The importance of establishing a national health security preparedness index.

Natural disasters, infectious disease epidemics, terrorism, and major events like the nuclear incident at Fukushima all pose major potential challenges to public health and security. Events such as the anthrax letters of 2001, Hurricanes Katrina, Irene, and Sandy, severe acute respiratory syndrome (SARS) and West Nile virus outbreaks, and the 2009 H1N1 influenza pandemic have demonstrated that public health, emergency management, and national security efforts are interconnected. These and other events have increased the national resolve and the resources committed to improving the national hea

Using Twitter and other social media platforms to provide situational awareness during an incident.

The recent use of social media by protesters in Iran, Egypt, Yemen and elsewhere has focused new attention on this communications medium. Government agencies and businesses, as well, are using social media to push information to their stakeholders. Those who are on the front lines of this information revolution, however, realise that social media is most effective when the communication is two-way. Unlike other media, social media allows information sharing. This, in turn, provides emergency managers with new situational-awareness resources when trying to mitigate an incident. As Federal Emerg

[Role of bilirubin in the prevention of cardiovascular diseases and cancer].

Oxidative stress contributes importantly to pathogenesis of numerous civilization diseases, including cardiovascular diseases, cancer, as well as autoimmune and neurodegenerative conditions. Bilirubin is the major product of the heme catabolic pathway in the intravascular compartment. For long time, bilirubin was considered to be only a waste product, however, recent data from the last two decades have proved its important protective properties, which contributes to defense against development of civilization diseases. Numerous experimental as well as clinical studies have demonstrated associa

Orbital biomanufacturing: the key to space resiliency, operational persistence, and ethical sustainability.

Space-based biomanufacturing has historically focused on long-duration crewed and exploration missions, but its greater potential lies in supporting in-space logistics, manufacturing, and servicing in Earth orbit. This article provides a strategic perspective on how shifting investments, policy, and R&D to orbital biomanufacturing could revolutionize defense, commercial, and civil sectors by enhancing supply chain resiliency, operational flexibility, ethical debris management, and commercial viability in Earth orbit.

Institutionalising public health emergency preparedness and responses in Africa: lessons learned during the 2022-2025 outbreaks with crossborder spread potential.

Lessons and best practices from outbreaks during 2022-25 in Africa were not comprehensively documented or shared to inform future outbreak responses. We conducted a narrative review of published articles and outbreak response reports of mpox, cholera, Ebola virus disease, and Marburg virus disease and captured experts' perspectives and lessons. We analysed and presented the data in themes. Evidence indicates that effective responses are built on routine investments maintained between outbreaks, particularly in decentralised laboratories, digital surveillance systems, community structures, and

A Novel Approach to Infectious Disease Preparedness: Incorporating Investigational Therapeutics and Research Objectives into Full-Scale Exercises.

The use of investigational therapeutics in the care of patients with Ebola virus disease was documented in the literature following the 2013-2016 outbreak. In order to access these types of therapeutics, facilities must have processes in place to quickly and efficiently activate study protocols, obtain the medications, and administer them in a timely manner. Testing these procedures in advance of a real-world event is crucial to ensuring successful implementation and execution. Drills and exercises are routinely used as part of the emergency preparedness planning process, as they provide a low

The Psychosocial Challenges of Caring for Patients with Ebola Virus Disease.

Caring for highly infectious patients in biocontainment units is a new phenomenon, and little is known about the behavioral health of workers in this setting. This is a qualitative study exploring the unique experiences of workers involved in the care of patients with Ebola virus disease (EVD) at Nebraska Medicine during the 2014 Ebola outbreak. Twenty-one in-depth interviews were conducted focused on topics of personal memories, interpersonal experiences, stress response, and patient management. Five themes were identified: (1) positive experiences were emotional while challenges were technic

Emergency Preparedness with People Who Sign: Toward the whole community approach.

The concept of the "whole community" involves including everyone in the community in preparing for emergencies, including members of often-overlooked groups. Deaf people who sign are one example of this type of group. An innovative model based on the whole community approach, Emergency Preparedness with People Who Sign (EPPS), is introduced in this article. This model focuses on members of the deaf community directly training first responders using a variety of techniques for effective communication and cultural understanding to achieve safety for all. This model was developed and field tested

Public health preparedness of post-Katrina and Rita shelter health staff.

INTRODUCTION: During 2005, Hurricanes Katrina and Rita struck the US Gulf Coast, displacing approximately two million people. With >250,000 evacuees in shelters, volunteers from the American Red Cross (ARC) and other nongovernmental and faith-based organizations provided services. The objective of this study was to evaluate the composition, pre-deployment training, and recognition of scenarios with outbreak potential by shelter health staff. METHODS: A rapid assessment using a 36-item questionnaire was conducted through in-person interviews with shelter health staff immediately following Hurr

Bioterrorism: an overview.

How real is the threat of bioterrorism? Experts may disagree on the likelihood of use, but the possibility cannot be totally dismissed. Complacent ignorance of a low-probability, high-cost risk is dangerous and can result in devastating global consequences. This is a US government work. There are no restrictions on its use.

A Public Health Preparedness Logic Model: Assessing Preparedness for Cross-border Threats in the European Region.

Improving preparedness in the European region requires a clear understanding of what European Union (EU) member states should be able to do, whether acting internally or in cooperation with each other or the EU and other multilateral organizations. We have developed a preparedness logic model that specifies the aims and objectives of public health preparedness, as well as the response capabilities and preparedness capacities needed to achieve them. The capabilities, which describe the ability to effectively use capacities to identify, characterize, and respond to emergencies, are organized int

Climate Change and Its Impact on the Incarcerated Population: A Descriptive Review.

This descriptive review article describes climate change and its detrimental effects on incarcerated populations. Case examples are provided of specific natural disasters and deaths due to overheating temperatures. Because public health and social work aims to improve the health and social welfare of vulnerable populations, the authors explain why climate change should be considered a priority area in both fields. Examples are provided on how to improve conditions for the 2.4 million men, women, and youth who are incarcerated.

Prevalence of Respiratory Protective Devices in U.S. Health Care Facilities: Implications for Emergency Preparedness.

An online questionnaire was developed to explore respiratory protective device (RPD) prevalence in U.S. health care facilities. The survey was distributed to professional nursing society members in 2014 and again in 2015 receiving 322 and 232 participant responses, respectively. The purpose of this study was to explore if the emergency preparedness climate associated with Ebola virus disease changed the landscape of RPD use and awareness. Comparing response percentages from the two sampling time frames using bivariate analysis, no significant changes were found in types of RPDs used in health

Transmission dynamics of primary pneumonic plague in the USA.

Plague is thought to have killed millions during three catastrophic pandemics. Primary pneumonic plague, the most severe form of the disease, is transmissible from person-to-person and has the potential for propagating epidemics. Efforts to quantify its transmission potential have relied on published data from large outbreaks, an approach that artificially inflates the basic reproductive number (R(0)) and skews the distribution of individual infectiousness. Using data for all primary pneumonic plague cases reported in the USA from 1900 to 2009, we determined that the majority of cases will fai

Criminal and legal responsibilities in Tourette's syndrome.

Tourette's Syndrome (TS) is a neuropsychological disorder characterized by the presence of multiple involuntary motor tics accompanied by one or more vocal tics. Articles about TS and criminal responsibility and the restriction of civil rights are limited. A person with TS was evaluated to consider his criminal responsibility after swearing at a referee during a football game. He was also evaluated as to whether or not he was capable of professionally driving a service bus. Additionally, medico-legal situations regarding military service, obtaining a shotgun license and marriages of patients w
