Hunger strikes in prisons: a narrative systematic review of ethical considerations from a physician's perspective.

OBJECTIVES: We sought to identify and review published studies that discuss the ethical considerations, from a physician's perspective, of managing a hunger strike in a prison setting. METHODS: A database search was conducted to identify relevant publications. We included case studies, case series, guidelines and review articles published over a 20-year period. Non-English language publications were translated. RESULTS: The review found 23 papers from 12 jurisdictions published in five languages suitable for inclusion. CONCLUSIONS: Key themes from included publications are identified and su

Acute ethics: risk assessment and the Doctrine of Double Effect in a referral to on-call urology.

The Doctrine of Double Effect has been described as often used in medicine and surgery but is seldom described outside the palliative medicine context. This paper presents a possible clinically focussed illustration of the doctrine of double effect used in an acute urological setting. The authors describe the case of an elderly patient with significant dementia and haematuria who required anticoagulation. Implicit double effect reasoning may have helped the surgical team reach their initial decision with regard to a treatment plan, reconciling a duty to beneficence with a duty not to cause har

Endovascular procedures for below the knee arteries in diabetics.

Patients with below the knee (BTK) lesions may present either with intermittent claudication (IC) or with critical limb ischemia (CLI). Generally, these patients with a high incidence of diabetes have a high rate of myocardial infarction and mortality. So, even if, the surgery is potentially a very good technique with excellent results when the best conduit as a vein graft is used, the ratio benefice/risk seems too low for the IC patients. On another hand for the CLI patients a lot of reasons contraindicate or are unfair to the surgical option: lack of conduit, bad or unclear run-off or distal

[Poliomyelitis, philanthropy and physiotherapy: the birth of the career of physiotherapist in Rio de Janeiro in the 1950s].

The polio epidemics in the country and especially in Rio de Janeiro left hundreds of children with sequels in the 1950s. The public outcry over the polio epidemic, the reports in the press and the association of experienced physicians with businessmen, bankers and relatives of victims created the conditions for the emergence of a philanthropic entity to combat infantile paralysis. The Brazilian Beneficent Association of Rehabilitation (BBAR) was founded in 1954, and two years later the association created the School of Rehabilitation of Rio de Janeiro (SRRJ), the first institution to graduate

[Therapeutic management of asthma].

Therapeutic management of asthma is based on the key role of lower airway inflammation. New treatments and inhalation devices in conjunction with international recommendations have contributed to improve asthma management. The aim of asthma treatment is to avoid acute asthma and to reduce or suppress symptoms, to permit normal life (familial, scholar or professional) and to normalize lung function. The final goal is to improve patient quality of life and must also integrate the benefice/risk ratio of treatment. Asthmatic patients need to be regularly followed by a physician. Recently, French A

Plastic surgery considerations for holoprosencephaly patients.

Holoprosencephaly (HPE) is considered the leading abnormality of the brain and face in humans and is frequently associated with a wide spectrum of specific craniofacial anomalies including mid-line facial clefts, cyclopia and nasal irregularities. A standard course of treatment has not been developed and management is symptomatic and supportive. In this work, the authors discuss the wide-ranging spectrum of HPE and propose surgical guidelines to provide more uniform and appropriate care to patients suffering from holoprosencephaly. Assessment of the patient's brain abnormality is essential in

Surveys on attitudes to active euthanasia and the difficulty of drawing normative conclusions.

PURPOSE: To present surveys on active euthanasia and to discuss what normative conclusions can be drawn. METHODS: Two summary articles and 30 recent surveys on attitudes to active euthanasia are discussed. RESULTS: According to the first summary article, acceptance of active euthanasia among the public has stabilized around 65%; according to the second, almost 60% of physicians are in favour of legalizing active euthanasia. As for the 30 recent surveys, physicians are most often respondents. while the general public is surveyed in only three. The differences in attitudes are striking: 21-78%

Epidemiology and moral philosophy.

To an increasing extent ethical controversies affect and sometimes obstruct public health work and epidemiological research. In order to improve communication between the concerned parties a model for identification and analysis of ethical conflicts in individual-based research has been worked out in co-operation between epidemiologists and moral philosophers. The model has two dimensions. One dimension specifies relevant ethical principles (as beneficence, non-maleficence, autonomy and justice). The other dimension specifies the groups of persons involved in the conflict under consideration (

Ethical analysis of the intrapartum management of pregnancy complicated by fetal hydrocephalus with macrocephaly.

The intrapartum management of pregnancy complicated by fetal hydrocephalus with macrocephaly often confronts the physician with a complex array of moral obligations to both mother and fetus that may sometimes come into direct conflict. The authors analyze the component beneficence-based and autonomy-based obligations of these conflicts and offer strategies for resolving them for each of three clinical subgroups. For isolated fetal hydrocephalus, we argue that cesarean section should be performed except when the woman with full informed consent unwaveringly refuses. For fetal hydrocephalus with

ETHICS IN RADIOLOGICAL PROTECTION FOR MEDICAL DIAGNOSIS AND TREATMENT.

Publication 138 defines the ethical foundations of the ICRP System of Radiological Protection based on core values (beneficence and non-maleficence, dignity, justice, and prudence) and procedural values (accountability, transparency, and inclusiveness). The purpose of the present publication is to propose a practical application of values for medical radiological protection professions. As medicine has a long history and strong culture of ethics, this publication starts by identifying the shared values, and defines a common language between biomedical ethics and radiological protection. Treatment of anorexia nervosa in Ireland: striking an ethical balance.

