Temporary placement of self-expanding oesophageal stents as bridging for neo-adjuvant therapy.

BACKGROUND: Placement of self-expanding stents is an effective palliation for dysphagia in non-resectable oesophageal or proximal gastric cancer. The aim of this analysis was to assess the efficacy of temporary stent placement for dysphagia relief during neo-adjuvant treatment for locally advanced disease. METHODS: A total of 38 patients scheduled for neo-adjuvant chemo(radio)therapy for locally advanced oesophageal cancer (n = 29), cardia cancer (n = 8) or subcardial gastric cancer (n = 1) underwent placement of self-expanding plastic stents (n = 13) or covered metal stents (n = 25) due to s

Bridging the gap between theory and practice: a multimedia tutorial for students of voice therapy.

A computer-based tutorial for speech and language therapy students is described in which a digitized video-recording of a diagnostic interview with a voice-disordered client was dynamically linked to an on-screen transcript window. A student-centered tutorial package, which provided guidance through questions, assessment tasks, and commentary from an experienced speech and language pathologist as well as the lecturer in voice disorder, was constructed. Evaluation of the tutorial as an aid to active learning, including feedback from students and supervising clinicians, is reported and the role

The Role of Intra-aortic Balloon Pump Therapy at Resource-Limited Institutions: A Bridge to Care Escalation.

Despite providing relatively modest circulatory support, the intra-aortic balloon pump (IABP) remains the most utilized mechanical support device. IABP therapy specifically provides utility in transferring critically ill patients from resource-limited hospital settings. In a single-center series, 71 patients who received IABP were identified from 2018 to 2023. In this group, 66 (93%) patients presented with acute myocardial infarction (AMI), of which 40 (61%) patients presented with STEMI. Sixty-three (89%) patients presented in cardiogenic shock. In total, 15 (21%) patients died during their

Percutaneous Coronary Intervention as a Bridge Therapy for Aortic Dissection Related Bilateral Coronary Malperfusion: A Case Report.

Dealing with coronary arteries caused by aortic dissection remains a great challenge in the treatment of aortic dissection. Here, we present the case of a 57-year-old woman, who initially detected chest pain, and electrocardiography showed ST-elevation myocardial infarction. After the primary percutaneous coronary intervention, aortic dissection was accidentally detected. She then received central repair and had a satisfactory outcome. We proved that percutaneous coronary intervention as a bridge therapy for aortic dissection-related coronary malperfusion might be efficient.

Multipoint left ventricular pacing as an addition to cardiac resynchronization therapy: a bridge to the holy grail?

Cardiac resynchronization therapy (CRT) constitutes a cornerstone to the treatment of advanced dyssynchronous heart failure (DyssHF); moreover it represents one of the few instances that a revolutionary approach was pursued, yielding previously unfathomable benefits to patients out of realistic therapeutic options. However, as is rather extensively established, nonresponse, or even negative response, to CRT continue to plague its course, precluding favourable effects in up to 40% of recipients, for a multitude of reasons. Given the scope of the issue of nonresponse, attempts to negate it by me

Surgically Implanted Percutaneous Assist Device After Left Ventricular Assist Device Malfunction as a Bridge to Definitive Therapy.

Patients receiving durable left ventricular assist device (dLVAD) support can experience device malfunction, cardiogenic shock, and/or incomplete unloading. Traditional guidance supports the use of emergent dLVAD exchange or orthotopic heart transplantation (OHT). We challenge this paradigm with evidence from four cases at our institution. This case series highlights the use of a microaxial flow pump (mAFP; Impella 5.5; AbiomedM, Danvers, MA) in the setting of dLVAD malfunction as a bridge to decision. Use of the mAFP in four distinct cases improved end-organ perfusion and increased left ventr

Statin Therapy before Percutaneous Coronary Intervention: A Novel Bridge between Thrombin and Thrombomodulin for Enhanced Cardiovascular Protection.

Percutaneous coronary intervention administration of statins enhances the thrombin-thrombomodulin (TM) interaction, which is critical for the prevention of post-procedural cardiovascular complications. Atomic force microscopy showed that statins increased the single molecule rupture forces between thrombin and TM, suggesting a strengthened complex. This was supported by surface plasmon resonance, which showed an enhanced binding affinity under the statin treatment. Molecular docking and dynamics simulations revealed that statins can bind to both thrombin and TM, forming stable complexes that f

Gemcitabine-oxaliplatin as a bridge therapy toward autologous hematopoietic stem cell transplantation in infant-type brain tumors.

