Horner's Syndrome Caused by Ultrasound-Guided Supraclavicular Nerve Block.

Horner's syndrome, caused by supraclavicular nerve block, is a rare case. It is mostly expected after interscalene nerve block, caused by anatomic reasons. Horner's syndrome results from neuronal paralysis of the post-ganglionic cervical sympathetic chain. For anatomic reasons, interscalene nerve block is very common but very uncommon in the case of supraclavicular nerve block. Horner's syndrome results from the paralysis of the ipsilateral sympathetic cervical chain. One common cause is interscalene nerve block. This effect occurs frequently due to anatomical proximity - the brachial plexus n

Diagnostic value of combined magnetic resonance imaging examination of brachial plexus and electrophysiological studies in multifocal motor neuropathy.

BACKGROUND/AIM: Multifocal motor neuropathy (MMN) is an immune-mediated disorder characterised by slowly progressive asymetrical weakness of limbs without sensory loss. The objective of this study was to investigate the involvement of brachial plexus using combined cervical magnetic stimulation and magnetic resonance imaging (MRI) of plexus brachialis in patients with MMN. We payed special attention to the nerve roots forming nerves inervating weak muscles, but without detectable conduction block (CB) using conventional nerve conduction studies. METHODS: Nine patients with proven MMN were inc

Anesthetic management of primary hyperparathyroidism during pregnancy: A case report.

RATIONALE: Primary hyperparathyroidism (PHPT) during pregnancy is rare. Nevertheless, hypercalcemia secondary to gestational PHPT may be masked by physiological changes in calcium homeostasis during pregnancy. Gestational PHPT constitutes a serious danger to mother and fetus. Surgery is the only curative treatment when conservative treatment could not control the condition. Due to the lack of guidelines concerning PHPT during pregnancy, the optimal anesthetic management of PHPT during pregnancy needs to be individualized. Patient concerns: We report a case of PHPT with successful surgical trea

Cervical root stimulation in a case of classic neurogenic thoracic outlet syndrome.

We performed C8 nerve root stimulation in addition to other electromyographic (EMG) studies in a surgically proven case of classic thoracic outlet syndrome (TOS). The patient was a 19-year-old woman with a 2-year history of right hand cramps and progressive weakness and atrophy of hand muscles, especially the thenar eminence. Routine EMG studies showed evidence for an axon-loss lower trunk brachial plexopathy. Stimulation studies of the C8 nerve roots demonstrated proximal conduction block on the affected side only. The diagnosis was further supported by cervical spine radiographs, which demon

Probable cervical midline epidural septum complicating the treatment of a patient with upper extremity sympathetically maintained pain.

We present a woman who developed left arm sympathetically maintained pain (SMP, or "shoulder-hand syndrome") as a result of brachial plexus injury. After confirmatory diagnosis with both stellate local anesthetic block and intravenous phentolamine infusion, the patient had a cervical epidural catheter placed and a local anesthetic infusion started. After numerous unilateral blocks were obtained, a cervical epidurogram demonstrated a probable cervical midline epidural septum. Catheter placement was adjusted, and a successful chemical sympathectomy was performed for 6 days. This resulted in sign

Motor root conduction in neuralgic amyotrophy: evidence of proximal conduction block.

OBJECTIVE: To determine the presence and role of proximal conduction block in neuralgic amyotrophy. METHODS: Percutaneous electrical stimulation of cervical roots and brachial plexus was employed in eight patients with neuralgic amyotrophy. Root to Erb's point compound muscle action potential amplitude ratios for abductor digiti minimi, extensor digitorum communis, biceps, and deltoid muscles were compared with results obtained from 10 healthy controls. RESULTS: Conduction block in the nerve to one muscle was found in three of eight patients (38%) suggesting focal proximal demyelination. Rep

Surgical Approach to Injuries of the Cervical Plexus and Its Peripheral Nerve Branches.

