Glenohumeral gliding manipulation following interscalene brachial plexus block in patients with adhesive capsulitis.

Previous studies describing nonconservative treatment of shoulder adhesive capsulitis include distention arthrography, brisement techniques, arthrotomy of the anterior/inferior axillary fold and subscapularis tendon, and manipulation under general anesthesia. The purpose of this study was to develop and describe an alternative treatment method that utilizes glide manipulation under interscalene brachial plexus block. Eight patients (four females and four males), age 31-55 years, with a mean age of 44 years, were treated conservatively for adhesive capsulitis of the shoulder and failed to produ

Effective range in the proximal shoulder by ultrasound-guided interscalene brachial plexus block.

BACKGROUND: Ultrasound-guided interscalene block (ISB) is a common anesthesia technique for shoulder surgery. However, its effective area remains unclear. Therefore, this study aimed to evaluate the specific area in the proximal aspect of the shoulder affected by ultrasound-guided ISB. METHODS: A total of 100 consecutive patients who underwent arthroscopic rotator cuff repair under ultrasound-guided ISB in a single institution were included in this study. The effective area of the block was investigated using a pinprick test after surgery along 7 uniquely preset lines: line A extended from th

Changes in pulse wave transit time variability after interscalene brachial plexus block placement.

BACKGROUND: The pulse wave transit time (PWTT) increases with decreased vascular tone resulting from sympathetic blockade caused by regional anesthesia. It oscillates, exhibiting variability due to the interaction between the autonomic nervous and cardiovascular systems. We hypothesized that interscalene brachial plexus block (ISBPB) placement increases the PWTT and reduces the low-frequency power of PWTT variability (LF). METHODS: Fifty-six patients receiving an ISBPB were analyzed. The PWTT was defined as the difference in milliseconds (ms) between the R peak of the electrocardiogram and th

Thermal Imaging to Predict Failed Supraclavicular Brachial Plexus Block: A Prospective Observational Study.

BACKGROUND: Successful brachial plexus blockade produces sympathetic blockade, resulting in increased skin temperature in the blocked segments. This study aimed to evaluate the accuracy of infrared thermography in predicting failed segmental supraclavicular brachial plexus block. METHODS: This prospective observational study included adult patients undergoing upper-limb surgery under supraclavicular brachial plexus block. Sensation was evaluated at the dermatomal distribution of the ulnar, median, and radial nerves. Block failure was defined as absence of complete sensory loss 30 min after bl

Subcoracoid tunnel block as an alternative infraclavicular brachial plexus approach -a case series.

BACKGRUOND: Magnetic resonance neurography shows the brachial plexus cords in the subcoracoid tunnel beneath the pectoralis minor. With an ultrasound scan along the brachial line, the brachial plexus cords in the subcoracoid tunnel can be targeted using an in-plane needle approach. We describe this new approach to the infraclavicular block called the "subcoracoid tunnel block." CASE: Twenty patients were administered with the ultrasound-guided subcoracoid tunnel block for the below-elbow surgery. The contact of the needle tip with cords was visible in all 20 patients. With neurostimulation, t

The effect of dexmedetomidine in coracoid approach brachial plexus block under dual stimulation.

BACKGROUND: Coracoid approach brachial plexus block (CABPB) is safe and effective for clinical anesthesia and analgesia. Dual stimulation can enhance the block effect of CABPB when using nerve stimulator. Dexmedetomidine is a highly selective α-adrenoceptor agonist and it can prolong the duration of anesthesia when it is added into local anesthetics. The aim of this study was to assess the effects of dexmedetomidine on the duration of anesthesia and the effective postoperative analgesia time when it was mixed with ropivacaine for CABPB under dual stimulation. METHODS: A total of 60 patients w

Median nerve injury caused by brachial plexus block for carpal tunnel release surgery.

Carpal tunnel release is required to treat patients with severe carpal tunnel syndrome. The regional anesthesia of the upper limb by brachial plexus block (BPB) may be a good alternative to general anesthesia for carpal tunnel release surgery, because it results in less complications. However, the regional anesthesia still has various side effects, such as hematoma, infection, and peripheral neuropathy. We hereby report a rare case of median nerve injury caused by BPB for carpal tunnel release.

Severe stridor and marked respiratory difficulty after right-sided supraclavicular brachial plexus block.

Brachial plexus block is commonly used for upper limb surgery. Although the procedure is safe, it may be associated with some life-threatening complications. We performed right-sided supraclavicular brachial plexus block for below-elbow amputation in a 45-year-old female. At completion of the block the patient developed marked respiratory difficulty with audible inspiratory stridor. Although SpO(2) decreased to 82% initially, it was increased to 100% by continuous positive airway pressure with a face mask. On conventional direct laryngoscopy, the left vocal cord was found to be in the midline

Restricted infraclavicular distribution of the local anesthetic solution after infraclavicular brachial plexus block.

