Lactose breath test in adults: Are symptoms predictive of lactose malabsorption?

BACKGROUND/OBJECTIVE: Lactose malabsorption (LM) is common, yet not all malabsorbers develop lactose intolerance (LI). We aimed to assess whether symptoms recorded during the hydrogen breath test (HBT) predict LM and whether methane (CH4) measurement adds diagnostic information. METHODS: In this retrospective study, 199 adults without organic gastrointestinal disease underwent a standardized lactose HBT between January 2022 and February 2025. Pre-test and intra-test gastrointestinal symptoms were graded on a 0-10 Numerical Rating Scale (NRS) and used to classify LI as mild, moderate, or sever

Hydrogen-methane breath testing results influenced by oral hygiene.

The measurement of hydrogen-methane breath gases is widely used in gastroenterology to evaluate malabsorption syndromes and bacterial overgrowth. Laboratories offering breath testing provide variable guidance regarding oral hygiene practices prior to testing. Given that oral dysbiosis has the potential to cause changes in breath gases, it raises concerns that oral hygiene is not a standard inclusion in current breath testing guidelines. The aim of this study was to determine how a pre-test mouthwash may impact hydrogen-methane breath test results. Participants presenting for breath testing who

Congruency of Genetic Predisposition to Lactase Persistence and Lactose Breath Test.

The physiological decline of lactase production in adulthood, in some individuals, is responsible for the so-called "Lactose Intolerance." This clinical syndrome presents with gastrointestinal and non-gastrointestinal symptoms following the consumption of dairy containing food. Lactose intolerance can be evaluated by means of the Lactose Breath Test (phenotype) and/or genetic evaluation of lactase-gene polymorphism (genotype). A comparison of the two tests was carried out in a large number of symptomatic adult subjects, which are selected and not representative of the general population. Congr

The Predictive Value of the Hydrogen Breath Test in the Diagnosis of Fructose Malabsorption.

BACKGROUND: Fructose malabsorption is commonly diagnosed by the hydrogen fructose (H2) breath test. However, the mechanisms behind fructose malabsorption in humans are not well understood and the clinical relevance of this test is considered controversial. Hence, the main aim of this study is to evaluate the predictive value of the H2 breath test. METHODS: Regarding exclusion criteria, the study enrolled 562 consecutive patients, enlisted to a gastroenterology clinic between 2009 and 2011 for testing malabsorption. In the final data analysis, 246 patients were included. Ecotrophologists used

Lactulose hydrogen breath test and functional symptoms in pediatric patients.

BACKGROUND: The role of small intestinal bacterial overgrowth (SIBO) in functional digestive disorders in the pediatric population is a matter of controversy, since methods currently used to establish this diagnosis are difficult to interpret. The aim of this work was to analyze the characteristics of the lactulose H(2) breath test (LHBT) in children with functional gastrointestinal symptoms according to more recent criteria. METHODS: Seventy-two patients and 17 controls were enrolled. A questionnaire was administered regarding digestive symptoms (abdominal pain, bloating, vomiting, and bowel

Home sleep testing in the diagnosis and treatment of sleep disordered breathing.

Sleep-disordered breathing is a growing public health concern and an integral part of head and neck surgery. Multichannel home sleep testing is a cost-effective, patient-friendly, scientifically valid technique of evaluating patients who present with symptoms of sleep-disordered breathing, typically snoring or daytime sleepiness. Home sleep tests can be dispensed from the physician's office. They have a 95% successful recording rate. Scoring can be autoscore or manual score. There are several protocols that can be followed based on diagnostic outcomes.

False-positive breath-alcohol test after a ketogenic diet.

A 59-year-old man undergoing weight loss with very low calorie diets (VLCD) attempted to drive a car, which was fitted with an alcohol ignition interlock device, but the vehicle failed to start. Because the man was a teetotaller, he was surprised and upset by this result. VLCD treatment leads to ketonemia with high concentrations of acetone, acetoacetate and beta-hydroxybutyrate in the blood. The interlock device determines alcohol (ethanol) in breath by electrochemical oxidation, but acetone does not undergo oxidation with this detector. However, under certain circumstances acetone is reduced

Applicability of short hydrogen breath test for screening of lactose malabsorption.

The gold standard for diagnosing lactose malabsorption is the H2 hydrogen breath test (HBT). Different methods of HBT have been proposed. However, in clinical practice the HBT is often shortened to 1-2 hr without proper validation. Our objective was to establish whether the usefulness of the HBT is influenced by shortening of the test and/or by substrate variations. In 62 patients with clinically suspected lactose intolerance and a positive lactose HBT we calculated the sensitivity of the HBT depending on the duration of the HBT. To determine whether substrate variations influence the sensitiv

Refinement of the 14C-urea breath test for detection of Helicobacter pylori.

BACKGROUND: Helicobacter pylori is recognized as the main etiological factor for chronic gastritis, peptic ulcer and possibly also gastric malignancies. A 14C-urea breath test was successively refined to correctly diagnose the presence of H. pylori. METHODS: After intake of a 14C-urea cocktail, 14CO2 in breath was trapped in benzethoniumhydroxide/ethanol. Serology by ELISA, followed by Western blot, was used as reference method for confirmation of presence or absence of H. pylori. RESULTS: Breath measurements at 10 and 20 min were determined to be optimal for diagnostic purposes. With 2.5 mi

[14C]-urea breath test to confirm eradication of Helicobacter pylori.

