Clofazimine nanoclusters show high efficacy in experimental TB with amelioration in paradoxical lung inflammation.

The rise of tuberculosis (TB) superbugs has impeded efforts to control this infectious ailment, and new treatment options are few. Paradoxical Inflammation (PI) is another major problem associated with current anti-TB therapy, which can complicate the treatment and leads to clinical worsening of disease despite a decrease in bacterial burden in the lungs. TB infection is generally accompanied by an intense local inflammatory response which may be critical to TB pathogenesis. Clofazimine (CLF), a second-line anti-TB drug, delineated potential anti-mycobacterial effects in-vitro and in-vivo and

Assessment of Clofazimine and TB47 Combination Activity against Mycobacterium abscessus Using a Bioluminescent Approach.

Mycobacterium abscessus is intrinsically resistant to most antimicrobial agents. The emerging infections caused by M. abscessus and the lack of effective treatment call for rapid attention. Here, we intended to construct a selectable marker-free autoluminescent M. abscessus strain (designated UAlMab) as a real-time reporter strain to facilitate the discovery of effective drugs and regimens for treating M. abscessus The UAlMab strain was constructed using the dif/Xer recombinase system. In vitro and in vivo activities of several drugs, including clofazimine and TB47, a recently reported cytochr

Clofazimine-induced interstitial lung disease with haemoptysis during therapy for Mycobacterium abscessus pulmonary disease.

We report a woman in her late 60s with a 6-month history of productive cough, low-grade fever and fatigue. Bronchoscopy yielded Mycobacterium abscessus, and she was diagnosed with M. abscessus pulmonary disease. She initially received intravenous imipenem and amikacin plus oral clarithromycin for 1 month, followed by maintenance therapy with clarithromycin and clofazimine (CFZ). One year after starting CFZ, she developed high fever, worsening cough, exertional dyspnoea and haemoptysis. Chest high-resolution CT revealed bilateral upper-lobe predominant reticular ground-glass opacities, bronchov

Challenges in Perioperative Cerebral Oximetry Monitoring: A Case of Clofazimine-Induced Skin Pigmentation Interference.

Perioperative stroke is a potentially devastating complication in patients undergoing noncardiac surgery. The most consistent risk factor associated with the condition is a history of a prior stroke. Cerebral oximetry is a simple, non-invasive, and continuous monitoring device that uses near-infrared spectroscopy (NIRS) to monitor cerebral oxygenation. However, like other monitoring devices, cerebral oximetry has certain limitations, and it must be interpreted cautiously and by taking into account all available clinical information related to the patient. We present a case of a 62-year-old Cau

Clofazimine enhances the efficacy of BCG revaccination via stem cell-like memory T cells.

Tuberculosis (TB) is one of the deadliest diseases, claiming ~2 million deaths annually worldwide. The majority of people in TB endemic regions are vaccinated with Bacillus Calmette Guerin (BCG), which is the only usable vaccine available. BCG is efficacious against meningeal and disseminated TB in children, but protective responses are relatively short-lived and fail to protect against adult pulmonary TB. The longevity of vaccine efficacy critically depends on the magnitude of long-lasting central memory T (TCM) cells, a major source of which is stem cell-like memory T (TSM) cells. These TSM

A phase II evaluation of clofazimine plus doxorubicin in advanced, unresectable primary hepatocellular carcinoma.

The riminophenazine compound clofazimine has been shown to be a potent inhibitor of hepatocellular carcinoma (HCC) in vitro. Therapeutic benefit was claimed for patients with HCC treated with clofazimine in a recent clinical trial. The current trial was initiated to evaluate response and survival of patients with HCC receiving clofazimine plus doxorubicin. Twenty-eight patients were entered into the study, of whom 27 were evaluable for response and survival. No patients had a complete or partial response, and 9 had stable disease. The median survival time was 7 weeks. Toxicity was mild with ye

Phagocytosis and intracellular degradation of 125I-labelled immune complexes by Clofazimine treated macrophage cultures.

Mouse peritoneal and calf alveolar macrophage cultures were exposed to various Clofazimine concentrations for 5 days. Cultures exposed to drug concentrations near the blood therapeutic level phagocytize and digest more immune complexes than control cultures. Our results allow us to suggest that the beneficial action of Clofazimine in lepromatous leprosy could be at least partially mediated by the removal and degradation of circulating immune complexes by macrophages.

Influence of counterion transfer on the physicochemical properties of fixed dose combinations containing bedaquiline and clofazimine.

Fixed-dose combinations (FDCs) offer significant advantages in the treatment of multidrug-resistant tuberculosis (MDR-TB), including reduced pill burden, increased treatment adherence, minimization of resistance development, and decreased relapse rates. A critical prerequisite for developing FDCs in single-unit formulations is ensuring physicochemical compatibility between the combined drugs, particularly for ionizable compounds. Bedaquiline (BDQ) and clofazimine (CFZ) are weakly basic drugs and promising candidates for next-generation antituberculosis FDCs. BDQ fumarate (BDQF) is widely used

[In vitro Activity of Rifabutin and Clofazimine to Macrolide-Resistant Mycobacterium abscessus Complex Clinical Isolates].

