Facial nerve stimulation with cochlear implantation. VA Cooperative Study Group on Cochlear Implantation.

The course of the facial nerve may place it within the current field generated by an activated cochlear implant to produce incidental facial movement. We investigated the presence of facial nerve stimulation associated with cochlear implants in the VA Cooperative Study of Advanced Cochlear implants. Twelve of 82 patients enrolled in this study demonstrated facial nerve stimulation within 2 years of implant activation. Facial nerve stimulation in six patients with multiple channel implants (Nucleus or ineraid devices) either resolved spontaneously (n = 2), or was eliminated by deactivating basa

Vestibular and radiological characteristics of children affected by unilateral auditory neuropathy spectrum disorder.

OBJECTIVE: Auditory neuropathy spectrum disorders (ANSD) are defined by the association of a preserved outer hair cell function and an impaired auditory nerve neural response, and present mostly bilaterally. Unilateral ANSD are consequently only seldom described, and most frequently as isolated cases. This study aims to describe the audiological, vestibular and radiological characteristics of a population of children with unilateral ANSD. MATERIAL AND METHODS: We isolated 22 patients with unilateral ANSD, 12 boys and 10 girls from 0 to 95 months, in a database of auditory evoked potentials. W

Ototoxicity in dogs and cats.

Ears are special sense organs whose principal functions are hearing and maintaining equilibrium. Aminoglycoside antibiotics, erythromycin, polymyxin B, and cisplatin can affect either or both of these functions by binding with, injuring, and/or destroying special receptor cells associated with these functions. Severe hearing loss manifests itself as deafness, whereas loss of equilibrium will present as abnormal righting reflexes, nausea, and vomiting. Damage is proportional to levels of these ototoxins in the endolymphatic fluids. Evidence suggests that toxicity may be influenced by endolympha

Creation of an incus recess for a middle-ear microphone using a drill or laser ablation: a comparison of equivalent noise level and middle ear transfer function.

PURPOSE: Studies have assessed the trauma and change in hearing function from the use of otological drills on the ossicular chain, but not the effects of partial laser ablation of the incus. A study of the effectiveness of a novel middle-ear microphone for a cochlear implant, which required an incus recess for the microphone balltip, provided an opportunity to compare methods and inform a feasibility study of the microphone with patients. METHODS: We used laser Doppler vibrometry with an insert earphone and probe microphone in 23 ears from 14 fresh-frozen cadavers to measure the equivalent no

Tonic efferent-induced cochlear damping in roosting and echolocating mustached bats.

The activity of the medial olivocochlear (MOC) efferent system in mustached bats, Pteronotus p. parnellii, was studied by monitoring changes in the mechanical properties of the cochlea. The changing properties were expressed by the decay time (DT) of cochlear microphonic potentials produced by transient-induced ringing (Henson et al., 1995). Tape-recorded roost noise (biosonar and communication sounds) produced sudden, marked decreases in DT when presented to the contralateral ear of animals adapted to the quiet. When the animals were first removed from their roosts the DT was relatively short

The effect of cervical sympathectomy on cochlear electrophysiology.

The purpose of this study was to examine the electrophysiologic responses to sound from guinea pig cochleas, 3--6 days after a unilateral cervical sympathectomy. After recovery from surgery the guinea pigs developed a Horner's syndrome on the sympathectomized side. Some days later their bullas were opened and electrodes were placed bilaterally on the round windows. Most of the sympathectomized cochleas showed signs of decreased sensitivity to sound. They had a smaller dynamic range for the click- and tone-burst evoked compound action potential (CAP) compared with the non-sympathectomized cochl

Evidence that inner hair cells are the major source of cochlear summating potentials.

The role of the inner hair cells (IHCs) in generating the cochlear summating potentials (SP) was assessed by measuring SP, cochlear nerve action potentials (CAP), cochlear microphonics (CM) and 2f1-f2 distortion product otoacoustic emissions (DPOAEs) in 15 chinchillas with either acute chemical de-afferentation, accomplished by applying kainic acid to the round window, or surgical de-afferentation and basal IHC loss, which developed within two months after sectioning the auditory nerve. In the auditory nerve sectioned ears, type I ganglion cells disappeared whereas most, if not all, type II ga

Auditory mechanics of the tectorial membrane and the cochlear spiral.

PURPOSE OF REVIEW: This review is timely and relevant because new experimental and theoretical findings suggest that cochlear mechanics from the nanoscale to the macroscale are affected by the mechanical properties of the tectorial membrane and the cochlea's spiral shape. RECENT FINDINGS: Main tectorial membrane themes addressed in this review are composition and morphology, nanoscale mechanical interactions with the outer hair cell bundle, macroscale longitudinal coupling, fluid interaction with inner hair cell bundles, and macroscale dynamics and waves. Main cochlear spiral themes are macro

Responses of the ear to low frequency sounds, infrasound and wind turbines.

Infrasonic sounds are generated internally in the body (by respiration, heartbeat, coughing, etc) and by external sources, such as air conditioning systems, inside vehicles, some industrial processes and, now becoming increasingly prevalent, wind turbines. It is widely assumed that infrasound presented at an amplitude below what is audible has no influence on the ear. In this review, we consider possible ways that low frequency sounds, at levels that may or may not be heard, could influence the function of the ear. The inner ear has elaborate mechanisms to attenuate low frequency sound compone

Frequency selectivity in the auditory periphery: similarities between damaged and developing ears.

Single fiber tuning curves (stimulus frequency versus neural threshold curves) were obtained from 198 auditory nerve fibers in 24 kittens between birth and the 16th postnatal day and from 74 auditory nerve fibers in adult cats. Three developmental stages during which adult-like frequency-resolving capacity was acquired were identified. During the early stage of postnatal development, all auditory nerve fibers were essentially untuned and responded to a narrow range of low to middle frequency tone bursts presented at intensities exceeding 110 dB sound pressure level (SPL) re 20 muPa. Morphological and physiological findings on hair cells with impaired metabolism (nitrogen, monoiodoacetate) in Caiman crocodilus with reference to sudden deafness.