INTRODUCTION: In neuro-oncological pediatric patients under 3 years of age, chemotherapy intensified to high doses (high-dose chemotherapy, HDC) represents the cornerstone to avoid the potential toxicity of radiotherapy. Combination treatment with gemcitabine-oxaliplatin (GemOx) was administered for infant- type cerebral tumors as a bridge toward autologous hematopoietic transplantation to achieve clinical and neuroradiological permissiveness to HDC and to raise the possibility of second-look neurosurgery. METHODS: From May 2017 to May 2023, at Meyer Children's Hospital IRCSS in Florence (Ita

Ruptured infective native thoracic aortic aneurysm treated by endovascular repair as a bridge therapy to open repair.

We report the case of a 70-year-old woman who presented with a ruptured infective native thoracic aortic aneurysm (INTAA), associated with spondylodiscitis and posterior mediastinitis. She underwent a staged hybrid repair: urgent thoracic endovascular aortic repair was first performed as a bridge therapy in the context of septic shock. Allograft repair using cardiopulmonary bypass was performed five days later. Given the complexity of INTAA, multidisciplinary teamwork was paramount to determine the most appropriate treatment strategy, including procedure planning with multiple operators as wel

Resolution of myocardial bridge-related wall motion abnormality and associated myocardial perfusion defect with beta-blocker therapy.

While not uncommon, a myocardial bridge may result in significant alterations in coronary flow hemodynamics to the point of being either clinically evident based on symptoms, or visually apparent on noninvasive imaging modalities. We report a case of a myocardial bridge of the mid-left anterior descending artery associated with anterior wall akinesis on left ventriculography, and evidence of a fixed perfusion defect in the anterior wall by gated cardiac single photon emission computed tomography that resolved with initiation of beta-blocker therapy.

Impact of inotuzumab ozogamicin as bridging therapy and tumor burden in CAR-T therapy for B-acute lymphoblastic leukemia.

INTRODUCTION: Inotuzumab ozogamicin is increasingly being used as bridging therapy (BT) prior to chimeric antigen receptor T-cell (CAR-T) in relapsed/refractory B-cell acute lymphoblastic leukemia (R/R ALL), but its impact on CAR-T expansion and clinical outcomes remains controversial. METHODS: This study analyzed 24 R/R ALL patients receiving tisagenlecleucel after BT (Inotuzumab [n=10] vs. chemotherapy/steroids [n=14]). CAR-T expansion was monitored via multiparametric flow cytometry at different time-points after infusion, with outcomes assessed by event-free survival (EFS), overall surviv

Bridging cone-beam CT and MRI to stopping power ratio maps: a modality-agnostic Brownian bridge approach to support robust adaptive proton therapy.

PURPOSE: Adaptive proton therapy (APT) relies on routine offline CT simulations for updated anatomy. We present a modality-agnostic regularized Brownian bridge (rBBrg) framework to deterministically predict stopping power ratio (SPR) maps from CBCTs or MRIs. METHODS: rBBrg consisted of (1) generation mapping where a Brownian bridge predicted SPR from input and (2) reconstruction mapping where a cGAN reconstructed input from predicted SPR. Class embedding was adopted for modality conditioning. Efficient, deterministic prediction was achieved via one-step sampling. Unimodality rBBrg and ResNet

Short-Course, Low-Dose Metoclopramide as Bridge Therapy for Dysautonomia-Associated Gastrointestinal Dysmotility in Adolescents: A Case Series.

BACKGROUND: Dysautonomia, including postural orthostatic tachycardia syndrome (POTS) and related orthostatic disorders, is frequently associated with debilitating gastrointestinal (GI) symptoms in adolescents, including chronic nausea, early satiety, postprandial fullness, and functional dysmotility. Despite the significant disease burden, pharmacological options for GI dysmotility in this population remain poorly studied. To our knowledge, no prior case series has described metoclopramide as a targeted prokinetic bridge therapy specifically in adolescents with dysautonomia-associated GI dysmo

A new step in understanding stem cell mobilization in patients with Fanconi anemia: A bridge to gene therapy.

BACKGROUND: Fanconi anemia (FA) is an inherited disorder characterized clinically by congenital abnormalities, progressive bone marrow failure (BMF), and a predisposition to malignancy. Gene therapy (GT) of FA, via the infusion of gene-corrected peripheral blood (PB) autologous hematopoietic stem cells (HSCs), may constitute a cure for BMF. GT bypasses the donor restrictions and adverse events associated with allogenic HSC transplantation. However, adequate harvesting of PB-HSCs is a crucial determinant of successful engraftment in gene therapy. Harvesting the low numbers of HSCs in patients w
