Preliminary dosimetric study on feasibility of multi-beam boron neutron capture therapy in patients with diffuse intrinsic pontine glioma without craniotomy.

Diffuse intrinsic pontine glioma is a very frustrating disease. Since the tumor infiltrates the brain stem, surgical removal is often impossible. For conventional radiotherapy, the dose constraint of the brain stem impedes attempts at further dose escalation. Boron neutron capture therapy (BNCT), a targeted radiotherapy, carries the potential to selectively irradiate tumors with an adequate dose while sparing adjacent normal tissue. In this study, 12 consecutive patients treated with conventional radiotherapy in our institute were reviewed to evaluate the feasibility of BNCT. NCTPlan Ver. 1.1.

A Monte Carlo dosimetry-based evaluation of the 7Li(p,n)7Be reaction near threshold for accelerator boron neutron capture therapy.

Advanced methods of boron neutron capture therapy (BNCT) use an epithermal neutron beam in conjunction with tumor-targeting boron compounds for irradiation of glioblastomas and metastatic melanomas. A common neutron-producing reaction considered for accelerator-based BNCT is 7Li(p,n)7Be, whose cross section increases very rapidly within several tens of keV of the reaction threshold at 1.88 MeV. Operation in the proton energy region near threshold will have an appreciable thick target neutron yield, but the neutrons produced will have relatively low energies that require little moderation to re

Comparative biological evaluation of [18F]fluciclovine, [18F]FBPA-Fr, and [18F]FET for boron neutron capture therapy: Preclinical validation and clinical feasibility.

PURPOSE: The PET tracer 4‑borono-2-[18F]-fluoro-phenylalanine fructose ([18F]FBPA-Fr) is commonly used to estimate the biodistribution of L-4-dihydroxy-borylphenylalanine fructose (BPA-Fr). However, its limited regulatory approval has prompted the exploration of alternative imaging agents. In this study, we evaluated the feasibility of [18F]fluciclovine and [18F]fluoroethyl-tyrosine ([18F]FET) PET for pre-BNCT assessment, with direct comparison to [18F]FBPA-Fr. METHODS: In vitro cellular uptake studies were performed using two different cancer cell lines. In vivo microPET/MR imaging and biodi

Lyophilized tumour cell-loaded 10B-doped carbon dots for simultaneous boron neutron capture therapy and enhancement of antitumor immunity of prostate cancer.

10B agents are critical components in the application of boron neutron capture therapy (BNCT) for tumor treatment. The development of a multi-functional 10B agent plays an important role in the application of BNCT. This study presents a pioneering lyophilized-tumour-cell-based approach for the enhanced accumulation of 10B-doped carbon dots (10B-CDs) within tumour tissue, irradiating thermal neutrons to treat prostate cancer. In this platform, lyophilized RM-1 cells function as both tumor cell antigens and delivery vehicles for 10B-CDs. This design facilitates the co-delivery of tumor antigens

Profile analysis of adverse events after boron neutron capture therapy for head and neck cancer: a sub-analysis of the JHN002 study.

The purpose of this study was to outline the course and profile of adverse events specific to boron neutron capture therapy (BNCT) for head and neck cancer. This was a sub-analysis of the phase II JHN002 trial. Patients received 400 mg/kg borofalan(10B), followed by neutron irradiation. The course of adverse events after BNCT was documented in the JHN002 Look Up study. Patients were grouped into face/front (FF), face/lateral (FL) and neck (N) beam groups according to the point of skin incidence of the epithermal neutron beam axis, and the profile of adverse events dependent on beam incidence p

Similar T/N ratio between 18F-FBPA diagnostic and BPA therapeutic dosages for boron neutron capture therapy in orthotropic tongue cancer model.

OBJECTIVE: The tumor-to-normal tissue (T/N) boron ratio is determined in a patient prior to boron neutron capture therapy (BNCT) using 4-borono-2-18F-fluoro-L-phenylalanine (18F-FBPA) positron emission tomography (PET). The T/N ratio is used as a reference parameter to calculate BNCT dose and to evaluate treatment effects. The boronophenylalanine (BPA) dosage for BNCT treatment is higher than the 18F-FBPA dosage for PET diagnosis. Therefore, we aimed to determine whether the T/N ratios between diagnosis and treatment were correlated. METHODS: In this study, SAS tongue cancer cells were used t

Boron neutron capture therapy for recurrent pediatric sialoblastoma: the first clinical experience and dosimetric considerations for skeletal safety with 4-year follow-up.

Sialoblastoma is a rare, aggressive pediatric salivary gland tumor often refractory to conventional therapies, which carry significant risks of growth impairment in children. We report the 52-month follow-up of a 6-year-old girl with recurrent sialoblastoma treated with two sessions of boron neutron capture therapy (BNCT). She achieved a 23-month complete response after the first session and a sustained partial response following the second. Dosimetric analysis using a conservative α/β ratio of 2.0 Gy for late-responding tissues showed that the equivalent dose in 2-Gy fractions to the mandible

Fluorine-18 labeling PEGylated 6-boronotryptophan for PET scanning of mice for assessing the pharmacokinetics for boron neutron capture therapy of brain tumors.