Mycobacterium abscessus complex (MABSC) is one of the most resistant bacteria against antimicrobial agents. The number of agents that can be used by oral route, such as macrolides, is limited in antimicrobial therapy. In recent years, rifabutin and clofazimine have gained importance as they can be administered by oral route and have shown synergistic effects with macrolides and aminoglycosides. The aim of this study was to determine the in vitro activity of rifabutin and clofazimine against clinical isolates of MABSC resistant to macrolides. A total of 48 MABSC isolates obtained from respirato

Repurposing clofazimine for malignant pleural mesothelioma treatment - In-vitro assessment of efficacy and mechanism of action.

AIMS: Malignant pleural mesothelioma (MPM) is a rare cancer of lungs' pleural cavity, with minimally effective therapies available. Thus, there exists a necessity for drug repurposing which is an attractive strategy for drug development in MPM. Repurposing of an old FDA-approved anti-leprotic drug, Clofazimine (CFZ), presents an outstanding opportunity to explore its efficacy in treating MPM. MAIN METHODS: Cytotoxicity, scratch assay, and clonogenic assays were employed to determine CFZ's ability to inhibit cell viability, cell migration, and colony growth. 3D Spheroid cell culture studies we

Using methotrexate to treat patients with ENL unresponsive to steroids and clofazimine: a report on 9 patients.

UNLABELLED: Erythema nodosum leprosum (ENL) is a chronic recurrent systemic complication of multi-bacillary leprosy frequently associated with the development of neuritis, iritis, orchitis, arthritis and dactylitis. It is well managed by thalidomide, but thalidomide is not available in Bangladesh. The World Health Organization recommends high doses of clofazimine and prednisolone. About 19% of patients do not respond completely to this regimen or experience relapse when reducing steroid dosage. OBJECTIVE: We undertook this study to determine whether oral prednisolone combined with methotrexat

Clofazimine-mediated enhancement of reactive oxidant production by human phagocytes as a possible therapeutic mechanism.

Clofazimine, at concentrations within the therapeutic range (0.01-5 micrograms/ml), stimulated human polymorphonuclear leucocytes (PMNL) to generate increased amounts of reactive oxidants (RO) when activated with the tripeptide leucoattractant N-formyl-L-methionyl-L-leucyl-L-phenylalanine (FMLP), calcium ionophore, phorbol myristate acetate and opsonised zymosan. Clofazimine per se did not activate the membrane-associated oxidative metabolism of PMNL, but rather primed these cells to hyperreact to the various stimuli. To investigate the therapeutic significance of these observations clofazimin

Pharmacokinetics and Safety of a Novel Clofazimine Formulation and Dosing Strategy in Children With Rifampicin-resistant Tuberculosis.

BACKGROUND: Clofazimine is commonly included in multidrug regimens for children with multidrug-resistant and rifampicin-resistant tuberculosis (MDR/RR-TB), but accurate pediatric dosing has been limited by 100-mg soft-gel capsules that cannot be reliably divided. We evaluated the exposure and safety of a novel 50-mg clofazimine tablet using revised once-daily weight-banded dosing informed by a previous study (Clofazimine PK1). METHODS: Children <18 years weighing <30 kg receiving MDR/RR-TB treatment, including clofazimine, were enrolled. Sparse and semi-intensive pharmacokinetic sampling was

Addition of Clofazimine Enhances the Activity of Standard Treatment Regimen in a Mouse Model of Tuberculous Meningitis.

Tuberculous meningitis causes substantial mortality due to limited penetration of antibiotics to the central nervous system and excessive neuroinflammation. This study evaluated clofazimine as an adjunct to the standard first-line tuberculosis regimen, in a mouse model. Mice were infected intracerebrally with Mycobacterium tuberculosis and treated for 6 weeks. Addition of clofazimine markedly reduced the brain bacterial burden, achieving measurable brain tissue concentrations despite minimal cerebrospinal fluid penetration, as well as attenuated brain tissue inflammation. These findings indica

Clofazimine-induced cutaneous hyperpigmentation as a source of stigma in the treatment of leprosy: A cross-sectional study.

OBJECTIVES: Cutaneous hyperpigmentation is one of the main adverse effects encountered in patients undergoing leprosy treatment with multidrug therapy (WHO-MDT). This adverse effect has been described as intolerable and capable of contributing to social stigma. The objectives of this study were to quantify the variation in skin colour induced by clofazimine during and after treatment and to assess the related stigma. METHODS: This observational cross-sectional study objectively measured skin colour in 51 patients by reading the individual typology angle (ITA°) with a spectrophotometer, follow

Fatal course of clofazimine-induced pulmonary crystal deposition in a patient with Melkersson-Rosenthal syndrome.

A wide variety of drugs and substances have the potential to damage the respiratory system by different mechanisms. Clofazimine is an anti-leprosy drug that is normally only prescribed for a few years. It has a very long half-life, and crystalline deposition of the drug in various tissues has been documented. But up to now, no fatalities due to pulmonary damage have been described. We report the case of a patient who took clofazimine for almost 27 years as off-label treatment for Melkersson-Rosenthal syndrome. He suffered from progressive dyspnea, productive cough, and occasional hemoptysis. X
