Development of and clinical experience with capecitabine (Xeloda) in the treatment of solid tumours.

The oral fluoropyrimidine capecitabine (Xeloda) delivers 5-FU to the tumour site, thereby limiting the side effects and other complications associated with intravenous (i.v.) 5-FU. As an oral drug, capecitabine is preferred to 5-FU by many patients as it can be conveniently taken at home. In first-line metastatic colorectal cancer (MCRC), capecitabine results in superior response rates and equivalent progression-free and overall survival compared with i.v. 5-FU/LV. There is also increasing evidence for replacing i.v. 5-FU with capecitabine in combination with other anticancer agents (e.g. oxal

Adequate lymphadenectomy and adjuvant capecitabine warrant survival benefit in gallbladder cancer.

BACKGROUND: Significant heterogeneity exists in the management of resectable gallbladder cancer (GBC), regarding the extent of lymphadenectomy and the role of adjuvant chemotherapy (aCTx). This study investigates outcomes of resected GBC according to contemporary surgical oncology principles. METHODS: The international database of the GBC Study Group was queried for patients with resected GBC between 2012 and 2022. Patients with ≥6 lymph nodes resected and aCTx were compared to those with inadequate lymphadenectomy or inadequate aCTX. Unadjusted and adjusted Cox regression models were employe

Meta-Analysis of Capecitabine versus 5-Fluorouracil in Advanced Gastric Cancer.

OBJECTIVE: To investigate the effect of capecitabine versus 5-fluorouracil in advanced gastric cancer patients. METHODS: We searched PubMed, Cochrane Library, Embase, and other databases from database establishment to June 2022, containing randomized controlled trials (RCT) on capecitabine and 5-fluorouracil in advanced gastric cancer patients. A meta-analysis was conducted to evaluate the effect of capecitabine versus 5-fluorouracil on overall response rate, neutropenia, thrombocytopenia, stomatitis, hand-foot syndrome, nausea and vomiting, alopecia, and diarrhea. RESULTS: Eight RCTs with a

Toxicity Associated with Capecitabine in Patients Suffering from Dihydropyrimidine Dehydrogenase Deficiency.

Dihydropyrimidine dehydrogenase (DPD) is a metabolic enzyme that is crucial in 5-fluorouracil (5-FU) degradation. A deficiency in it is associated with the occurrence of adverse events following fluoropyrimidine-based therapies. We describe a case of toxicity grade 5 after the administration of capecitabine and oxaliplatin in a patient with stage III colorectal cancer and DPD congenital deficiency, which was identified later. Several polymorphisms have been associated with the global toxicity of 5-FU; however, genetic tests are low in sensitivity and therefore they cannot as yet be used as pre

Self-reported compliance with capecitabine: findings from a prospective cohort analysis.

OBJECTIVES: While oral anticancer treatment has increased the convenience for patients with no risk of venous access complications compared to intravenous drug administration, a high level of compliance cannot always be assumed. The aim of the present report was to evaluate real-life drug adherence in a prospective cohort analysis of patients with gastrointestinal or breast cancer treated with capecitabine-based chemotherapy. METHODS: Twenty-nine Swiss oncologists recruited patients receiving capecitabine, either as monotherapy or in combination with other chemotherapeutic agents, in a prospe

Radiation recall phenomenon secondary to capecitabine: possible role of thymidine phosphorylase.

BACKGROUND: The first reported case of radiation (XRT) recall related to capecitabine described dermatitis in a previously radiated field in a breast cancer patient (Ortman; JCO). We previously reported the first case of recall syndrome manifesting as diffuse gastritis and duodenitis related to capecitabine with prior XRT with 5-FU in a pancreatic cancer patient (Saif; JARCET). We report here another pancreatic cancer patient with a radiation recall receiving capecitabine following capecitabine-XRT. PATIENTS AND METHODS: From April 2004 to June 2005, 20 patients with locally advanced pancreat

Oral capecitabine in anthracycline- and taxane-pretreated advanced/metastatic breast cancer.

An open-label, non-randomized, compassionate-use study was carried out to investigate the effects of oral capecitabine at a dose of 1 250 mg/m2 twice daily on days 1 to 14 every 21 days in anthracycline- and taxane-pretreated advanced/metastatic breast cancer patients. Forty-eight patients were enrolled from April 2000 to December 2001. Twenty-four patients (50%) had metastases to the liver, 18 to bone, 13 to lung, 10 to regional lymph nodes, 8 to pleura, 7 to the thoracic wall, 5 to skin, 3 to the mediastinum, 1 to breast and 1 had metastasis to the abdomen. [Capecitabine: an oral chemotherapeutic agent against metastatic breast and colorectal cancer].

