It is reported on 4 patients aged 69 to 79 years suffering from rupture of aberrant subserosal bile ducts. In the first patient the gallbladder did not obtain stones, the common duct stones causing increased pressure in the intrahepatic ducts and rupture were not diagnosed, the supposed "cyst" was excised, followed by suture of the liver tissue. Due to peritonitis relaparotomy had to be done. Since now intrahepatic obstruction was supposed a hepatojejunostomy was performed. The patient died from peritonitis following insufficiency of the anastomosis and bronchopneumonia. The other patients wer

Five patients with choledocholithiasis refractory to standard extraction methods after endoscopic sphincterotomy underwent peroral cholangioscopy with the "mother-daughter" endoscopy system. electrohydraulic lithotripsy of nine stones was performed successfully and without complication, other than minor transient bleeding in one patient. The outer sheath of the "daughter scope" sustained a linear burn in one instance. This method of stone fragmentation is a useful adjunct to achieve ductal clearance in difficult cases.

The results of treatment of 72 patients with ampullary choledocholithiasis (16% of the total number of patients with choledocholithiasis) are presented. In 15 patients, bringing down a concrement was performed, in 3--its retrograde removal via the incision of the common bile duct with subsequent creation of choledochoduodenoanastomosis (CDA), in 24--endoscopic papillosphincterotomy (with subsequent cholecystectomy--in 6, with cholecystectomy and CDA creation--in 18), in 30--cholecystectomy with transduodenal papillosphincteroplasty. After the operation, 5 patients died.

The relative indications for operative common duct exploration (CDE) and endoscopic sphincterotomy (ES) in treating common duct stones are often unclear. This prospective study compared CDE and ES in treating choledocholithiasis after excluding patients with acute cholecystitis, idiopathic pancreatitis, sphincter of Oddi dysfunction and malignant disease. One hundred and two patients had 105 CDE and a further 50 patients had 57 ES. Of the patients having CDE, 76 also had cholecystectomy for gall-bladder (GB) disease while 26 had prior cholecystectomy. With ES, in 16 the GB was present and not

During the period 1982-1988 the author has performed 532 endoscopic papillosphincterotomies. In 288 patients (53.13%) the gall bladder had been previously surgically removed and they suffered from a well-expressed postcholecystectomy syndrome. In 73.6% of the 532 patients there were one or more stones in the main bile duct. In 76 patients a stenosing papillitis was found. The endoscopic papillosphincterotomy was performed mainly in patients over 65 years of age--244 patients (45.86%) suffering from various cardiovascular and other accompanying diseases which increase the risk of operation. The purpose of this study was to evaluate the results of transduodenal papillostomy as a routine procedure in managing choledocholithiasis in treating common bile duct (CBD) stones. From 1973 to 1978, 117 patients underwent transduodenal papillostomy for CBD lithiasis. The operation was carried out in standard manner, and all patients had preoperative telecholangioscopy, cholangiography, and biliary manometry. The mean age of patients was 53.7 years, and women predominated in a ratio of 4.5:1.0. Papillostomy was performed together with cholecystectomy for CBD stones in 111 patients (group 1).

The work is based on the examination of 106 patients, operated in the clinic for acute cholecystitis associated with choledocholithiasis and strictures of Vater's papilla. The operation of choice is a removal of pathologically changed gallbladder, elimination of the cause of cholestasis with restoration of bile passage in the bowel. The technic of choledochoduodenostomy adopted in the clinic is described. The immediate mortality was observed in 2.8%. Choledochoduodenostomy yielded good results in 81.1%, external drainage -- in 70.6%.

Choledocholithiasis occurring 40 years postcholecystectomy surgery is not commonly seen and is one of the longest reported latency period in literature today. Herein, we report a case of a 72 year old lady with previous open cholecystectomy 40 years ago, presented with acute onset right upper quadrant pain. Her bloods result revealed an obstructive jaundice picture with total bilirubin 125 μmol/L. Computed tomography imaging revealed high burden calculi in the common bile duct. The choledocholithiasis was subsequently completely retrieved by endoscopic retrograde cholangiopancreatography proce

BACKGROUND Endogenous bacterial endophthalmitis is caused by a breach of the blood-ocular barrier by pathogens originating from distant infective foci. Here, we report a case of endogenous endophthalmitis due to cholangitis complicated by common bile duct stones, which is a rare source of infection. CASE REPORT A 73-year-old man with type II diabetes mellitus underwent endoscopic choledocholithotripsy 20 years ago and laparoscopic cholecystectomy 18 years ago. He had choledocholith-related cholangitis 6, 5, and 1 years previously and 4 times in the last year and underwent endoscopic choledocho

