Obstructive Pyelonephritis With Enterococcal Bacteremia Despite Unremarkable Urinalysis.

Obstructive pyelonephritis can be diagnostically challenging when urinary findings are unremarkable. Enterococcus faecalis is a well-known cause of complicated urinary tract infections and bacteremia; however, its clinical presentation may be atypical in patients with obstruction. A 73-year-old man presented with fever and left flank pain. Urinalysis and urinary sediment examination on admission were normal, and a urine culture was not obtained during the initial evaluation. Two sets of blood cultures yielded E. faecalis. Non-contrast computed tomography revealed a 12-mm left ureteral stone wi

Interaction of 31 beta-lactam antibiotics with the H+/peptide symporter PEPT2: analysis of affinity constants and comparison with PEPT1.

The activity of the renal peptide transporters PEPT2 and PEPT1 determines-among other factors such as metabolic stability in liver and plasma-the circulatory half-life of penicillins and cephalosporins during therapy. This study was initiated to examine systematically the interaction of beta-lactam antibiotics with PEPT2. Interaction of 31 cephalosporins and penicillins with the carrier protein was characterized by measuring their ability to inhibit the uptake of [(14)C]Gly-Sar into renal SKPT cells. Cefadroxil, cefaclor, cyclacillin, cephradine, cephalexin and moxalactam were recognized by PE

Invasive Infection Caused by a Hypervirulent ST367-KL1 Klebsiella quasipneumoniae subsp. similipneumoniae: First Case Report in Japan.

Klebsiella quasipneumoniae is a member of the Klebsiella pneumoniae complex that is difficult to distinguish from K. pneumoniae using conventional laboratory methods, and information on its clinical manifestations is limited. We describe the first known case of invasive infection with hypervirulent K. quasipneumoniae subsp. similipneumoniae in Japan. A 65-year-old woman with pancreatogenic diabetes mellitus presented with fever and nausea. Computed tomography revealed a liver abscess and multiple pulmonary nodules with cavitation suggestive of septic pulmonary emboli. Blood cultures yielded an

A pediatric case of ureterolithiasis due to cystinuria accompanied by acute appendicitis; a case report.

INTRODUCTION: Acute abdominal pain, a chief complaint frequently seen in the emergency department, can be triggered by a vast range of conditions. Although ureterolithiasis is a less common cause in children, renal colic can be caused by calculi due to hereditary metabolic diseases among patients in those age groups. PRESENTATION OF CASE: We report a 12-year-old girl with abdominal pain who was diagnosed with concurrent acute appendicitis and ureterolithiasis due to cystinuria. Acute appendicitis was successfully treated with cefmetazole, and the calculus was eliminated after adequate fluid l

Frequent isolation of extended-spectrum beta-lactamase-producing bacteria from fecal samples of individuals with severe motor and intellectual disabilities.

Extended-spectrum beta-lactamase (ESBL) producing bacteria spread worldwide and became major concern for antibiotic treatment. Although surveillance reports in general hospitals and long-term care facilities are increasing, their frequencies in individuals with severe motor and intellectual disabilities (SMID) are so far unknown. In this study, we examined the frequency of ESBL in stool samples collected from 146 asymptomatic SMID subjects hospitalized in a single institution. With their clinical information, we evaluated possible risk factors for ESBL colonization. From 146 fecal samples, ESB

Recurrent Liver Abscess Caused by Mucoid-Type Klebsiella pneumoniae Successfully Treated With Hepatic Artery Antibiotic Infusion Therapy: A Case Report.

Liver abscesses typically respond well to a combination of antibiotics and drainage. However, some cases can be resistant to treatment and pose a clinical challenge. Infections caused by mucoid-type Klebsiella pneumoniae are rare, but highly invasive, and can lead to conditions such as liver abscesses and hematogenous dissemination, resulting in systemic complications. Herein, we present the case of an 80-year-old woman with recurrent multilocular liver abscess caused by mucoid-type K. pneumoniae who was successfully treated with hepatic artery antibiotic infusion therapy. The patient was init

Effects of Prospective Audit and Feedback in Patients with Extended-Spectrum β-Lactamase-Producing Escherichia coli Bacteremia.

Antimicrobial stewardship (AS) Guidelines by the Infectious Diseases Society of America recommend employing prospective audit and feedback (PAF) as an effective intervention in AS programs. Since July 2022, our hospital has implemented PAF for all patients with positive blood cultures, including those with extended-spectrum β-lactamase (ESBL)-producing Escherichia coli (EC) bacteremia. Our study examined the effect of PAF on clinical outcomes in patients with ESBL-EC bacteremia. We enrolled 62 patients diagnosed with ESBL-EC via blood culture who were undergoing antibiotic treatment. The patie

Investigation of the Importance of Multidrug Resistance-Associated Protein 4 (Mrp4/Abcc4) in the Active Efflux of Anionic Drugs Across the Blood-Brain Barrier.

The importance of multidrug resistance-associated protein 4 (Mrp4/Abcc4) in limiting the penetration of Mrp4 substrate compounds into the central nervous system across the blood-brain barrier was investigated using Mrp4-/- mice. Significant adenosine triphosphate-dependent uptake by MRP4 was observed for ochratoxin A, pitavastatin, raltitrexed (Km = 43.7 μM), pravastatin, cyclic guanosine monophosphate, 2,4-dichlorophenoxyacetate, and urate. The defect in the Mrp4 gene did not affect the brain-to-plasma ratio (Kp,brain) of quinidine and dantrolene. Following intravenous infusion in wild-type a

Chimeric TK-NOG mice: a predictive model for cholestatic human liver toxicity.

Due to the substantial interspecies differences in drug metabolism and disposition, drug-induced liver injury (DILI) in humans is often not predicted by studies performed in animal species. For example, a drug (bosentan) used to treat pulmonary artery hypertension caused unexpected cholestatic liver toxicity in humans, which was not predicted by preclinical toxicology studies in multiple animal species. In this study, we demonstrate that NOG mice expressing a thymidine kinase transgene (TK-NOG) with humanized livers have a humanized profile of biliary excretion of a test (cefmetazole) drug, wh

In vitro and in vivo effects of combinations of cefotaxime or other beta-lactams with rolitetracycline on methicillin-resistant Staphylococcus aureus.

Combinations of cefotaxime (CTX) or other five beta-lactams with rolitetracycline (RTC) were examined using the checkerboard method for their synergistic effects against 27 strains of methicillin-resistant Staphylococcus aureus (MRSA), and the combination of CTX and RTC was further evaluated for its synergistic effect in vitro and in vivo against 1 or 2 strains. Synergy occurred against 44% of the strains when RTC was combined with CTX, 22 to 30% with cefazolin, methicillin, and ceftizoxime, and 4 to 11% with latamoxef and cefmetazole. No antagonism was found with any combinations tested. Kill
