[A case of interval form of carbon monoxide poisoning with a remarkable recovery].

A 69-year-old woman was admitted to our hospital due to an interval form of carbon monoxide (CO) poisoning one month after acute CO poisoning. On admission, she had disorientation, memory disturbance, apathy, masked face, muscle rigidity, bradykinesia and parkisonian gait. An MRI (FLAIR image) revealed high signal intensity lesions in the bilateral globus pallidus and the white matter of the frontal lobe. Hyperbaric oxygen (HBO) therapy at 2 atmospheres for 60 min was given every day, in addition to citicoline, levodopa/DCI and selegiline hydrochloride. Cognitive disturbance and parkinsonism g

[State of the organ of hearing and equilibrium in acute carbon monoxide poisoning].

The examinations covered 50 patients (28 females and 24 males), aged 30.1 (+/- 12.1) hospitalized for accidental and suicidal acute CO poisoning. 17 patients were exposed for 50 minutes to 2--3 hr and 9 patients for 6 hr. The remaining patients failed to establish for how long they had had contact with the poison. All the patients underwent tonal threshold and suprathreshold audiometric examinations and electronystagmographic examination at rest following kinetic and optokinetic stimulation. 21 (42%) patients developed bilateral receptive hearing impairment of various degree, mostly (80%) extr

Carbon Monoxide Poisoning (Reprinted from the 2023 Hyperbaric Indications Manual 15th edition).

Despite established exposure limits and safety standards, and the availability of carbon monoxide (CO) alarms, each year an estimated 50,000 people in the United States visit emergency departments for CO poisoning. Carbon monoxide poisoning can occur from brief exposures to high levels of CO or from longer exposures to lower levels. If the CO exposure is sufficiently high, unconsciousness and death occur quickly, and without symptoms. With non-lethal exposures to CO, common symptoms include headaches, nausea and vomiting, dizziness, general malaise, and altered mental status. Some patients may

Hypothyroidism following carbon monoxide poisoning: An epidemiological study supported by animal experiments.

Previous studies have suggested a possible association between carbon monoxide poisoning (COP) and hypothyroidism, but the evidence is limited. Therefore, the aim of this study was to further investigate this relationship. Using data from the Taiwan National Health Research Database, we identified 32,162 COP patients and matched with 96,486 non-COP patients by age and index date for an epidemiological study. The risk of hypothyroidism was compared between the two cohorts until 2018. Independent predictors of hypothyroidism were analyzed using competing risk analysis. An animal study was also c

Surface-based morphometry study of brain in patients with carbon monoxide poisoning.

PURPOSE: Although cortical volume abnormalities are frequently detected in patients with carbon monoxide poisoning (COP), particularly delayed neurological sequelae (DNS), the associated changes in cortical thickness and shape patterns remain unknown. MATERIALS & METHODS: Using surface-based morphometry, we investigated the differences in cortical thickness and shape indices between a COP group (n = 44) vs healthy controls (HCs, n = 36), and between the DNS (n = 21) vs non-DNS (n = 23) subgroups. Additionally, the influence of cortical damage on neurological disorders was explored. RESULTS:

Brain stem and spinal cord demyelination due to acute carbon monoxide poisoning.

Demyelination throughout the brain stem and spinal cord caused by acute carbon monoxide (CO) poisoning has not been previously reported. Magnetic resonance imaging (MRI) has revealed that acute CO poisoning primarily affects the subcortical white matter of the bilateral cerebral hemispheres and basal ganglia. Here we report the case of a patient with delayed neuropsychological sequelae (DNS) due to acute CO poisoning. A 28-year-old man was admitted to our department following a suicide attempt by acute CO poisoning. After a six-month pseudo-recovery period, he was diagnosed with DNS, with MRI

Carbon monoxide poisoning: a prediction model using meteorological factors and air pollutant.

BACKGROUND: While the influence of meteorology on carbon monoxide (CO) poisoning has been reported, few data are available on the association between air pollutants and the prediction of CO poisoning. Our objective is to explore meteorological and pollutant patterns associated with CO poisoning and to establish a predictive model. RESULTS: CO poisoning was found to be significantly associated with meteorological and pollutant patterns: low temperatures, low wind speeds, low air concentrations of sulfur dioxide (SO2) and ozone (O38h), and high daily temperature changes and ambient CO (r absolu

Demographic characteristics and delayed neurological sequelae risk factors in carbon monoxide poisoning.

AIM: Carbon monoxide (CO) is a colorless, odorless gas and tasteless. CO poisoning (COP) is one of the most frequently encountered inhalation poisonings. The most common cause of morbidity in COP is delayed neurological sequelae (DNS). DNS is the occurrence of neuropsychiatric findings within 2-240 days after discharge of patients with COP and there are no definitive diagnostic criteria. The aim of our study is; to determine the risk factors and incidence of DNS. METHOD: Our study is a retrospective, observational study. Patients with the diagnosis of COP in the emergency department between 2

Hemiplegia and bilateral globus pallidus infarcts after carbon monoxide poisoning: case report.

The vast clinical manifestations of carbon monoxide (CO) poisoning can involve the neurological, neuropsychological and cardiac systems as well as others. In this case report, we describe our management of a 64-year-old woman exposed to CO in her apartment. Her presentation was unusual in that she had symmetric globus pallidus lesions, no evidence of thrombosis, but the lateralizing neurologic manifestation of severe hemiplegia.

Central Diabetes Insipidus and Hyperglycemic Hyperosmolar State Following Accidental Carbon Monoxide Poisoning.

