Deep brain stimulation for psychiatric diseases: what are the risks?

Despite the application of deep brain stimulation (DBS) as an efficient treatment modality for psychiatric disorders, such as obsessive-compulsive disorder (OCD), Gilles de la Tourette Syndrome (GTS), and treatment refractory major depression (TRD), few patients are operated or included in clinical trials, often for fear of the potential risks of an approach deemed too dangerous. To assess the surgical risks, we conducted an analysis of publications on DBS for psychiatric disorders. A PubMed search was conducted on reports on DBS for OCD, GTS, and TRD. Forty-nine articles were included. Only r

[Disruption of the blood-brain barrier in inflammatory neurological diseases].

The blood-brain barrier (BBB) restricts the exchange of humoral factors and cells between the blood and brain and thus plays a crucial role in maintaining cerebral homeostasis. Disruption of the BBB has been considered the initial key step of the disease process in some inflammatory neurological diseases, including multiple sclerosis (MS), neuromyelitis optica (NMO), and neuropsychiatric systemic lupus erythematosus (NPSLE). In MS, the BBB may be impaired before the formation of demyelinating foci or T-cell infiltration around small vessels; however, once the BBB is disrupted, massive infiltra

Role of COX-2 in inflammatory and degenerative brain diseases.

In the last decade, the potential role of cyclooxygenase-2 (COX-2) and prostaglandins (PGs) in brain diseases has been extensively studied. COX-2 over-expression has been associated with neurotoxiticy in acute conditions, such as hypoxia/ischemia and seizures, as well as in inflammatory chronic diseases, including Creutzfeldt-Jakob disease (CJD) and Alzheimer's disease (AD). However, the role played by COX-2 in neurodegenerative diseases is still controversial and further clinical and experimental studies are warranted. In addition, the emerging role of COX-2 in behavioural and cognitive funct

[Cytopathological changes in human and animal brain in prion diseases].

The study of brain histological sections of humans and animals afflicted by prion diseases has shown that neuronal vacuolization began in and intensely spread from the distal portions of dendrites. Two types of neuronal cell body death were demonstrated, including cytolysis and pyknosis, the latter being usually associated with dendrite vacuolization. The involvement of major glial types in the pathogenesis of prion diseases was established, which argues against the concept of astrocytosis in favor glyosis. The accumulation of phagocytes (microgliocytes, blood macrophages, pericytes) and emigr

Drosophila as a model to study human brain degenerative diseases.

Drosophila has been an ideal system in which to identify molecules and define pathways involved in development, in part because of the powerful genetic approaches that are possible. Many of the molecules and pathways important in development in Drosophila are evolutionarily conserved between fly and human. With its highly evolved nervous system, amenability to genetic analysis, and the full genomic sequence available, Drosophila is a valuable tool for investigating and understanding the molecular mechanisms of neurodegenerative diseases. In order to have neurodegenerative Drosophila mutants, I

[Dyskinesia in various diseases--cerebrovascular disease, head injury, brain tumor].

In the broad sense of the term, dyskinesia refers to the overall spectrum of abnormal involuntary movement. Abnormal involuntary movement includes parkinsonism, chorea, athetosis, myoclonus and ballism. Clinically, the most frequently encountered form of dyskinesia is parkinsonism. In addition to parkinsonism, typical forms of dyskinesia caused by cerebrovascular disease, head injury and brain tumor, based on a review of the literature and our own experience are discussed.

Clinical study of brain retraction in different approaches and diseases.

Pressure of brain retraction was measured with a strain gauge spatula in 31 cases of 21 tumours and 10 aneurysms. They were operated on via different approaches: subfrontal (10 cases), interhemispheric (5), subtemporal (7) and suboccipital (9). The patients' age ranged from 11 to 74 years (average 50). The recorded data were averaged for every 5 minutes and divided into two groups: the first was data taken in the initial stage during the approach to the lesions, and the second were those taken during procedures at and around the lesion. Significant difference in the pressure among different ap

Brain-Derived Neurotrophic Factor and Associated Signaling in Kidney Diseases.

Brain-derived neurotrophic factor (BDNF) is a key member of the neurotrophin family that promotes neuronal function via its receptor kinase TrkB. However, recent work has demonstrated BDNF as a multifaceted regulator beyond its classic roles in the nervous system, particularly in renal patho-physiology. This review summarizes the recent advances about BDNF/TrkB signaling pathway in various kidney diseases, including chronic kidney disease (CKD) and acute kidney injury (AKI). Upon binding TrkB, BDNF initiates specific intracellular signaling pathways, such as PI3K-Akt, MAPK-ERK, and PLCγ, to su

Therapeutic potential of apigenin in neurodegenerative diseases and brain health.

