Cogan's syndrome (CS) is a rare chronic inflammatory disorder, classically characterized by interstitial keratitis and sensorineural hearing loss. Recurrent episodes of inner ear disease might result in deafness. In some patients, it may also be accompanied by systemic vasculitis. Diagnosis of CS is often missed or delayed due to its rarity, the nonspecific clinical signs at onset, and the lack of a confirmatory diagnostic test. The mechanisms responsible for CS are unknown; however, in the last decade, the pathogenesis has been somewhat elucidated, suggesting that the disease is a result of i

We report a case of atypical Cogan's syndrome presenting as bilateral endogenous endophthalmitis in a woman with ovarian cancer. A 62-year-old woman with ovarian cancer developed bilateral interstitial keratitis and panuveitis accompanied by bilateral sensorineural hearing loss and chondritis. Auricular cartilage biopsy ruled out relapsing polychondritis and the diagnosis of atypical Cogan's syndrome was set clinically.

BACKGROUND: Cogan's syndrome is a chronic inflammatory disease of unknown origin, characterised by sensorineural hearing loss, episcleritis, and vasculitis. An autoimmune origin has been suggested but not proven. Our aim was to establish whether or not an autoimmune process is the cause of the disease.
METHODS: We used pooled IgG immunoglobulins derived from eight patients with Cogan's syndrome to screen a random peptide library to identify disease relevant autoantigen peptides. Among the identified peptides, one was recognised by all the patients' sera. Antibodies against peptides were affin

The purpose of this report is to describe the effective treatment of severe anterior segment inflammation due to Cogan syndrome through the use of topical administration of cyclosporin A. A 47-year-old female patient had been experiencing headaches and difficulties with her vision. Subsequent examination revealed the sudden onset of bilateral conjunctival injection and swelling of bilateral auricles. Despite the multiple treatment (systemic and topical corticosteroid and antibiotic therapy), necrotizing scleritis had appeared bilaterally and the scleral wall was thinning. Topical administratio

Cogan's syndrome is an idiopathic inflammatory disease which may present as interstitial keratitis, inflammation of other ocular structures, Meniere's-like attacks, or systemic vasculitis. Although the ocular manifestations respond to topical steroids and are rarely serious, permanent deafness may result if systemic steroid therapy is not promptly instituted for audiovestibular dysfunction, while major morbidity and even death may occur if systemic sequelae such as vasculitis and aortic insufficiency are not recognized. For these reasons, ophthalmologists must suspect Cogan's syndrome in any p

A 55-year-old woman with a six-year history of Sjögren's syndrome (SS) and the positivity of IgG and IgM antiphospholipid antibodies (aPL) developed a sudden onset of sensorineural hearing loss associated with vertigo. This suggested the presence of an atypical Cogan's syndrome (CS), which might be a focal, neurological complication of aPL.

We performed visual evoked potential testing and electroretinography in five patients with Cogan's oculomotor apraxia. The visual evoked potentials were normal, confirming previously reported data. Electroretinograms were abnormal in two of the five patients examined. A previous study showed normal electroretinogram responses bilaterally in one patient. Our findings suggest the existence of two variants of the syndrome; the one with electroretinogram anomalies has a worse visual prognosis.

Cogan's syndrome (CS) is a rare autoimmune disorder involving audio-vestibular and ocular inflammation. While systemic vasculitis is a known complication, its association with Moyamoya-type cerebrovascular remodeling has not been previously documented. A 19-year-old Asian woman was diagnosed with atypical CS following a presentation of hyperpyrexia, unintentional weight loss, and severe bilateral sensorineural hearing loss. After five years of appropriate immunosuppressive treatment, she developed subacute headaches, hypertension, and left hemiparesis and paresthesia. Despite normal inflammato

A woman in her 20 s with a remote history of chronic iritis and bilateral sensorineural hearing loss presented with proptosis, eye pain, blurred vision and elevated intraocular pressures. Her medical history was significant for end-stage renal disease and congestive heart failure. Imaging was limited due to cochlear implants and a cardioverter defibrillator. Infectious and autoimmune investigations were negative. After initial management with topical pressure-lowering drops, she was started on systemic corticosteroids with rapid improvement in visual acuity and resolution of symptoms. The diag

Ocular myasthenia gravis (OMG) is a challenging condition to diagnose, with Cogan's lid twitch (CLT) serving as a key clinical sign. Early and accurate diagnosis of OMG is crucial for timely intervention and improved patient outcomes. However, current diagnostic methods for CLT rely primarily on visual assessment by clinicians, which is inherently subjective and prone to interobserver variability, highlighting the need for more objective diagnostic tools. This pilot study was the first to investigate the potential of video-based eyelid tracking for simplified diagnosis of Cogan's lid twitch (C

INTRODUCTION: Cogan's syndrome (CS) is a rare autoimmune disease, characterized by ocular and vestibulo-auditory involvement. Treatment of CS could be challenging, and the only evidence-based data comes from case reports or series.
AREAS COVERED: There have recently been several reports of new treatment strategy involving the use of biological disease-modifying anti-rheumatic drugs such as TNFα inhibitors, anti-CD20 or anti-IL6 receptor antibodies, in cases resistant to first- or second-line drugs.
EXPERT COMMENTARY: Corticosteroids are the cornerstone of CS therapy at disease onset and duri

PURPOSE: To describe clinical features and visual and therapeutic outcomes in a patient affected by Cogan's syndrome with retinal angiomatosis and cystoid macular edema (CMO).
CASE REPORT: A 46-year-old Caucasian man reported frequent episodes of eye redness and pain, conjunctivitis, xerophthalmia, photophobia, and progressive worsening of visual acuity. Slitlamp examination revealed interstitial keratitis. Pan-fundus fluorescein and indocyanine green angiographies revealed the presence of CMO, optic disc edema, and a peripheral angiomatous-like lesion in OS. The audiogram revealed bilateral

Cogan's syndrome is a rare idiopathic chronic inflammatory disease of the eye and the inner ear in young adults. Cogan's syndrome can be associated with large vessel vasculitis. We report a young female, with a history of interstitial keratitis and audiovestibular disease, who presents with large vessel vasculitis with left main coronary artery occlusion and develops heart failure. Cogan's syndrome was diagnosed on the basis of the presence of large vessel vasculitis with the typical inner ear and ocular involvement.

INTRODUCTION: Cogan's Syndrome is typically characterised by a non syphilitic interstitial keratitis (IK), with or without conjunctivitis, iritis or subconjunctival bleeding and progressive sensorineural hearing loss within two years of ocular signs. Atypical ocular manifestations include episcleritis, scleritis, posterior scleritis, retinal artery occlusion, choroiditis, retinal vasculitis, and optic disc oedema. We report a case of Cogan's syndrome in with recurrent cystoid macular oedema was the main feature.
CASE PRESENTATION: A patient was diagnosed with Cogan's syndrome nearly 2 years a

Cogan's syndrome is an immune-mediated systemic disorder characterized by ocular and audiovestibular inflammation. Although interstitial keratitis in the absence of an infectious cause is the classic form of eye involvement, other nonspecific inflammatory eye diseases can also occur, often making prompt diagnosis of the underlying disease challenging. The audiovestibular disease is difficult to treat and can lead to profound hearing loss. A poor outcome, especially complete hearing loss, can sometimes be prevented through a timely recognition and initiation of glucocorticoid therapy at the ons
