A surgically corrected case of tetralogy of Fallot with anomalous origin of left pulmonary artery from the ascending aorta is presented. The patient was under-developed 3-year-old girl. She weighed only 1,000 grams at birth and had a systolic heart murmur and slight cyanosis. Echocardiography demonstrated the features of tetralogy of Fallot shortly after birth. Chest roentgenogram showed right ventricular enlargement, but vascular marking of lung was almost normal. Catheterization and cineangiography demonstrated the findings of tetralogy of Fallot. This study investigated whether the jet diameter measured by a multigated pulsed Doppler system could be used to assess the severity of valve disease in children with pulmonary (n = 11) or aortic (n = 4) valve stenosis. The results obtained were compared with those obtained at cineangiography and at operation. Persistent stenosis or recoarctation occurs in 16 to 33% of infants who undergo repair of coarctation of the thoracic aorta by use of end-to-end anastomosis. This report describes the results in 12 infants of repair of coarctation of the aorta using a left subclavian artery flap procedure. Cardiac catheterization and cineangiography performed in four patients from 4 to 66 months following repair showed a 10 mm systolic gradient in one patient and no evidence of significant stenosis in any patient. Nine patients with complete atrioventricular canal and tetralogy of Fallot underwent intracardiac repair of both anomalies between 1982 and 1989. The ages of the patients ranged from 6 months to 7 years. Six of the 9 had one or more previous systemic-pulmonary artery shunts. Two-dimensional echocardiography showed diagnostic characteristics of both malformations in all patients. The diagnosis was confirmed by cardiac catheterization and cineangiography. The purpose of this study was to validate the accuracy of determining left ventricular (LV) volumes by echocardiography in infants and children. Thirty-one patients between the ages of two months and 15 years underwent left ventriculography (biplane antero-posterior and lateral cine). Ventricular volumes using an ellipsoidal model were calculated for end diastole from the cineangiograms. An 18-month-old infant diagnosed as having endomyocardial fibrosis by echocardiography is presented. The left ventricular cineangiograms of 22 asymptomatic, apparently healthy male aviators without noninvasive (echocardiographic or auscultatory) evidence of mitral valve prolapse were reviewed and compared with those of 12 men with noninvasive evidence of mild mitral valve prolapse. The maximal protrusion of the posterior mitral valve leaflet superior and posterior to a line perpendicular to the long axis of the left ventricle at end-systole was measured from the right anterior oblique left ventricular cineangiogram by repeated observation of left ventricular inflow.