INTRODUCTION: Choledocholithiasis may be treated following an endoscopic approach or by laparoscopic common bile duct exploration (LCBDE). Stone recurrence following endoscopic management has been extensively investigated. We analyze the risk factors associated with stone recurrence following LCBDE.
METHODS: Patients who underwent LCBDE from February 2004 to July 2016 were examined in an univariate and multivariate analysis to assess the association of stone recurrence with the following variables: gender; age; hepatopathy; dyslipidemia, obesity or diabetes mellitus; previous abdominal surger

Solitary pyogenic liver abscess is usually caused by a metastatic infection through the portal blood flow or through the hepatic arterial blood flow from extra-abdominal pyogenic foci. Besides, it may be the result of local inflammatory diseases, such as cholecystitis, hydatid cyst, haematomas particularly with retained foreign bodies, etc. Suppurative cholangitis usually causes multiple pyogenic liver abscesses. Solitary pyogenic abscess is rarely caused by cholangitis, but practically always by suppurative cholangitis. Giant pyogenic liver abscess due to asymptomatic or mild cholangitis is a

In this case of choledocholithiasis in a patient with previous Billroth-II surgery and cholecystectomy we demonstrate the advantages of a heavily T2-weighted half-Fourier turbo-spin-echo (HASTE) sequence. This technique allows thin-slice snapshot imaging with 1.4 s per slice eliminating motion artifacts and still has the necessary heavy T2-weighting to depict biliary fluid with high contrast. In the presented case endoscopic retrograde cholangiography (ERCP) could not be performed prior to MRI due to technical problems. In a second attempt, ERCP was successful and a common bile duct stone as d

To define the clinical significance of delayed postsphincterotomy hemorrhage, we reviewed 476 consecutive ERCP procedures performed over a three-year period. Of 250 patients who underwent endoscopic sphincterotomy (ES), five (2%) developed postprocedure hemorrhage, two of whom had immediate, self-limited bleeding that resolved after endoscopic injection of epinephrine and did not require transfusion. The other three had delayed hemorrhage characterized by: onset 20-48 hr after the procedure, melena without hematemesis as the index clinical manifestation of bleeding, and atraumatic balloon extr

Three patients with symptomatic intra- and extrahepatic choledocholithiasis who were not good candidates for retrograde endoscopy, surgery, or extracorporeal shock wave lithotripsy (ESWL) were treated successfully with endoscopically guided tunable dye laser lithotripsy via a 12-F transhepatic sheath. There were no complications secondary to the use of the laser. On the basis of this initial experience, transhepatic laser lithotripsy is a technically feasible and safe alternative when choledocholithiasis cannot be managed with retrograde endoscopy, ESWL, or surgery. Its role in the management

A prospective randomized trial was conducted of preoperative endoscopic sphincterotomy and surgery (ES&S) or surgery alone (SA) in 52 patients with cholecystolithiasis and choledocholithiasis that were candidates for elective surgery. After ES&S 65% of patients were stone free. Eighty-eight per cent of patients with SA were stone free after surgery (p less than 0.05). Three patients in each group had residual stones at the completion of the operation. Five of these six had more than 20 common bile duct (CBD) stones. There was one episode of major hemorrhage in a patient in each group and no de

INTRODUCTION: Pediatric cholelithiasis incidence is increasing. Adult guidelines and the recently developed pediatric DUCT criteria aim to identify patients at risk for choledocholithiasis, but pediatric validation is lacking. This study aimed to identify predictive factors for pediatric choledocholithiasis.
METHODS: We conducted a retrospective cohort study of children ≤18 y undergoing cholecystectomy from January 1, 2018 to May 31, 2023 at an academic medical center. Patients with cancer, chronic pancreatitis, or acalculous cholecystitis were excluded. Demographic and clinical data were abs

Retrieval of stone by endoscopic papillotomy, laparoscopic choledochotomy or open choledochotomy is the treatment of choice for choledocholithiasis. Published literature shows that the recurrence rate is 4% to 24% with existing method of treatment. We have treated 8 patients who admitted with recurrent choledocholithiasis in the department of Hepato-Biliary-Pancreatic and Liver Transplant Surgery, Bangabandhu Sheikh Mujib Medical University (BSMMU), Bangladesh in the period of January 2016 to December 2019. None had intrahepatic duct abnormality or stones. All patients underwent either ERCP st

BACKGROUND AND OBJECTIVES: Foreign material in the biliary tree may serve as a nidus for stone formation and would usually present as choledocholithiasis with jaundice or cholangitis. Overall it is a rare occurrence, but there are many anecdotal reports of ingested matter or surgical material such as suture or clips causing biliary stones. Especially interesting are the cases in which there is migration of a metallic clip used in laparoscopic cholecystectomy. Cholecystectomy is such a common operation that although the phenomenon is rare, it is important because it is preventable, and as such
