Genitogluteal porokeratosis: an unusual clinical presentation.

A 51-year-old man presented with a 12-year history of an expanding, irritable rash on his buttocks, groin and scrotum. He gradually developed erythematous, annular plaques with ridged borders and depressed centres. He also had a verruciform eruption in his perianal area. A clinical diagnosis of porokeratosis confined to the genitogluteal area was confirmed histopathologically. Oral acitretin resulted in symptomatic and cosmetic improvement. He continues to be followed up to evaluate treatment outcomes.

Topical spermine and putrescine stimulated DNA synthesis in the hairless mouse epidermis.

Polyamines were applied topically to the skin of the hairless mouse. Putrescine stimulated the incorporation of thymidine after a 24-h application period. The effect of polyamines upon skin pretreated with a potent topical steroid was also examined; in this model thymidine incorporation was stimulated by both spermine and putrescine. Pretreatment was performed in order to reduce endogenous polyamine biosynthesis and increase the sensitivity of the epidermis to exogenous polyamines. Depletion of the activity of ornithine decarboxylase, the rate-limiting polyamine biosynthetic enzyme, by topical

Topical hydrocortisone 17-butyrate 21-propionate in the treatment of inflammatory skin diseases: pharmacological data, clinical efficacy, safety and calculation of the therapeutic index.

Hydrocortisone 17-butyrate 21-propionate (hydrocortisone buteprate, HBP) is a medium potent, non-halogenated double-ester of hydrocortisone with a favorable benefit/risk ratio for the treatment of inflammatory skin disorders. HBP is available as a 0.1% cream or ointment formulation. Good results were obtained with a once-daily topical treatment. HBP is characterized by a strong topical anti-inflammatory activity and weak systemic action. It is considered to have potency comparable to that of betamethasone 17-valerate (BV), but its systemic effects are less pronounced. HBP was shown to have a g

Clinical recommendation: pediatric lichen sclerosus.

Lichen sclerosus is a chronic inflammatory condition affecting the anogenital region that may present in the prepubertal or adolescent patient. Clinical presentations include significant pruritus, labial adhesions, and loss of pigmentation. Treatment includes topical anti-inflammatory agents and long-term follow-up as there is a high risk of recurrence and an increased risk of vulvar cancer in adult women with history of lichen sclerosus. These recommendations are intended for pediatricians, gynecologists, nurse practitioners and others who care for pediatric/adolescent girls in order to facil

Effects of Intermittent Treatment with Topical Corticosteroids and Calcineurin Inhibitors on Epidermal and Dermal Thickness Using Optical Coherence Tomography and Ultrasound.

INTRODUCTION: Proactive therapy with topical corticosteroids (TCSs) is the standard treatment for chronic inflammatory diseases such as atopic dermatitis; however, skin atrophy as TCS side effect remains a concern. METHODS: This 16-week, evaluator-blinded, within-patient placebo-controlled, randomized study enrolled volunteers with healthy skin conditions. For 12 weeks, their volar forearm and the back of their hand were applied with hydrocortisone acetate 1% cream (HC), methylprednisolone aceponate 0.1% cream (MPA), betamethasone valerate 0.1% cream (BMV), or an active agent-free base cream

Vitamin D analogues in the treatment of psoriasis.

Psoriasis is a chronic hyperproliferative skin disease in which inflammatory and immunologic processes may play important pathophysiologic roles. Recently the skin has been identified as a target tissue for vitamin D. Because 1,25-dihydroxy vitamin D3 inhibits epidermal proliferation and promotes epidermal differentiation, it has been introduced for the treatment of psoriasis vulgaris. In addition to 1,25-(OH)2-D3, synthetic vitamin D3 analogues have undergone clinical evaluation. Calcipotriol (INN) (calcipotriene [USAN]) has been studied most extensively. Compared with 1,25-(OH)2-D3, calcipot

Contact allergies to topical corticosteroids: 10 cases of contact dermatitis.

Patients who noticed worsening of their skin disease after using topical corticosteroid preparations were patch tested both with the commercial preparation and the corticosteroid itself. Between 1987 and 1989, 10 cases of contact dermatitis due to topical corticosteroids were detected in this way. The corticosteroids wee amcinonide (2 patients), hydrocortisone butyrate, clobetasol propionate (2), betamethasone valerate (2), prednicarbate and fluocortolone (2). Patch tests with the commercial preparations and the corticosteroids themselves elicited reactions almost identical in time course and

Dysphonia caused by inhaled steroids: recognition of a characteristic laryngeal abnormality.

Nine of 14 asthmatic patients who presented with persistent dysphonia while taking inhaled corticosteroids had a bilateral adductor vocal cord deformity with bowing of the cords on phonation. This causes the dysphonia and usually occurs without candidiasis. It was seen with beclomethasone dipropionate (in both pressurised aerosol and dry powder preparations), betamethasone valerate, and budesonide. It was related to the dose and potency of inhaled steroid and may represent a local steroid myopathy. It was reversed when the inhaled steroid was stopped, although resolution sometimes took weeks.

Locoid vs betnovate lotion in the treatment of seborrhoeic and atopic dermatitis of the scalp.

In a randomised, double-blind clinical trial, the relative efficacy of Locoid scalp lotion and Betnovate scalp application was assessed in the treatment of 30 patients, 15 in each treatment group, suffering from seborrhoeic or atopic dermatitis of the scalp. Both therapies produced a statistically significant improvement and clearance of the lesions by the end of the four weeks of treatment. Slight differences were observed in favour of Betnovate with respect to global efficacy but this difference was not statistically significant. Side-effects were not spontaneously complained of by any of th

A trial of the Actiderm dermatological patch and topical corticosteroids in the treatment of psoriasis vulgaris. The Actiderm Multi-Center Study Group.

The use of the Actiderm dermatological patch in conjunction with topical corticosteroids was evaluated in a multi-centered, paired-comparison study of 189 patients with chronic psoriasis. In each patient, two lesions of comparable severity were selected for treatment. One plaque was treated with a twice-daily application of a steroid cream (triamcinolone acetonide 0.1 percent, betamethasone valerate 0.1 percent, or halcinonide 0.1 percent) while the second plaque was treated with a forty-eight hour application of the same steroid cream under Actiderm. Harnessing the potential of fatty Acid-Surfactant-Based micellar gel for enhanced topical delivery of Apremilast in psoriasis treatment.

Apremilast (APR) is a potent anti-psoriatic agent that inhibits the phosphodiesterase 4 enzyme. Due to the poor oral bioavailability and associated systemic side effects the clinical applicability of APR has been constrained. Nanotechnology-based carrier system presents a novel option to increase the efficacy of the topical treatment of APR. The current investigation deals with the development of fatty acid-surfactant conjugate-based hybrid mixed micellar gel (HMMG) for the topical delivery of APR. The developed micelles exhibited an average size of 83.59 ± 4.46 nm, PDI of 0.239 ± 0.047, % ent

Effects of glucocorticosteroids on primary human skin fibroblasts. I. Inhibition of the proliferation of cultured primary human skin and mouse L929 fibroblasts.

Various glucocorticosteroids were added to logarithmically growing cultures of primary human skin fibroblasts and of mouse L929 fibroblasts. These steroids inhibited proliferation of the human fibroblasts at concentrations which fall in a range expected to occur during the topical treatment of skin disorders. In terms of the concentrations required for the inhibition hydrocortisone was least and clobetasol-17-propionate most effective. All other steroids studied (hydrocortisone-17-butyrate, triamcinolone acetonide, betamethasone-17-valerate and hydrocortisone-21-acetate) showed medium effectiv