Neurodegenerative disorders are characterized by progressive neuronal loss and remain a major global health concern, largely due to their complex and multifactorial nature. These conditions, including Alzheimer's disease, Parkinson's disease, and Huntington's disease, involve overlapping mechanisms such as oxidative stress, chronic inflammation, mitochondrial dysfunction, and impaired neurotransmission. In recent years, natural bioactive compounds have gained attention as potential therapeutic candidates. Among them, apigenin, a dietary flavonoid widely found in fruits and vegetables, exerts n

Blood-Brain Barrier: Structure, Function, Diseases, and Drug Delivery Systems.

The blood-brain barrier (BBB) is a highly selective and dynamic neurovascular interface essential for maintaining central nervous system homeostasis. This specialized barrier comprises brain microvascular endothelial cells interconnected by tight junctions, supported by pericytes and astrocytic end-feet within the neurovascular unit. While protecting the brain from circulating pathogens and toxins, the BBB presents formidable obstacles to drug delivery, restricting approximately 98% of small-molecule therapeutics and nearly all large biomolecules from reaching the brain parenchyma. BBB dysfunc

Brain photobiomodulation: a potential treatment in Alzheimer's and Parkinson's diseases.

Alzheimer's Disease (AD) and Parkinson's Disease (PD) are common neurodegenerative diseases, characterized by the progressive loss of synapses and neurons, leading to cognitive and motor decline. Their pathophysiology includes cerebral lesions, oxidative stress, neuroinflammation as well as brain-gut axis microbiota dysbiosis. Preclinical investigations demonstrated that brain photobiomodulation (bPBM) reduces oxidative stress and inflammation, increases cerebral blood flow and enhance neurogenesis and synaptogenesis, which makes bPBM a promising treatment in AD and PD. This review focuses on

Targeting PAAN/MIF nuclease activity in parthanatos-associated brain diseases.

Current FDA-approved drugs for neurodegenerative diseases primarily aim to reduce pathological protein aggregation or alleviate symptoms by enhancing neurotransmitter signaling. However, outcomes remain suboptimal and often fail to modify the course of neurodegenerative diseases. Acute neurologic injury that occurs in stroke and traumatic brain injury (TBI) also suffer from inadequate therapies to prevent neuronal cell death, resulting from both the acute insult and the subsequent reperfusion injury following recanalization of the occlusion in stroke. Approaches to prevent neuronal loss in neu

Mitochondrial Dysfunction: A Key Player in Brain Aging and Diseases.

Mitochondria are thought to have become incorporated within the eukaryotic cell approximately 2 billion years ago and play a role in a variety of cellular processes, such as energy production, calcium buffering and homeostasis, steroid synthesis, cell growth, and apoptosis, as well as inflammation and ROS production. Considering that mitochondria are involved in a multitude of cellular processes, mitochondrial dysfunction has been shown to play a role within several age-related diseases, including cancers, diabetes (type 2), and neurodegenerative diseases, although the underlying mechanisms ar

Metabolic Crosstalk between Liver and Brain: From Diseases to Mechanisms.

Multiple organs and tissues coordinate to respond to dietary and environmental challenges. It is interorgan crosstalk that contributes to systemic metabolic homeostasis. The liver and brain, as key metabolic organs, have their unique dialogue to transmit metabolic messages. The interconnected pathogenesis of liver and brain is implicated in numerous metabolic and neurodegenerative disorders. Recent insights have positioned the liver not only as a central metabolic hub but also as an endocrine organ, capable of secreting hepatokines that transmit metabolic signals throughout the body via the bl

Molecular mechanisms of ferroptosis and their involvement in brain diseases.

Ferroptosis is a type of regulated cell death characterized by intracellular accumulation of iron and reactive oxygen species, inhibition of system Xc-, glutathione depletion, nicotinamide adenine dinucleotide phosphate oxidation and lipid peroxidation. Since its discovery and characterization in 2012, many efforts have been made to reveal the underlying mechanisms, modulating compounds, and its involvement in disease pathways. Ferroptosis inducers include erastin, sorafenib, sulfasalazine and glutamate, which, by inhibiting system Xc-, prevent the import of cysteine into the cells. RSL3, stat

Insulin action in the brain: cell types, circuits, and diseases.

Since its discovery over 100 years ago, insulin has been recognized as a key hormone in control of glucose homeostasis. Deficiencies of insulin signaling are central to diabetes and many other disorders. The brain is among the targets of insulin action, and insulin resistance is a major contributor to many diseases, including brain disorders. Here, we summarize key roles of insulin action in the brain and how this involves different brain cell types. Disordered brain insulin signaling can also contribute to neuropsychiatric diseases, affecting brain circuits involved in mood and cognition. Und
