Teenage parenthood among child welfare clients: a Swedish national cohort study of prevalence and odds.

To assess prevalence and odds for teenage parenthood among former child welfare clients, we used national register data for all children born in Sweden 1972-1983 (n=1,178,207), including 49,582 former child welfare clients with varying intervention experiences. Logistic regression models, adjusted for demographic, socio-economic and familial background factors, were used to estimate odds ratios. Among youth who received interventions in adolescence, 16-19% of the girls and 5-6% of the boys became teenage parents, compared to 3% for girls and 0.7% for boys without child welfare experiences. Workarounds to Mandated Data Practices: Rethinking Digital Infrastructure in Child Welfare and Homeless-Serving Organizations.

Over the past decade, much of the infrastructure used for social service delivery in Canada has been digitized. Drawing on institutional ethnographic research practices, we undertook observational research, facilitated focus groups with twenty five key informants, and interviewed 121 leaders and workers in the child welfare and youth homelessness sectors to learn how data practices in these sectors are shaped by government-mandated infrastructure and tools associated with the adoption of digital information management systems in the public sphere. Despite promises that these new digital techno

Risk and Protective Factors for Alcohol, Marijuana, and Cocaine Use Among Child Welfare-Involved Youth.

Youth involved in child welfare services (CWS) are at elevated risk for substance use. CWS involvement may provide an opportunity for intervention to prevent subsequent use; however, little is known about mitigating substance use risk in this population. Using data from the second National Survey of Child and Adolescent Well-Being (NSCAW II), the present study examined individual, psychological, and contextual risk factors (e.g., prior substance use, depression, posttraumatic stress, maltreatment experiences) and protective factors (e.g., caregiver monitoring, peer relationships) following CWS

The linkages among childhood maltreatment, adolescent mental health, and self-compassion in child welfare adolescents.

OBJECTIVES: Childhood maltreatment is a robust risk factor for poor physical and mental health. Child welfare youths represent a high-risk group, given the greater likelihood of severe or multiple types of maltreatment. This study examined the relationship between childhood maltreatment and self-compassion - a concept of positive acceptance of self. While not applied previously to a child welfare sample, self-compassion may be of value in understanding impairment among maltreatment victims. This may be most pertinent in adolescence and young adulthood, when self-identity is a focal development

Characteristics of Sexually Abused Children and Their Nonoffending Mothers Followed by Child Welfare Services: The Role of a Maternal History of Child Sexual Abuse.

Considering the importance of mother's support in the adaptation of a sexually abused child, it is relevant to determine if the mothers and children involved in an intergenerational cycle of child sexual victimization differ from dyads in which only the child has been abused. The purpose of this study was to compare mother-child dyads with sexually abused children according to whether the mother had herself been victim of child sexual abuse. The sample included 87 dyads with sexually abused children aged 3-18 years old and their mothers (44 reporting maternal and child abuse), followed by soci

Validation of a Small Language Model for DSM-5 Substance Category Classification in Child Welfare Records.

BACKGROUND: Recent studies have demonstrated that large language models (LLMs) can perform binary classification tasks on child welfare narratives, such as detecting the presence or absence of constructs such as substance-related problems, domestic violence, and firearms involvement. However, whether smaller locally deployable models can move beyond binary detection to classify specific substance types from these narratives remains untested. OBJECTIVE: Validate a locally hosted LLM classifier for identifying specific substance types aligned with DSM-5 categories in child welfare investigation

Variation in decision-making on out-of-home placements in child and family welfare services: A new approach.

BACKGROUND: Variation across local administrative units (municipalities) is critical in child welfare, as inconsistencies in out-of-home placement decisions can create inequalities in children's access to support. Given the impact of placement decisions on a child's well-being, and long-term outcomes, ensuring fairness and consistency is essential in promoting equal opportunities for all children. OBJECTIVE AND SETTING: This study explores municipal variation in out-of-home placement decisions following referrals in Danish child and family welfare using 52.649 out-of-sample predicted risk sco

Prevention Adaptation of an Evidence-Based Treatment for Parents Involved With Child Welfare Who Use Substances.

