, variant I-A/B virus was transported from a maj

Oligogonotylus mayae n.sp. is described from the intestine of the Mayan cichlid Cichlasoma urophthalmus (Günther) in Ría Lagartos, Ría Celestún, and Estero Progreso, Yucatán State. This is the second species described for Oligogonotylus Watson, 1976, the other being O.manteri Watson, 1976. The new species is readily distinguished from O. manteri by the anterior extension of the vitelline follicles. In O. Manteri, Vitelline follicles are found entirely in the hindbody, extending posteriorly to mid-testicular level. Vitelline follicles in the new species extend from teh anterior margin of poster

Predictions of tropical cyclone (TC) frequencies are hampered by insufficient knowledge of their natural variability in the past. A 30-m-long sediment core from the Great Blue Hole, a marine sinkhole offshore Belize, provides the longest available, continuous, and annually resolved TC-frequency record. This record expands our understanding, derived from instrumental monitoring (73 years), historical documentations (173 years), and paleotempestological records (2000 years), to the past 5700 years. A total of 694 event layers were identified. They display a distinct regional trend of increasing
