A diagnostic test for Ménière's Disease and Cochlear Hydrops: impaired high-pass noise masking of auditory brainstem responses.

HYPOTHESIS: Endolymphatic hydrops in patients diagnosed with Ménière's disease causes changes in the response properties of the basilar membrane that lead to impaired high-pass noise masking of auditory brainstem responses to clicks. BACKGROUND: Ménière's disease is defined as the idiopathic syndrome of endolymphatic (cochlear) hydrops, which is an abnormal increase in the volume of cochlear fluid (endolymph) in the inner ear. Accurate detection and diagnosis are important but difficult because of the lack of sufficiently sensitive tests. METHODS: Two populations were compared: (1) 38 non-Mé

Hearing loss in Paget's disease of bone: the relationship between pure-tone thresholds and mineral density of the cochlear capsule.

We have developed a unique method of quantitative computed tomography (QCT) that enables measurement of the density of the cochlear capsule in vivo. We performed pure-tone audiometry and QCT on 67 ears from 35 subjects with radiographically confirmed Paget's disease of the skull and on 40 ears from twenty volunteer subjects. The Pearson product-moment correlation coefficients (age- and sex-adjusted) in the group affected by Paget's disease were -0.63 for left ears and -0.73 for right ears for high-frequency air conduction pure-tone thresholds (mean of 1, 2, and 4 kHz) versus cochlear capsule d

Imaging meets function: radiological and vestibular predictors of long-term cochlear implant outcomes in patients with Ménière disease.

INTRODUCTION: Although the proportion of Ménière disease (MD) patients who meet candidacy for CI remains relatively low, this subgroup has garnered increasing clinical interest. The main objective of this work is to evaluate the influence of preoperative vestibular function and radiological markers of endolymphatic hydrops (EH) on long-term auditory outcomes after cochlear implantation (CI) in patients with definite Ménière’s disease (MD). METHODS: All patients underwent CI and were assessed using a dedicated 3T MRI hydrops protocol. Audiological outcomes were measured using pure-tone audiomet

Role of prior intratympanic gentamicin and corticosteroids therapy on speech understanding in patients with Menière's disease after cochlear implantation.

AIM: Intratympanic injection of corticosteroids (ITC) and gentamicin therapy (ITG) are widely used treatments for vertigo in Meniere's disease (MD). Even though studies show good results after cochlea implantation (CI) in MD patients when compared to non-MD groups, there is no indication on the effect of ITC and ITG prior to CI on hearing after CI. This study compares the post-operative hearing of CI patients with and without MD and patients who have received ITG or ITC prior to CI. METHODS: In a retrospective case control study, adult patients with MD who received CI from 2002 till 2021 were

[A case of Sjögren syndrome suspected, rheumatoid arthritis and Behcet's disease complicated with cochlear nerve and trigeminal nerve disorders].

We described a patient with secondary Sjögren's syndrome suspected showing multiple cranial neuropathies at the different time. A 68-year old woman has been treated as rheumatoid arthritis since 1963. In 2001, she was also diagnosed with Sjögren's syndrome suspected. Afterward, abducent and trigeminal neuropathies and uveites occurred. This case is important to insight into the cranial neuropathy with Sjögren's syndrome.

Clinical characteristics of audio-vestibular impairment in Ménière's disease: does vestibular function deteriorate in accordance with cochlear function?

The clinical course of Ménière's disease and the outcomes of functional examinations were found to be correlated, and a difference was found between the progressions of vestibular and cochlear dysfunction. Neither the total number of definitive vertiginous spells nor the duration of illness correlated significantly with the degree of audio-vestibular dysfunction. In contrast, frequent vertiginous spells were clearly associated with preservation of audio-vestibular function. Moreover, the clinical characteristics of those patients who experienced less than one definitive vertiginous spell per y

K+-induced motility and depolarization of cochlear hair cells. Direct evidence for a new pathophysiological mechanism in Ménière's disease.

In addition to an endolymphatic hydrops in various models of Ménière's disease, ruptures of the membranes lining the endolymphatic spaces or massive changes in their biochemical permeability are believed to allow large amounts of potassium-rich endolymph to deluge the normally low [K+] perilymphatic fluid. Our clinic has shown that exposure of the lateral and basal membrane parts of living isolated outer hair cells to increasing concentrations of potassium ions resulted in a sustained reversible cellular depolarization. Furthermore, potassium intoxication was accompanied by a strictly longitud

Cochlear Amyloid-β42 Accumulation Drives Progressive Auditory Neuropathy in 5XFAD Mice: A Potential Biomarker for Early Alzheimer's Disease.

BACKGROUND: Emerging evidence suggests auditory dysfunction may serve as an early biomarker of Alzheimer's disease (AD). This study investigates amyloid-beta 42 (Aβ42) accumulation in the cochlea and its relationship to auditory dysfunction in 5XFAD mice. METHODS: Immunofluorescence imaging assessed Aβ42 deposition in cochlear structures (spiral ganglion neurons [SGNs], vasculature) at 8 weeks. Auditory brainstem responses (ABR) were analyzed using multimetric methods (Wave I amplitude, signal-to-noise ratio [SNR], phase-locking precision, cross-correlation) at 8 and 16 weeks. RESULTS: Aβ42

GFAP aggregates in the cochlear nerve increase the noise vulnerability of sensory cells in the organ of Corti in the murine model of Alexander disease.

Outer hair cell (OHC) loss in the auditory sensory epithelium is a primary cause of noise-induced sensory-neural hearing loss (SNHL). To clarify the participation of glial cells in SNHL, we used an Alexander disease (AxD) mouse model. These transgenic mice harbor the AxD causal mutant of the human glial fibrillary acidic protein (GFAP) under the control of the mouse GFAP promoter. It is thought that GFAP aggregates compromise the function of astrocytes. In the auditory pathway, the formation of GFAP aggregates was observed only in GFAP-positive cells of the cochlear nerve. The presence of GFAP
