Antibody therapy targeting the CD47 protein is effective in a model of aggressive metastatic leiomyosarcoma.

Antibodies against CD47, which block tumor cell CD47 interactions with macrophage signal regulatory protein-α, have been shown to decrease tumor size in hematological and epithelial tumor models by interfering with the protection from phagocytosis by macrophages that intact CD47 bestows upon tumor cells. Leiomyosarcoma (LMS) is a tumor of smooth muscle that can express varying levels of colony-stimulating factor-1 (CSF1), the expression of which correlates with the numbers of tumor-associated macrophages (TAMs) that are found in these tumors. We have previously shown that the presence of TAMs

Targeting MAN1B1 potently enhances bladder cancer antitumor immunity via deglycosylation of CD47.

BACKGROUND: Only a few bladder cancer patients benefit from anti-programmed cell death protein 1/programmed cell death ligand 1 immunotherapy. The cluster of differentiation 47 (CD47) plays an important role in tumor immune evasion. CD47 is a highly glycosylated protein, however, the mechanisms governing CD47 glycosylation and its potential role in immunosuppression are unclear. Therefore, this study aimed to evaluate the function of CD47 glycosylation in bladder cancer. METHODS: Western blotting, immunohistochemistry, and flow cytometry were used to measure protein expression, protein-protei

Is drug interference still an issue for pretransfusion testing of patients on anti CD38 and other monoclonal antibody therapies?

Certain therapies that target CD markers on some blood cells can affect pretransfusion testing. Key examples are anti-CD38, CD47 monoclonal antibody (mAb) therapies such as daratumumab (DARA) and magrolimab, which have presented a challenge for transfusion medicine laboratories around the globe. Scientists have been faced with not only introducing a protocol to provide safe blood to patients but also investigating the most effective method to remove the pretransfusion pan-agglutinating interference caused. A number of papers in the last 5 years have reported on various methods to remove pretra

CD47 expression in urothelial carcinoma and its correlation with histopathological findings.

BACKGROUND: The incidence of bladder cancers (BCs) is increasing day by day in both males and females with increased smoking. New treatment protocols are required due to the long follow-up times and high recurrence rates. CD47 (integrin-associated protein) is a membrane receptor that is effective in normal and pathophysiological events such as apoptosis, proliferation, adhesion, and immunity. Phagocytosis of both normal and tumor cells is prevented, by binding to the ligand signal regulatory protein-1 (SIRP-1)α on macrophages. Anti-CD47 antibodies can eliminate the binding and may be effective

Effects of human TFPI and CD47 expression and selectin and integrin inhibition during GalTKO.hCD46 pig lung perfusion with human blood.

BACKGROUND: Loss of barrier function when GalTKO.hCD46 porcine lungs are perfused with human blood is associated with coagulation pathway dysregulation, innate immune system activation, and rapid sequestration of human formed blood elements. Here, we evaluate whether genetic expression of human tissue factor pathway inhibitor (hTFPI) and human CD47 (hCD47), alone or with combined selectin and integrin adhesion pathway inhibitors, delays GalTKO.hCD46 porcine lung injury or modulates neutrophil and platelet sequestration. METHODS: In a well-established paired ex vivo lung perfusion model, GalTK

Targeting the CD47-SIRPα Axis: Present Therapies and the Future for Cutaneous T-cell Lymphoma.

The loss of CD47 on aging cells serves as a signal to macrophages to eliminate the target. Therefore, CD47 is a "do-not-eat-me" sign preventing macrophagal phagocytosis via interaction with its ligand SIRPα. Malignant lymphocytes of mycosis fungoides and Sézary syndrome express CD47 highly, thus, being ideal candidates for targeted anti-CD47 therapies. The classes of current anti-CD47-SIRPα therapeutic molecules present in a large variety and include monoclonal antibodies against CD47 and SIRPα, bioengineered SIRPα proteins, miRNAs, and bispecific antibodies. We provided a detailed analysis of

The immunopathogenesis and immunotherapy of cutaneous T cell lymphoma: Current and future approaches.

In the past few decades, immunotherapy has emerged as an effective therapeutic option for patients with cutaneous T cell lymphoma (CTCL). CTCL is characterized by progressive impairment of multiple arms of the immune system. Immunotherapy targets these deficits to stimulate a more robust antitumor response, thereby both clearing the malignant T cells and repairing the immune dysfunction. By potentiating rather than suppressing the immune system, immunotherapy can result in longer treatment responses than alternatives such as chemotherapy. In recent years, advances in our understanding of the p

Clinical outcomes and safety of CD47-targeted immunotherapies across hematologic malignancies: a systematic review of monoclonal antibodies and fusion proteins in combination strategies.

