INTRODUCTION: Twenty-five per cent of tuberculosis patients have pleural tuberculosis, which is the third most common form of presentation. Most cases present as an exudative pleural effusion with just few cases reported as chylothorax in the literature. All pleural effusions from confirmed cases, including tuberculous chylothorax, had exudate features.
AIM: To describe a patient with Mycobacterium tuberculosis affecting the lungs and pleura, which laboratory testing demonstrated had features of transudate chylothorax.
PATIENT AND METHODS: A 70-year-old man presented with constitutional symp

A chylothorax, also known as chylous pleural effusion, is an uncommon cause of pleural effusion with a wide differential diagnosis characterized by the accumulation of bacteriostatic chyle in the pleural space. The pleural fluid will have either or both triglycerides >110 mg/dL and the presence of chylomicrons. It may be encountered following a surgical intervention, usually in the chest, or underlying disease process. Management of a chylothorax requires a multidisciplinary approach employing medical therapy and possibly surgical intervention for post-operative patients and patients who have

AIM: to improve the results and current pathogenetic treatment of chylorrhea.
MATERIAL AND METHODS: Thirty seven patients with chylothorax have been treated for the period 2004-2014. In 34 cases traumatic chylothorax developed after surgery and in 3 cases - after intensive care for therapeutic diseases and great veins catheterization. Chylothorax was predominantly diagnosed by X-ray method. Herewith, hydrothorax was established and its nature was defined using laboratory survey. Presence of neutral fat in pleural drainage is the sign of chylothorax. Pathogenetic treatment was often delayed (u

Chylothorax is a disease in which chyle leaks and accumulates in the thoracic cavity. Interstitial pneumonia and pneumomediastinum are common thoracic manifestations of dermatomyositis, but chylothorax complicated with dermatomyositis is not reported. We report a case of dermatomyositis with interstitial pneumonia complicated by chylothorax. A 77-year-old woman was diagnosed as dermatomyositis with Gottron's papules, skin ulcers, anti-MDA5 antibody and rapid progressive interstitial pneumonia. Treatment with betamethasone, tacrolimus and intravenous high-dose cyclophosphamide was initiated, an

Hennekam syndrome was defined as a syndrome characterized by a new autosomal recessive, severe lymphedema in legs, face and genitalia with intestinal lymphangiectasia, various face anomalies and severe mental retardation. A 21 years old male patient was examined due to bilateral pleural effusion. There were edema in both legs and eyelids, swelling in the scrotum and operation scar, broad forehead and face, depressed nasal bridge, epicanthal folds and micrognathia in the physical examination. Chylothorax was diagnosed due to level of pleural triglyceride (650 mg/dL). Lymphatic flow delayed in b

We report the case of an 8-year-old boy who presented to the emergency department of another hospital and was referred to our pediatric intensive care unit with dyspnea and tachypnea of recent onset. The diagnosis of massive chylothorax with mediastinal shift was made on chest radiographs and CT scan. Initial investigations revealed no definite cause for the chylothorax. On later radiography and CT imaging with 3D surface rendering, a marked bone loss of the left ribs, clavicle, and shoulder joint was shown. The diagnosis of Gorham-Stout disease associated with chylothorax was suspected and hi

BACKGROUND: Chylothorax resulting from chest tube injury to the thoracic duct is very rare and underreported.
OBJECTIVE: The purpose of this case report is to exemplify this rare but potentially significant complication of chest tube thoracostomy.
CASE REPORT: An 86-year-old woman presented with sepsis and a massive right pleural effusion; she developed a chylous effusion with the pleural fluid triglyceride level of 158 mg/dL 2 days after a traumatic chest tube insertion. All investigations excluded common causes of non-traumatic chylothorax. The chylothorax improved after fasting and implem

We report a case of follicular lymphoma with chylothorax. A 45-year-old man visited our hospital complaining of shortness of breath and abdominal distension. Chest X-rays showed bilateral pleural effusion, and an abdominal CT scan revealed a large intraperitoneal tumor around the abdominal aorta and pancreas. Bilateral cervical and inguinal lymph nodes were swollen. Biochemical study of the pleural fluid revealed the presence of chylomicrons, and an inguinal lymph node biopsy led to a follicular lymphoma diagnosis. The patient achieved complete remission, with disappearance of pleural effusion

