Intraocular femtosecond laser use in traumatic cataract.

PURPOSE: To report the use of femtosecond laser cataract surgery in a traumatic case. METHODS: A 38-year-old man had a penetrating eye injury while working with wire. The corneal laceration was sutured first. In the postoperative period, cortical cataract appeared in the crystalline lens; therefore, in the second session, cataract surgery was performed using a femtosecond laser system (Alcon-LenSx Lasers Inc.). RESULTS: Corneal incisions, an intact 4.5 mm capsulorhexis, and nucleus liquefaction were performed with the femtosecond laser. Corrected distance visual acuity was 0.9 1 month after

Intraocular pinhole implantation for irregular astigmatism after planned and unplanned posterior capsule opening during cataract surgery.

We present two cases for which intraocular pinhole implants were used to treat irregular corneal astigmatism after planned and unplanned posterior capsule opening during cataract surgery. The first case had a fibrotic posterior capsule that was eliminated with a posterior capsulorhexis. After that, the intraocular lens (IOL) and pinhole were implanted in the capsular bag. In the second case, a posterior capsule tear after phacoemulsification was converted to a posterior capsulorhexis, allowing implantation of an IOL in the capsular bag and the intraocular pinhole in the ciliary sulcus. With th

[The specificity of surgery of congenital cataract with implantation of folding "Acrysof" IOL in children].

Described in the paper are the results of surgical treatment of 25 children (37 eyes), aged 3 months to 12 years, with congenital and secondary cataracts. The treatment schemes were based on new technologies including tunnel incisions, continuous round anterior capsulorhexis and aspiration-irrigation as well as implantation of different-model posterior-chamber flexible "Acrysof' IOLs into the capsular sac or ciliary sulcus. A high treatment efficiency as well as a need in a differentiated approach to choosing an IOL fixation, diameter and technique of anterior capsulorhexis and a model of the

Surgical approach for management of complete anterior capsular contraction syndrome.

Anterior capsular contraction syndrome is an uncommon but well-known complication associated with continuous curvilinear capsulorhexis performed during cataract surgery. It remains asymptomatic unless the constriction progresses to involve the visual axis or causes late intraocular lens-related complications. A male patient in his 50s presented with severely decreased vision in his right eye 2 years after uneventful cataract surgery. Slit-lamp biomicroscopy revealed capsular phimosis and a dense, central, fibrous plaque with total occlusion of the anterior capsulorhexis opening. Neodymium-dope

[Use of of the high frequency capsulotomy instrument in cataract surgery--effect of coagulation on corneal endothelium].

UNLABELLED: Intumescent or hypermature cataracts make a safe capsulorhexis impossible. High frequency capsulotomy represents a satisfying solution for this problem. Primary goal of the present study was to investigate a possible damage to the corneal endothelium by this method. MATERIALS AND METHODS: 55 patients with an uncomplicated senile cataract were enclosed into a prospective randomized study undergoing cataract surgery with capsulorhexis or with high frequency capsulotomy. Corneal endothelium was examined preoperatively as well as postoperatively at several intervals. RESULTS: Concern

Effect of the Femtosecond Laser on an Intracorneal Inlay for Surgical Compensation of Presbyopia during Cataract Surgery: Scanning Electron Microscope Imaging.

PURPOSE: To investigate the effect of the femtosecond laser-assisted cataract surgery (FLACS) on porcine eyes implanted with a Kamra corneal inlay and to describe how the inlay may change the effect of the femtosecond laser on the lens. METHODS: FLACS was performed on six porcine eyes and a Kamra corneal inlay had been implanted, exploring the lens under the surgical microscope. Another Kamra corneal inlay was attached to the upper part of the transparent hemisphere used for calibration of the femtosecond laser. Capsulorhexis, arcuate incisions, and phacofragmentation were carried out. The Ka

Phaco rolling technique.

We describe a phacoemulsification technique for soft and medium-hard cataracts to decrease phaco time and enhance the safety of the procedure. After conventional hydrodissection and hydrodelamination are performed, a 15- or 30-degree phaco tip is positioned on the peripheral lens beside the capsulorhexis edge and in contact with the nucleus-epinucleus interface. The lens is then aspirated onto the phaco tip. Phacoemulsification is started with the ultrasound energy level limited to 15% to 25% depending on the nuclear hardness and with linear aspiration power up to 250 mmHg. The phaco tip is sl

Three-step technique for staining the anterior lens capsule with indocyanine green or trypan blue.

