Predictors of self-reported psychopathology in children abused repeatedly by a parent.

OBJECTIVE: This study was designed to test the influence of gender, type of abuse, and other factors on the development of psychopathology in abused children. METHOD: Forty-one children who had been abused repeatedly either physically or sexually by a parent or parent figure were given the revised Diagnostic Interview for Children and Adolescents. RESULTS: Logistic regression analyses showed: conduct disorder was predicted by male gender and increasing age regardless of type of abuse; post-traumatic stress disorder was predicted by number of stressors other than abuse and not by sexual abuse

Attached to a diagnosis: the quandary of social deficits and reactive attachment disorder.

: Alex is a 9-year-old boy brought to you, his primary care provider, for a "fifth opinion." You have cared for Alex since he was adopted from a Romanian orphanage at 3 years of age. He has been physically healthy with normal growth parameters and no evidence of fetal alcohol syndrome. Alex has long-standing history of social difficulties, impulsivity, lying, controlling, manipulative behaviors, violent outbursts at home with subsequent lack of remorse, and excessive chatter. You referred Alex to an interdisciplinary child development clinic 2 years ago, where he was diagnosed with reactive at

[Psychosomatic aspects of adolescence].

During adolescence physical, psychic and social changes occure more rapidly and to a greater extent than during any other period of live. The marked changes concern not only the outern appearence, but also its intrapsychic representations, the body image. The complex integrative processes towards the identity as man or woman cannot be mastered without a certain emotional and physical instability. This is particularly the case if, in addition, nearly infantile disturbances in the socialisation or conflicts specific for this life period are, or have been, present. They can in the sense of a disp

Sexual abuse: somatic and emotional reactions.

A chart review and telephone interview of 72 sexual abuse victims was conducted to determine if children and adolescents who are victims of sexual abuse suffer from symptoms similar to the "rape trauma syndrome," which has been reported in adults. Symptoms similar to the rape trauma syndrome were found in 48 of the 72 abused children and only 26 of the matched control group, p less than .01. Common somatic complaints in the sexual abuse patients included dysuria, vaginal discharge and chronic abdominal pain. Some of the emotional and behavioral problems noted during the follow-up period among

Parental death or divorce and the school adjustment of young children.

This study examined the effects of two potentially crisis-producing experiences, parental divorce or death, on the school adjustment of young children. Children with such "crisis" histories were found to show greater overall school maladaptation than children without such histories. Children of divorce had significantly more acting-out problems than noncrisis controls or death children and those with histories of parental death more serious shy-anxious problems than the other groups. These effects were stable across (a) independent year samples, (b) referred and "normal" nonreferred groups, an

Associations Between Child Maltreatment, Inflammation, and Comorbid Metabolic Syndrome to Depressed Mood in a Multiethnic Urban Population: The HELIUS Study.

BACKGROUND: Child maltreatment is a common negative experience and has potential long-lasting adverse consequences for mental and physical health, including increased risk for major depressive disorder (MDD) and metabolic syndrome. In addition, child maltreatment may increase the risk for comorbid physical health conditions to psychiatric conditions, with inflammation as an important mediator linking child maltreatment to poor adult health. However, it remains unresolved whether experiencing child maltreatment increases the risk for the development of comorbid metabolic syndrome to MDD. Theref

Radial nerve palsy owing to localized hypertrophic neuropathy (intraneural perineurioma) in early childhood.

Localized hypertrophic neuropathy, also termed intraneural perineurioma, is a rare disorder of unknown etiology that produces a slowly progressive painless focal lesion of a peripheral nerve. It is characterized histologically by concentric whorls ("onion bulbs") of epithelial membrane antigen-reactive, S-100 protein-negative perineurial cells surrounding nerve fibers. We report a radial nerve palsy in a child aged 2 years in whom the diagnosis of localized hypertrophic neuropathy was made by biopsy. Resection of the affected nerve segment and sural nerve grafting produced no useful recovery a

Clinical features and subjective/physiological responses to emotional stimuli in the presence of emotion dysregulation in attention-deficit hyperactivity disorder.

INTRODUCTION: Emotion dysregulation (ED) has long been recognized in clinical descriptions of attention-deficit hyperactivity disorder (ADHD), but a renewed interest in ED has advanced research on the overlap between the two entities. Autonomic reactivity (AR) is a neurobiological correlate of emotion regulation; however, the association between ADHD and AR remains unclear. Our aim was to explore the clinical differences, AR, and subjective emotional responses to visual emotional stimuli in ADHD children with and without ED. METHOD: School-aged ADHD children with (n = 28) and without (n = 20)

Sex differences in children's response to parental divorce: 1. Research methodology and postdivorce family forms.

This two-part review documents inconsistencies in the empirical basis for the hypothesis that boys are more negatively affected than are girls by parental divorce. An attempt is made to move beyond a global hypothesis to one specifying circumstances for the pattern. Part 1 asks whether methodological adequacy or postdivorce family forms in the studies help explain the discrepancy across studies that examine sex differences in children's response to parental divorce. Part 2, reviewing other possible sources for the discrepancy, will appear in a forthcoming issue of this Journal.

