Impact of 2 years of COVID-19 pandemic on preterm birth: Experience from a tertiary center of obstetrics in western Germany.

OBJECTIVE: To compare preterm birth rates and reasons before and during the COVID-19-pandemic using a monocentric, retrospective study. METHODS: Univariate analysis identified differences in rates and reasons for preterm birth and neonatal outcomes between the pre-pandemic period (January 1, 2018 to December 31, 2019) and during the pandemic (January 1, 2020 to December 31, 2021) among all births at our tertiary obstetrical center, the University Hospital of Essen. RESULTS: The cohort consisted of 6086 deliveries with 593 liveborn preterm singletons. During the pandemic, the incidence of pre

Impact of advanced maternal age on perinatal outcomes in Tanzania: Insights from Kilimanjaro Christian Medical Center Birth Registry.

The increasing prevalence of advanced maternal age (AMA) births necessitates the exploration of associated pregnancy outcomes within the healthcare-limited context of northern Tanzania to elucidate potential region-specific risks and implications. This study explored the influence of AMA on pregnancy outcomes in northern Tanzania, where healthcare resources and infrastructure are constrained in comparison to developed countries. This cross-sectional hospital-based study utilized maternally linked data from the Kilimanjaro Christian Medical Center (KCMC) Medical Registry and included 32,798 wom

Development and validation of a spontaneous preterm birth risk prediction algorithm based on maternal bioinformatics: A single-center retrospective study.

BACKGROUND: Spontaneous preterm birth (sPTB) is a primary cause of adverse neonatal outcomes. The objective of this study is to analyze the factors influencing the occurrence of sPTB in pregnant women and to construct and validate a predictive model for sPTB risk based on big data from clinical and laboratory assessments during pregnancy. METHODS: A retrospective analysis was conducted on the clinical data of 3,082 pregnant women, categorizing those who delivered before 37 weeks of gestation as the sPTB group and those who delivered at or after 37 weeks as the full-term group. The performance

Use of transpyloric feeds in extremely low birth weight infants at risk of severe bronchopulmonary dysplasia-a single center experience.

INTRODUCTION: The incidence of severe BPD (sBPD), defined as needing oxygen or positive pressure at 36 weeks corrected gestational age (CGA), has remained unchanged. These infants are at risk for developing late pulmonary hypertension (LPHN) or needing surgical interventions such as Gastrostomy Tubes (GT) or Tracheostomy Tubes (TT). The finding of pepsin in the lungs of infants who were extremely low birth weight (ELBW) with sBPD has led to the speculation that gastroesophageal reflux (GER) and aspiration could contribute to their lung disease. Micro-aspiration-reducing strategies such as Tran

The interval between births and the risk of recurrent preeclampsia among predominantly high risk women in urban tertiary care center.

INTRODUCTION: Women with a history of preeclampsia have a higher risk of recurrent preeclampsia. This study sought to ascertain the relationship between the interbirth interval and the risk of recurrent preeclampsia and difference in angiogenic markers between the two groups. METHODS: Data was collected from an ongoing cohort study of women with hypertensive disorders of pregnancy (HDP) enrolled at the admission to the labor and delivery floor. From this dataset, multigravida women with a prior diagnosis of preeclampsia were identified and compared to women with no prior history of preeclamps

Increasing access to midwifery models of care: A case study of the first Midwifery-Led Birth Center in a public facility in Malawi.

BACKGROUND: Midwifery-Led Birthing Centers offering the midwifery model of care, as defined by the International Confederation of Midwives, are an evidenced-based intervention proven to offer high-quality, patient-centered, cost-effective care and reduce maternal and neonatal mortality. There are very few of these types of birthing centers in low-middle income countries, where maternal and neonatal mortality are highest. Additionally, there is limited research and understanding of how to implement such centers. From 2020 to 2025, multi-stakeholders collaborated to develop the first Midwifery-L

Term birth and levothyroxine dosage are significant factors associated with permanent congenital hypothyroidism: experience from a medical center in Taiwan.

BACKGROUND: Before the introduction of newborn screening, congenital hypothyroidism was the leading cause of intellectual disability in infants and children. Patients with permanent congenital hypothyroidism require lifelong levothyroxine supplementation to prevent intellectual disability and growth failure. With progressively lower thyrotropin (TSH) cutoffs in newborn screening programs, more transient congenital hypothyroidism cases-requiring only temporary treatment-have also been identified. To avoid unnecessary medication use and reduce the burden on healthcare systems, early differentiat

Iatrogenic Esophageal Perforation in Extremely Premature Babies With Low Birth Weight: A Case Series at a Single Tertiary-Care Center.

BACKGROUND: Iatrogenic esophageal perforation (EP) is a rare but serious complication of nasogastric tube (NGT) or orogastric tube (OGT) insertion in neonates. Early recognition of this condition is often challenging, as clinical and radiological findings may mimic esophageal atresia. Reported mortality is high, largely owing to the sequelae of extreme prematurity and associated comorbidities. The purpose of this study was to review cases of iatrogenic EP in extremely premature babies with low birth weight and to describe the presentation, management, and outcomes. METHODS: Retrospective char

Impact of physical activity on maternal and neonatal outcomes among women with vaginal births-single center prospective cohort study.

BACKGROUND: Optimal levels of physical activity during pregnancy are associated with numerous health benefits for both the mother and fetus. Additionally, maternal exercise has been linked to improved cardiovascular fitness, reduced lower back pain, enhanced mental wellbeing and favorable neonatal outcomes, including a lower risk of macrosomia and improved placental function. However, both insufficient and excessive physical activity levels may have adverse effects, highlighting the need for a balanced approach. OBJECTIVE: The aim of this study was to evaluate the impact of physical activity

Recovery of the Total Birth Brachial Plexus Palsy without Surgical Treatment: A Single-Center, Retrospective Study and Literature Review.

Background  Spontaneous recovery of a birth brachial plexus disorder is difficult to predict. Although root avulsion and total plexus injury is indicated for surgical management, early nerve surgery is still doubtful. Hand motion is obviously an important indicator for predicting the function of an affected limb. However, the timing for diagnosing a transient or true total plexus injury from hand recovery is controversial. This study aimed to report the recovery time of total birth brachial plexus palsy in patients who did not undergo surgery due to various reasons. Methods  In this 15-year re

Synergistic Clinico-Radiological Assessment of Term Newborns Afflicted by Birth Asphyxia in a Tertiary Medical Center: A Prospective Observational Study.

Background and objectives Hypoxic-ischemic encephalopathy (HIE) remains a critical concern in newborns, with therapeutic hypothermia (TH) serving as a key treatment strategy. However, assessing long-term outcomes requires a comprehensive approach, as children may develop neuropsychological and learning challenges even in the absence of noticeable motor impairments. This study aims to explore the correlation between electroencephalography (EEG) and neuroimaging findings with the clinical severity of HIE. Materials and methods This prospective observational study included a sample of 110 term ne
