Molecular assessment of erythroid lineage chimerism following nonmyeloablative allogeneic stem cell transplantation.

OBJECTIVE: Nonmyeloablative conditioning regimens for allogeneic stem cell transplantation are now commonly used in the treatment of patients with hematologic malignancies. Since this treatment often results in the establishment of mixed hematopoietic chimerism, this approach may also prove to be useful in the treatment of nonmalignant disorders, such as sickle cell disease and thalassemia major. To apply this approach to these diseases, it will be necessary to determine the levels of donor erythropoiesis required to correct hemolysis and ameliorate disease symptoms. Current methods for measur

Detection of relapse by sequential monitoring of chimerism in circulating CD34+ cells.

Relapse of the underlying malignant disease represents one of the major causes of treatment failure in patients treated with stem cell transplantation, especially in patients with acute leukemia. Detection of minimal residual disease (MRD) after allogenic stem cell transplantation (SCT) is important to schedule therapeutic intervention, e.g. donor lymphocyte infusion. We asked, whether chimerism analysis in circulating CD34+ cells might be a feasible method to monitor MRD in patients after allogeneic SCT. Eighty-seven patients undergoing allogeneic SCT were prospectively analyzed. CD34+ cells

Chimerism and donor-specific nonreactivity 27 to 29 years after kidney allotransplantation.

Chimerism was demonstrated with immunocytochemical and/or polymerase chain reaction techniques in kidney allografts and in the native skin, lymph nodes, or blood of 5 of 5 patients who received continuously functioning renal transplants from 1 or 2 haplotype HLA mismatched consanguineous donors (4 parents, 1 aunt) 27-29 years ago. In the 4 cases where the kidney donor still was alive to provide stimulator lymphocytes for testing, these provoked no (n = 2) or modest (n = 2) MLR in contrast to vigorous MLR to third party lymphocytes. In all 4 cases, the donor cells failed to generate in vitro cy

Microhaplotype-based chimerism analysis for sensitive monitoring following allogeneic hematopoietic stem cell transplantation.

BACKGROUND: Relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT) remains a leading cause of treatment failure in patients with hematologic malignancies. Short tandem repeat (STR)-based chimerism analysis has limited analytical sensitivity, whereas minimal residual disease (MRD) assays are disease-specific and not universally applicable. We developed a microhaplotype (MH)-based next-generation sequencing assay for sensitive quantification of low-level recipient DNA after allo-HSCT. METHODS: Analytical performance was evaluated using artificial donor-recipient DNA mixtur

Body-wide chimerism and mosaicism are predominant causes of naturally occurring ABO discrepancies.

Routine ABO blood group typing of apparently healthy individuals sporadically uncovers unexplained mixed-field reactions. Such blood group discrepancies can either result from a haematopoiesis-confined or body-wide dispersed chimerism or mosaicism. Taking the distinct clinical consequences of these four different possibilities into account, we explored the responsible cause in nine affected individuals. Genotype analyses revealed that more than three-quarters were chimaeras (two same-sex females, four same-sex males, one sex-mismatched male), while two were mosaics. Short tandem repeat analyse

Twin anemia-polycythemia sequence with blood chimerism in monochorionic dizygotic opposite-sex twins.

We report a case of twin anemia-polycythemia sequence with blood chimerism in monochorionic dizygotic twins born to a 30-year-old woman who conceived via ovulation induction. The neonates developed twin anemia-polycythemia sequence; the female twin had anemia and the male had polycythemia. We detected blood chimerism using fluorescence in situ hybridization (FISH). Twin anemia-polycythemia sequence carries not only perinatal risks, but also genetic and immunological implications due to blood chimerism. Although previous reports have described twin-to-twin transfusion syndrome in monochorionic

Rare case of an oligospermic male with 46,XX/46,XY tetragametic chimerism.

Chimerism, a rare human disorder, is assumed to be the result of an amalgamation of two separate zygotes in a single embryo. Studies have shown that the phenotypic spectrum of chimerism is variable and there is no definite genotype-phenotype correlation in patients with chimerism, therefore a majority of cases might remain undiagnosed. This study aims to investigate the possible mechanism of the chimerism in a 46,XX/46,XY infertile and phenotypically normal male, with 46,XX blood karyotype and normal spermatogenesis. We have used Interphase-FISH analysis to study the CEPX and CEPY regions on b

Combined immunodeficiency evolving into predominant CD4+ lymphopenia caused by somatic chimerism in JAK3.

PURPOSE: Idiopathic CD4 lymphopenia constitutes a heterogeneous group of immunodeficiencies with characteristically low CD4+ T-cell counts with largely unknown genetic etiology. We here sought to determine the underlying molecular cause in an index family with two patients suffering from combined immunodeficiency that evolved into predominant CD4+ lymphopenia. The more severely affected index patient also presented with selective antibody deficiency against bacterial polysaccharide antigens. METHODS: For the genetic analysis, we used combined homozygosity mapping and exome sequencing. Functio

Long-term follow-up of donor chimerism and tolerance after human liver transplantation.