Carbon monoxide poisoning is common and carries significant morbidity and mortality. The nervous system, particularly the brain, is frequently affected by it, owing to its high metabolic activity and oxygen requirements. Carbon monoxide damages the nervous system by both hypoxic and inflammatory mechanisms. Central diabetes insipidus is an extremely rare complication of carbon monoxide poisoning. Herein, we report the case of a young lady, who developed this complication and severe hypernatremia after accidental carbon monoxide poisoning. She also developed a hyperglycemic hyperosmolar state d

Neurologic and radiologic manifestations of three girls surviving acute carbon monoxide poisoning.

We report the neurologic and radiologic manifestations of three adolescent girls with acute carbon monoxide poisoning. The girls were found collapsed and unconscious in a bathroom where liquid petroleum gas was being used as heating fuel. As hyperbaric oxygen therapy was not available locally, they only received oxygen supplementation via nasal cannula (4 L/minute) as treatment in the first 2 days. On transfer to a tertiary center in Hong Kong, evolving neurologic manifestations of visual acuity and field deficits, confusion, and focal motor weaknesses were observed. Focal infarctions were evi

Variability of neuropsychological deficits associated with carbon monoxide poisoning: four case reports.

Carbon monoxide (CO) poisoning is associated with variable neuropsychological deficits, depending on levels of CO exposure and individual differences. Studies to date have reported variable findings, as their subjects have been exposed to different levels of CO from different poisoning sources. Four unique case studies are presented, all of whom experienced the same level of CO poisoning (17-29%) in the same accident. Two of the individuals were brothers with an identical genetic disorder (i.e. syndactylism) and the other two were brother/sister. The results indicated: (1) variable neuropsycho

Cardiac and Thrombotic Effects of Severe Carbon Monoxide Poisoning: A Case Series.

INTRODUCTION: Carbon monoxide (CO) poisoning is the most common source of poisoning in the United States. Acute poisoning can cause a wide range of symptoms. CO poisoning can have long-term morbidity as well, the most well-described of which is the development of delayed neurologic sequelae. CO can cause acute and chronic cardiovascular and thrombotic effects as well. CASE REPORT: We describe four cases of severe CO poisoning with distinct cardiac and thrombotic effects. Carboxyhemoglobin levels ranged from 23.1%-42.5%. All four were critically ill and intubated. All four were evaluated with

Arterial blood gas analysis in risk stratification of acute carbon monoxide poisoning.

This single-centre retrospective cohort study demonstrates that arterial blood gas (ABG) parameters independently stratify ICU admission risk in acute carbon monoxide poisoning (ACOP) beyond carboxyhemoglobin (COHb). Among 124 ACOP patients, the acidosis group (pH<7.35) had a higher ICU admission rate [44.8%, 95% Confidence Interval (CI): 26.8%∼63.7%] than the normal pH (17.7%, 95%CI: 10.1%∼27.5%) and alkalemia groups (12.5%, 95%CI: 1.6%∼38.3%, P<0.001), with a median COHb of 33.1%. The acidosis group presented severe metabolic disturbance (median lactate 7.60 mmol/L, GCS 7). Spearman analysis

Hypocapnia in acute carbon monoxide poisoning: A complex interaction beyond metabolic compensation.

INTRODUCTION: Acute carbon monoxide poisoning frequently induces complex acid-base disturbances; however, whether hypocapnia serves solely as a compensatory response to metabolic acidosis or involves additional factors remains insufficiently characterized. METHODS: We retrospectively analyzed 940 emergency department patients with acute carbon monoxide poisoning (January 2019-February 2025), stratified by base excess into an acidosis group (<-3 mmol/L) and a non-acidosis group (≥ -3 mmol/L). Group comparisons used Mann-Whitney U tests, Spearman's correlations assessed bivariate relationships,

Hyperbaric oxygen for paroxysmal sympathetic hyperactivity syndrome after acute carbon monoxide poisoning.

Paroxysmal sympathetic hyperactivity syndrome (PSH) is common in patients with severe craniocerebral injuries. Carbon monoxide poisoning (ACOP) may lead to secondary PSH, and hyperbaric oxygen (HBO) is an important treatment method for ACOP that can promote the dissociation of carboxyhemoglobin and reduce the long-term sequelae of ACOP. To explore the risk factors and clinical characteristics of PSH secondary to acute ACOP and to investigate the efficacy of HBO treatment, a retrospective analysis was performed on patients with moderate to severe ACOP admitted to the Hyperbaric Oxygen Departmen

Association between carbon monoxide poisoning and adrenal insufficiency: a nationwide cohort study.

Carbon monoxide poisoning may damage the brain and adrenal glands, but it is unclear whether it is associated with adrenal insufficiency. We identified all COP patients diagnosed between 1999 and 2012 in Taiwan using the Nationwide Poisoning Database and selected a reference cohort (participants without COP) from the same database by exact matching of age and index date at a 1:2 ratio. Participants with a history of adrenal insufficiency or steroid use of more than 14 days were excluded. We followed up participants until 2013 and compared the risk of developing adrenal insufficiency between th

Carbon Monoxide poisoning and Air Embolism following Hysteroscopic Myomectomy: a case report.

INTRODUCTION: Uterine myomas are the most common benign uterine tumors. Hysteroscopic myomectomy has grown as a standard minimally invasive surgical procedure, but this technique is not free from complications. CASE: An hysteroscopic myomectomy was performed on a 38 years-old woman. During the awakening after the procedure, she presented focal neurological deficits, thus arterial blood gas test and total body computerized tomography (CT) scan were urgently carried out. They revealed a very high carboxyhemoglobin level and abdominal venous air embolism. The patient stayed in Trendelenburg posi