The occurence of chylothorax is uncommon and usually is caused by trauma or neoplastic process of the mediastinum. Primary lymphatic lesions of the lungs are extremly rare. One of them is lymphangiomatosis --diffuse lesions characterized primarily by an increased number of complex anastomosing lymphatic channels in which dilatation is secondary phenomenon. These lesions can involve lungs, mediastinum and pleura. The prognosis for the patients with this disease limited to the thorax is guarded and progressive although some patients have realtively indolent course. We present 2 patients : 18-yea

BACKGROUND: The addition of somatostatin to the conventional treatment of neonatal chylothorax has been described in isolated cases.
OBJECTIVE: To describe the results obtained in a series of five patients with neonatal chylothorax treated with somatostatin.
PATIENTS: Five neonates (gestational age range: 29-39 weeks) diagnosed with chylothorax of various etiologies were included. Chylothorax was congenital in two neonates, secondary to congenital diaphragmatic hernia repair in two neonates and secondary to thrombosis in the superior vein cava in one neonate. All the neonates were started on

Chylothorax is an uncommon condition of dogs and cats defined by the accumulation of chylous effusion within the pleural space. Chylothorax has been experimentally created in dogs and cats by ligation of the cranial vena cava and has been reported to occur spontaneously in dogs with naturally occurring obstruction of the cranial vena cava. In the cat of this report, iatrogenic chylothorax was caused by surgical ligation and transection of the left brachiocephalic vein during a tracheal resection and anastomosis procedure. The chylous effusion resolved with medical management 9 weeks after surg

OBJECTIVE: To report a case of post-cardiac surgery-induced chylothorax treated with octreotide and review the literature on octreotide efficacy.
CASE SUMMARY: A 5-month-old boy with Down syndrome was admitted for atrioventricular canal repair. On admission, he was taking captopril and furosemide. On postoperative day 4, he exhibited signs of chest wheezing and crackles, but was without cough or fever. Chest X-ray revealed a moderate right-sided pleural effusion. Accordingly, a pleural catheter was inserted and drained an average of 7.14 mL/h of chylous fluid that day. Laboratory analysis of

A 76-year-old female was referred to our hospital for examination of milky pleural effusion. We diagnosed her illness as chylothorax because of the high concentration of triglyceride in the effusion. There was neither obstruction nor damage of the thoracic duct. Systemic evaluation disclosed an abdominal mass in the umbilical region. Fasting with intravenous hyperalimentation followed by pleurodesis with minocycline successfully eliminated the effusion. On the other hand, the abdominal mass was diagnosed as mesenteric panniculitis by open biopsy. Since she also had chylous ascites, the tumor c

A 47 year old woman receiving oestrogen replacement therapy for primary amenorrhoea presented with recurrent chylothorax. The clinical and radiological features of her illness were characteristic of pulmonary lymphangio-leiomyomatosis, a rare hormone-dependent disease. The condition was treated with pleurodesis and withdrawal of oestrogen therapy.

We have described the case of a patient with AIDS in whom a chylothorax developed, probably as a result of lymphatic obstruction from KS. This case emphasizes that the loss of proteins, fats, and lymphocytes into chylous pleural effusions can be particularly devastating in AIDS patients, who have profound nutritional problems and a marked depletion in the level of helper T-lymphocytes.

An 11-year-old boy with a capillary/cystic lymphangioma of the posterior mediastinum is described, in whom a symptomatic chylothorax proved to be refractory to surgical management. The addition of low dose mediastinal radiotherapy (2,000 cGy/10 fractions) resulted in prompt and permanent resolution of the chylothorax. Although operation is the primary modality of treatment for these lesions, the addition of radiotherapy in such refractory patients appears to be well tolerated and efficacious and should result in few long-term complications.

Insertion complications of subclavian vein catheter placement are well documented. Thoracic duct injury is a potential problem which has rarely been reported. A patient with duct injury and resultant chylothorax is presented. The management of this problem is discussed with emphasis placed on chyle leakage from both the nutritional and mechanical aspect. The anatomy and function of the thoracic duct as well as the pathophysiology and treatment of chylothorax are detailed.
