Femoroacetabular impingement is a cause of hip pain that in selected cases can be treated by removal of impinging bone (osteoplasty). No absolute parameters for osteoplasty exist. We present a technique for dynamic evaluation of bony impingement and control of arthroscopic osteoplasty using intraoperative fluoroscopy. With the patient supine on a fracture table, the C-arm is positioned between the legs. Femoral anteversion is measured with the C-arm vertical and the hip flexed 90° (Dunn view). The C-arm is then tilted back 25° from vertical to create a profile view of the anterior rim, and the

The results in 102 acetabuloplasties have been assessed in eighty-six patients, with a minimum follow-up of five years. Clinical and radiological assessment was carried out according to a score system very similar to that used in previous studies, and the results were expressed as excellent, good, fair and poor according to the score obtained. Any hip requiring further stabilising procedures was automatically grouped as a failure. Subtrochanteric femoral ostestomy was used when femoral anteversion was excessive. The results revealed that 73 per cent of the hips were satifactory (excellent or g

PURPOSE: The study of the interrelation between hip and spine disorders is gaining increasing importance in the last years, but the link between Hip Osteoarthritis (HOA) and Low Back Pain (LBP) remains still unclear. Aim of the study is to assess the relationship between Femoral Neck Anteversion (FNA), LBP, and spinopelvic parameters in patients undergoing Total Hip Replacement (THR) for unilateral severe primary HOA.
MATERIALS AND METHODS: 91 patients were recruited. Inclusion criteria were: grade 5 or 6 unilateral HOA, according to Turmezei, and Harris Hip score (HHS) <60. Exclusion criteri

BACKGROUND: The use of a cemented monoblock dual-mobility implant into a fully porous cup is indicated for patients with acetabular bone loss who have a high risk of postoperative hip instability. Patients undergoing lumbar fusion for sagittal spinal deformities have an increased risk of hip dislocation (7.1%) and should be assessed on sitting and standing radiographs1. Gabor et al. conducted a multicenter, retrospective study assessing the use of a cemented monoblock dual-mobility bearing in a porous acetabular shell in patients with acetabular bone loss and a high risk of hip instability2. BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has increasingly become a reliable intervention to improve function and pain for a variety of indications. Glenoid fixation is critical to the success of rTSA, and traditionally the standard anatomic centerline is used, allowing for central baseplate fixation. However, in cases of severe bone loss, baseplate fixation along the alternative centerline has been described. The baseplate axis is anteverted and inferiorly tilted, allowing for fixation into a column of bone where the base of the scapular spine and coracoid meet, thereby prioritiz

BACKGROUND: Femoral neck anteversion (FNA) is a key determinant of hip biomechanics and component positioning in total hip arthroplasty (THA). Most reference values for femoral version are derived from Western populations and non-robotic measurement techniques. Data defining native femoral version in the Indian population using CT-based robotic planning are limited.
METHODS: This multicenter cross-sectional study analyzed 742 hips undergoing primary robotic-assisted THA using the MAKO THA 4.0 system. Native femoral version was calculated automatically from preoperative CT scans as the angle b

BACKGROUND: Most femoral stem designs used in total hip arthroplasty (THA) take into account the proximal femoral morphotype in terms of lateralization and neck-shaft angle (NSA) but not version. The objective of this study was to analyze the acetabular anteversion and femoral anteversion (FA) values in a large cohort according to the 3-dimensional (3D) morphotype of the proximal femur. Our hypothesis was that FA is an anatomic parameter independent of the coronal morphotype (varus, neutral, valgus).
METHODS: A retrospective study based on prospectively collected data included all patients wh

Although total hip arthroplasty is now a classic procedure that is well controlled by orthopedic surgeons, some cases remain complex. Difficulties may be due to co-morbidities: obesity, skin problems, muscular problems, a history of neurological disease or associated morphological bone deformities. Obese patients must be informed of their specific risks and a surgical approach must be used that obtains maximum exposure. Healing of incisions is not a particular problem, but adhesions must be assessed. Neurological diseases may require tenotomy and the use of implants that limit instability. Spe

