The Missing Piece in Chronic Disease Prevention: Dental Caries.

INTRODUCTION: OpenStreetMap is an open-source and crowd-sourced geospatial database and one of the few available sources of sidewalk data for walkability research; however, its accuracy and completeness have not been systematically assessed. This study aimed to compare OpenStreetMap sidewalk data with Google Street View. METHODS: We clipped an OpenStreetMap map of pedestrian-accessible roads in Maryland to 141 census tracts (~10.0% of census tracts in Maryland) using stratified random sampling across tertiles of population density. Fifty points within each census tract were randomly distribut

Burden of enteric fever at three urban sites in Africa and Asia: a multicentre population-based study.

BACKGROUND: Enteric fever is a serious public health concern in many low-income and middle-income countries. Numerous data gaps exist concerning the epidemiology of Salmonella enterica serotype Typhi (S Typhi) and Salmonella enterica serotype Paratyphi (S Paratyphi), which are the causative agents of enteric fever. We aimed to determine the burden of enteric fever in three urban sites in Africa and Asia. METHODS: In this multicentre population-based study, we did a demographic census at three urban sites in Africa (Blantyre, Malawi) and Asia (Kathmandu, Nepal and Dhaka, Bangladesh) between Ju

Identifying Communities at Risk for COVID-19-Related Burden Across 500 US Cities and Within New York City: Unsupervised Learning of the Coprevalence of Health Indicators.

BACKGROUND: Although it is well-known that older individuals with certain comorbidities are at the highest risk for complications related to COVID-19 including hospitalization and death, we lack tools to identify communities at the highest risk with fine-grained spatial resolution. Information collected at a county level obscures local risk and complex interactions between clinical comorbidities, the built environment, population factors, and other social determinants of health. OBJECTIVE: This study aims to develop a COVID-19 community risk score that summarizes complex disease prevalence to

Social Vulnerability Modifies the Effects of Geographic Proximity on Engagement in Latent Tuberculosis Infection Care in a United States Safety Net Healthcare Network.

BACKGROUND: Latent tuberculosis (TB) infection care often requires engagement with multiple teams in several locations throughout the diagnostic and treatment steps of the TB infection care cascade. The intersecting effects of geographic proximity and social drivers on care cascade retention have not been well examined. METHODS: We conducted a retrospective cohort study of patients with a positive TB infection test between 2018-2019 within a health system in Boston, Massachusetts. The primary outcome was attendance at a TB clinic after a referral was placed. The primary exposure was geographi

Life Expectancy and Causes of Premature Death by Subgroup for Community-Based Action in Marin County, California, 2017-2021.

Marin is a medium-sized county in California's San Francisco Bay Area. Despite its historically higher-than-average life expectancy and socioeconomic level, known economic and health disparities by race, ethnicity, and geography became more visible during the COVID-19 pandemic.  Methods: We calculated life expectancy, measured years of potential life lost (YPLLs), and described premature mortality for the five years of 2017-2021 by race, ethnicity, census tract, and resource level (as measured by Healthy Places Index [HPI]) to provide data on inequities to guide community-centered action to re

Understanding how the concentration of neighborhood advantage and disadvantage affects spina bifida risk among births to non-Hispanic white and Hispanic women, Texas, 1999-2014.

BACKGROUND: This is the first study to utilize the index of concentration at the extremes (ICE) to examine risk factors for spina bifida in Texas. The ICE is a useful measure for providing the degree to which residents in a certain area are concentrated into groups at the extremes of disadvantage and privilege. We introduce two novel ICE measures (language and nativity), and three existing ICE measures (race/ethnicity, income, and education), which we applied specifically to Texas residents. METHODS: We used multivariable mixed-model Poisson regression analyses to estimate spina bifida birth

Geographic and socioeconomic disparities to retinopathy of prematurity treatment.

PURPOSE: To identify geographic and socioeconomic factors predictive of travel burden on families to neonatal intensive care units (NICUs) that provide acute treatment for retinopathy of prematurity (ROP) and long-term, outpatient follow up care. DESIGN: This is a retrospective cross-sectional study. METHODS: This cross-sectional analysis crosslinked data from a nationwide survey of NICU ROP treatment, with multiple large publicly available datasets. Geographic information systems analysis identified travel time from each census tract to the nearest NICU offering ROP treatment. Multivariable

Air pollution exposure among people with limitations in activities of daily living in the United States.