Capecitabine is the first drug in a new fluoropyrimidine class that offers distinct characteristics. On one hand its oral administration mimics continuous fluorouracil infusion while remaining convenient with higher patient acceptance and compliance rates and avoiding intravenous administration-associated complications and financial costs. On other hand it provides intra-tumour selective activation, thus potentially facilitating local management and therefore improved anti-tumour activity as well as reduced systemic toxicity. The concentration of thymidine phosphorylase an enzyme essential for

Topical Delgocitinib for Capecitabine-Induced Hand-Foot Syndrome: A Therapeutic Option?

INTRODUCTION: Hand-foot syndrome (HFS) is a frequent and potentially dose-limiting adverse event of capecitabine. Current therapeutic strategies are not always effective. Since inflammatory pathways, including JAK-STAT3 signaling, may contribute to capecitabine-induced HFS, topical JAK inhibition could represent a potential therapeutic approach. METHODS: We report two women with metastatic breast cancer receiving capecitabine monotherapy who developed Common Terminology Criteria for Adverse Events (CTCAE) grade 3 HFS involving both palms and soles. Both patients had previously received clobet

Vasospastic Angina Following Capecitabine Initiation: A Rare But Serious Cardiotoxic Effect.

BACKGROUND: Fluoropyrimidines are widely used chemotherapy agents in the treatment of solid tumors, particularly colorectal cancer (CRC). The cardiotoxic effect most commonly associated with 5-fluorouracil (5-FU) is coronary vasospasm, which typically occurs within the first hours or days following administration and can present with a spectrum of manifestations ranging from stable angina to acute coronary syndrome with hemodynamic instability. Capecitabine, a prodrug of 5-FU, has less defined cardiovascular side effects. CASE REPORT: A 66-year-old man with lower rectal adenocarcinoma, receiv

Re-thinking the possible interaction between proton pump inhibitors and capecitabine.

Proton Pump Inhibitors (PPI) rank within the top ten most prescribed medications in Europe and USA. A high frequency of PPI use has been reported amongst patients undergoing chemotherapy, to mitigate treatment-induced gastritis or gastro-oesophageal reflux. Several recent, mostly retrospective, observational studies have reported inferior survival outcomes among patients on capecitabine who concomitantly use PPI. Whilst this association is yet to be definitively established, given the prominence of capecitabine as an anti-cancer treatment with multiple indications, these reports have raised co

Acute myocardial infarction after capecitabine treatment: not always vasospasm is responsible.

Capecitabine is an orally available chemotherapeutic agent that is converted to 5-fluorouracil (5-FU) after absorbtion. Capecitabine and its active metabolite, 5-FU, have cardiotoxic effects with reported instances of acute coronary syndromes caused due to coronary vasospasm. However, these agents exert toxic effects on cardiovascular system and beyond vasospasm provacation. We report a 46-year-old patient diagnosed as acute inferior infarction who is treated with capecitabine for 3 months due to metastatic breast carcinoma, in whom thrombotic coronary occlusion was observed in angiography. [Severe toxicity following capecitabine administration because of dihydropyrimidine deshydrogenase (DPD) deficiency].

Capecitabine is an anticancer agent, prodrug of 5 fluorouracil (5-FU) administered orally and with a narrow therapeutic index. In gastrointestinal cancer, capecitabine is indicated for the treatment of colorectal cancer and metastatic unresectable gastric cancer. The 5-FU is active by incorporation in the biosynthesis of nucleic acids. Inhibition of endogenous synthesis of thymidine is the main way of toxicity of 5-FU. 5-FU is metabolised by the dihydopyrimydine dehydrogenase (DPD). Patients with a DPD deficiency can experience severe toxicity of 5-FU. We report the case of a patient who prese

Prognostic and safety analysis of capecitabine monotherapy versus CapeOx (capecitabine plus oxaliplatin) in elderly patients with high-risk stage II and stage III MRD-negative colorectal cancer.

BACKGROUND: Postoperative adjuvant chemotherapy is a cornerstone of treatment for high-risk stage II and stage III colorectal cancer (CRC). However, the optimal regimen for elderly minimal residual disease (MRD)-negative patients remains controversial. This study aimed to compare the efficacy and safety of capecitabine monotherapy versus the CapeOx regimen (capecitabine plus oxaliplatin) in this population. METHODS: We conducted a retrospective analysis of 117 elderly patients (aged 65-85 years) with high-risk stage II and stage III CRC who underwent curative surgery and tested MRD-negative.

[Evaluation of the Leftover Capecitabine Tablets in Adjuvant CAPOX Chemotherapy for Gastric Cancer].

Since it is important that patients take their oral anticancer therapy as prescribed, pharmacists need to assess adherence. In addition, oral anticancer drugs are expensive, and reuse of leftover drugs at outpatient pharmacy clinics is useful in reducing drug costs. The present study aimed to clarify when and why patients have leftover capecitabine tablets, and the cost of leftover capecitabine tablets reused at an outpatient pharmacy clinic, focusing on adjuvant capecitabine plus oxaliplatin (CAPOX) chemotherapy for gastric cancer. We retrospectively studied patients who received adjuvant CAP
