Papillary fibroelastoma (PFE) is a benign tumor that arises mostly from left-sided valves. PFE can cause stroke, and surgical resection may be needed. Lambl's excrescence (LE) is a filiform valvular lesion and is considered a possible cause of stroke. A 79-year-old man with light-headedness and left-sided hemiparesis was diagnosed with stroke. Transesophageal echocardiography (TEE) revealed a round-shaped mobile mass in the left ventricular outflow tract (LVOT), which was considered the cause of the stroke. Surgical resection was performed transaortically, and during surgery, a mass was incide

Papillary fibroelastomas (PFE) are rare benign cardiac tumours mainly originating on aortic and mitral valvular surfaces. Management is individualised, but most recommend surgical excision due to thromboembolic risk. We report a 75-year-old man with symptomatic severe aortic stenosis compounded by PFE. Redo sternotomy aortic valve replacement was deferred in favour of the trans-apical (TAVR) approach. This report highlights, for the first time, the application of TAVR as a strategy for aortic valve stenosis and PFE to mitigate risk posed by injury to patent internal mammary arterial graft in c

Cardiac tumors are a group of rare disorders with a frequency that varies in population studies between 0.0017% and 0.33%. There are primary cardiac tumors with an incidence of 5% of all cardiac tumors and secondary tumors (metastases of the heart) in 95% of cases. Symptoms are nonspecific and can mimic many other heart diseases. This fact makes the diagnosis of cardiac tumors very difficult. Approximately 75% of the primary cardiac tumors are benign; the most frequent histological type encountered is myxoma in 50% of cases, followed by cardiac fibromas, lipomas, rhabdomyomas, hemangiomas, ter

A 63 year-old woman who had had mitral valve commissurotomy 12 years earlier was seen because of rheumatic mitral stenosis and left brachial paresis due to cerebral embolism. On clinical evaluation, a diastolic rumble was heard over the mitral area, and the echocardiogram revealed a mass attached to the mitral subvalvular apparatus. The patient was operated on, and both the surgical and histologic findings depicted papillary fibroelastoma. This tumor may occur as an isolated lesion or be associated with mitral valve stenosis or other cardiac abnormalities, and it is an important source of embo

Cardiac papillary fibroelastomas (PFEs) are the most common tumors of the cardiac valves and the third most common cardiac tumor. They are usually detected accidentally on echocardiography, but have the potential to manifest with catastrophic embolic phenomena, resulting in stroke and myocardial infarction. Echocardiography is currently the preferred diagnostic modality, while magnetic resonance imaging and computed tomography are helpful in the differential diagnosis of cardiac tumors. The management of PFEs is empiric, as no large randomized trials have been conducted to support specific tre

A 72-year-old female had persistent severe chest pain while climbing stairs. She was diagnosed as having acute coronary syndrome, and underwent an emergency coronary angiography (CAG). The right coronary artery (RCA) ostium was sub-totally occluded. Echocardiography revealed a 10 mm mobile mass at the right coronary cusp of the aortic valve. To avoid total obustruction on two drug eluting stents were placed at the RCA ostium so as to have the proximal end protrude into the right Valsalva sinus. Thus, her hemodynamic condition was stabilized. The tumor was surgically resected and the stents wer

Cardiac papillary fibroelastoma (PFE) is a rare benign endocardial tumor with significant embolic potential despite its non-malignant histology. We report the case of a 51-year-old woman with palpitations and a history of ischemic stroke involving the left medulla oblongata, accompanied by multiple posterior fossa infarcts. Extensive thrombophilic and autoimmune investigations were negative, and a patent foramen ovale was excluded. Multimodal cardiac imaging, including transesophageal echocardiography (TEE) and cardiac magnetic resonance imaging (CMR), identified a highly mobile pedunculated m

A 68-year-old man was referred to our hospital with a left ventricular (LV) mass and obstructive hypertrophic cardiomyopathy (HOCM). Although he was treated for the LV thrombosis and received anticoagulation therapy, the mass did not decrease in size for three years. His past history was colon cancer which was resected in its early phase. Laboratory studies revealed the absence of any inflammatory and tumor marker symptoms. Transthoracic and transesophageal echocardiography revealed a mass of 24x11 mm in diameter attached to the septal wall of LV and another two or three masses detected in LV

Papillary fibroelastoma (PFE) is an uncommon benign primary cardiac tumor whose detection has increased with the widespread use of advanced cardiac imaging, particularly transesophageal echocardiography and cardiac magnetic resonance imaging. Its clinical expression is heterogeneous, ranging from incidental findings in asymptomatic individuals to serious complications related to embolic events or myocardial ischemia. Surgical excision remains the definitive treatment to eliminate this risk. We report the case of a 69-year-old woman with a medical history of diabetes mellitus, hypertension, and

