Beware of "curare cleft" like changes during unilateral capnothorax.

Capnography is a standard monitoring tool during general anaesthesia. Diaphragmatic movement with the weaning of muscle relaxant effect produces the characteristic "curare cleft" on capnography. Various artefacts can mimick this trace intraoperatively. Cautious interpretation and identification of these is essential to avoid any undue overdosing of the patients with muscle relaxants. We report "curare cleft" like artefact during ventilation with a single lumen tube in a patient with unilateral capnothorax undergoing minimally invasive esophagectomy.

Attention-demand effects on respiration in chronic low back pain patients.

Increasing attention is being drawn towards the involvement of systems other than the musculoskeletal one in the presence of low back pain (LBP). Recent evidence suggests both cognitive and respiratory functions to be affected in LBP patients. The aim of this study was to compare the effect of performance of a cognitive task on the respiratory function in LBP patients with that in control participants. Capnography and spirometry parameters of 48 participants (24 in each group) were assessed under 3 cognitive loading conditions (no, easy and difficult cognitive task). The results showed that in

Reducing Lung Injury from Blind Insertion of Small-Bore Feeding Tubes.

Using a blind insertion technique to insert small-bore feeding tubes can result in inadvertent placement in the lungs, leading to lung perforation and even mortality. In a Magnet-designated, 500-bed, level 2 trauma center, two serious patient safety events occurred in a four-week period due to nurses blindly inserting a small-bore feeding tube. A patient safety event review team convened and conducted an assessment of reported small-bore feeding tube insertion events that occurred between March 2019 and July 2021. The review revealed six lung perforations over this two-year period. These event

Blind Nasal Intubation Revisited: No Longer a Blind Technique?

BACKGROUND: Advancements in airway management have made the practice of blind nasal intubation obsolete. We report on successful blind nasal intubation performed with the help of capnography and real-time ultrasonography in two patients with tempormandibular joint ankylosis. CASE REPORT: Blind nasal intubation was performed in a 12-year-old patient and a 17-year old patient under general anesthesia with spontaneous respiration. Capnography was used as an aid during insertion and dynamic ultrasonography was performed to guide and confirm proper tracheal tube placement. Use of capnography helps

Evaluation of the Integrated Pulmonary Index® during non-anesthesiologist sedation for percutaneous endoscopic gastrostomy.

Standard monitoring of heart rate, blood pressure and arterial oxygen saturation during endoscopy is recommended by current guidelines on procedural sedation. A number of studies indicated a reduction of hypoxic (art. oxygenation < 90% for > 15 s) and severe hypoxic events (art. oxygenation < 85%) by additional use of capnography. Therefore, U.S. and the European guidelines comment that additional capnography monitoring can be considered in long or deep sedation. Integrated Pulmonary Index® (IPI) is an algorithm-based monitoring parameter that combines oxygenation measured by pulse oximetry (a

End-Tidal CO2 Monitoring in Critical Illness: Applications in Resuscitation, Shock, and Ventilation.

BackgroundContinuous end tidal carbon dioxide (ETCO2) monitoring provides a noninvasive, real-time assessment of ventilation, pulmonary perfusion, and indirectly cardiac output. Although historically limited to anesthetic practice, capnography has emerged as an increasingly valuable physiologic monitoring tool in critical care settings.MethodsA review was conducted using MEDLINE/PubMed and the Cochrane Library from 1988 to 2025 to evaluate the clinical applications of ETCO2 monitoring in critically ill patients. Eighteen studies met predefined inclusion criteria after screening and were includ

The use of carbon dioxide monitoring to determine orogastric tube placement in premature infants: a pilot study.

Enteral nutrition, frequently given through gastric tubes inserted through the nose or mouth, is an important part of supportive care for children unable to maintain adequate nutrition orally. To provide safe enteral nutrition, however, correct tube position must be achieved. Capnography, a noninvasive monitoring technique designed to measure expired carbon dioxide (CO2) levels, has been used previously to identify respiratory placement of nasogastric tubes in adults; however, its use in children is understudied. The purpose of this pilot study was to determine the potential of CO2 monitoring

Capnographic monitoring of propofol-based sedation during colonoscopy.

