Circulation and transmission of clones of Vibrio cholerae during cholera outbreaks.

Cholera is still a major public health problem. The underlying bacterial pathogen Vibrio cholerae (V. cholerae) is evolving and some of its mutations have set the stage for outbreaks. After V. cholerae acquired the mobile elements VSP I & II, the El Tor pandemic began and spread across the tropics. The replacement of the O1 serotype encoding genes with the O139 encoding genes triggered an outbreak that swept across the Indian subcontinent. The sxt element generated a third selective sweep and most recently a fourth sweep was associated with the exchange of the El Tor ctx allele for a classical

BACILLI OF THE HOG-CHOLERA GROUP (BACILLUS CHOLERAE SUIS) IN MAN.

1. The organisms isolated by Hirschfeld from febrile cases resembling paratyphoid fever and named Paratyphoid C can be placed in the hog-cholera bacillus group by their agglutination absorption properties though they are not typical culturally. 2. When fed to a pig a febrile disease resulted from which the animal recovered. After injection of hog-cholera virus the organisms fed were found generally, distributed and some of them had lost cultural characters so that they are brought into the class of typical hog-cholera bacilli except for their low virulence for rabbits. 3. While hog-cholera bac

Oral Cholera Vaccines as Transmission-Interrupting Tools for Cholera Control: A Narrative Review.

BACKGROUND: Cholera remains a significant global health challenge, with an estimated 1.3 billion people at risk in endemic regions and recurrent outbreaks increasingly reported in fragile, conflict-affected, and humanitarian settings. Persistent transmission continues despite advances in case management and water, sanitation, and hygiene (WASH) interventions. OBJECTIVE: To synthesize current evidence on oral cholera vaccines (OCVs) as transmission-interrupting tools and to examine their implications for integrated cholera control strategies. METHODS: This narrative review synthesized publish

Impact of oral cholera vaccines in cholera-endemic countries: A mathematical modeling study.

BACKGROUND: Impact evaluation of vaccination programs is necessary for making decisions to introduce oral cholera vaccines (OCVs) in cholera-endemic countries. METHODS: We analyzed data to forecast the future global burden of cholera. We developed a mathematical model of cholera transmission in three countries as examples: Nigeria, Uganda, and Indonesia. After fitting the model, we evaluated the impact of OCVs delivered in four vaccination strategies varying by target age group and frequency of vaccination over the period of 2015-2030. RESULTS: Data suggest that the global annual incidence o

Vibrio cholerae-induced inflammation in the neonatal mouse cholera model.

Vibrio cholerae is the causative agent of the acute diarrheal disease of cholera. Innate immune responses to V. cholerae are not a major cause of cholera pathology, which is characterized by severe, watery diarrhea induced by the action of cholera toxin. Innate responses may, however, contribute to resolution of infection and must be required to initiate adaptive responses after natural infection and oral vaccination. Here we investigated whether a well-established infant mouse model of cholera can be used to observe an innate immune response. We also used a vaccination model in which immunize

[Influence of plant extracts on the activity of cholera toxin of Vibrio cholerae].

AIM: Study the activity of plant extracts against cholera toxin (CT) of Vibrio cholerae O1. MATERIALS AND METHODS: Antitoxic activity of plant extracts was determined by using enzyme immunoassay and CHO-K1 cell culture. RESULTS: 8 water extracts of plants were studied. Extracts of nut, tutsan, milfoil, basil do not have effect on CT activity in EIA or CHO-K1 cell culture. Celandine and rhubarb extracts do not reduce CT immunochemical activity but prevent elongation of CHO-K1 cells. Oak and hop extracts suppress binding in EIA of cholera toxin and GM1 receptors and insignificantly reduce its

Structural characterization of metalloprotease vibriolysin of cholera pathogen Vibrio cholerae.

Vibriolysin is among several zinc metalloproteases produced by Vibrio cholerae. It is involved in the molecular pathogenicity of cholera. Here, we cloned and expressed full-length vibriolysin gene from V. cholerae. Electrophoretic and mass spectrometric data showed that the N-terminal pro-peptide was removed from pro-vibriolysin generating a 45-kDa segment containing the metalloprotease plus the C-terminal domains, and the 35 kDa metalloprotease. The 35 kDa metalloprotease segment of vibriolysin was purified to homogeneity using ion-exchange and gel filtration chromatography. Circular dichrois

Capsaicin, a potential inhibitor of cholera toxin production in Vibrio cholerae.

The use of natural compounds as inhibitory agents for virulence factor production is a new approach to overcome increased antimicrobial resistance in pathogenic bacteria. In this study, we examined whether red chilli (Capsicum annuum) contains any such compound(s) that can repress the cholera toxin (CT) production in Vibrio cholerae. We found that the methanol extract of red chilli could inhibit CT production in recently emerged V. cholerae O1 El Tor variant strains without affecting their viability. Interestingly, capsaicin, a well-studied active component of red chilli, also drastically inhi

The Epidemiology of Cholera in Zanzibar: Implications for the Zanzibar Comprehensive Cholera Elimination Plan.