BACKGROUND: The CD47-SIRPα axis is a key innate immune checkpoint that enables tumor cells to evade macrophage-mediated clearance. CD47 is overexpressed in a spectrum of hematologic malignancies, contributing to poor outcomes, particularly in high-risk biological subgroups. While early clinical trials of CD47 blockade demonstrated limited efficacy as monotherapy, combination strategies have emerged as promising approaches. This systematic review synthesizes the current clinical evidence on the outcomes and safety of CD47-targeted monoclonal antibodies and fusion proteins administered in combin

CD47 expression in classic follicular lymphoma is associated with event-free survival.

CD47 inhibits host cell phagocytosis by macrophages, leading to immune evasion. Upregulation of CD47 in many cancers including hematopoietic neoplasms correlates with poor prognosis. We evaluated CD47 expression in classic follicular lymphoma (FL) to ascertain its potential utility for targeted therapy. A cohort of 97 cases were studied by immunohistochemistry using a rabbit anti-CD47 monoclonal antibody and the results were correlated with clinicopathologic features, gene expression and clinical outcome. Our findings show that high CD47 expression (H-score ≥150) was significantly associated w

An integrated "Engage & Evasion" approach for mononuclear phagocyte system escape and efficient extracellular vesicle therapy.

Ischemic diseases are major contributors to global morbidity and mortality, posing a substantial threat to human health. Extracellular vesicles (EVs) play an essential role in enhancing neovascularization in ischemic tissues, thereby facilitating tissue repair and regeneration. However, the utilization of EVs is hindered by their rapid uptake and clearance by the mononuclear phagocyte system (MPS), which markedly impedes their therapeutic efficacy and organ-specific accumulation. Notably, CD47, upon binding to signal regulatory protein alpha, initiates a "don't eat me" signal, enabling immune

Deletion of CD47 from Schwann cells and macrophages hastens myelin disruption/dismantling and scavenging in Schwann cells and augments myelin debris phagocytosis in macrophages.

BACKGROUND: Myelin that surrounds axons breaks in trauma and disease; e.g., peripheral nerve and spinal cord injuries (PNI and SCI) and multiple sclerosis (MS). Resulting myelin debris hinders repair if not effectively scavenged by Schwann cells and macrophages in PNI and by microglia in SCI and MS. We showed previously that myelin debris evades phagocytosis as CD47 on myelin ligates SIRPα (signal regulatory protein-α) on macrophages and microglia, triggering SIRPα to inhibit phagocytosis in phagocytes. Using PNI as a model, we tested the in vivo significance of SIRPα-dependent phagocytosis in

New Frontiers in Monoclonal Antibodies for the Targeted Therapy of Acute Myeloid Leukemia and Myelodysplastic Syndromes.

Acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS) represent an unmet clinical need whose prognosis is still dismal. Alterations of immune response play a prominent role in AML/MDS pathogenesis, revealing novel options for immunotherapy. Among immune system regulators, CD47, immune checkpoints, and toll-like receptor 2 (TLR2) are major targets. Magrolimab antagonizes CD47, which is overexpressed by AML and MDS cells, thus inducing macrophage phagocytosis with clinical activity in AML/MDS. Decoding the immune microenvironment: precision immunotherapy for medulloblastoma subtypes.

Medulloblastoma is a severe pediatric brain tumor with distinct molecular subtypes-WNT, SHH, Group 3, and Group 4-each having unique genetic drivers and immune microenvironments. This review highlights the immune characteristics of each subtype: SHH is rich in tumor-associated macrophages (TAMs), whose role in tumorigenesis is debated; Group 3 features cytotoxic T cells often neutralized by immune checkpoints like PD-L1, causing T cell exhaustion; and Group 4 is marked by natural killer (NK) cells and B cells. These immune landscapes, including tumor-associated astrocytes (TAAs) and abnormal v

Clinicopathological analysis of immunohistochemical CD47 and signal-regulatory protein-α expression in Extranodal Natural killer/T-cell lymphoma.