BACKGROUND AND OBJECTIVES: The distribution of local anesthetic after different approaches for brachial plexus anesthesia could be responsible for the varying rates of side effects, such as phrenic block, hoarseness, and Horner's syndrome associated with each approach. We compared the distribution of local anesthetic within the neurovascular space in infraclavicular block with that of interscalene and supraclavicular block. METHODS: In a prospective analysis using fluoroscopy, we studied the distribution of a solution of local anesthetic containing radiologic contrast medium in 18 patients. S

Relationship between evoked motor response and sensory paresthesia in interscalene brachial plexus block.

BACKGROUND AND OBJECTIVES: This study sought to define the relationship between a paresthesia and a motor response (MR) to electrical nerve stimulation using a peripheral nerve stimulator (PNS) during interscalene block. We sought to determine if at a low amperage (< or =1.0 mA) a MR would precede a paresthesia. METHODS: Twenty-two interscalene blocks were performed using insulated needles and a PNS. A MR was obtained at 0.5 mA and then the PNS was turned off. The needle was further advanced until a paresthesia was elicited. The PNS was again turned on, the needle held immobile, and the amper

Brachial plexus block with the nerve stimulator: motor response characteristics at three sites.

BACKGROUND AND OBJECTIVES: Differences in motor response patterns, minimum electrical currents, and success rates using a nerve stimulator for brachial plexus block were determined for the interscalene, supraclavicular, and axillary approaches. METHODS: Supervised, resident physicians performed 106 brachial plexus blocks. For each block, the minimum stimulating current, the strongest joint motion at the minimum current immediately before local anesthetic injection, presence or absence of other joint motion, and anesthetic success or failure were recorded. RESULTS: In the interscalene group (

[Evaluation of brachial plexus block. Comparison between supraclavicular and interscalene approach (author's transl)].

A scheme for evaluating brachial plexus block was developed, which is based on anatomical structures and which utilizes simple neurological examination techniques. It facilitates the localization of the tip of the cannula near the plexus, gives an idea of the spread of the local anesthetic in the region of the trunks and cords, gives well timed judgement on the success of the block and allows a comparison of the development of the block when using various techniques. Using this scheme, the development of the block was examined following use of the supraclavicular and interscalene approach. Wit

A comparison of the continuous supraclavicular brachial plexus block using the proximal longitudinal oblique approach and the interscalene brachial plexus block for arthroscopic shoulder surgery: A randomised, controlled, double-blind trial.

BACKGROUND: Continuous interscalene brachial plexus block (ISB) is widely used for arthroscopic shoulder surgery, but the incidence of hemidiaphragmatic paresis (HDP) has been reported to reach 100%. Several methods, including injections distal to the C5-C6 nerve roots, have been attempted to reduce the HDP incidence. However, catheter placement distal to the C5-C6 nerve roots interferes with the surgical site. OBJECTIVE: Our primary objective was to describe a new technique, the supraclavicular brachial plexus block (SCB), using the proximal longitudinal oblique approach (PLO-SCB), which can

Critical structures in the needle path of the costoclavicular brachial plexus block: a cadaver study.

PURPOSE: The costoclavicular block is a relatively novel alternative to the infraclavicular block. We aimed to determine the anatomical structures vulnerable to needle injury during a costoclavicular block. METHODS: The needle path consistent with a costoclavicular block approach was performed bilaterally on four lightly embalmed cadavers using ultrasound guidance. Careful dissection was performed with 18-G Tuohy needles in situ and photographs were taken. RESULTS: The needle penetrated the deltoid in six of eight cases and the pectoralis minor in three of eight cases. The subclavius tendon

[The use of neurostimulation with ultrasound-guided brachial plexus block: Does it increase success?].

OBJECTIVES: Infraclavicular brachial plexus blockade is an anesthetic technique used for operations of the hand, wrist, and elbow. Ultrasound (US)-guidance is a recent addition to the surgical technique. The aim of this study was to compare the use of US alone and US with a nerve stimulator in an infraclavicular brachial plexus blockade in terms of the performance time, successful blockade rate, and the quality of sensory block. METHODS: A total of 40 patients who were scheduled for hand, wrist, or elbow surgery were included in the study. The patients were divided into 2 groups: US and USSS

Clinical comparison of two techniques of brachial plexus block for forelimb surgery in dogs.

The objective of this study was to compare two different local anaesthesia techniques for intraoperative analgesia in dogs undergoing surgical procedures distal to the mid humeral diaphysis. Thirty-two dogs were divided into two groups: the US group received an ultrasound-guided brachial plexus block at the level of the first rib and the N group received a neurostimulation-guided paravertebral brachial plexus block. Ropivacaine 0.75% at 1.5mg/kg was injected in each case. Dogs were monitored during the surgical procedure, and rescue analgesia was administered whenever the heart rate, respirato
