Erythroid proliferations in myeloid neoplasms.

Prominent erythroid proliferations (in which erythroid elements comprise ≥50% of total bone marrow cells) can be seen in various hematopoietic stem cell neoplasms. The myeloproliferative neoplasm polycythemia vera exhibits effective, overexuberant erythropoiesis resulting in an increased red blood cell mass; in contrast, most other diseases characterized by erythroid predominance exhibit ineffective hemopoiesis. The latter include acute erythroid leukemia (erythroid-myeloid and pure erythroid leukemia subtypes) as well as some cases of myelodysplastic syndromes, acute myeloid leukemia with mye

Presumed tumor-infiltrating clonal hematopoiesis unmasking a concomitant systemic mastocytosis with associated myeloid neoplasm in a patient with metastatic melanoma.

We report a unique case in a 67-year-old male patient in  whom melanoma tumor sequencing identified DNMT3A and SF3B1 mutations suggesting tumor-infiltrating clonal hematopoiesis. Following comprehensive bone marrow evaluation, the patient was discovered to have a well-differentiated (WD) systemic mastocytosis with an associated myeloid neoplasm (SM-AMN) best classified as myelodysplastic syndrome with SF3B1 mutation. Notably, the SM component was negative for KIT D816V mutation, as expected for WD-SM; however, the immunophenotype was atypical with aberrant expression of CD25. This case highlig

Myeloid tumors accompanying systemic mastocytosis, basophilia, and abnormal platelet-derived growth factor receptor β: A case report.

INTRODUCTION: Myeloid neoplasms with platelet-derived growth factor receptor β (PDGFRB) rearrangement usually present with eosinophilia in the peripheral blood and bone marrow. Here we report a case of systemic mastocytosis related myeloid neoplasms with basophilia and PRKG2-PDGFRB fusion gene. PATIENTS CONCERNS: A 53-year-old male patient felt fatigue with thrombocythemia and normal hemoglobin over 2 years. Considering the possibility of primary thrombocytosis, the patient was treated with hydroxyurea and interferon. Then the therapy was stopped due to adverse events and worsen condition. D

Philadelphia chromosome-positive acute myeloid leukemia with masses and osteolytic lesions: finding of 18F-FDG PET/CT.

Philadelphia chromosome-positive acute myeloid leukemia is controversial and difficult to distinguish from the blast phase of chronic myeloid leukemia. As a myeloid neoplasm, rare cases of this leukemia manifest multiple soft-tissue tumors or bone lytic lesions. In this paper, we describe a 49-year-old male patient who had an abrupt onset with sharp chest pain, fever, fatigue, emaciation, and splenomegaly. 18F-fluoro-deoxy-glucose positron emission tomography/computed tomography (18F-FDG PET/CT) result showed diffuse and uneven hypermetabolic lesions in the bone marrow with peripheral bone mar

Extramedullary masses as presenting features of acute monoblastic leukemia.

Acute monoblastic leukemia is an uncommon type of non-lymphocytic leukemia that generally first manifests with signs and symptoms of bone marrow failure. This study describes four patients with acute monoblastic leukemia in whom the initial manifestations of the disorder were related to extramedullary masses. Three of the four patients were children. All patients had bone marrow involvement. The extramedullary mass lesions of acute monoblastic leukemia may be difficult to distinguish from other hematopoietic neoplasms, such as granulocytic sarcoma, malignant lymphoma and malignant histiocytosi

Neutrophils in BCR::ABL1 negative MPN: Contributors or bystanders of fibrosis?

BCR::ABL1 negative myeloproliferative neoplasms (MPNs) are a heterogenous group of disorders characterized by clonal proliferation of hematopoietic stem and progenitor cells (HSPCs) within the bone marrow. Although the identification of somatic key driver mutations significantly increased both understanding and diagnostic accuracy of MPNs, many questions about the exact pathophysiology remain unanswered. Increased neutrophil count at diagnosis is a well-recognized predictor of worse disease evolution and survival, nonetheless the exact role of neutrophilic granulocytes within MPN pathophysiolo

Synoptic Diagnostics of Myeloproliferative Neoplasms: Morphology and Molecular Genetics.

The diagnosis of a myeloid neoplasm relies on a combination of clinical, morphological, immunophenotypic and genetic features, and an integrated, multimodality approach is needed for precise classification. The basic diagnostics of myeloid neoplasms still rely on cell counts and morphology of peripheral blood and bone marrow aspirate, flow cytometry, cytogenetics and bone marrow trephine biopsy, but particularly in the setting of Ph- myeloproliferative neoplasms (MPN), the trephine biopsy has a crucial role. Nowadays, molecular studies are of great importance in confirming or refining a diagno

FLOCK cluster analysis of plasma cell flow cytometry data predicts bone marrow involvement by plasma cell neoplasia.

We analyzed plasma cell populations in bone marrow samples from 353 patients with possible bone marrow involvement by a plasma cell neoplasm, using FLOCK (FLOw Clustering without K), an unbiased, automated, computational approach to identify cell subsets in multidimensional flow cytometry data. FLOCK identified discrete plasma cell populations in the majority of bone marrow specimens found by standard histologic and immunophenotypic criteria to be involved by a plasma cell neoplasm (202/208 cases; 97%), including 34 cases that were negative by standard flow cytometric analysis that included cl

Disseminated extramedullary myeloid tumor of the gallbladder without involvement of the bone marrow.