BACKGROUND: The interaction between CD47 and signal-regulatory protein-alpha (SIRPα) inhibits phagocytosis, and their clinicopathological characteristics have been evaluated in various diseases. However, the significance of CD47 and SIRPα expression, as well as the combined effect, in Extranodal Natural killer/T-cell Lymphoma (ENKTL) remains uncertain. METHODS: In total, 76 newly diagnosed ENKTL patients (mean age 49.9 years, 73.7% male) were included in this study. CD47 and SIRPα expression were examined by immunohistochemistry. Survival analyses were conducted through Kaplan-Meier curves an

Erythrophagocytosis in sickle cell anemia: statistical evidence for a biological phenomenon.

The precise role of erythrophagocytosis in sickle cell disease is not known. Using hematological data from three studies and 791 subjects comprising of eight epidemiological groups, we found a strong statistical support for the hypothesis that erythrophagocytosis is increased in sickle cell trait, that neutrophils and lymphocytes are the most likely cells involved in erythrophagocytosis in these subjects and that increased erythrophagocytosis may for a mechanistic explanation for an increased risk of vaso-occlusive crisis in sickle cell trait. Statistically, erythrophagocytosis was not increas

SOHO State of the Art Updates and Next Questions | Novel Immunotherapy Combinations for the Treatment of Indolent B-Cell Lymphoma.

Chemoimmunotherapy (CIT) is the standard frontline treatment for advanced indolent non-Hodgkin lymphomas (iNHL). While lenalidomide-based immunotherapy remains the standard of care for relapsed iNHL, its frontline use is limited, due to nonsuperiority as compared to CIT. Agents that engage T-cells, polarize macrophage phenotype to a more antitumoral phenotype, and/or to target epigenetic pathways could enhance immunotherapy. We summarize in this review safety plus efficacy data from published and/or ongoing clinical trials investigating the combination of lenalidomide-based immunotherapy with

Proteomic and Physicochemical Analysis of Erythrocyte Membrane Vesicles from Fresh and Frozen Blood.

Erythrocyte membrane vesicles (EMVs) have emerged as promising nanocarriers for drug delivery due to their biocompatibility and immune evasion through retention of erythrocyte surface proteins. The main limitation is their dependence on freshly drawn blood, which limits batch production. Long-term storage of EDTA-anticoagulated blood at -80°C without cryoprotectant represents an accessible alternative, but its effect on EMV physicochemical properties and protein composition has not been investigated. We prepared EMVs from fresh and frozen blood and characterized the resulting fractions by dyna

Chimeric CD47-CD63 Enables Small Extracellular Vesicles to Evade the Undesirable Phagocytosis.

Extracellular vesicles (EVs) play a pivotal role in cell-cell communication and possess numerous advantages that make them promising candidates for clinical applications. Notably, both natural and engineered small EVs (sEVs) have been widely explored as drug delivery systems (DDSs). However, their short circulating half-life and accumulation in specific organs remain major obstacles to therapeutic use, primarily due to immune clearance by the mononuclear phagocyte system (MPS). In this study, we demonstrate that a truncated form of CD63 directs the sorting of a chimeric CD47-CD63 protein into

Synergic cancer chemo-immunotherapy comprising combined doxorubicin and siRNA targeting CD47 co-delivered by a bola-amphiphilic dendrimer.

Restoring antitumor immunity while enhancing the chemotherapy efficacy represents an optimistic strategy for cancer treatment. Enabling macrophage-mediated phagocytosis of cancer cells is pivotal for rehabilitating effective immune responses. However, its efficacy is often limited by elevated anti-phagocytic signals, such as CD47, and insufficient pro-phagocytic signals on cancer cells. We employed a synergic chemo-immunotherapeutic approach involving the chemotherapeutic agent doxorubicin (DOX) together with siRNA targeting CD47 (siCD47), co-delivered by a bola-amphiphilic dendrimer (bola4A)

Tumor-associated protease-activated anti-CD47 antibody precisely maintains phagocytic ability of macrophages with minimal effect on healthy tissue.

(BACKGROUND): CD47 is highly expressed on many cancer cells and acts as an innate immune checkpoint. Its binding to signal regulatory protein alpha (SIRPα) on macrophages enables cancer cells to evade phagocytosis. Although anti-CD47 antibody (αCD47 Ab) has been employed to restore phagocytic capacity, the ubiquitous expression of CD47 on normal cells results in significant toxicities during Ab treatment, such as anemia, thrombocytopenia, and sepsis. METHODS: To mitigate these side effects, we used an autologous hinge region as a spatial-hindrance-based Ab lock and connected it to the N-termi
