Neonatal invasive meningococcal disease: Clinical case from Saudi Arabia and updated review of reported cases.

Neonatal invasive meningococcal disease (IMD) is rare but associated with severe morbidity and mortality. We report an 11-day-old male neonate who presented with fever, apnea, and poor feeding and was diagnosed with meningitis due to Neisseria meningitidis. Diagnosis was confirmed by blood culture and cerebrospinal fluid (CSF) multiplex PCR, although CSF culture remained negative. The patient developed ventriculitis, requiring prolonged antimicrobial therapy. To contextualize this case, we reviewed 71 previously reported neonatal IMD cases published between 1916 and 2024. The present case is d

Endoscopic evidence of ventricular and cisternal inflammatory changes after intraoperative cysticercal rupture during endoscopic third-ventriculostomy removal.

Two patients who suffered recurrent hydrocephalus after cysticercal removal by means of endoscopic transventricular (ETV) approach are presented. Severe inflammatory lesions within the ventricular system and basal cisterns, with a patent third-ventriculostomy were demonstrated during a second endoscopic observation. Mandatory shunting with prolonged steroid therapy may be indicated after intraoperative cysticercal rupture after ETV removal, as showed by sequential endoscopic observations.

Suspected bacterial ventriculitis in a nine-month-old Goldendoodle.

Bacterial ventriculitis is an uncommon, often fatal complication of intracranial bacterial infection. This case report describes a nine-month-old spayed female Goldendoodle that presented with a history of acute onset generalised seizures. The initial clinical examination, neurological examination and minimum database laboratory tests were unremarkable, and the dog was diagnosed with probable idiopathic epilepsy. Three weeks thereafter, the patient developed breakthrough clustering orofacial and generalised seizures. Magnetic resonance imaging (MRI) findings were consistent with bacterial vent

From Headache to Hydrocephalus: The Hidden Pulmonary Arteriovenous Malformation Behind Multiple Brain Abscesses.

A 24-year-old man experienced high-grade fever, chills, and headaches for 2 months. From August 18 to 28, 2021, he received intraperitoneal treatment, then remained at home. One month passed before he underwent external ventricular drainage for hydrocephalus. Two days later, he presented with vomiting, altered sensorium, and a foreign body sensation. Upon examination, the patient's vitals were stable. Clubbing was noted on all digits. He was afebrile but had an abnormal central nervous system exam with a Glasgow Coma Scale score of E4V3M5. He was disoriented and nonresponsive to commands. The

Salmonella enteritidis meningitis in an immunocompetent adult.

Invasive Salmonella infection is associated with high morbidity and mortality. It commonly affects patients who are immunocompromised or at extremes of age. We report an unusual case of a previously well 50-year-old gentleman with disseminated Salmonella infection. He presented with left hemispheric syndrome from an acute left middle cerebral artery stroke, complicated by status epilepticus requiring admission to the intensive care unit (ICU). Investigations revealed a large retroperitoneal abscess, and evidence of meningeal inflammation, ventriculitis and empyema. He required an abdominal dra

Fungal and Parasitic CNS Infections.

Central nervous system fungal infections can be broadly divided into those that infect a healthy host such as Cryptococcus, Coccidioides, Histoplasma, Blastomyces, Sporothrix spp., and those that cause opportunistic infections in an immunocompromised host such as Candida, Aspergillus, Zygomycetes, Trichosporon spp. The clinical manifestations of central nervous system fungal infections commonly seen in children in clinical practice include a chronic meningitis or meningoencephalitis syndrome, brain abscess, rhino-cerebral syndrome and rarely, a fungal ventriculitis. Fungal central nervous syst

The National Neuroscience Institute External Ventricular Drain Study: A Pragmatic Multisite Risk-Stratification Pathway to Reduce Ventriculostomy-Related Infection.

OBJECTIVE: Ventriculostomy-related infection (VRI) is associated with potential serious morbidity, extended hospitalization duration, increased health care costs, and mortality. We assessed the effectiveness of a pragmatic risk-stratification pathway for external ventricular drain (EVD) management, allowing for surgical decision making, in reducing the rate of VRIs. METHODS: Two studies were performed concurrently. A retrospective audit of EVD infection rates and outcomes in our unit across 3 hospitals was conducted from January to December 2014. The second prospective study compared the same

Cerebrospinal fluid leak-associated ventriculitis - a case report.