BACKGROUND: Located in the neck beneath the sternocleidomastoid muscle, the cervical plexus comprises a coalition of nerves originating from C1 through C4, which provide input to four cutaneous, seven motor, and three cranial nerves and the sympathetic trunk. Sporadic instances of injury to these superficial nerves have been reported. Nevertheless, this specific anatomical cause of neurogenic pain remains incompletely described and underrecognized. METHODS: Twelve patients presented with pain and were diagnosed with various combinations of injury to the lesser occipital, great auricular, tran

A rare anomaly of the cervicobrachial plexus: penetration of the anterior scalene muscle tendon by the T1 nerve root.

The cervicobrachial plexus is known to exhibit anatomical variation, particularly the brachial plexus, which maintains a close anatomical relationship with the subclavian artery and the scalene muscles. Knowledge of these structural variations is critical for clinicians involved in the diagnosis and management of brachial plexus injuries. This case report describes a rare case involving multiple anomalies in the root formation of the brachial plexus, observed in a Japanese male cadaver. In this case, both brachial plexuses were formed from C4 to T1, and the both subclavian arteries passed betw

The middle interscalene block: cadaver study and clinical assessment.

BACKGROUND AND OBJECTIVES: A variety of brachial plexus block techniques via the interscalene approach have been proposed. We describe here a new middle interscalene perivascular approach to the brachial plexus. To verify its effectiveness, we studied 719 patients scheduled for shoulder arthroscopy. Furthermore, to verify the accuracy of the proposed bony landmarks to use in the case of inability to palpate the subclavian artery pulse, we simulated the block on 10 cadavers. METHODS: The aim of our technique is to cannulate the neurovascular bundle by inserting a 35-mm needle lateral to the su

Single-Entry Selective Trunk Block (S-SeTB): An Innovative Approach to Complete Upper Extremity Anesthesia in Weapon-Wounded Patients in Low-Resource Settings.

BACKGROUND: Regional anesthesia for upper extremity surgery in weapon-wounded patients is challenging, particularly in low-resource settings. Existing techniques often require multiple needle entries and ultrasound probe repositioning, increasing complexity and risk. There is a need for a simpler, effective technique providing complete anesthesia from the shoulder to the hand. METHODS: We developed a novel ultrasound-guided regional anesthesia technique-the Single-entry Selective Trunk Block (S-SeTB)-which targets the superior trunk (ST), middle trunk (MT), and C8 ventral ramus (C8VR) with lo

A case report of complex auricular neuralgia treated with the great auricular nerve and facet blocks.

BACKGROUND: The great auricular nerve is a cutaneous branch of the cervical plexus originating from the C2 and C3 spinal nerves. It innervates the skin over the external ear, the angle of the mandible and the parotid gland. It communicates with the ansa cervicalis. Great auricular neuralgia is rarely diagnosed in clinical practice and can be refractory. We present a new approach using ultrasound-guided nerve blocks. CASE: We present a case of a 41-year-old female with paroxysmal ear pain accompanied by dysautonomia, tingling in the tongue, dysphagia, dysarthria and abdominal symptoms. No sign

Ultrasound-guided supraclavicular brachial plexus block in patient with halo device.

Ultrasound guided regional blocks are on the rise, many institutes are training their staff to master this technique of regional anesthesia. Regional anesthesia in case of an emergency surgery or elective surgery can be the best choice. The case described here is an example - patient with a halo fixation device after motor vehicle accident scheduled for surgery of the extremity. The main aim of management of this case is to achieve a safe anesthesia with minimal interference of the cervical fixation. Supraclavicular brachial plexus block is a good choice for surgeries of the arm and hand and u

Comparison of anesthesia efficacy between subclavius muscle plane block and interscalene brachial plexus block for open reduction and internal fixation of clavicle fractures: study protocol for a randomized non-inferiority clinical trial.