OBJECTIVE: The reconstruction of acetabular bone defect in developmental dysplasia of the hip (DDH) is a great challenge. Although several successful solutions have been raised, their efficacy and reliability have not been fully substantiated. This work aims to present a simple, economic and effective acetabular reconstructive technique to resolve the massive acetabular bone defect in DDH scenario.
METHODS: This is a case series and observational study investigating the effectiveness and safety of extra-articular blocking technique in patients diagnosed as DDH of Crowe type II-III and Hartofi

BACKGROUND: Many anatomic factors contribute to patellar instability, including excessive femoral anteversion and external tibial torsion. Derotational osteotomies may be performed concomitantly with medial patellofemoral ligament (MPFL) reconstruction to address multiple factors and improve patellofemoral engagement.
INDICATIONS: Proper identification of axial plane abnormalities is important as recurrence of instability may be higher in patients when these factors are not addressed. Femoral anteversion or tibial torsion of >20° can generate abnormal lateral forces in the patellofemoral join

BACKGROUND Increased femoral anteversion (FA) has been demonstrated in patients with recurrent patellar dislocation (RPD). However, the effect of FA on the patellar tilt angle (PTA) in patients with RPD is unclear. The aim of this study was to compare the FA and PTA between patients with RPD and healthy controls and to investigate the relationship between FA and PTA. MATERIAL AND METHODS A total of 30 knees with RPD and 30 knees from healthy volunteers were evaluated with computed tomography (CT). The FA and PTA were measured and compared between the RPD and control groups. Correlations betwee

BACKGROUND: Anteverted pelvis (AP) subgroup is unusual subtype in asymptomatic population characterized by low pelvic incidence (PI), high lumbar lordosis (LL), and a sloped sacrum. However, the contribution of paraspinal muscles (PSM) to this specific alignment has not been clarified. The purpose of this study was to determine if the AP subgroup was associated with different characteristics in the cross-sectional area (CSA) and fat infiltration (FI) of PSM.
METHODS: A cross-sectional study was conducted with 168 asymptomatic volunteers. MRI was used to assess the CSA and FI of the multifidus

A tool is presented to assess the lower extremities using linear and angular measurements under full weight-bearing in three dimensions. While the patient is standing on both legs in a frame representing a calibrated three-dimensional (3D) space, standard radiographs of the hips (anteroposterior view, AP) as well as of the knees (AP and lateral views) are taken. The coordinates for localisation of the malleoli are determined clinically in respect to the coordinate system of the 3D space. Characteristic landmarks are digitized on the calibrated radiographic films, and the so determined coordina

OBJECTIVE: To summarize the biomechanical characteristics, diagnosis, and hip arthroscopic treatment of borderline developmental dysplasia of hip (BDDH) with Cam-type femoroacetabular impingement (Cam FAI).
METHODS: The literature on BDDH with Cam FAI at home and abroad in recent years was extensively reviewed and analyzed.
RESULTS: In patients with BDDH and Cam FAI, the femoral neck anteversion angle and femoral neck shaft angle increase, the pelvis tilts, and the acetabulum rotates, resulting in instability of the hip joint. In order to maintain the stability of the hip joint, the directio

INTRODUCTION: While outcomes after total hip arthroplasty (THA) are generally excellent, prosthetic dislocation remains a multifactorial complication. This study hypothesized that differences in combined anteversion (CA) exist between patients with and without dislocation. The objectives were to (1) compare postoperative alignment parameters between dislocated and stable hips, (2) assess differences of alignement according to surgical approach, and (3) evaluate patient-related risk factors for dislocation.
MATERIALS AND METHODS: In this retrospective case-control study, 37 dislocated hips wer

BACKGROUND: Transtrochanteric anterior rotational osteotomy (ARO) is joint-preserving surgery for patients with osteonecrosis of the femoral head (ONFH). During ARO, femoral neck-shaft varus angulation by changing intertrochanteric osteotomy plane is often designed to obtain a sufficient postoperative intact ratio. However, the effect of intertrochanteric osteotomy plane on postoperative femoral anteversion has not been well examined. Therefore, we performed a simulation study of ARO to determine how intertrochanteric osteotomy plane and preoperative femoral anteversion affect both femoral nec