Morphological and physiological studies were carried out on Caiman crocodilus under experimentally produced metabolic changes. During N2-respiration the cochlear potentials responded differently. The negative component (CM-) of the cochlear microphonics decreased continuously, whereas the CM+ component showed only a little change. The summation action potential (AP) exhibited a similar behaviour to that of CM-. These alterations were reversible up to periods of 30 min N2-respiration. The morphological findings, after N2-respiration, showed an intracellular oedema of the hair cells. The afferen

The CAPOS mutation in ATP1A3 alters Na/K-ATPase function and results in auditory neuropathy which has implications for management.

Cerebellar ataxia, areflexia, pes cavus, optic atrophy and sensorineural hearing impairment (CAPOS) is a rare clinically distinct syndrome caused by a single dominant missense mutation, c.2452G>A, p.Glu818Lys, in ATP1A3, encoding the neuron-specific alpha subunit of the Na+/K+-ATPase α3. Allelic mutations cause the neurological diseases rapid dystonia Parkinsonism and alternating hemiplegia of childhood, disorders which do not encompass hearing or visual impairment. We present detailed clinical phenotypic information in 18 genetically confirmed patients from 11 families (10 previously unreport

Effects of endolymphatic and perilymphatic application of salicylate in the pigeon. I: Single fiber activity and cochlear potentials.

The effects of salicylate on the mammalian cochlea function are well documented. However, there is a lack of reports on salicylate effects on the avian auditory periphery and it might well be that salicylate is not ototoxic at all in submammalian vertebrates. We therefore recorded single fiber activities, compound action potential (CAP) and endocochlear potential (EP) during application of salicylate (calculated final concentration of about 2-18 mmol/l) into the scala media of pigeons. We furthermore recorded CAP and EP during perilymphatic perfusion of salicylate (2-20 mmol/l). Salicylate app

Bilateral electrocochleographic findings in unilateral Menière's disease.

At present, electrocochleography is the only proven investigation that can demonstrate objectively the presence of endolymphatic hydrops. The electrophysiologic recordings in response to sound stimuli show an enhancement of the negative summating potential in these cases. It is well established that patients with unilateral Meniere's disease have a high likelihood of development of the disease bilaterally in the fullness of time. Using transtympanic electrocochleography in 40 patients who manifested unilateral clinical Meniere's disease, we have recorded bilateral abnormalities indicative of e

Comparative study of effects of impact tone and steady state tone exposure: EP and concentration of K+ ion and Na+ ion.

To test the adequacy of equal energy principle (EEP), guinea pigs were exposed to impact tone. The changes in electrophysiological data, namely endocochlear potential (EP) and the change in K+ ion and Na+ ion concentrations in the endolymph were investigated. The frequency of impact tone was 1 pulse/second or 1 pulse/3 seconds. The steady state tone had Leq24h = 100, 95, 90 or 85 dB, and impact tone had Leq24h = 95, 90 or 85 dB. The results are the following. Both steady state and impact tone exposure cause changes of electrophysiological data. The effects on the absolute value of negative EP

[Measurement of Ca2+ concentration and endocochlear potential in experimental endolymphatic hydrops in vivo].

This study was based on observations on 14 normal guinea pigs and 17 guinea pigs in which the left endolymphatic duct and sac had been surgically obliterated to induce endolymphatic hydrops. Three groups of hydropic animals were tested 1, 2 and 3 months after surgery. The Ca2+ concentration and endocochlear potential (EP) were recorded by means of double-barrelled ion selective micro-electrode in the third turn of the cochlea. The results indicated that a decrease in EP and an increase in Ca2+ in hydropic cochlea, a negative correlation was present. In the control ear, the values of Ca2+ and E

Turn-specific differences in the endocochlear potential between albino and pigmented guinea pigs.

Recent findings indicate that structural differences exist in the stria vascularis (SV) between albino and pigmented guinea pigs. In the higher cochlear turns, volume density for marginal cells in the albino SV is abnormally large, while that for intermediate cells (melanocytes) is abnormally small. These anatomical variations suggest that functional differences between albino and pigmented inner ears also may be found. To examine this possibility, four strains of guinea pigs were studied, consisting of Hartley albino (N = 9) and NIH outbred pigmented (N = 15) guinea pigs, as well as albino (N

Experimental vibratory damage of the inner ear.

The aim of the experiment was to determine the effect of whole-body vibration on the inner ear. The investigations were carried out on 40 guinea pigs, subjected to sinusoidal vibration (10 Hz/5 mm/1.4 g rms) for 1 to 6 months in a noiseless apparatus. Cochlear microphonic measurements were done with a phase-sensitive detection technique for the levels 70, 80 and 90 dB and the frequencies of 0.26, 0.5, 1 and 2 kHz from the apex of the cochlea and for 4 and 8 kHz from the region of the round window. Analysis of 1,440 measurements suggested the possibility of damage appearing in the upper turning

[Summating potential evoked by long-tone burst stimuli in Menière's disease].

It is now well documented that in cases of Menière's disease/endolymphatic hydrops, the electrocochleography (ECochG) evoked by a broad-band click is often characterized by an enlarged summating potential (enlarged-SP) and a large negative summating potential to action potential ratio (large -SP/AP) but these mechanism have not been clarified. The aim of this study was to find clues to the mechanism of enhanced -SP and large -SP/AP in Menière's disease, by using long-tone burst stimuli. We also sought to determine the significance of SP measurement for evaluating the cochlear function in Meniè