Papillary fibroelastoma (PFE) is a rare benign cardiac tumor, usually left-sided and valvular. Right-sided, non-valvular PFEs are uncommon, and systemic embolization from these lesions is particularly rare, typically requiring an interatrial shunt. We report a surgically confirmed right-atrial, non-valvular PFE presenting with paradoxical embolic stroke through an ostium secundum atrial septal defect (ASD). A 72-year-old woman with recurrent ischemic cerebrovascular events was admitted with an acute posterior circulation stroke. Transthoracic echocardiography was nondiagnostic. Transesophageal

Primary cardiac tumors are rare, with an incidence of 0.056% according to autopsy reports. The most common type is myxoma, while other types, including sarcoma, lipoma, papillary fibroelastoma, rhabdomyoma, fibroma, hemangioma, teratoma, lymphoma and mesothelioma also occur. Primary cardiac tumors usually cause embolization, pericardial effusion and arrhythmia, leading to heart failure. Only 10% of primary cardiac tumors are malignant, approximately 95% of which are sarcomas, while the remaining 5% are cardiac lymphomas and mesotheliomas. The present study reported a case of primary cardiac ly

We report the case of a 75-year-old female with atypical chest pain followed by non-Q wave myocardial infarction. At coronary angiography and ventriculography severe left main stenosis and severe mitral regurgitation were evidenced. Transesophageal echocardiography, performed in order to evaluate a possible mitral repair, showed an aortic tumor (11 x 14 mm in diameter) attached to the commissure between the left and non-coronary cusp, without mitral regurgitation. Cardiac magnetic resonance showed a relationship between the tumor and the left coronary ostium which explained a paroxysmal mitral

Embolic stroke secondary to cardiac tumors is uncommon. However, 25-30% of cardiac tumors may cause systemic emboli. We report a 29-year-old male consulting for a sudden episode of aphasia and right hemiparesis, compatible with infarct of the left middle cerebral artery territory. Transthoracic echocardiography reported an ovoid tumor of 8 × 7 × 7 mm in relation to the sub valvular apparatus of the mitral valve. After neurologic stabilization, surgical treatment was performed. Approached by median sternotomy and in cardiopulmonary bypass, the mitral valve was explored. A macroscopic tumor cons

A 75-year-old woman presented with recurrent severe chest pain radiating to her back. The initial ECG was normal, and clinically an aortic dissection was suspected. The patient became hypotensive during a chest computed tomography scan with infusion, had subsequent repeated cardiac arrests, and died. Autopsy revealed a 1.5-cm polypoid lesion, a papillary fibroelastoma, on the aortic valve, which effectively occluded the ostium of the left coronary artery in a ball-valve effect. In addition, an embolic fragment of the tumor was found in the left anterior descending artery. This is the sixth rep

BACKGROUND: Papillary fibroelastoma (PFE) is a rare benign cardiac tumor that may lead to embolic complications, particularly in patients with prosthetic valves.
CASE SUMMARY: A 62-year-old woman presented with dizziness, nausea, and left ocular paralysis. She had a history of hypertension, dyslipidemia, and mitral valve replacement. Electrocardiogram showed atrial fibrillation; transthoracic and transesophageal echocardiography revealed a mobile, pedunculated left ventricular mass (1.4 × 1.0 cm). Magnetic resonance imaging showed old cerebral infarcts. She underwent thoracotomy with mass res

Primary benign and malign pericardial ind heart tumors are very rare (0.002-0.03%). Benign tumors of heart and pericardium are more frequent (myxoma, rhabdomyoma, lipoma, and papillary fibroelastoma of endocardium: 70% of all primary tumors). Because of low frequency, clinicians rarely and hardly diagnose these types of tumors. The affected patients wander from one healthcare center to another, usually with misleading diagnosis, such as cardiomyopathy or mitral stenosis. Pericardial and cardiac tumors cause pressure to surrounding structures. Because of the specific anatomy and function, all h

An 80-year-old woman with a history of congestive heart failure, atrial fibrillation, and hypertension was transferred to our institution with hematemesis. Her drug regimen included 2 mg warfarin potassium/day to prevent thromboembolic events. Transthoracic echocardiography (TTE) performed at 78 years of age revealed a mass attached to the noncoronary cusp and a cardiac tumor was suspected. The patient declined surgery and was meticulously followed up with periodic TTE. Upper gastroendoscopy revealed a gastric ulcer with an exposed blood vessel; anticoagulant therapy was ceased. On day 15 of a
