Femoroacetabular impingement predisposes to traumatic posterior hip dislocation.

BACKGROUND: Traumatic posterior hip dislocation in adults is generally understood to be the result of a high-energy trauma. Aside from reduced femoral antetorsion, morphologic risk factors for dislocation are unknown. We previously noticed that some hips with traumatic posterior dislocations had evidence of morphologic features of femoroacetabular impingement (FAI), therefore, we sought to evaluate that possibility more formally. QUESTIONS/PURPOSES: We asked whether hips with a traumatic posterior hip dislocation present with (1) a cam-type deformity and/or (2) a retroverted acetabulum. METH

Recovery potential after postnatal shoulder paralysis. An animal model of neonatal brachial plexus palsy.

BACKGROUND: Injury to the brachial plexus during birth results in paralysis of the upper extremity in as many as one in 250 births and can lead to substantial functional deficits in the shoulder. The goal of this study was to characterize the development of bone and joint deformities in paralyzed neonatal shoulders and to assess the improvement of these deformities after muscle function recovery with use of an animal model. METHODS: Intramuscular injections of botulinum toxin were used to paralyze the supraspinatus, infraspinatus, and posterior deltoid of the left shoulders of mice at birth.

The importance of glenoid version in patients with anterior dislocation of the shoulder.

BACKGROUND: Although increased retroversion of the glenoid has been shown to be an important factor in posterior instability of the shoulder, there are few studies reporting glenoid bone structure as a risk factor in anterior dislocation of the shoulder. This study aimed to compare glenoid version in patients with anterior dislocation of the shoulder and individuals in a control group with no shoulder problems before undergoing computed tomography and to assess a possible relationship between demographic characteristics and glenoid version angle. METHODS: The study group comprised 63 patients

Combination of modified free vascularized fibular grafting and reverse Less Invasive Stabilization System (LISS) for the management of femoral neck nonunion in patients thirty years of age or younger.

Femoral neck nonunion (FNN) is a potential complication in patients with displaced femoral neck fractures, occurring in 33%. This may lead to early hip dysfunction or arthroplasty. Combination of modified free vascularized fibular grafting (FVFG) and a reverse LISS may provide a reasonable means to salvage a painless, functional and native hip. Between August 2010 and August 2012, sixteen patients with femoral neck nonunion were treated with a combined procedure involving modified free vascularized fibular grafting and a reverse LISS. The average age of them is 20.3 years (range 12.0-28.0 year

Morphological variations of residual hip dysplasia in the adult.

Common understanding of residual hip dysplasia of the adult characterizes it as a uniform, anterolateral insufficiency of femoral head coverage; the proximal femur may or may not have increased valgus and antetorsion. This view is reflected by the parameters used for quantification but also by the classic surgical approaches to compensate or correct the deformity. More than two decades of dedication to hip dysplasia and its surgical treatment, but also the application of modern diagnostic tools such as magnetic resonance arthrography have allowed us to portray hip dysplasia more polymorphicall

Biomechanical Differences in Young Patients With True Femoral Retrotorsion: Correlations Between Gait Deviations, Clinical Measurements, and Imaging Data.

BACKGROUND: Torsional abnormalities of the femur, such as femoral retrotorsion, can significantly impact gait and joint biomechanics in children and adolescents. However, the resulting gait patterns as well as the relationship between static imaging measurements and dynamic gait deviations remain unclear. This study aimed to investigate biomechanical differences in gait between young individuals with true femoral retrotorsion and typically developing peers, examining correlations between gait deviations, clinical measurements, and imaging data. METHODS: A multicenter retrospective study was c

Minimum two-year follow-up of a reverse total shoulder arthroplasty using a wedged baseplate.

BACKGROUND: Avoiding inclination of the glenoid baseplate in reverse shoulder arthroplasty often requires considerable glenoid reaming. It is proposed that the use of a metal wedged baseplate in all patients can achieve neutral inclination with reduced glenoid reaming. MATERIALS AND METHODS: A prospective clinical single-centre study with minimum two-year follow-up was carried out. Glenoid deformity was classified on CT and surgery planned using BluePrintTM. The Tornier Perform® Reversed Wedged Augmented Glenoid was used in all cases. Clinical outcome scores and radiographs were assessed. RE

[Torsion and torsional development of the lower extremities].

