Experiential aspects of bulimia nervosa. Implications for cognitive behavioral therapy.

Fifty female bulimic patients were asked to complete a questionnaire describing their experience during a binge-vomit episode. Findings indicated that starvation was a common precipitant of bingeing, as were situations such as being alone and eating something, and dysphoric feelings such as anxiety and frustration. Significant others, particularly the mother and the boyfriend or husband, might also precipitate a binge. Although many feelings increased or decreased in a linear fashion as the episode progressed, others such as depression and relief waxed and waned at different points in the epis

Cognitive reactivity and dysfunctional attitudes in cognitive behavioral and psychodynamic therapy for adult depression.

OBJECTIVE: It is unclear whether cognitive reactivity (CR), an indicator of depression vulnerability, is reduced after cognitive behavioral therapy (CBT), as theorized. It is also unclear whether baseline CR and dysfunctional attitudes (DA) are associated with treatment outcome in CBT for depression. This study aims to examine these questions and assess if these effects are specific to CBT for depression. METHOD: Depressed adult outpatients (n = 159) randomized to 16 sessions CBT or short-term psychodynamic supportive therapy (SPSP) completed measures of depression severity and CR at baseline

Cognitive Behavioral Therapy, Mindfulness-Based Cognitive Therapy and Acceptance Commitment Therapy for Anxiety Disorders: Integrating Traditional with Digital Treatment Approaches.

Anxiety disorders are an enormous societal burden given their high lifetime prevalence among adult populations worldwide. A variety of anxiety disorders can be successfully treated with psychological treatments such as cognitive behavioral therapy (CBT), mindfulness-based cognitive therapy (MBCT), and acceptance and commitment therapy (ACT), either as stand-alone individual or group treatment or as adjunctive treatment to pharmacotherapy. Furthermore, a growing body of evidence suggests that therapist-guided Internet-delivered CBT (iCBT) and, to some degree, digitalized mindfulness- and accept

Exploring the Credibility of Psilocybin-assisted Therapy and Cognitive-behavioral Therapy for Depression.

Depression treatments succeed with many but leave others unimproved, and they can generate concerns about side effects, time, and cost. Psilocybin has generated media attention and empirical support for antidepressant effects, but lay impressions of its effectiveness are unclear. Although perceptions of treatment credibility contribute to outcome, beliefs about the credibility of psilocybin-assisted therapy (PAT) among potential patients remain uninvestigated, especially relative to cognitive-behavioral therapy (CBT), a common, empirically-validated approach. The present study examined credibi

Examination of the core cognitive components of cognitive behavioral therapy and acceptance and commitment therapy: an analogue investigation.

We aimed to examine the core elements of cognitive behavioral therapy and acceptance and commitment therapy that target distressing negative cognitions, cognitive restructuring (CR) and cognitive defusion (CD), respectively. Participants (N=142) recalled a saddening autobiographical event, identified a distressing thought it triggered, and completed a task that induced rumination on these cognitions. They then completed one of four brief interventions that targeted these emotionally charged cognitions: analogue versions of CR and CD, and two control interventions. The personal negative cogniti

Cognitive behavioral group therapy vs phenelzine therapy for social phobia: 12-week outcome.

BACKGROUND: This article presents results of the acute treatment phase of a 2-site study comparing cognitive behavioral group therapy (CBGT) and treatment with the monoamine oxidase inhibitor phenelzine sulfate for social phobia. METHODS: One hundred thirty-three patients from 2 sites received 12 weeks of CBGT, phenelzine therapy, pill placebo administration, or educational-supportive group therapy (an attention-placebo treatment of equal credibility to CBGT). Cognitive remediation therapy and cognitive behavioral therapy with an older adult with anorexia nervosa: A brief case report.

Cognitive remediation therapy represents a new approach to the treatment of anorexia nervosa (AN) emerging from research, suggesting that adults with chronic AN have specific neurocognitive inefficiencies. Specifically, adults with AN demonstrate an overly detailed cognitive processing bias (Roberts, Tchanturia, & Treasure, 2013) and difficulties shifting set quickly and efficiently (Roberts, Tchanturia, Stahl, Southgate, & Treasure, 2007). These characteristics manifest as rigid, rule-bound, and detail-focused cognitions, beliefs, and behaviors. Versions of these problems appear to persist af

Cognitive behavior therapy of binge eating disorder.

Binge eating disorder (BED) is characterized by recurrent episodes of uncontrollable eating, even when not hungry, until uncomfortably full, occurring at least twice a week for a 6-month period. This is differentiated from bulimia nervosa (BN) by the lack of compensatory mechanisms such as purging/laxative abuse. There are significantly higher levels of psychiatric symptoms in patients with BED as compared to those without BED. Furthermore, depressive symptomatology may increase the patient's vulnerability to binge eating as well as to relapse after treatment. Grazing is defined as eating smal

Cognitive behavior therapy (CBT) in anxiety disorders.