BACKGROUND AND STUDY AIMS: Capnography enables the measurement of end-tidal CO2 and thereby the early detection of apnea, prompting immediate intervention to restore ventilation. Studies have shown that capnographic monitoring is associated with a reduction of hypoxemia during sedation for endoscopy and early detection of apnea during sedation for colonoscopy. The primary aim of this prospective randomized study was to evaluate whether capnographic monitoring without tracheal intubation reduces hypoxemia during propofol-based sedation in patients undergoing colonoscopy. PATIENTS AND METHODS:

Improvements in Exhaled Partial Pressure Carbon Dioxide After Interventional Bronchoscopy: Two Case Reports of Malignant and Benign Bronchial Obstruction.

Bronchial obstruction disrupts gas exchange, thus leading to a ventilation-perfusion mismatch. This report presents two cases demonstrating the utility of intrabronchial capnography in the management of airway obstruction. In case 1, stenting for recurrent lung cancer rapidly increased left lung perfusion from 13% to 59% with normalized carbon dioxide levels and improved imaging. In Case 2, tuberculosis-induced obstruction required secondary intervention, thus gradually increasing perfusion from 7% to 28% over time. Both cases highlighted the role of capnography in detecting airway collapse, a

Identifying Early Opioid-Induced Respiratory Depression and Rapid Response Team Activation.

BACKGROUND: Opioids can cause respiratory depression, which could lead to patient harm. The project site noted a gap in identifying and monitoring postsurgical thoracic patients at risk for opioid-induced respiratory depression (OIRD), so an evidence-based solution was sought. AIMS: The purpose of this quality improvement project was to determine if translating the research by Khanna et al. (2020) on implementing the prediction of opioid-induced respiratory depression in patients monitored by capnography (PRODIGY) risk prediction tool would affect rapid response team (RRT) activation among po

A modified CO2/O2 Guedel airway improves capnographic accuracy compared with a CO2/O2 nasal cannula: An infant manikin study.

BACKGROUND: Capnography via a CO2/O2 nasal cannula is commonly used for respiratory monitoring during sedation. However, signal disturbances are frequently encountered, especially in young children. OBJECTIVE: Sampling ports placed closer to the trachea have been shown to result in improved signal quality. In a manikin model of a 6-month-old infant we compared capnography from a modified Guedel airway with a CO2 port located at the tip with that from a CO2/O2 nasal cannula. DESIGN: A comparison study using an artificial model of a breathing 6-month-old infant. SETTING: Department of Paediat

Application of End-Tidal CO2 Monitoring to ICU Management.

Waveform capnography is a noninvasive measurement of ventilation and perfusion commonly employed in the prehospital setting. It is easy to apply, and modern cardiac monitors are equipped with the necessary ports and capability to display results. Despite its ease of use, end-tidal CO2 monitoring has not yet achieved widespread adoption within the hospital setting. It is routinely used in the emergency department and by anesthesiologists, but its application could support ICU management in critically ill patients. Its use is routinely supported by multiple professional societies, and it has bee

Relationship between ETCO2 and PaCO2 under Changing Capnogram in Ventilated Infants with NAVA: An Observational Study.

This observational study evaluated the validity of end-tidal CO2 (ETCO2) as a surrogate for arterial PCO2 (PaCO2) in infants on neurally adjusted ventilatory assist (NAVA), particularly considering the influence of variable spontaneous breathing on capnography waveforms. The study involved 16 infants, analyzing 50 paired ETCO2 and PaCO2 values. Deming regression analysis highlighted a notably stronger correlation for maximum ETCO2 (r2 = 0.6783, p <0.0001) compared to mean ETCO2 (r2 = 0.5686, p <0.0001) and demonstrated a significantly weaker association for minimum ETCO2 (r2 = 0.1838). Enhancement of capnogram waveform in the presence of chest compression artefact during cardiopulmonary resuscitation.

BACKGROUND: Current resuscitation guidelines emphasize the use of waveform capnography to help guide rescuers during cardiopulmonary resuscitation (CPR). However, chest compressions often cause oscillations in the capnogram, impeding its reliable interpretation, either visual or automated. The aim of the study was to design an algorithm to enhance waveform capnography by suppressing the chest compression artefact. METHODS: Monitor-defibrillator recordings from 202 patients in out-of-hospital cardiac arrest were analysed. Capnograms were classified according to the morphology of the artefact.