BACKGROUND: Torsion is a frequent reason for consultation in paediatric orthopaedics. Torsion of the femur and the tibia in children change during growth. Depending on the age and possibility for compensation, this can be reflected in the gait pattern. Different causes can affect the normal development of torsion. DIAGNOSTICS: In the context of paediatric orthopaedic assessment, the distinction between physiological and pathological torsion is essential. In addition to the patient history and observation of the gait pattern, as well as a detailed clinical examination, additional imaging techn

Midterm radiographic outcomes of anatomic total shoulder arthroplasty in biplanar glenoid deformities.

BACKGROUND: Optimal management of retroversion in anatomic total shoulder arthroplasty (aTSA) remains controversial, and limited attention has been directed to the impact of glenoid inclination. Prior biomechanical study suggests that residual glenoid inclination generates shear stresses that may lead to early glenoid loosening. Combined biplanar glenoid deformities may complicate anatomic glenoid reconstruction and affect outcomes. The goal of this matched-cohort analysis was to assess the relationship between biplanar deformities and midterm radiographic loosening in aTSA. METHODS: The stud

[Femoroacetabular impingement related to Legg-Calvé-Perthes disease].

Femoroacetabular impingement is an undesirable contact between acetabular rim and femoral neck and presents abnormality of proximal femoral part and acetabulum, as well. Two forms may appear: cam impingement and pincer impingement. Femoroacetabular impingement related to Legg-Calvé-Perthes disease may be caused by various reasons, as the consequence of the disease itself, and as the consequence of its treatment. Coxa magna deformity (large femoral head and neck) and coxa brevis deformity (shortened femoral neck) may produce cam femoroacetabular impingement during hip flexion. After the disease

A Posterior Acromial Bone Block Augmentation Is Biomechanically Effective at Restoring the Force Required To Translate the Humeral Head Posteriorly in a Cadaveric, Posterior Glenohumeral Instability Model.

PURPOSE: To assess the biomechanical utility of a posterior acromial bone block (PABB) for the treatment of posterior glenohumeral instability. METHODS: Ten fresh-frozen cadaveric specimens were obtained based upon an a priori power analysis. A 2.5-cm scapular spine autograft was harvested from all shoulders. A custom robot device was used to apply a 50-N compressive force to the glenohumeral joint. The humeral head was translated 10 mm posteroinferiorly at 30 degrees from the center of the glenoid at a rate of 1.0 mm/s in 6 consecutive conditions: (1) intact specimen, (2) intact with PABB, (

Comparison of 3 Pediatric Pelvic Osteotomies for Acetabular Dysplasia Using Patient-specific 3D-printed Models.

BACKGROUND: Children with developmental dysplasia of the hip may require a pelvic osteotomy to treat acetabular dysplasia. Three osteotomies are commonly performed in these patients (Pemberton, Dega, and San Diego), though comparative studies of each are limited. The purpose of this study was to compare changes in acetabular morphology (acetabular version, volume, and octant coverage angles) created by these 3 osteotomies using matched patient-specific 3D-printed pelvic models. METHODS: Fourteen patients with developmental dysplasia of the hip and preoperative computed tomography (CT) imaging

The effect of glenohumeral radial mismatch on different augmented total shoulder arthroplasty glenoid designs: a finite element analysis.

BACKGROUND: Augmented glenoid implants to correct bone loss can possibly reconcile current prosthetic failures and improve long-term performance for total shoulder arthroplasty. Biomechanical implant studies have suggested benefits from augmented glenoid components, but limited evidence exists on optimal design. METHODS: An integrated kinematic finite element analysis (FEA) model was used to evaluate optimal augmented glenoid design based on biomechanical performance in translation in the anteroposterior plane similar to clinical loading and failure mechanisms with osteoarthritis. The Reliability of Commonly Used Radiographic Parameters in the Evaluation of the Pre-Arthritic Hip: A Systematic Review.

BACKGROUND: Radiographs play a central role in the evaluation of the extent of arthritic change and the quantification of morphological parameters characteristic of femoroacetabular impingement (FAI) and/or dysplasia. Reporting of these parameters is widespread in the literature, occasionally as an instrument for data pooling in collaborative research endeavors. Yet, the metrological properties of commonly used radiographic descriptors of acetabular and femoral architecture relevant to the field of hip preservation have not been reviewed systematically. The purpose of the present review was tw