A 3-step technique for staining the anterior lens capsule was developed to optimize the use of a capsule dye (trypan blue or indocyanine green) during capsulorhexis. After the injection of a viscoadaptive ophthalmic viscosurgical device (OVD) (sodium hyaluronate 2.3% [Healon 5]), which fills the anterior chamber, balanced salt solution (BSS) is gently injected onto the anterior capsule, creating a wafer-thin, fluid-filled space beneath the OVD. The dye is slowly injected through a 27-gauge cannula (Duckworth & Kent), selectively mixing with the BSS and allowing relatively accurate painting of

"Inverted flap rhexis" - Salvation of intumescence.

Intumescent cataract can be a challenge even for an experienced surgeon, particularly for creating a continuous curvilinear capsulorhexis (CCC) because of increased endolenticular pressure. CCC in white intumescent cataract is associated with increased risk of extension of rhexis margin leading to radial tear or biradial extension causing "Argentinian flag sign" and associated complications. We describe a novel technique for CCC, in which we manipulate the vector forces acting in creating a capsulorhexis. After making a mini CCC, a flap is raised and then inverted under the rhexis margin and C

Silent rupture: unmasking a posterior capsular rupture in a seemingly uncomplicated cataract.

Cataract surgery is typically considered routine, especially when preoperative things are normal. We present a rare intraoperative complication where the cataractous lens nucleus dropped into the vitreous cavity immediately following viscoelastic injection-prior to capsulorhexis-in a patient with Fuchs' heterochromic iridocyclitis and no overt signs of zonular instability or capsular pathology. The case underscores the need for heightened surgical awareness in seemingly uncomplicated cataract cases, particularly in the presence of subtle clinical history. Surgical management included anterior

The Ganesh-Grewal cystitome maker - A step in standardizing cataract surgery.

A well-centered, adequately sized continuous curvilinear capsulorhexis (CCC) is a prerequisite for successful cataract surgery. A perfect capsulorhexis ensures safe and effective performance of various steps of surgery as well as a correctly positioned intraocular lens (IOL) with optimal rotational stability. Ganesh and Grewal (GG) cystitome maker is a step toward standardizing the creation of a cystitome to reduce variations and complications associated with the crucial step of CCC in cataract surgery. We conducted a study to measure the repeatability and precision of cystitomes made by the G

[The state-of-the-art of surgical treatment of congenital cataract in prematures].

Described in the paper are results of a clinical treatment of 127 children (174 eyes), aged 2 to 15 years, with different-type congenital cataract; the treatment was based on modem technologies including tunnel limbic incision, continuous anterior round capsulorhexis, aspiration-irrigation of the contents of the capsular sac, implantation of soft Acrysof SA30A1 IOLs by forceps or "Monarch" injector and suturing of incision. The anterior continuous capsulotomy was found to be the most complicated surgery stage in removal of congenital cataract in children it could be made without errors in 75.3

Anterior Capsular Tear During Phacoemulsification: Successful Management With the Two-Y Crushing Technique.

An anterior capsular tear or radial extension of the capsulorhexis is a serious complication during phacoemulsification and may lead to significant sequelae. In the present case, an anterior capsular tear occurred during capsulorhexis in phacoemulsification surgery. We opted to use the Two-Y crushing technique to safely emulsify and aspirate the nucleus in the presence of an anterior capsular tear. This technique was originally described for nuclear cracking in posterior polar cataracts (PPCs) under the protection of an epinuclear shell, with the advantage of minimizing capsular stress. In thi

[Anterior capsule contraction syndrome after cataract phacoemulsification surgery].

Cataract facoemulsification with continuous anterior capsulorhexis and foldable intraocular lens implantation into the capsular bag has become basic, standard method in cataract surgery and curvilinear capsulorhexis s the most common technique for opening the anterior lens capsule. The opening, made during the operation, within several months of postoperative evolution gradually constricts and in extreme cases it may even close completely. That course of healing is not desirable because of evoking a noted impairment of vision. Decreasing in the anterior capsule opening is observed mostly in ey
