[Comment on the article "Severe coronavirus-2 infection background for cardiovascular complications"].

Current information is presented on a phenomenon that occurs in patients who had COVID-19 that could contribute to long-term cardiovascular risk. It deals with the mechanisms underlying cardiovascular and coagulation alterations that perpetuate cardiac symptoms caused by alterations induced by the virus or by myocardial inflammation, such as cardiac endothelial dysfunction, which could cause vascular microthrombosis. The intentional search for this in patients under follow-up becomes relevant since there is specific treatment and long-term complications, such as pulmonary hypertension and righ

Preparations for the Assessment of COVID-19 Infection and Long-Term Cardiovascular Risk.

Studies showing that coronavirus disease 2019 (COVID-19) is associated with an increased risk of cardiovascular disease continue to be published. However, studies on how long the overall cardiovascular risk increases after COVID-19 and the magnitude of its long-term effects have only been confirmed recently. This is partly because the distinction between cardiovascular risk as an acute complication of COVID-19 or post-acute cardiovascular manifestations is ambiguous. Long-COVID has arisen as an important topic in the second half of the pandemic. This term indicates that symptoms persist for mo

Circulating T Cells and Cardiovascular Risk in People With and Without HIV Infection.

BACKGROUND: Lower CD4+ cell count in people with HIV infection (PWH) is associated with increased cardiovascular disease (CVD) risk. Whether subsets of CD4+ T helper cells are linked with CVD is unclear. OBJECTIVES: The aim of this study was to explore the association between peripherally circulating CD4+ T cell subsets and incident CVD. METHODS: Data from 1,860 participants (1,270 PWH) without prevalent CVD from the VACS (Veterans Aging Cohort Study), a prospective, observational cohort of veterans with and without HIV infection, were analyzed. T cell subsets were quantified in baseline sam

A Turtle Disaster: Salmonella enteritidis Cardiovascular Implantable Electronic Device Infection.

Cardiovascular implantable electronic device (CIED) infections have high mortality and morbidity. CIED infections secondary to gram-negative pathogens are rare, and there are few data regarding their treatment. We report a case of a 60-year-old male who developed recurrent Salmonella enteritidis bacteremia leading to CIED infection and nonsusceptibility to ciprofloxacin.

Persistent Coxiella burnetii cardiovascular infection on Bentall-De Bono prosthesis.

Coxiella burnetii cardiovascular prosthetic infections are associated with high morbidity and mortality and represent a major health problem due to the lack of standardized management. We were confronted with a C. burnetii infection on Bentall-De Bono prosthesis characterized by a history of vascular infection with relapse that prompted us to screen for cases of C. burnetii on Bentall-De Bono vascular prosthesis monitored in our center. We screened patients between 1991 and 2019, from the French national reference center for Q fever. A microbiological criterion in addition to a lesional criter

Regional fat deposition and cardiovascular risk in HIV infection: the FRAM study.

HIV-infected individuals are at increased risk for cardiovascular disease (CVD) and lipodystrophy, but the relationship between regional adipose tissue (AT) depots and CVD risk is not well described. We determined regional AT volumes and CVD risk in an analysis of 586 HIV-infected and 280 control FRAM study subjects using whole-body magnetic resonance imaging (MRI) and the Framingham Risk Score (FRS). Median FRS and FRS >10% were higher in HIV than control men (4.7% vs. 3.7%, p=0.0002; 16% vs. 4%, p<0.0001). HIV and control women had similarly low FRS (1.1% vs. 1.2%, p=0.91). In controls, tota

Subclinical carotid atherosclerosis and cardiovascular risk factors in HIV-infected patients.

BACKGROUND: HIV infected patients, especially those treated with antiretroviral (ARV) drugs, show an increased risk and incidence of cardiovascular disease. OBJECTIVES: The aim of this study was to evaluate the progression of subclinical atherosclerosis in the carotid arteries, expressed as the value of carotid intima-media thickness (cIMT) and the amount of atherosclerotic plaques, and to analyze the correlation between cIMT and risk factors for cardiovascular diseases in a cohort of HIV infected patients. METHODS: The analysis included 72 HIV infected patients, mean age 39.4 years, and 27

Cardiovascular implantable electronic device infection in patients with Staphylococcus aureus bacteremia.

