Open Versus Closed Reduction in Subtrochanteric Femur Fractures: The Critical Role of Acceptable Reduction in Healing Outcomes.

Introduction Subtrochanteric femur fractures are challenging injuries due to high mechanical stress and compromised vascularization, resulting in higher complication rates compared with other proximal femur fractures. While intramedullary nailing is the treatment of choice, controversy persists regarding whether open or closed reduction provides superior outcomes. Some evidence suggests that open reduction may achieve better alignment, whereas others highlight risks of impaired biology, blood loss, and infection. This study aimed to compare open versus closed reduction in subtrochanteric fract

Distal metaphyseal radius fractures in children following closed reduction and casting: can loss of reduction be predicted?

PURPOSE: The aim of this study was to identify factors which contribute to loss of reduction (LOR). METHODS: Outpatient records and initial, post-reduction (PR) and follow-up radiographs of patients with a distal radial metaphyseal fracture were reviewed to determine demographic factors; fracture characteristics (obliquity, comminution, intact ulna); three-point cast index (3PI); and initial, PR, and follow-up displacement (angulation and translation in the sagittal and coronal planes). Univariate and multivariate regression were used to identify significant risk factors for LOR. RESULTS: A

Open reduction versus closed reduction in internal fixation of displaced femoral neck fracture in children: a systematic review and meta-analysis.

BACKGROUND: The quality of reduction is an important factor affecting clinical outcomes for displaced femoral neck fractures (FNFs). However, concerns remain about the invasiveness of open reduction and internal fixation (ORIF) as compared to that of closed reduction and internal fixation (CRIF), and the choice between ORIF and CRIF as an optimal treatment strategy for displaced pediatric FNF remains controversial. MATERIALS AND METHODS: MEDLINE, Embase, and the Cochrane Library were systematically searched for studies published up to December 22, 2022, that compared ORIF and CRIF techniques

Analysis of risk factors for failed closed reduction in pediatric Gartland Type III supracondylar humerus fracture.

BACKGROUND: Gartland Type III supracondylar humerus fractures (SCHFs) are commonly treated using closed reduction followed by percutaneous pin fixation. However, conversion to open reduction may be necessary if closed reduction fails. This study aimed to identify risk factors associated with failed closed reduction and provide a theoretical basis for clinical decision-making in the treatment of Gartland Type III fractures. METHODS: A retrospective analysis was conducted on children with Gartland Type III SCHF who underwent surgical treatment between April 2017 and June 2018. Based on whether

Should closed reduction be attempted in the emergency department for an acute anterior shoulder fracture dislocation?

BACKGROUND: Whether an anterior shoulder fracture dislocation should be reduced under sedation in the emergency department is still a dilemma. This retrospective study aimed to determine when it is safe to perform a closed reduction based on the fracture pattern. METHODS: Surgically treated anterior shoulder fracture dislocations over eight years were classified into three groups. Group 1 involved an isolated greater tuberosity fracture. Group 2 and 3 involved surgical and or anatomical neck fractures. In group 2, the head and the shaft fragments were displaced together anteriorly and inferio

Comparison between closed reduction with percutaneous pinning and open reduction with pinning in children with closed totally displaced supracondylar humeral fractures: a randomized controlled trial.

This was a randomized controlled trial of 28 children 1 year to 12 years of age with closed totally displaced supracondylar humeral fracture. The purpose of the study was to compare closed reduction and pinning (group A) and open reduction and pinning (group B). Each group consisted of 14 children. The general characteristics of both groups (age, sex side, displacement, nerve injury preoperatively) were statistically the same (P > 0.05). All cases healed with good alignment without cubitus varus, without infection and with a good range of motion except for one. The mean +/- standard deviation

[Closed reduction and minimally invasive fixation of a sacral avulsion fracture : Minimal invasiveness with maximum stability].

Surgical stabilization of high-energy sacral avulsion fractures with spinopelvic dissociation places high demands on the surgeon. The goal is to achieve maximum stability while minimizing invasiveness. The present case of a dislocated U‑type fracture in a 25-year-old motocross rider exemplifies how a reduction with the targeted application of closed reduction techniques through hyperextension of the hip joints and lordosis in the lumbosacral hinge and through a standardized procedure in intraoperative fluoroscopic imaging, a minimally invasive stabilization by means of transsacral screw fixati

Two Different Concentrations of Ketofol for Procedural Sedation and Analgesia in Closed Reduction of Nasal Fracture.

OBJECTIVES: Ketofol is a mixture of propofol and ketamine in various concentrations. It is accepted as a suitable combination in procedural sedation and analgesia. The mixture of propofol and ketamine with different respiratory and hemodynamic effects may result in fewer dose-related side effects and a probable synergistic effect. This study was designed to compare the adverse effects and quality of analgesia of 2 different intravenous concentrations of ketofol in patients undergoing closed reduction of the nose. METHODS: This randomized double-blinded study was conducted on 100 patients who

Thoracolumbar fracture-dislocation in child abuse: case report, closed reduction technique and review of the literature.

There have been very few reports in the literature of management of thoracolumbar fracture dislocations in very young children. The following is a presentation of one of these rare cases that was treated using a closed reduction method that has been little described in the medical literature. A 14-month-old child suffered a fracture dislocation at T(12)-L(1) from nonaccidental trauma. The patient was neurologically intact upon admission. Treatment consisted of closed manual reduction and hyperextension casting under sedation. At 22-month follow-up, the patient displayed excellent clinical and

Closed anatomic reduction of a juvenile tillaux fracture by dorsiflexion of the ankle. A case report.

The juvenile Tillaux fracture is a Salter-Harris Type III intraarticular fracture of the ankle in adolescent children whose distal tibial epiphyses are partially closed. Although there is general agreement that the major force of injury is one of external rotation, when closed reduction is attempted for displaced fractures by reversing the direction of this injuring force, i.e., by internal rotation of the foot on the leg as recommended, there is often failure to achieve satisfactory reduction. Consequently, the fracture is often treated by open reduction. Anatomic reduction was achieved and m

The Use of a CAD/CAM Thermoformed Splints System in Closed Reduction of Condylar Fractures.

(1) Background: Mandibular fractures are very common. Common indications of closed treatment for mandibular fractures are non-displaced or minimally displaced simple fractures in adult compliant patients with good dentition, the absence of occlusal disruption, and fractures in growing children. In closed treatment, the mandible is maintained in centric occlusion with a maxillomandibular fixation (MMF) with orthodontic elastics. Many methods of MMF have been described, often using orthodontic appliances. In recent years, CAD-CAM technology has improved many procedures used in maxillofacial surg
