We examined the influence of cinacalcet on vascular calcification by measuring the coronary artery calcification score (CACS) in hemodialysis patients who received cinacalcet. The cinacalcet treatment group consisted of eight hemodialysis patients with secondary hyperparathyroidism who received cinacalcet for 7-14 months. The mean CACS change in the treatment group was -0.094/year, showing a decreasing tendency, while that in the control group showed an increasing tendency of 0.034/year. BACKGROUND: Secondary hyperparathyroidism in patients with end stage renal disease on dialysis is associated with bone pain and fractures in addition to cardiovascular morbidity. Cinacalcet is the most commonly used drug to treat such patients, but it has never been compared to surgery. Primary hyperparathyroidism (PHPT) is a common endocrine disease. Although surgical treatment is curative in most cases, there are few alternative therapies for the hypercalcemia caused by PHPT. INTRODUCTION Secondary hyperparathyroidism (SHPT) is a common hormonal disorder associated with chronic kidney disease (CKD). The treatment of SHPT should lead to a reduction in parathormone concentrations by calcimimetics or active vitamin D administration and stabilization of calciumand phosphate metabolism. In the event of failure of conservative treatment, complete or partial parathyroid resection should be considered. OBJECTIVE: Persistent hyperparathyroidism (HPT) with hypercalcemia and hypophosphatemia is common after renal transplantation, resulting in the need for parathyroidectomy. Cinacalcet may be a therapeutic option for these patients. We sought to analyze the efficacy of treatment with cinacalcet for patients with hypercalcemia (Ca > 10.5 mg/dL) secondary to HPT.
PATIENTS AND METHODS: We undertook a prospective study of 29 kidney transplant recipients with HPT who started treatment with 30 mg of cinacalcet. The mean follow-up was 13 months (range, 3-29 months).