BACKGROUND: Cholera poses a public health and economic threat to Zanzibar. Detailed epidemiologic analyses are needed to inform a multisectoral cholera elimination plan currently under development. METHODS: We collated passive surveillance data from 1997 to 2017 and calculated the outbreak-specific and cumulative incidence of suspected cholera per shehia (neighborhood). We explored the variability in shehia-specific relative cholera risk and explored the predictive power of targeting intervention at shehias based on historical incidence. Using flexible regression models, we estimated cholera'

An Ogawa cholera outbreak 6 months after the Inaba cholera outbreaks in India, 2006.

BACKGROUND/PURPOSE: Cholera has been reported in the state of Orissa, India during the last decades. An explosive outbreak of diarrhea occurred in Central Cuttack Ward 22 of Orissa (population approximately 10,621), between March 12-23, 2006. This outbreak was investigated by a team from the Regional Medical Research Centre of Bhubaneswar to identify the causative agents and to determine the antimicrobial susceptibility pattern and associated virulent genes. METHODS: Clinical and epidemiological data were collected from 100 hospitalized patients with diarrhea from the Sriram Chandra Bhanja Me

Cholera incidence in a population offered cholera vaccination: comparison of cooperative and uncooperative groups.

From May 1964 to December 1965, a controlled field trial of the effectiveness of cholera and cholera El Tor vaccines was conducted in Negros Occidental, Philippines. Some people did not volunteer for vaccination, and of those who did some received cholera vaccine and others a control (typhoid) vaccine. After analysing he incidence of cholera among these three groups it was found that the morbidity and mortality rates were significantly higher in the unvaccinated group than in either the control vaccine group or the cholera vaccine group. This would indicate that the unvaccinated group is basic

Cost-effectiveness of oral cholera vaccine in a stable refugee population at risk for epidemic cholera and in a population with endemic cholera.

Recent large epidemics of cholera with high incidence and associated mortality among refugees have raised the question of whether oral cholera vaccines should be considered as an additional preventive measure in high-risk populations. The potential impact of oral cholera vaccines on populations prone to seasonal endemic cholera has also been questioned. This article reviews the potential cost-effectiveness of B-subunit, killed whole-cell (BS-WC) oral cholera vaccine in a stable refugee population and in a population with endemic cholera. In the population at risk for endemic cholera, mass vacc

Promotion of Cholera Awareness Among Households of Cholera Patients: A Randomized Controlled Trial of the Cholera-Hospital-Based-Intervention-for-7 Days (CHoBI7) Intervention.

Previous studies have demonstrated that household contacts of cholera patients are highly susceptible to cholera infections for a 7-day period after the presentation of the index patient in the hospital. However, there is no standard of care to prevent cholera transmission in this high-risk population. Furthermore, there is limited information available on awareness of cholera transmission and prevention among cholera patients and their household contacts. To initiate a standard of care for this high-risk population, we developed the Cholera-Hospital-Based-Intervention-for-7-Days (CHoBI7), whi

Household and Individual Risk Factors for Cholera among Cholera Vaccine Recipients in Rural Haiti.

Oral cholera vaccination was used as part of cholera control in Haiti, but the vaccine does not provide complete protection. We conducted secondary data analyses of a vaccine effectiveness study in Haiti to evaluate risk factors for cholera among cholera vaccine recipients. Individuals vaccinated against cholera that presented with acute watery diarrhea and had a stool sample positive for Vibrio cholerae O1 were included as cases. Up to four vaccinated individuals who did not present for treatment of diarrhea were included as controls for each case, and matched by location of residence, enroll

When, how, and where can oral cholera vaccines be used to interrupt cholera outbreaks?

Cholera continues to be a major global health problem, at times causing major and prolonged outbreaks in both endemic and nonendemic settings in developing countries. While improved water quality, sanitation, and hygiene (WASH) will provide the ultimate solution to prevention of this disease burden, this is a far-off goal for most developing countries. Oral cholera vaccines cholera vaccines (OCVs) have been demonstrated to be effective in the control of cholera outbreaks, and constitute useful tools to be used in conjunction with efforts to improve WASH. Two killed OCVs are prequalified by WHO

Cholera in the Time of MINUSTAH: Experiences of Community Members Affected by Cholera in Haiti.

In 2010, Haiti experienced one of the deadliest cholera outbreaks of the 21st century. United Nations (UN) peacekeepers are widely believed to have introduced cholera, and the UN has formally apologized to Haitians and accepted responsibility. The current analysis examines how Haitian community members experienced the epidemic and documents their attitudes around accountability. Using SenseMaker, Haitian research assistants collected micronarratives surrounding 10 UN bases in Haiti. Seventy-seven cholera-focused micronarratives were selected for a qualitative thematic analysis. The five follow

Conditions to eliminate cholera in Mozambique - the pathway for the development of the national cholera plan.

Cholera disproportionately affects the most vulnerable segments of the population, particularly those who have low or no access to basic water, sanitation, and hygiene (WASH). Despite some improvements in WASH conditions, cholera still represents a persistent challenge in Mozambique, where outbreaks occur almost every year, with high case fatality rates, posing a threat to the country's economic development. The Government of Mozambique has started developing a revised National Cholera Plan (NCP), which aligns with "ending cholera-a global roadmap to 2030" launched by the Global Task Force on
