We discuss a case of an asymptomatic cervical rib presenting as a hard neck lump in a 56-year-old woman. Cervical rib is a common condition associated with thoracic outlet syndrome. The diagnosis, investigation and management of this condition is an important differential diagnosis that an ENT surgeon should know in a patient presenting with a neck mass.

Results of surgical treatment for thoracic outlet syndrome in 7 patients are reviewed. The symptom has been caused by cervical rib in 5 cases, in 1 case by the first rib anomaly and in 1 case by disturbed attachment of m.scalenus anterior. A case of misdiagnosed cervical rib is described in details; the patient has been operated on peripherally and serious iatrogenic complications followed. Ineffectiveness of pharmacotherapy and physiotherapy is pointed out. Surgical treatment resulted in remission of symptoms in 6 cases; in case of patient with iatrogenic complications marked improvement has

Reported here is a 13-year-old girl suffering from pain in her right lower neck region and the shoulder for the preceding 2-3 years. She was diagnosed to have right thoracic outlet syndrome due to bifid ribs--caused by the bony fusion of the variant cervical rib. She was operated via supraclavicular approach, whereby the variant cervical rib and the fibrous bands were excised. She was found to be quite improved 1 month after surgery. Thoracic outlet syndrome, a rare disease of childhood, is discussed in the light of the relevant literature.

Cervical ribs may cause thoracic outlet syndrome in adults, but symptoms are poorly described in children. In our series, 88.8% of the 322 children were asymptomatic. The most common symptoms were neck mass and pain. Useful diagnostic tools were cervical spine and chest radiographs. Differential diagnosis of a supraclavicular mass includes cervical ribs.

Thoracic outlet syndrome (TOS) is a broad term for compression of the neurovascular structures in the area of the 1. rib and the clavicle. The cause can be either fibrous bands, cervical ribs, anomalous muscles or posttraumatic changes as well as tumors. Symptoms depend on the affected structure, in most cases (up to 97% of TOS patients) neurologic symptoms are present. In case of an arterial compression, for example due to a cervical rib like in our case, embolism of the arm and finger arteries can occur. For mild or moderate symptoms a conservative approach with physiotherapy can be helpful.

Careful anatomic studies during 1,120 operations for thoracic outlet syndrome indicate that most TOS patients have anomalous fibromuscular bands near the brachial plexus and subclavian vessels that predispose them to neurovascular symptoms in the shoulder and upper extremity. Nine different types of these "soft tissue" anomalies not visible on roentgenograms have been identified during these operations. The anomalies often are multiple, and may or may not be associated with bone abnormalities as well. These anomalies are clearly associated with the symptoms of thoracic outlet syndrome, particu

Thoracic outlet syndrome (TOS) refers to an abnormal compression of the subclavian vessels and the brachial plexus at the base of the neck or thoracic outlet. The authors described a novel case of a 66-year-old male with arterial TOS and unilateral upper rib dysplasia following cervical spine disease. Owing to the comorbidity of cervical spondylotic amyotrophy and a history of sloping shoulder due to rib dysplasia, TOS occurred, which subsequently led to subclavian arterial thrombosis and eventually resulted in the gangrene of the hand. Thrombectomy and surgical resection of the first and seco

The thoracic outlet syndromes encompass the diverse clinical entities affecting the branchial plexus or subclavian artery including cervical ribs or bands. Thoracic outlet syndrome are often difficult to diagnose on existing clinical and electrophysiological criteria and new diagnostic methods are necessary. This study reports our experience with magnetic resonance imaging (MRI) of the brachial plexus in 20 patients with suspected thoracic outlet syndrome. The distribution of pain and sensory disturbance varied widely, weakness and wasting usually affected C8/T1 innervated muscles, and electro

The authors present immediate and long-term results of treatment of 117 patients with superior thoracic outlet syndrome (STOS). There were different reasons for compression of neu- rovascular fascicle in outlet of the thorax. The costaclavicular syndrome was a reason in 48 patients, additional cervical ribs had 36 patients. Skalenus syndrome was noted in 26 cases, rudimentary cervical ribs or hypertrophy of cervical vertebrae C7 had 7 patients. Raynaud's syndrome took place in 19 cases. The required volume of diagnostic procedures and surgical treatment of STOS were determined according to the

