Decreased neonatal tibial bone ultrasound velocity in term infants born after breech presentation.

OBJECTIVE: Fetuses found to be in the breech presentation have limited motion of their lower limbs. The aim of this study was to test the hypothesis that bone speed of sound (SOS) would be lower in infants born after breech presentation than in those born after vertex presentation. STUDY DESIGN: We studied 127 singleton, appropriate for gestational age, term infants delivered by a scheduled cesarean delivery at approximately 38 weeks of gestation because of breech presentation or repeat elective cesarean section with vertex presentation. We used the Sunlight Omnisense 7000p device to measure

Cesarean vs. vaginal birth for term breech presentation in 2 different study periods.

OBJECTIVE: To compare outcomes for fetuses at term in breech presentation during 2 periods when different delivery policies were in effect. METHODS: Outcomes of the 392 planned vaginal deliveries and 1160 elective cesarean sections (CSs) performed from January 1, 1988, through December 31, 2000, were compared with those of the 24 emergency vaginal deliveries, the 403 planned CSs, and 75 emergency CSs performed from January 1, 2001 through December 31, 2004 at Alexandra Hospital, Athens, Greece. RESULTS: A significant difference was found in rates of low 5-minute Apgar score, birth trauma, se

Planned vaginal delivery versus Caesarean section for breech presentation in Ile-Ife, Nigeria.

BACKGROUND: The optimum mode of breech delivery remains a matter of controversy among obstetricians worldwide. OBJECTIVE: To determine whether term breech babies born by planned vaginal delivery are at higher risk of neonatal mortality and morbidity than those born by planned caesarean delivery. DESIGN: A hospital based non-experimental comparison of outcome of breech delivery. SETTING: Ife State Hospitals Complex, Ile-Ife. SUBJECTS: Two hundred and fourty four singleton breech deliveries occurring at term. MAIN OUTCOME MEASURES: They include low 5-minute Apgar score, birth trauma, matern

[Breech presentation of the second twin. Effect on early morbidity and perinatal mortality].

Twin delivery is often complicated by breech presentation of the second twin. To investigate the influence of breech presentation of the second twin, we reviewed all 259 twin deliveries from the 18,404 deliveries at our institution over 11 years between 1978 to 1989. The neonatal morbidity of the second twin in breech presentation (pH less than 7,15; 1-minute-Apgar less than 7, transfer to newborn hospital) was significantly higher in comparison to vertex presentation of the second twin. A dependence of neonatal morbidity on the time interval was not found. Where the second twin was in vertex

Antepartum detection of cord presentation by transvaginal ultrasonography for term breech presentation: potential prediction and prevention of cord prolapse.

AIM: We evaluated the efficacy of antepartum screening for cord presentation by trans-vaginal ultrasonography (TVS) on predicting and preventing umbilical cord prolapse (UCP) in term breech delivery. METHODS: We investigated every woman with a breech-presenting fetus for cord presentation by weekly TVS after 36 weeks of gestation since 1995. If the cord was found in advance of fetal presenting parts, we recommended her to undergo elective cesarean section to avoid UCP. We studied the incidence of cord presentation by TVS and the clinical courses of the cases with it for 198 women who delivere

[Mode of Delivery for Breech Presentation in Cases of Very or Extremely Preterm Birth].

The mode of delivery in cases of very preterm or extremely preterm breech presentation remains a matter of debate. Available randomized trials are too few and underpowered to provide a definitive answer, and most of the evidence comes from observational, hospital-based or population-based studies, often limited by bias, particularly indication bias. Practices vary widely depending on teams and institutional policies. Analyses from different cohorts, including EPIPAGE 1 and 2, show no clear superiority of planned cesarean section over planned vaginal delivery with respect to neonatal mortality,

Breech presentation: Clinical practice guidelines from the French College of Gynaecologists and Obstetricians (CNGOF).

OBJECTIVE: To determine the optimal management of singleton fetuses in breech presentation. MATERIALS AND METHODS: Consultation of the PubMed database, the Cochrane Library and guidelines issued by the French and foreign obstetrical societies or colleges. RESULTS: In France, 5% of women have breech deliveries (level of evidence [LE] 3). One third of them have a planned vaginal delivery (LE3), and 70% of these give birth vaginally (LE3). External cephalic version (ECV) is associated with lower rates of both breech presentation at birth (LE2) and of cesarean deliveries (LE3) without any increa

In 16 cases from a total of 36 (44%) the external cephalic version was successful. 18 times the relaxation of the uterus was performed by general anaesthesia with a successrate of 50% and 17 times by Fenoterol with a successrate of 35%. A V-para had a successful external cephalic version without any relaxation of the uterus. In average the external cephalic version was performed in the 39th week of pregnancy. From these 16, in whose case an external cephalic version was successful, there were 6 caesarian deliveries because of fetal indication. From 5 of these 6 caesarian deliveries it was disc

Clinical Tips of Cesarean Section in Case of Breech, Transverse Presentation, and Incarcerated Uterus.

Cesarean section in breech or transverse presentation involves more complicated procedures than cesarean section in cephalic presentation because the former requires additional manipulations for guiding the presenting part of the fetus, liberation of the arms, and the after-coming head delivery; therefore, those cesarean sections are likely to be more invasive. Making a rather wide uterine incision to prevent uterine injury during delivery of the fetus facilitates smooth delivery of the fetus. Furthermore, in cases of breech or transverse presentation, it is important to initially identify the

Cesarean section and right femur fracture: a rare but possible complication for breech presentation.

Background. The breech extraction of the fetus through the vagina has a greater risk of hip fracture compared with the extraction by abdominal route. Case. A 2390 g female infant was delivered at 39 weeks by elective cesarean section for breech presentation. The newborn sustained a fracture of the right femur. A simple immobilization of the limb in extension led to a complete healing of the fracture without sequelae. Conclusion. Caesarean delivery reduces the risk of causing a traumatic injury of the newborn compared to vaginal delivery, especially with breech presentation but does not elimina

A simple patient-initiated intervention to increase antenatal detection of breech presentation at term.

Antenatal detection of breech presentations at 35-37 weeks is necessary to allow adequate time for decision making about external cephalic version (ECV) and/or caesarean section. This study aimed to increase antenatal detection of breech presentation and referral for ECV using an inexpensive patient prompt (a simple brochure encouraging pregnant women to ask how their baby is presenting) and posters reminding clinicians to assess presentation. The interventions were evaluated using a before-after (single time series) study design. The records of women who had a breech presentation in late preg

The premature breech presentation: outcome of newborn infants born by vaginal or abdominal delivery.

There is ample documentation that breech full-term infants delivered vaginally have a higher perinatal morbidity and mortality rate than breech infants born via cesarean section. Until now, little emphasis has been placed on the risks to premature newborns born in breech presentation. Therefore, the authors have considered all singleton pregnancies with infants in breech presentation admitted to the Department of Obstetrics, University of Padova, from January 1978 to December 1979 and delivered before 36-weeks gestation. On the basis of obstetric management, the authors have obtained two group