Two tryptophan compound classes 5- and 6-borono PEGylated boronotryptophan derivatives have been prepared for assessing their aqueous solubility as formulation of injections for boron neutron capture therapy (BNCT). The PEGylation has improved their aqueous solubility thereby increasing their test concentration in 1 mM without suffering from toxicity. In-vitro uptake assay of PEGylated 5- and 6-boronotryptophan showed that the B-10 concentration can reach 15-50 ppm in U87 cell whereas the uptake in LN229 cell varies. Shorter PEG compound 6-boronotryptophanPEG200[18F] was obtained in 1.7 % radi

Development of a practical and high-speed deep learning-based dose calculation model in boron neutron capture therapy for head and neck cancer.

BACKGROUND: In boron neutron capture therapy (BNCT), Monte Carlo (MC) dose calculations are commonly employed because of the complicated neutron reactions. However, MC dose calculations are generally time-consuming. Recently, deep learning (DL)-based dose prediction/calculation has attracted increasing attention; however, the applications of DL models in BNCT are limited and have not been investigated extensively. In addition, there are no practical DL models that can be employed in BNCT clinical practice. PURPOSE: We propose a practical DL model for head and neck cancers using a commercial t

Feasibility evaluation of neutron capture therapy for hepatocellular carcinoma using selective enhancement of boron accumulation in tumour with intra-arterial administration of boron-entrapped water-in-oil-in-water emulsion.

INTRODUCTION: Hepatocellular carcinoma (HCC) is one of the most difficult to cure with surgery, chemotherapy, or other combinational therapies. In the treatment of HCC, only 30% patients can be operated due to complication of liver cirrhosis or multiple intrahepatic tumours. Tumour cell destruction in boron neutron-capture therapy (BNCT) is due to the nuclear reaction between (10)B atoms and thermal neutrons, so it is necessary to accumulate a sufficient quantity of (10)B atoms in tumour cells for effective tumour cell destruction by BNCT. Water-in-oil-in-water (WOW) emulsion has been used as

Automated radiosynthesis of 2-[18F]BPA for PET-based planning of boron neutron capture therapy (BNCT): rational precursor design, radiofluorination, and characterization of methodology.

BACKGROUND: Boron neutron capture therapy relies on the selective accumulation of boron-containing compounds in tumor tissue, making accurate quantification of boron distribution essential for effective treatment planning. The amino acid analog boronophenylalanine is widely used as a boron delivery agent, yet direct assessment of its biodistribution remains challenging. A fluorine-18 labeled analog, 2-fluoro-boronophenylalanine, offers the potential to visualize and quantify uptake through positron emission tomography. However, reported radiosynthetic methods often suffer from low radiochemica

Boron neutron capture therapy of brain tumors: functional and neuropathologic effects of blood-brain barrier disruption and intracarotid injection of sodium borocaptate and boronophenylalanine.

Sodium borocaptate (BSH) and boronophenylalanine (BPA) are two drugs that have been used clinically for boron neutron capture therapy (BNCT) of brain tumors. We previously have reported that hyperosmotic mannitol-induced disruption of the blood-brain barrier (BBB-D), followed by intracarotid (i.c.) administration of BPA or BSH, either individually or in combination, significantly enhanced tumor boron delivery and the efficacy of BNCT in F98 glioma bearing rats. The purpose of the present study was to determine the short-term neuropathologic consequences of this treatment and the long-term effe

A LAT1-selective PET tracer, 5-[¹⁸F]F-αMe-3BPA, as a companion to its structurally matched ¹⁰B analog in boron neutron capture therapy.

PURPOSE: This study aimed to develop and evaluate 5-[¹⁸F]F-αMe-3BPA, a novel PET probe designed as a theranostic partner for 5F-αMe-3[¹⁰B]BPA in Boron Neutron Capture Therapy (BNCT). The goal was to address limitations of the clinically used BPA/[¹⁸F]FBPA pair, including poor water solubility, limited LAT1 specificity, and suboptimal diagnostic performance, thereby improving tumor-selective drug delivery and enabling accurate prediction of therapeutic biodistribution through structure-matched PET/BNCT. METHODS: LAT1 dependency was tested in vitro using cancer cell lines with differential LAT1

Boron Neutron Capture Therapy (BNCT) for Cutaneous Malignant Melanoma Using 10B-p-Boronophenylalanine (BPA) with Special Reference to the Radiobiological Basis and Clinical Results.

BNCT is a radiotherapeutic method for cancer treatment that uses tumor-targeting 10B-compounds. BNCT for cutaneous melanoma using BPA, a phenylalanine derivative, was first initiated by Mishima et al. in 1987. This article reviews the radiobiological basis of melanoma control and damage to normal tissues as well as the results of clinical studies. Experimental studies showed that the compound biological effectiveness (CBE) values of the 10B (n, α)7Li reaction for melanoma control ranged from 2.5 to 3.3. The CBE values of the 10B (n, α)7Li reaction for skin damage ranged from 2.4 to 3.7 with mo

Investigation of the usability of cone-beam computed tomography images using digital radiography equipment for boron neutron capture therapy treatment planning in the sitting position.

In boron neutron capture therapy (BNCT), treatment planning images are acquired in the recumbent position. However, treatment is occasionally performed in the sitting position. For BNCT treatment planning, we investigated the usability of cone-beam computed tomography (CBCT) images using digital radiography equipment that allows imaging in the sitting position. The dose calculation results in both CBCT and fan beam CT were in good agreement. This method will eliminate the posture difference between planning and treatment.