This review examines CBT as a leading evidence-based psychotherapy. It describes briefly the major components of CBT: exposure, reduction of safety behaviors, attention focus modification and cognitive restructuring. Specific CBT strategies suited for the main anxiety disorders are reported (specific phobias, panic disorder and agoraphobia, social phobia, generalized anxiety disorder, obsessive compulsive disorder and post-traumatic stress disorder). The review emphasizes the efficiency of the use of CBT as a psychotherapeutic method in anxiety disorders. Finally, it points to the lack of the

The Association Between Cognitive Behavioral Therapy for Insomnia and Incident Cognitive Impairment Among Older Veterans.

Among older adults, chronic insomnia is a risk factor for cognitive decline. Cognitive behavioral therapy for insomnia (CBT-I) is recommended as the initial treatment. We conducted a retrospective cohort study of 84,739 Veterans, using national Veterans Health Administration data, to evaluate the association between CBT-I completion and incident cognitive impairment among older Veterans. Veterans were classified as administration of Any CBT-I versus Referral only using electronic documented notes. Incident cognitive impairment was defined by dementia-related diagnosis codes or an associated me

Does cognitive behavioral therapy alter mental defeat and cognitive flexibility in patients with panic disorder?

OBJECTIVE: Mental defeat and cognitive flexibility have been studied as explanatory factors for depression and posttraumatic stress disorder. This study examined mental defeat and cognitive flexibility scores in patients with panic disorder (PD) before and after cognitive behavioral therapy (CBT), and compared them to those of a gender- and age-matched healthy control group. RESULTS: Patients with PD (n = 15) received 16 weekly individual CBT sessions, and the control group (n = 35) received no treatment. Patients completed the Mental Defeat Scale and the Cognitive Flexibility Scale before th

An adaptive randomized trial of dialectical behavior therapy and cognitive behavior therapy for binge-eating.

BACKGROUND: Early weak treatment response is one of the few trans-diagnostic, treatment-agnostic predictors of poor outcome following a full treatment course. We sought to improve the outcome of clients with weak initial response to guided self-help cognitive behavior therapy (GSH). METHOD: One hundred and nine women with binge-eating disorder (BED) or bulimia nervosa (BN) (DSM-IV-TR) received 4 weeks of GSH. Based on their response, they were grouped into: (1) early strong responders who continued GSH (cGSH), and early weak responders randomized to (2) dialectical behavior therapy (DBT), or

Failures in cognitive behavior therapy: The state of the art.

The aim of this review is to evaluate the state of the art of studies investigating failures in cognitive behavior therapy (CBT). The last couple of years relatively few studies have specifically focused on failures in CBT. Generally, clinicians underestimate the number of therapies that fail and therapists are no better than chance at predicting effectiveness classification. In patients treated in outpatient clinics, effects of therapy were lower than effects of therapy in disorder-specific CBT efficacy randomized controlled trials. In eating disorders, results of CBT are moderate with high r

Mediators of transdiagnostic group cognitive behavior therapy.

The efficacy of cognitive-behavioral therapy (CBT) for anxiety is well established. Investigations into the mechanisms of change in CBT report changes in cognitive vulnerabilities mediating improvements over the course of treatment. As anxiety disorders share certain risk factors, there is a trend toward CBT emphasizing these vulnerabilities, including negative affectivity (NA) and also more specific constructs such as anxiety sensitivity (AS) and intolerance of uncertainty (IU). The purpose of this investigation was to analyze potential mediators of anxiety reduction over the course of transd

Cognitive behavior therapy in the treatment of panic disorder.

BACKGROUND: Comprehensive cognitive behavior therapies have been proved to be more effective than behavioral interventions. However, the efficacy of CBT is not studied in the Indian context and also, the amount of change brought about by CBT is not known. AIMS: This study aims to examine the efficacy of cognitive behavioral intervention (CBI) in the treatment of panic disorder. Our specific objectives were to assess the effectiveness of CBI in reducing symptom severity as well as cognitions related to panic and panic-related behaviors. DESIGN: The study adopted a two-group comparison with pr

[Cognitive behavior therapy of a patient with chronic schizophrenia].

A case of successful cognitive behavioural treatment of a 63-year-old woman with chronic schizophrenia is reported. She suffered from medication-resistant auditory hallucinations and was therefore referred for therapy. She continued throughout the therapy on olanzapine 10 mg, the maximum dose tolerated. The therapy went through the stages of engagement, psychological formulation, normalisation, challenging beliefs and providing alternative explanations to her experiences. Socratic questioning was used throughout the therapy as part of the ongoing engagement process. The therapy lasted 21 sessi

Brief Cognitive-Behavioral Therapy to Mitigate Subacute Suicidal Ideation Among Youth.

There is a critical need to develop brief, scalable interventions for youth that can facilitate accessible, cost-efficient, and least-restrictive support, while addressing key drivers of suicidal ideation (SI). We present an overview of a novel cognitive-behavioral therapy module which targets perceived burdensomeness toward others (PB), an established driver of SI. Empirical work indicates that PB precipitates SI and is modifiable in youth. Hence, the primary aims of the module are to decrease levels of PB and downstream SI in youth who present with subacute suicidal ideation (i.e., SI not re