Intestinal pseudo-obstruction and acute pandysautonomia associated with Epstein-Barr virus infection.

We report the association of neurological and intestinal disorders with the reactivation of Epstein-Barr virus (EBV) in a child. This previously healthy 13-yr-old boy presented with pharyngitis and acute abdominal ileus. Laparotomy excluded a mechanical obstruction. Postoperatively, he suffered from prolonged intestinal obstruction, pandysautonomia, and encephalomyelitis. Histological examination of the appendix and a rectal biopsy taken 3 months after the onset showed an absence of ganglion cells (appendix) and hypoganglionosis (rectum), with a mononucleate inflammatory infiltrate in close co

Intracranial Hemorrhage From Cerebral Venous Thrombosis With Hypereosinophilia and Positive Dengue Serology in a Child: A Rare Case and Challenges in Management.

Hypereosinophilia is a rare condition, defined as a persistent elevation of absolute eosinophil count greater than 1.5x109/L and/or tissue eosinophilia. This condition can be caused by numerous different etiologies, both hematological (clonal) and non-hematological (reactive). Reactive hypereosinophilia encompasses all disorders, including infections. Patients with hypereosinophilia may experience a spectrum of clinical consequences due to multiple organ damage, including neurologic and thrombotic complications, associated with organ dysfunction and potentially life-threatening sequelae. [Psychopathologic structure of reactive depression in young children].

Thirty-four children, aged 1 to 3.5 years, with a reactive depressive state were examined. Using the ontogenetic approach, the authors analyze affective as well as somatovegetative, psychomotor and behavioural disorders in the structure of the abnormal state. The authors have identified stages of disease development, as well as productive and negative dysontogenetic disturbances secondary to protracted reactive depressions in young children.

Disturbances of attachment and parental psychopathology in early childhood.

As the field of attachment has expanded over the past four decades, the perturbations in the relational context which give rise to disturbances of attachment are increasingly, though by no means conclusively, understood. In Part I, this article reviews the historical and current state of research regarding normative attachment classification, the diagnosis of Reactive Attachment Disorder, and the proposed categories of Secure Base Distortions and Disrupted Attachment Disorder. In Part II, the article explores the role of parental psychopathology and the manner in which disturbed caregiver self

An adolescent with a mediastinal mass, diagnosed with Graves disease and thymic hyperplasia.

Graves disease is an autoimmune disorder characterized by thyroid enlargement and hyperthyroidism. Thyrotropin receptor (TSHR) autoantibodies bind the TSHR on the membrane of thyroid follicular cells and stimulate cell proliferation and thyroid-hormone synthesis. TSHR has also been identified in extrathyroidal organs, including the human thymus. Thus far, radiologically detectable thymic enlargement has only been reported in adults with Graves disease. We present here the case of a child with Graves disease and significant thymic hyperplasia. L. K. was a 15-year-old girl evaluated for cough an

Acute Periflexural Purpura as a Revealing Sign of Parvovirus B19 Infection in a Child.

Purpura is a frequent dermatologic manifestation in children, arising from a broad spectrum of conditions ranging from benign viral infections to systemic disorders. Among viral causes, parvovirus B19 has been reported to occasionally induce purpuric eruptions in children, sometimes mimicking more severe hematologic or inflammatory disorders. Parvovirus B19 is classically associated with erythema infectiosum, but it can occasionally manifest with atypical cutaneous eruptions, including purpuric lesions in unusual distributions. We report the case of an 11-year-old child with no prior medical h

Prenatal exposure to air pollution and childhood internalizing problems: roles of shyness and anterior cingulate cortex activity.

BACKGROUND: Prenatal exposure to air pollution increases the risk for psychiatric disorders characterized by internalizing problems. In this study, we examined the roles of shyness and anterior cingulate cortex (ACC) activity in the association between prenatal exposure to polycyclic aromatic hydrocarbons (PAH) and children's internalizing problems at 7-9 years old. METHODS: Participants include 53 children (31 girls, 22 boys). Personal air monitoring was conducted over 48 continuous hours during the third trimester of pregnancy to measure 8 PAHs. Mothers reported children's shyness (Emotiona

Potentially traumatic events in foster youth, and association with DSM-5 trauma- and stressor related symptoms.

BACKGROUND: In DSM 5, three disorders are related to trauma and/or maltreatment: Post-traumatic Stress Disorder (PTSD), Reactive Attachment Disorder (RAD) and Disinhibited Social Engagement Disorder (DSED) but how these disorders relate to each other and to traumatic events is unknown. OBJECTIVE: We examined 1. Prevalence of Potentially Traumatic Events (PTEs) and poly-victimization for youths in foster care. 2. Associations between single/multiple PTEs and PTSD, DSED, and the two symptom-clusters that constitute RAD: Failure to seek/accept comfort (RAD A), and Low social-emotional responsive