Structural barriers including limited healthcare access and disability-related health conditions make disabled people differentially susceptible to air pollution-related adverse health outcomes compared to nondisabled people. We used 2020 census-tract level counts of individuals with limitations in activities of daily living (ADLs) to identify a subset of disabled people. We described geographic areas where this population was highly exposed to air pollution in the contiguous U.S., indicsating health risk. We assessed census tract-level exposure to PM2.5, O3, NO2 (2016-2020), and wildfire PM2.

Multilevel modelling of the incidence of visceral leishmaniasis in Teresina, Brazil.

Epidemics of visceral leishmaniasis (VL) in major Brazilian cities are new phenomena since 1980. As determinants of transmission in urban settings probably operate at different geographic scales, and information is not available for each scale, a multilevel approach was used to examine the effect of canine infection and environmental and socio-economic factors on the spatial variability of incidence rates of VL in the city of Teresina. Details on an outbreak of greater than 1200 cases of VL in Teresina during 1993-1996 were available at two hierarchical levels: census tracts (socio-economic ch

A Retrospective, Cross-Sectional Study of Geographic Food Environment and Diabetes in Pregnancy.

OBJECTIVE: Insufficient access to healthy food has been linked to poor health outcomes in under-resourced communities. The relationship between neighborhood-level food insecurity and diabetes in pregnancy remains understudied, with previous studies reporting inconsistent results. This study examined the association between living in a low-income, low access (LILA) census tract and the prevalence of pregestational type 2 diabetes (T2D) and gestational diabetes (GDM) among pregnant individuals. STUDY DESIGN: This cross-sectional study included patients who delivered a singleton pregnancy at ≥20

Preventable avoidable mortality: evolution of socioeconomic inequalities in urban areas in Spain, 1996-2003.

This study describes the inequalities in preventable avoidable mortality in relation to socioeconomic levels and analyses their evolution during the period 1996-2003 in the cities of Alicante, Castellon and Valencia. Four causes of preventable avoidable mortality were analysed according to sex: malignant tumour of the trachea, bronchus and lung, cirrhosis and other chronic diseases of the liver, motor vehicle accidents and AIDS, which had caused the death of non-institutionalised residents in the three cities during the period 1996-2003. The different census tracts were grouped into three soci

Predictors of early life residential mobility in urban and rural Pennsylvania children with acute lymphoblastic leukemia and implications for environmental exposure assessment.

BACKGROUND: Residential mobility can introduce exposure misclassification in pediatric epidemiology studies using birth address only. OBJECTIVE: We examined whether residential mobility varies by sociodemographic factors and urbanicity/rurality among children with cancer. METHODS: Our study included 400 children born in Pennsylvania during 2002-2015 and diagnosed with leukemia at ages 2-7 years. Addresses were obtained from state registries at birth and diagnosis. We considered three aspects of mobility between birth and diagnosis: whether a child moved, whether a mover changed census tract,

Identifying pre-diabetes 'hotspots' in Northern California using geospatial analysis: opportunities to target diabetes prevention strategies and improve health equity.

OBJECTIVES: The US Preventive Services Task Force recommends screening of adults aged 35-70 with a body mass index ≥25 kg/m2 for type 2 diabetes and referral of individuals who screen positive for pre-diabetes to evidence-based prevention strategies. The diabetes burden in the USA is predicted to triple by 2060 necessitating strategic diabetes prevention efforts, particularly in areas of highest need. This study aimed to identify pre-diabetes hotspots using geospatial mapping to inform targeted diabetes prevention strategies. A 'hotspot' is defined as a cluster of 3 or more neighbouring census

Relationship Between Sexually Transmitted Infections and Social Determinants of Health in Durham County, North Carolina, United States.

BACKGROUND: Sexually transmitted infections (STIs) are increasing in the United States, and certain populations are more at risk than others. One explanation for this is inequities in underlying social determinants of health (SDOH). METHODS: We analyzed chlamydia, gonorrhea, and syphilis cases in Durham County, North Carolina, from 01/01/2020 to 12/31/2020 by select SDOH at the census tract level. We included 48 variables of interest, including variables related to income, education, transportation, and health insurance. For each variable, we modeled STI incidence at the census tract level us

Relationships between neighbourhood characteristics and current STI status among HIV-infected and HIV-uninfected women living in the Southern USA: a cross-sectional multilevel analysis.

OBJECTIVES: Neighbourhood characteristics (eg, high poverty rates) are associated with STIs among HIV-uninfected women in the USA. However, no multilevel analyses investigating the associations between neighbourhood exposures and STIs have explored these relationships among women living with HIV infection. The objectives of this study were to: (1) examine relationships between neighbourhood characteristics and current STI status and (2) investigate whether the magnitudes and directions of these relationships varied by HIV status in a predominantly HIV-infected cohort of women living in the Sou