We present a case of a 74-year-old woman with headaches, pyrexia and intermittent right-sided otorrhoea and rhinorrhoea. Her nasal discharge tested positive for beta-2-transferrin, confirming a cerebrospinal fluid (CSF) leak. High-resolution CT (HRCT) mastoids showed a defect in the right tegmen, and CSF within the middle ear and mastoid air cells. A gadolinium-enhanced MRI brain showed high signal in the occipital horns of the lateral ventricles with diffusion-weighted imaging (DWI) and a low signal with DWI-apparent diffusion coefficient (DWI-ADC), consistent with ventriculitis. She made an

Intraventricular colistin sulphate as a last resort therapy in a patient with multidrug-resistant Acinetobacter baumannii induced post-neurosurgical ventriculitis.

Limited therapeutic options exist for multidrug-resistant/extensively drug-resistant Acinetobacter baumannii (MDR/XDR-Ab) meningitis/ventriculitis. A combination of intravenous and intraventricular (IVT)/intrathecal (IT) polymyxins achieves good therapeutic outcomes for cases of healthcare-associated MDR/XDR-Ab meningitis/ventriculitis. Colistin is commercially available as colistin sulphate and its sulphomethylated derivative. However, the effect and safety of colistin sulphate in the treatment of MDR/XDR-Ab meningitis/ventriculitis has not been reported. We report on a 66-year-old male patie

Ventriculitis and meningitis: by methicillin-resistant S. epidermidis: comparison of serum and CSF vancomycin concentrations and possible effect on MIC.

A case of HCAMV due to methicillin-resistant S. epidermidis (MRSE) in a new-born treated with vancomycin is described. Despite adequate vancomycin serum concentrations, an increase in vancomycin MIC (from 1 to 2 mg/L) in presence of persistently low CSF concentrations were observed. The eGFR during the treatment period was normal (no evidence of hyperfiltration). This report confirms the observation of a relationship among antibiotic PK/PD, cure of localized infections and resistance selection and specifically suggests the need for a first-line alternative to vancomycin in case of HCAMV due to

Neonatal bacterial meningitis versus ventriculitis: a cohort-based overview of clinical characteristics, microbiology and imaging.

Central nervous system (CNS) infections are potentially life threatening in neonates and can lead to the ill-defined diagnosis of ventriculitis. With this study we aimed to explore and describe ventriculitis regarding clinical, microbiological and ultrasonographic characteristics. We performed a retrospective cohort study including all neonates with a culture-proven CNS infection admitted to our tertiary NICU over a 12-year period (2004-2016). For each case clinical data was gathered, and three timed cranial ultrasounds were anonymized and retrospectively reviewed and assessed for signs of ven

Iatrogenic Ventriculitis Due to Mycoplasma Hominis: A Case Report and Review of the Literature.

BACKGROUND Mycoplasma hominis, which rarely causes infection after neurosurgical procedures, is a small free-living organism, belonging to the genus Mycoplasma. M. hominis lacks a rigid cell wall and cannot be clearly visualized by routine light microscopy. Thus, it is challenging to diagnose infections caused by this pathogen. Here, we report a case of Mycoplasma hominis causing iatrogenic ventriculitis secondary to extraventricular drain. CASE REPORT A 25-year-old man who was a victim of a road traffic accident developed M. hominis ventriculitis secondary to extraventricular drain. Despite a

Decrease in proven ventriculitis by reducing the frequency of cerebrospinal fluid sampling from extraventricular drains.

OBJECT: Ventriculitis associated with extraventricular drains (EVD) increases rates of morbidity and mortality as well as costs. Surveillance samples of CSF are taken routinely from EVD, but there is no consensus on the optimum frequency of sampling. The goal of this study was to assess whether the incidence of ventriculitis changed when CSF sampling frequency was reduced once every 3 days. METHODS: After receiving institutional ethics committee approval for their project, the authors compared a prospective sample of EVD-treated patients (admitted 2008-2009) and a historical comparison group