BACKGROUND: Interscalene brachial plexus block (ISBPB) combined with supraclavicular nerve block or superficial cervical plexus block has been reported to be a reliable, efficacious anesthesia technique for clavicle fracture surgery. However, several serious adverse events impeded the development of this technique. Herein, we propose a novel nerve block technique-the subclavius muscle plane block (SMPB) combined with supraclavicular nerve block-that selectively targets the sensory nerves innervating the surgical region of the clavicle. We hypothesize that SMPB may achieve non-inferior anesthes

Ultrasound-guided Interscalene Block as a Safer Alternative to General Anesthesia in a Patient with Ankylosing Spondylitis Undergoing Proximal Humerus Fracture Fixation.

Ankylosing spondylitis (AS) presents significant anesthetic challenges due to restricted cervical spine mobility and an anticipated difficult airway. General anesthesia may increase perioperative risk, particularly in emergency settings. We report the anesthetic considerations of a 58-year-old male of short stature with suspected AS and severely restricted neck movements, posted for emergency fixation of a proximal humerus fracture. Considering the anticipated difficult airway, ultrasound-guided interscalene brachial plexus block was administered as the sole anesthetic technique. Ultrasound-gu

Anesthesia for tracheoplasty in a patient with tracheal stenosis: a case report.

BACKGROUND: Tracheal stenosis is a serious complication often following prolonged endotracheal intubation. Surgical tracheoplasty is definitive treatment but poses significant airway management challenges during anesthesia, especially in patients with severe stenosis and comorbidities like coronary artery disease. CASE PRESENTATION: We present a 28-year-old male patient scheduled for tracheoplasty due to severe upper tracheal stenosis (narrowest diameter ~ 4 mm) following cardiac arrest and prolonged intubation. He also had pre-existing coronary artery disease (moderate stenosis). Traditional

[Blockade of the suprascapular nerve in shoulder pain].

The pathological course of the most common diseases of the shoulder includes pain as a main symptom. Besides other methods of treatment the infiltration of the suprascapular nerve can improve the therapeutic methods in many cases. The technique of Gordh, used as an adjuvant procedure, is shown. Since we used this technique of local anaesthesia, in 32 of 42 cases a temporary release from pain could be achieved. In 6 more patients it led to an significant reduction of pain. In conclusion this method of local anaesthesia can be recommended as an additional treatment of patients with pain of the s

Postoperative analgesia after major shoulder surgery with interscalene brachial plexus blockade: etidocaine versus bupivacaine.

Postoperative pain is commonly treated with significant doses of narcotics, occasionally resulting in side effects including nausea, pruritus, and respiratory depression. One potential advantage of regional anesthesia is profound postoperative analgesia that reduces exposure to potent narcotics. To evaluate the efficacy of two long-acting local anesthetics, bupivacaine and etidocaine, in providing pain relief after major shoulder surgery, we randomized 20 patients to receive either bupivacaine or etidocaine for brachial plexus block as the primary anesthetic for shoulder surgery. The presence of transverse cervical and dorsal scapular arteries at three ultrasound probe positions commonly used in supraclavicular brachial plexus blockade.

BACKGROUND: Ultrasound-guided supraclavicular brachial plexus block carries a risk for puncture of vascular structures. In this study, we determined the frequency with which the transverse cervical artery (TCA) and the dorsal scapular artery (DSA) are detected by ultrasound evaluation at 3 probe positions during supraclavicular block. METHODS: Ultrasound examinations of the supraclavicular region were performed in 53 healthy adult volunteers. Ultrasound images of the supraclavicular region were acquired at 3 probe positions: position A (the brachial plexus and the subclavian artery both lying

Unexpected Anatomical Variation While Performing an Ultrasound-Guided Interscalene Block for Shoulder Surgery.

Anatomical variations of the brachial plexus are very common. Knowledge of the possible anatomical variations encountered in ultrasound imaging is crucial for the safe and effective practice of regional anesthesia. The interscalene block (ISB) targets the brachial plexus roots in the interscalene groove, between the anterior and middle scalene muscles (MSM), at the level of the sixth cervical vertebra. Blockade of the brachial plexus roots anesthetizes the shoulder region, making the ISB one of the preferred regional anesthesia options in shoulder surgeries. Abnormalities of the muscular struc