We aimed to quantify peripheral donor chimerism (DC) and to analyze its association with graft and recipient outcome. Forty-two liver transplant recipients and their respective donors were studied, providing a total of 148 posttransplantation serum samples. DC was assessed with real-time quantitative polymerase chain reaction (qPCR) to detect polymorphic markers. DC did not decrease with time post-transplantation and was higher in child recipients versus adults and in recipients of deceased donor liver transplants versus recipients of live donor liver transplants. Higher levels of DC were dete

Chimerism studies as an approach for the induction of tolerance to extremity allografts.

Recent advances in the field of transplant immunology and reconstructive surgery have resulted in an increased interest in extremity allograft. Until now, more than 20 hand transplants have been performed in humans. Rejection is well controlled by currently available immunosuppressive drugs. The hand transplant, however, is not a life-supporting organ transplant and these drugs are unlikely to represent the final solution for hand transplantation due to serious adverse effects. The ultimate goal of extremity allograft is the induction of donor-specific immunotolerance. The major strategies for

Differing mechanisms of early and late B cell hyporesponsiveness induced by mixed chimerism.

Mixed hematopoietic chimerism induced via nonmyeloablative bone marrow transplantation (BMT) leads to unresponsiveness of anti-Gal alpha1,3Gal beta1,4G1cNAc (Gal) natural antibody (NAb)-producing cells in alpha1,3-galactosyltransferase deficient (GalT(-/-)) mice. We analyzed the mechanisms of anti-Gal-producing B cell unresponsiveness induced by Gal(+/+) BMT. C57BL/6 (B6) GalT(-/-) mice received 3Gy whole-body irradiation and BMT from B6-CD45 congenic mice. BMT led to marked reductions in serum anti-Gal IgM levels and in the numbers of splenic anti-Gal-producing cells by 2 weeks post-BMT. B ce

Mixed chimerism and tolerance without whole body irradiation in a large animal model.

Mixed hematopoietic chimerism may provide a treatment for patients with nonmalignant hematologic diseases, and may tolerize patients to organ allografts without requiring chronic immunosuppression. However, the toxicity of the usual conditioning regimens has limited the clinical applicability of this approach. These regimens generally include some level of whole body irradiation (WBI), which is thought to facilitate engraftment either by making room for donor hematopoietic stem cells or by providing sufficient host immunosuppression to enable donor cells to engraft. Here, we have established m

Analysis of chimerism following allogeneic bone marrow transplantation by fluorescent-in-situ hybridization.

Twelve patients (9 males and 3 females) with chronic myelogenous leukemia, underwent CD8+ T cell depleted allogeneic bone marrow transplantation with a sex-mismatched donor. To assess chimerism we performed fluorescent in-situ hybridization for the X and Y chromosome at different time points after BMT. Patient median age was 33 years (range, 27-48); median time to transplant was 28 months (range, 5-87). All patients received thiotepa 10 mg/kg; cyclophosphamide 120 mg/kg and 12.0 Gy of fractionated total body irradiation. CD8+ cells were depleted from the normal donor marrow with anti-CD8 murin

Chimerism after whole organ transplantation: its relationship to graft rejection and tolerance induction.

Bone marrow-derived "passenger" leukocytes that normally reside in the interstitial tissues of whole organs migrate into the recipient after transplantation and establish microchimerism. An equally important phenomenon is the reciprocal migration of circulating recipient leukocytes, which repopulate the interstitium of whole organ allografts. This bidirectional exchange and interaction of bone marrow-derived cells after organ transplantation is considered a seminal event in the acceptance of allografts and in the induction of donor-specific tolerance. The apparent dichotomous role of donor cel

Mixed chimerism and permanent specific transplantation tolerance induced by a nonlethal preparative regimen.

The use of allogeneic bone marrow transplantation as a means of inducing donor-specific tolerance across MHC barriers could provide an immunologically specific conditioning regimen for organ transplantation. However, a major limitation to this approach is the toxicity of whole body irradiation as currently used to abrogate host resistance and permit marrow engraftment. The present study describes methodology for abrogating host resistance and permitting marrow engraftment without lethal irradiation. Our preparative protocol involves administration of anti-CD4 and anti-CD8 mAbs in vivo, 300-rad

Hyperactivity of donor B cells after neonatal induction of lymphoid chimerism in mice.

Balb/c mice made chimaeric by neonatal injection of semi-allogeneic (A/J x Balb/c)F1 hybrid spleen cells develop anti-DNA and rheumatoid factor-like antibodies in the context of hypergammaglobulinaemia with marked elevation of IgG1 and IgE serum levels. Chimaeric mice also display increased levels of antibodies to different haptens and to tobacco mosaic virus (TMV). The allotypic marker of the A/J strain is present on anti-DNA and anti-hapten antibodies. In addition, spleen cells of chimaeric mice spontaneously produce high levels of IgG1 and anti-DNA antibodies in vitro and this hyperactivity