Extramedullary myeloid tumors (myeloid sarcomas) are rare neoplasms that are composed of myeloid precursors. They usually arise concurrently with a diagnosis of acute myeloid leukemia, chronic myeloid leukemia, or other myeloproliferative disorders. They may also indicate relapsing disease in a patient with a prior history of leukemia or myeloproliferative disorder. We present our findings of a 63-year-old female diagnosed with extramedullary myeloid tumor first presenting in the gallbladder. She subsequently developed respiratory failure; pre- and postmortem bone marrow studies were negative

Primary bone marrow T-cell lymphoma in a Golden Retriever.

A 6.2-year-old 28-kg (61.7 lb) intact female Golden Retriever was referred due to persistent and multiple cytopenias noted on a routine CBC prior to a mature ovariohysterectomy procedure. The patient's physical examination was unremarkable, and staging of the thorax and abdomen identified no abnormalities. At the referral hospital, moderate hypercalcemia, borderline anemia, and neutropenia were noted. Assessment of bone marrow samples by cytology, histology, immunohistochemistry, and flow cytometry indicated a T-cell neoplasm. The patient was treated with a multi-agent chemotherapy protocol fo

Role of the STING pathway in myeloid neoplasms: a prospero-registered systematic review of principal hurdles of STING on the road to the clinical practice.

Myeloid neoplasms are a group of bone marrow diseases distinguished by disruptions in the molecular pathways that regulate the balance between hematopoietic stem cell (HSC) self-renewal and the generation of specialized cells. Cytokines and chemokines, two important components of the inflammatory process, also influence hematological differentiation. In this scenario, immunological dysregulation plays a pivotal role in the pathogenesis of bone marrow neoplasms. The STING pathway recognizes DNA fragments in the cell cytoplasm and triggers an immune response by type I interferons. The role of ST

Controlled thermal injury of bone. Report of a percutaneous technique using radiofrequency electrode and generator.

Percutaneous ablation of the trigeminal ganglion using radiofrequency energy is an accepted treatment for trigeminal neuralgia. To test the applicability of this system to the percutaneous ablation of bone neoplasms, the authors studied its effects in the normal femurs of four living dogs. Lesions were made of various duration and spaced over time to determine the extent of injury and the natural history of healing. Bone and bone marrow necrosis was limited to a sphere approximately 1 cm in diameter, regardless of the probe size or duration of heating. No complications were encountered. This t

Bone marrow and peripheral blood HLA-G in hematologic diseases presenting with impaired erythropoiesis: HLA-G in disorders with impaired erythropoiesis.

The HLA-G molecule is expressed in few tissues and cells, such as erythroid precursors, in which it has been associated with cell signaling, maturation, and proliferation. Considering that the daily production of erythrocytes is massive, the bone marrow sHLA-G levels are higher than in peripheral plasma, and little attention has been devoted to the role of HLA-G in disorders presenting with defective erythropoiesis, we hypothesized that bone marrow could be a major source of sHLA-G to plasma. We investigated HLA-G (soluble-sHLA-G and membrane-bound) in patients presenting hematologic disorders

Histiocytic Sarcoma.

Histiocytic sarcoma (HS) is a rare hematopoietic malignancy originating from the monocyte/macrophage bone marrow lineage. HS can occur in isolation or in association with other hematological neoplasms such as non-Hodgkin lymphoma (NHL), myelodysplasia, or acute leukemia. Clinically, HS can affect lymph nodes, gastrointestinal tract, skin, bone marrow, and spleen as well as the central nervous system. Most cases of HS follow an aggressive clinical course, with most patients dying of progressive disease within one year of diagnosis.

Aggressive natural killer (NK)-cell leukaemia and extranodal NK/T-cell lymphoma are two distinct diseases that differ in their clinical presentation and cytogenetic findings.

AIMS: Aggressive natural killer (NK)-cell leukaemia (ANKCL) and extranodal NK/T-cell lymphoma (ENKTCL) with secondary bone marrow involvement are rare bone marrow NK/T-cell neoplasms and share similar features. This study aimed to distinguish these two entities. METHODS AND RESULTS: We studied bone marrow NK/T-cell neoplasms by classifying them into those with no extramedullary mass (group 1, eight cases) and those with extramedullary mass (group 2, 13 cases). The two groups showed similar clinical presentations and pathological features. Fever and cytopenia were the most common clinical pres

Image-based explainable artificial intelligence accurately identifies myelodysplastic neoplasms beyond conventional signs of dysplasia.

Cytomorphological assessment of bone marrow smears (BMS) is essential in the diagnosis of myelodysplastic neoplasms (MDS), yet manual evaluation is prone to inter-observer variability. We trained end-to-end deep learning models to distinguish between MDS, acute myeloid leukemia, and bone marrow donor BMS with high accuracy in internal tests and external validation. Occlusion sensitivity mapping revealed the high importance of nuclear structures beyond canonical dysplasia, demonstrating accurate, interpretable MDS detection without labor-intensive cell-level annotation.