Anorexia nervosa (AN) is a complex psychiatric illness with severe and life-threatening medical sequelae, including death. Existing evidence-based treatments are linked to good prognosis and full recovery in many. For a small minority of critically ill patients, treatment decisions extend beyond voluntary engagement. Severe cases may involve involuntary hospitalisation, nasogastric feeding, physical restraint, and other coercive measures. While these interventions are sometimes necessary to prevent death, they raise profound ethical concerns. This article explores the ethical tensions in treat

Pain in Pets: Beyond Physiology.

Animals do not speak a language humans understand, making it easy to believe that they do not experience pain the way humans do. Despite data affirming that companion animals can and do experience pain much as do humans, there remains a gap between companion animal acute pain management knowledge and its execution. Companion animal pain is not simply a physiological issue. Veterinary clinicians can and should embrace the foundational principles of clinical bioethics-respect for autonomy, nonmaleficence, beneficence, and justice-translated from human medicine for the benefit of their patients.

Compassion in Corrections: The Struggle Between Security and Health Care.

Many incarcerated individuals have chronic health conditions and mental health issues that have not been addressed by health care providers in years, if not decades. Patients in correctional settings are isolated from society and have reduced access to health care. Prison is a lonely scary place. How then do nurses impact the lives of these patients? It can be challenging given safety concerns, resource issues, and the bias of not only the nurse but also the security staff with whom they are working. Nurses have a responsibility to their patients to beneficence, justice, nonmaleficence, and au

Implied consent for HIV testing in the UK: time for a new approach?

Despite HIV infection being a treatable chronic illness and the many advances in testing for HIV, late diagnosis is still common, with associated avoidable morbidity and mortality. Requirements for explicit consent for HIV testing in the UK differ from those for other blood tests and are major barriers to testing. We argue that the disparity is illogical and outdated. We propose a model for normalising HIV testing that allows for routine testing in various health-care settings via implied consent, where other blood tests are performed. Inclusion of testing for hepatitis B and hepatitis C might

Reconciling ethical and legal aspects in neonatal intensive care.

During the last two decades there has been an enormous development in treatment possibilities for the extremely premature infants and the Neonatologists have to face in their daily practice many decisional problems and ethical, moral and legal dilemmas. These concern decisions to initiate or withhold treatment directly at birth, decision to withdrawn treatment with the possible consequence that the child will die. The debate between "sanctity" and "quality" of life, aggressive treatment in relation to discrimination toward the disabled, the principle of "beneficence" and the question of "propo

Sleep deprivation, physician performance, and patient safety.

Long work hours, overnight call duty, and rotating shifts are implicit features of hospital medical practice. Rigorous schedules have been deemed necessary to fulfill the professional obligation of patient beneficence, to optimize trainee learning, and to respond to economic realities. However, the resultant disruption and restriction of physicians' sleep produce demonstrable neurobehavioral impairments that may threaten other fundamental professional mandates, such as that of primum non nocere ("first, do no harm"). This article provides a basic overview of sleep/wake regulatory processes, ex

To help and not to harm: ethical issues in the treatment of chronic pain in patients with substance use disorders.

Patients with both chronic pain and substance use disorders are increasingly encountered in a variety of treatment settings. The treatment of these patients raises a number of ethical and patient care issues. Consultation-liaison psychiatrists possess the knowledge and skills to constructively address these issues. This chapter provides clinicians with a review of clinical and ethical dilemmas related to opioid treatment of chronic pain in patients with substance use disorders. The core conflict of beneficence and nonmaleficence will be explored in relation to the concepts of autonomy, justice

Ethical Obligations to Screen for Psychiatric Disorders and Suicidal Ideation in Patients with Alopecia Areata.

Alopecia areata (AA) is associated with substantial psychosocial morbidity. Population-based studies have identified increased suicide-related risks, although findings are heterogeneous, which raises an ethical question regarding dermatologists' obligation to screen for psychiatric morbidity. Routine screening for depression and psychologic distress may reasonably form part of AA care. In contrast, formal suicide risk assessment should follow identification of suicidal ideation or other concerning clinical findings rather than be universally administered. This proportionate approach balances b

Resistance or Cooperation? Actor and Partner Effects of Hostile and Benevolent Sexism on Observed Behavior in Couples' Interactions.

Hostile sexism evokes women's resistance necessitating benevolent sexism to promote cross-gender cooperation. Addressing inconsistent, primarily self-report evidence for ambivalent sexism theory principles, we test the actor and partner effects of sexist attitudes on behaviors exhibited within heterosexual couples' interactions (N = 855 dyadic interactions). Showing that men's hostile sexism likely incites resistance, men's hostile sexism predicted more hostile and less responsive behavior by men and their women partners. Providing new behavioral evidence that benevolent sexism may promote het

An "ethics of strangers"? On knowing the patient in clinical ethics.

The shape and function of ethical imperatives may vary if the context is an interaction between strangers, or those who are well acquainted. This idea, taken up from Stephen Toulmin's distinction between an "ethics of strangers" and an "ethics of intimacy", can be applied to encounters in healthcare. There are situations where healthcare personnel (HCP) know their patients (corresponding to an "ethics of intimacy") and situations where HCP do not know their patients (corresponding to "an ethics of strangers"). Does it make a difference for normative imperatives that follow from central concept
