It has become widely accepted that modern titanium dental implants are successful and predictable. However, there is bone loss in relation to any interface (microgap) that occurs between the implant parts when the microgap is in proximity or apical to the osseous crest. The principle of biologic width may be responsible for such bone remodeling in an apical manner. A new, biologically derived implant design that conceptually may minimize bone remodeling and promote better bone and overlying gingival contours and stability is suggested. The parabolic occlusal platform of this design is in harmo

Osteoporosis-induced implant loosening can lead to surgical failure and impede patient recovery. Various approaches, including anti-osteoporosis drugs, have been explored to improve implant stability, however, they fail to improve bone formation or restore bone balance, underscoring the need for novel therapeutic strategies. Certain natural products, such as Chikusetsusaponin IVa (CHSs IVa) exhibit strong antioxidant and bone-protective properties, making them viable options for improving implant stability. This study aims to investigate the inhibitory effect of CHS IVa on ferroptosis and its

Plaque-induced lesions can produce peri-implant bone loss with ultimate implant loss. Although the peri-implant tissues seem to be more resistant than the periodontal ones to plaque and calculus, they can produce a more extensive spread of the infection to the deeper tissues around implants. The case of a 45-year-old female patient is presented in which, over a three year period, there was a progressive loss of peri-implant bone and the formation of a periapical radiolucency with an external fistula. The implant was removed and examined with the cutting-grinding system. Microscopy examination

The primary objective was to compare the subsidence resistance properties of a novel 3D-printed spinal interbody titanium implant versus a predicate polymeric annular cage. We evaluated a 3D-printed spinal interbody fusion device that employs truss-based bio-architectural features to apply the snowshoe principle of line length contact to provide efficient load distribution across the implant/endplate interface as means of resisting implant subsidence. Devices were tested mechanically using synthetic bone blocks of differing densities (osteoporotic to normal) to determine the corresponding resi

Oral squamous cell carcinoma (OSCC) around a dental implant is a rare pathologic condition. This report describes a case of recurrent OSCC surrounding a dental implant, histopathologic findings, and a literature review of this condition. A 58-year-old Japanese woman underwent chemoradiotherapy for OSCC in the right lower gingiva, resulting in a complete response. Nine years after primary chemoradiotherapy, a dental implant was placed in her atrophic mandible. Three years later, an OSCC developed around the dental implant in the right lower premolar region. Marginal mandibulectomy was performed

BACKGROUND: Endoprosthetic reconstruction after oncologic resection of bone tumors requires stable fixation between the prosthesis and residual host bone. Compressive osseointegration has been developed as an alternative to traditional stemmed implants to address the challenges and complications of achieving this fixation. Sufficient time has now passed from the advent of compressive implants to allow for an assessment of the intermediate-term and long-term results of this form of fixation.
QUESTIONS/PURPOSES: At a minimum follow-up of 10 years after implantation of a compressive osseointegra

Osseointegration (OI) is the direct anchorage of a metal implant into bone, allowing for the connection of an external prosthesis to the skeleton. Osseointegration was first discovered in the 1960s based on the microscopic analysis of titanium implant placed into host bone. New bone was observed to attach directly to the metal surface. Following clinical investigations into dentistry applications, OI was adapted to treat extremity amputations. These bone anchored implants, which penetrate the skin and soft tissues, eliminate many of the challenges of conventional prosthetic sockets, such as po

The success of dental implants is highly dependent on integration between the implant and intraoral hard/soft tissue. Initial breakdown of the implant-tissue interface generally begins at the crestal region in successfully osseointegrated endosteal implants, regardless of surgical approaches (submerged or nonsubmerged). Early crestal bone loss is often observed after the first year of function, followed by minimal bone loss (< or =0.2 mm) annually thereafter. Six plausible etiologic factors are hypothesized, including surgical trauma, occlusal overload, peri-implantitis, microgap, biologic wid

Glenoid implants used in anatomic total shoulder arthroplasties typically incorporate peripheral pegs as a design feature to support eccentric loads. These peripheral pegs and the implant-cement-bone interface undergo substantial cyclic tensile-compressive loads during normal activity. Therefore, these pegs are of interest in translating the micromechanics of local implant fixation failure to the biomechanics of gross anatomic failure of the glenoid implant after total shoulder arthroplasty. This study used an in vitro peg-cement-bone construct which incorporated bone tissue acquired from oste

The aim of the study was to assess the eventual presence, cellular localization and extent of expression of the osteoclast activating cytokine M-CSF (CSF-1) in the periprosthetic tissues around loose total hip replacement (THR). Synovial-like membrane was obtained from the implant-to-bone interface and pseudocapsule from ten total hip revisions performed for aseptic loosening and compared to ten hip synovial tissue samples obtained from ten patients who had primary THR for osteoarthritis. Osseointegration at the bone-implant interface is a complex biological process that is triggered by the immune-inflammatory response and mediated by various cell types such as, bone-forming cells and immune cells, especially macrophages. The polarization of macrophages to inflammatory/regenerative (M1/M2) phenotypes, as well as the osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) at the bone-implant interface, significantly affects implant osseointegration and even causes implant failure. Graphene oxide (GO) is a promising candidate for performing implant surface functi