Background: Parental substance use, especially opioid misuse and/or methamphetamine use, is a key driver for recent increases in family involvement with child welfare and foster care placements in the United States. There is an urgent need for programs that prevent parental substance use disorders, yet few prevention programs exist that target parents' unique needs and strengths. Adapting evidence-based treatment approaches for prevention might be an efficient, effective way to address this gap. The current study informed the rigorous adaptation of an evidence-based treatment that supports fam

Profiles of Maltreatment in the Child Welfare System: Predicting Mental Health Outcomes and Examining Age as a Moderator.

This longitudinal study explored the unique profiles of maltreatment among youth in the child welfare system and examined their relation to mental health outcomes over time. We additionally examined the moderating role of age. Participants included 316 youth in the foster care system (age range: 6-13 years). Data were collected through the Department of Children and Family Services upon youths' entry into foster care, then quarterly for the duration of care. A latent profile analysis identified five profiles of maltreatment, categorized as primary neglect (PN), neglect and physical abuse (PA),

Improving outcomes for babies and toddlers in child welfare: A model for infant mental health intervention and collaboration.

Children under three comprise a sizable and growing proportion of foster care placements. Very young children who enter the child welfare system experience disruptions of critical attachments that are essential to this formative stage of brain development, as well as other traumatic events, leaving them at great risk for lifelong impairments. To reverse these concerning outcomes, babies who have been removed from their homes require intensive, relationship-based interventions that promote secure attachment to a primary caregiver and holistic attention the child's developmental needs. Implementation of AF-CBT by community practitioners serving child welfare and mental health: a randomized trial.

The Partnerships for Families project is a randomized clinical trial designed to evaluate the implementation of Alternatives for Families: A Cognitive-Behavioral Therapy (AF-CBT), an evidence-based treatment for family conflict, coercion, and aggression, including child physical abuse. To evaluate the effectiveness of a training program in this model, 182 community practitioners from 10 agencies were randomized to receive AF-CBT training (n = 90) using a learning community model (workshops, consultation visits) or Training as Usual (TAU; n = 92) which provided trainings per agency routine. The physical, developmental, and mental health needs of young children in child welfare by initial placement type.

The American Academy of Pediatrics recommends comprehensive assessments for children entering foster care. These children may be placed with biological parents, kin, or in nonrelative foster care. It is not known whether health-related needs differ by placement. Chart abstractions were conducted of child welfare and medical records of 1542 children, ages 3 months to 5 years 11 months, admitted to San Diego's sole emergency shelter/receiving facility from April 1, 1998, through June 30, 1999, for investigation of alleged maltreatment. Children were discharged to three placement types: biologica

Reducing Family Violence Through Child Welfare Intervention: A Propensity Score-Matched Study of Fathers for Change.

Intimate partner violence (IPV) is prevalent, costly, and detrimental to children's health and development. It often co-occurs with child abuse and neglect. Most children referred to child protective services (CPS) have witnessed IPV and are at increased risk for subsequent exposure, as well as repeat maltreatment. For CPS referred children, there is often a missed opportunity to interrupt family violence and prevent future occurrences. Fathers for Change (F4C) is a family level intervention designed to reduce IPV by improving emotion regulation and reflective functioning in fathers. To date,

Ophthalmic screening of 38,000 children, age 1 to 2 1/2 years, in child welfare clinics.

Screening of 38,000 infants, age 1 to 2 1/2 years, showed a prevalence of 1.3% of strabismus and 0.5% of strabismic amblyopia. Esotropia was more than three times as frequent as exotropia. Approximately half of the cases with esotropia were amblyopic. Eighty-one percent of the cases with exotropia were intermittent, and in 29% the V-pattern was found. Significant ametropia was found in over 50% of the cases with strabismus. Although hypermetropia was the most frequent refraction in children with esotropia, myopia was a frequent finding in both esotropia and exotropia. Anisometroia was particul

Informal Support Networks of Parents Involved With the Child Welfare System: Needs and Mental Health Symptoms.

Studies show the protective power of informal support networks for parents, however, most research in this area is not specific to the child welfare context. With a sample of parents with children involved with the U.S. child welfare system due to substantiated child maltreatment (N = 118), latent profile analyses revealed three distinct profiles of parents' informal support networks including perceived support, received support, and network demands. The profiles were associated with differences in sociodemographic risks, ACEs, and symptoms of depression and stress. Parents with four or more A