BACKGROUND: Treatment for Helicobacter pylori reduces ulcer recurrence. Eradication rates of the organism vary with different drug regimens from 30% to 90%. There is a need to identify patients who have failed treatment. [14C]-Urea breath test (UBT) is non-invasive, sensitive, safe and highly reliable test for diagnosis of H. pylori infection. As there is a paucity of reports on the utility of [14C]-UBT in confirming H. pylori eradication, this study was undertaken. METHODS: Thirty-eight patients (age 34 +/- 17 years, range 16-84 years, 27 men) with upper gastrointestinal symptoms underwent u

Drunken driving and breath alcohol test at the scene of violence in Japan.

Road Traffic Law prescribes that no person shall drive any vehicle under the influence of intoxicating liquor. Actually, determination of "influence of alcohol" is based on the standard set by the Cabinet Order that alcohol level exceeds 0.5 mg/ml of blood or 0.25 mg/l of expiration. In 1988, number of traffic accidents by drunken driving is 4,808 cases. Among the accidents by drunken driving the rate of fatal causes is 12%. During about last ten years, both of accidents and fatal cases by drunken driving have been decreasing in number, on the other hand, charged or cited number of violation i

Milk lactose malabsorption in Gabon measured by the breath hydrogen test.

It is usual to consider that the greatest part of the black African population is lactose intolerant. Also, milk lactose malabsorption was studied by a breath hydrogen technique in 87 Gabonese children and 20 Gabonese adults (central Africa). The prevalence of malabsorption was 64.2% in rural schoolchildren, 65% in the urban hospitalized, and 60% in adults. Twelve children and six adults had clinical symptoms after a lactose load. All subjects were Bantus, with no tradition of consuming dairy products. These data must be considered in programs of nutritional support in Africa.

Campylobacter pylori detected noninvasively by the 13C-urea breath test.

The high endogenous urease activity of Campylobacter pylori was exploited in a non-invasive test for the presence of this organism in the stomach. When 13C-urea was administered orally after a test meal, urea-derived 13CO2 appeared in the respiratory CO2 of infected individuals at a constant rate for greater than 100 min. The test was validated in 26 individuals who underwent both the 13C-urea breath test and endoscopic biopsy of the antral mucosa for culture and histological examination. Each positive breath test proved to be correlated with a positive culture or Warthin-Starry silver stain o

Breath hydrogen test in infants and children with blind loop syndrome.

Breath hydrogen production after oral lactose loading was examined in infants and children with stagnant loop syndrome, blind loop syndrome, or both. All six infants under 3 years of age had bacteriological evidence of small intestinal colonization. The characteristics of the breath hydrogen test in this syndrome are: (a) extremely high basal excretion of breath hydrogen (after overnight fasting); (b) an earlier and greater breath hydrogen value (0.293 +/- 0.201 ml/min/m2: mean +/- SD) after oral lactose administration than formed in lactose malabsorption alone (0.050 +/- 0.041 ml/min/m2); and

Bile acid deconjugation in intestinal obstruction studied by breath test.

Bile acid deconjugation was assessed by measuring specific activity of expired 14CO2 after the oral administration of cholyl-glycine-1-14C in 17 cases of intestinal obstruction, i.e. 15 mechanical and two paralytic. In the former, nine cases were operated and the remaining six cases were managed by the conservative treatment. Cumulative output of 14CO2 in breath for six hours before the treatment was 40.20=11.30 (mean+/-SEM) (control 2.96+/-1.16) but decreased to 6.86+/-3.64 after the treatment. Enteric bacteria capable of splitting amino moiety of the conjugated bile acid were found to be pre

Mechanisms of symptom generation in patients with lactose intolerance: lessons learned from validated symptom assessment during lactose breath tests.

BACKGROUND: Lactose intolerance is a common cause of functional gastrointestinal symptoms. Traditional diagnostic strategies primarily focus on detecting malabsorption despite symptoms often occurring without it. OBJECTIVES: This study examined the role of malabsorption and nonmalabsorptive mechanisms in the development of symptoms in patients with lactose intolerance. DESIGN: We analyzed 1015 patients undergoing standardized 50-g lactose hydrogen (H2) breath testing with simultaneous symptom assessment using the adult Carbohydrate Perception Questionnaire. Malabsorption was defined as an H2

Glucose breath test for the detection of small intestine bacterial overgrowth: Impact of diet prior to the test.

BACKGROUND: Glucose breath test (GBT) is used for the diagnosis of small intestine bacterial overgrowth. A restrictive diet without fibers and/or fermentable food is recommended on the day before the test. The aim of our retrospective study was to evaluate the impact of two different restrictive diets on the results of GBT. METHODS: A change of the pretest restrictive diet was applied in our lab on September 1, 2020. The recommended diet was a fiber-free diet before this date, and a fiber-free diet plus restriction of all fermentable food afterward. We thus compared the results of GBT perform

Validation of a breath-holding test as a screening test for exercise-induced hypoxemia in chronic respiratory diseases.

The detection of exercise-induced hypoxemia is important for evaluating disease status in patients with chronic respiratory diseases. The 6-min walk test (6MWT) is useful for detecting exercise-induced hypoxemia. This pilot study aimed to validate the breath-holding test (BHT) as a screening for exercise-induced hypoxemia and compare its utility with that of the 6MWT in patients with chronic respiratory diseases. Fifty-nine patients with chronic respiratory diseases underwent BHTs lasting 10, 15, and 20 s. Percutaneous oxygen saturation (SpO2), pulse rate, and severity of dyspnoea were measure