In previous finite element studies of cementless hip stems reported in the literature, the effect of bone quality on the initial micromotion and interface bone strain has been rarely reported. In this study, the effect of varying cortical and cancellous bone modulus on initial stem micromotion and interface bone strain was examined and the potential consequence of these changes on bone ingrowth and implant migration was reported. A finite element (FE) model of a total hip replacement (THR) was created and the Young's moduli of cortical and cancellous bone were systematically varied to study th

A porous material for bone ingrowth with adequate pore structure and appropriate mechanical properties has long been sought as the ideal bone-implant interface. This study aimed to assess in vivo the influence of three types of porous titanium implant on the new bone ingrowth. The implants were produced by means of a powder metallurgy technique with different porosities and pore sizes: Group 1 = 30 % and 180 µm; Group 2 = 30% and 300 µm; and Group 3 = 40% and 180 µm;. Six rabbits received one implant of each type in the right and left tibiae and were sacrificed 8 weeks after surgery for histol

OBJECTIVES: To evaluate the accuracy of radiographic filters to assess bone graft loss adjacent to dental implants.
METHODS: Twenty-one dental implants were inserted into dry mandible alveoli. The interface between the implant and the alveolar cortex was filled with particulate synthetic bone graft. Three levels of bone graft loss (GL) were simulated: exposure of the implant-implant cover interface, exposure of the first implant thread, and exposure of the second implant thread. Periapical radiographs were acquired with VistaScan system, and all images were saved without an image filter (orig

A streaming potential experiment was conducted on in vitro bone to determine the effects polyvalent salts have on bone zeta potential. Polyvalent metal salts would be introduced into local physiological electrolytes by corrosion of a metal implant or dissolution of a ceramic implant. It was hypothesized that a change in zeta potential of mineralized tissue, resulting from the introduction of polyvalent salts, could be a cause of tissue instability at the implant interface. Zeta potentials measured in bone were on the order of a few millivolts. Strongly hydrolytic metal cations such as Al+3 and

It is a clinical challenge to obtain a sufficient orthopaedic implant fixation in weak osteoporotic bone. When the primary implant fixation is poor, micromotions occur at the bone-implant interface, activating osteoclasts, which leads to implant loosening. Bisphosphonate can be used to prevent the osteoclastic response, but when administered systemically its bioavailability is low and the time it takes for the drug to reach the periprosthetic bone may be a limiting factor. Recent data has shown that delivering bisphosphonate locally from the implant surface could be an interesting solution. Lo

One of the main factors affecting the survival of a total knee arthroplasty (TKA) is the fixation method. The constraints placed on the bone-implant interface of a mobile-bearing TKA must be taken in account during the design and evaluation phases. For more than two decades, calcium phosphate ceramics, particularly hydroxyapatitis, have been used in Europe to accelerate the bone integration of cementless implants. A prospective study of patients continuously recruited by three senior surgeons at three French private hospitals has been carried out. There were no exclusion criteria. Eighty-four

BACKGROUND: The long-term success of total ankle replacement (TAR) depends on both bone ingrowth and remodelling. The extreme values of implant-bone micromotion hinder bone ingrowth. Whereas, bone resorption due to bone remodelling is triggered by stress shielding. This study aims to investigate the biomechanical performance of three popular tibial designs (STAR, Salto and Mobility) for TAR with different implant-bone interfacial conditions and bone qualities.
METHODS: In this study, CT data were used for the geometric modelling of bone. The cancellous bone was considered to be heterogeneous

Long-term survival of orthopedic implants requires a strong and compliant interface between the implant and surrounding bone. This paper further explores the in-vivo response to a novel, macro-scale osseointegration surface morphology. In this study, we examine the effects of material composition on osseointegration in relation to the controlled surface geometry. The pillared surface is constructed of discontinuous surface geometry which creates an open space for unencumbered bone migration. In creating an open, macro-scale morphology we have demonstrated a bone migration and integration that

Electrical signals serve as important modulators in bone regeneration, yet recreating this self-sustaining bioelectrical microenvironment at the peri-implant interface remains challenging. In this study, we engineered pyroelectric nanostructure coatings on titanium (Ti) implants to enhance implant osteointegration through temperature fluctuation-driven electrical stimulation. The pyroelectric titanium (pyroTi) implants leverage the combined effects of the pyroelectric effect and cold stimulation to amplify calcium ion (Ca2+) influx by activating cold-sensitive transient receptor potential mela