BACKGROUND: Staphylococcus aureus bacteremia (SAB) in patients with cardiovascular implantable electronic devices (CIED), including permanent pacemakers (PPMs) and implantable cardioverter-defibrillators (ICD), can be the sole manifestation of device infection. METHODS: To assess clinical factors associated with CIED infection, we retrospectively reviewed all patients with both CIED and SAB seen at Mayo Clinic Rochester between 2001 through 2006. CIED infection was defined using microbiological and clinical criteria. RESULTS: Of the 62 patients with SAB and a CIED, 22 patients (35.5%) had CI

Metabolic and cardiovascular complications of highly active antiretroviral therapy for HIV infection.

Highly active antiretroviral therapy (HAART) regimens, especially those including protease inhibitors have been shown to cause, in a high proportion of HIV-infected patients, a metabolic syndrome (lipodystrophy/lipoatrophy, dyslipidemia, type 2 diabetes mellitus, insulin resistance) that may be associated with an increased risk of cardiovascular disease. A careful stratification of the cardiovascular risk of HIV-infected patients under HAART is needed according to the most recent clinical guidelines.

Mycobacterium smegmatis Infection in a Cardiovascular Implantable Electronic Device in an Immunocompetent Patient.

Cardiovascular implantable electronic devices (CIEDs) are increasingly utilized for arrhythmia management, but device-related infections remain a significant complication. Rapidly growing non-tuberculous mycobacteria (RGM), including Mycobacterium smegmatis, are rare causes of CIED infections, typically reported in immunocompromised patients or in association with indwelling catheters. These infections pose diagnostic and therapeutic challenges due to non-specific clinical presentations and intrinsic antimicrobial resistance. We report an immunocompetent elderly patient who developed complete

Risk of Cardiovascular Implantable Electronic Device Infection in Patients With Cardiac Sarcoidosis.

BACKGROUND: Immunosuppressive therapy plays a key role in managing cardiac sarcoidosis (CS). The decision to implant a cardiovascular implantable electronic device (CIED) is frequently made alongside the initiation of immunosuppression. However, it remains unclear whether being on immunosuppressive therapy at the time of CIED implantation increases the risk of infection. As both interventions can be lifesaving, it is important to determine whether adjustments to immunosuppressive regimens are necessary to mitigate infection risk. OBJECTIVE: To assess the correlation between CIED-related infec

COVID-lateral damage: cardiovascular manifestations of SARS-CoV-2 infection.

Early in the pandemic, concern that cardiovascular effects would accompany COVID-19 was fueled by lessons from the first SARS epidemic, knowledge that the SARS-COV2 entry receptor (Angiotensin-converting enzyme 2, ACE2) is highly expressed in the heart, early reports of myocarditis, and first-hand accounts by physicians caring for those with severe COVID-19. Over 18 months, our understanding of the cardiovascular manifestations has expanded greatly, leaving more new questions than those conclusively answered. Cardiac involvement is common (∼20%) but not uniformly observed in those who require

SARS-CoV-2 Infection and Cardiovascular Disease: COVID-19 Heart.

Coronavirus disease (COVID-19) is a serious illness caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The symptoms of the disease range from asymptomatic to mild respiratory symptoms and even potentially life-threatening cardiovascular and pulmonary complications. Cardiac complications include acute myocardial injury, arrhythmias, cardiogenic shock and even sudden death. Furthermore, drug interactions with COVID-19 therapies may place the patient at risk for arrhythmias, cardiomyopathy and sudden death. In this review, we summarise the cardiac manifestations of COVID-19 i

Cardiovascular involvement and manifestations of systemic Chikungunya virus infection: A systematic review.

BACKGROUND: In the last three years, chikungunya virus disease has been spreading, affecting particularly the Americas, producing more than two million cases. In this setting, not only new disease-related epidemiological patterns have been found, but also new clinical findings have been reported by different research groups. These include findings on the cardiovascular system, including clinical, electrocardiographic and echocardiographic alterations. No previous systemic reviews have been found in major databases about it. METHODS: We performed a systematic review looking for reports about c