Arterial thoracic outlet syndrome comprises a collection of symptoms due to compression of the neurovascular structures of the thoracic outlet. Cervical ribs are rare congenital abnormalities that are a cause of thoracic outlet syndrome, leading to upper limb complications depending on the compressed structure. Management tends to be surgical in the form of rib resection. We report a case of arterial thoracic outlet syndrome secondary to a cervical rib in a 45-year-old male who presented with left-sided cerebellar stroke caused by subclavian artery thrombosis. Medical management in the form of

Cervical rib is a rare anatomical anomaly with an incidence of 0.2%-1% and is an important cause of thoracic outlet syndrome. We present a case series of five female patients with a mean age of 20.6 (15-26) years, symptoms present were neck pain, neck mass, tingling sensation and weakness in the affected side. Symptoms develop in adolescence probably due to sagging of the shoulders and a disproportion between chest and neck growth at this age. X-rays of cervical spine was a common mode of diagnosis and showed bilateral cervical rib in three cases and unilateral in two cases. They were managed

This case report discusses the incidental discovery of a cervical rib in a 53-year-old woman presenting with acute cholecystitis. While cervical ribs are rare, their identification holds clinical significance due to their potential implications for vascular compression or thoracic outlet syndrome. Despite the patient's primary complaint of cholecystitis, a chest X-ray incidentally revealed the cervical rib. This finding underscores the importance of a thorough radiographic examination. The report discusses similar cases, emphasizing varying clinical presentations and associated vascular compli

This is a description of a great variety of abnormal compressions of nerve, arterial, and rather seldom venous structures at the base of the neck or thoracic outlet, such as supernumerary cervical rib. abnormal ligaments, musculus scalenus anterior hypertrophy and positional alterations, interfering with the normal inter-reactions between first rib and structures overlying it. With a special reference to the hypothetical mechanism of the symptomatology a wide range of denominations of this particular syndrome have been adopted, namely: scalenus anticus syndrome, cervical rib syndrome, costocla

The costoclavicular compression syndrome and the cervical rib syndrome can cause arterial complications of the subclavian artery: mural thrombosis, occlusion and aneurysma. These complications can result in peripheral emboli or cerebrovascular insufficiency. Such sequelae can be avoided by timely surgical treatment of these compression syndromes. The therapy of choice is transaxillary rib resection. The same axillary approach can also be used for directly reconstructing the subclavian artery.

In 225 patients requiring reoperation for recurrent thoracic outlet syndrome, "pseudorecurrences" were noted in 43 patients not relieved of symptoms after the initial operation. Such recurrences were associated with technical errors at the initial operation including resection of the second rib instead of the first, resection of the first with a cervical rib left in place, or resection of a cervical rib with an abnormal first rib left. True recurrences occurred in 182 patients, 154 of whom had a substantial piece of rib remaining from the initial procedure. Indications for reoperation included

We present 4 patients, 4 months to 10 years of age, with thoracic outlet syndrome. All were referred to the brachial plexus clinic. Three patients were diagnosed with vascular thoracic outlet syndrome after clinical evaluation and diagnostic imaging. Three had a cervical rib and 1 had an anomalous first rib. All patients were treated surgically through a supraclavicular approach and had resolution of the symptoms. No postoperative complications were noted.

UNLABELLED: Neurovascular compression syndrome of the upper thoracic outlet is a collective term encompassing various entities: cervical ribs, structural variations of the first rib, costoclavicular syndrome, fibromuscular anomalies of anterior and middle scalene muscles, pectoralis minor syndrome and various pathologies of subclavian vessels. Their combination is based on the common substrate subject to compression (neurovascular bundle passing through narrow cervicoaxillary canal).
OBJECTIVE: To analyze research of etiology, pathogenesis and treatment of neurovascular compression syndrome o
