BACKGROUND: Cognitive remediation is an evidence-based intervention designed to improve the functional cognition of people with mental health conditions. Employ Your Mind is a vocational rehabilitation program with embedded cognitive remediation that is facilitated by trained mental health workers, referred to as Learning Coaches within this context. Providing effective training and support to Learning Coaches forms a significant component to the implementation of the program. To date, there has been limited examination of Learning Coaches' experiences of delivering the Employ Your Mind progra

INTRODUCTION: Adolescents with anorexia nervosa (AN) often present cognitive rigidity, impaired decision-making, and difficulties processing emotions, all of which can impede engagement in treatment. The marked rise in paediatric hospitalisations for AN during the COVID-19 pandemic highlights the urgent need for brief, targeted interventions in inpatient settings.
METHODS: This case series explored the feasibility and preliminary effects of combining Cognitive Remediation Therapy (CRT) and Cognitive Remediation and Emotion Skills Training (CREST) for eight female adolescents (12-16 years) adm

Autistic traits or autism spectrum disorder (ASD) can be found in 4% to 52% of anorexic patients, which makes the treatment of these patients very challenging. In this review, possible ways to treat ASD and anorexia nervosa (AN) comorbidity in children and adolescents are summarized. Over recent years, the focus has shifted from searching for the evidence of connections between these two disorders, which have started with Gillberg's study in 1983, to searching for more effective and holistic treatment of this comorbidity. The latter is known to contribute to more severe courses and worse progn

In this chapter, we develop a model of μ- and δ-opioid receptor (OR) effects on cellular activity in the corticostriatal circuit after reviewing clinical data on cognitive and mood impairments in opioid substance use disorder (OUD), we use this model to derive information on the relevance of opioid actions in this circuit for cognition and reward. We find that the cognitive impairments and rewarding properties of acute μ-OR activation can reasonably explained by pharmacological actions in the corticostriatal circuit. However, long-term cognitive impairments and mood dysfunction observed in OUD

OBJECTIVE: Cognitive remediation therapy (CRT) is an evidence-based behavioral intervention that enhances functional outcomes in schizophrenia patients by improving cognition. However, not all patients benefit equally from CRT, and predictors of real-world functional improvement are poorly understood. This study aimed to identify patient-related baseline predictors of functional improvement by evaluating two distinct CRT approaches targeting different types of cognition: social cognition (Integrated Social Cognition and Social Skills Training, ISST) and neurocognition (Neurocognitive Remediati

BACKGROUND: By the time Alzheimer's disease and related disorders (ADRD) are diagnosed, efficacy of treatments is limited. Preventive interventions are urgently needed.
OBJECTIVE: To design a randomized controlled trial to assess a novel intervention that aims to prevent ADRD in high-risk groups.
METHODS: We report on the rationale and describe the design of a multisite randomized controlled trial that aims to prevent ADRD in older persons with: (1) mild cognitive impairment (MCI); (2) remitted major depressive disorder (MDD) without MCI; or (3) remitted MDD with MCI.
RESULTS: PACt-MD (Prev

BACKGROUND: Cognitive remediation therapy (CRT) improves cognition in psychosis, yet response variability remains high. The hypothalamic-pituitary-adrenal (HPA) axis may influence cognitive plasticity. This study examined whether the cortisol awakening response (CARi) and diurnal cortisol slope predict cognitive change during CRT in recent-onset psychosis (ROP).
METHODS: In a randomized, crossover pilot trial (first 12-week phase analyzed), 28 outpatients with ROP were assigned to CRT (n = 14) or treatment as usual (TAU; n = 14). The CRT program (NeuroPersonalTrainer®) consisted of twice-week

BACKGROUND: Neurocognitive impairment is a prominent characteristic of bipolar disorder (BD), linked with poor psychosocial functioning. This study's purpose is to evaluate the effectiveness of functional remediation (FR) in enhancing neurocognitive dysfunctions in a sample of remitted patients with diagnosis of BD in comparison to treatment as usual-TAU. To quantify the neurocognitive damage, the Brief Assessment of Cognition in Affective Disorders (BAC-A) will be used, and the overall psychosocial functioning will be measured with the Functioning Assessment Short Test-FAST.

BACKGROUND: Non-invasive brain stimulation (NIBS), such as transcranial direct current stimulation (tDCS) and repetitive transcranial magnetic stimulation (rTMS), has emerged as a promising alternative in the precise treatment of clinical symptoms, such as the cognitive impairment of bipolar disorder (BD). Optimizing the neurocognitive effects by combining tDCS and rTMS to strengthen the clinical outcome is a challenging research issue.
OBJECTIVE: In this randomized, controlled trial, we first combined tDCS and neuronavigated rTMS targeting the V1 region to explore the efficacy on neurocognit

BACKGROUND: Adolescents with anorexia nervosa have set-shifting inefficiencies that can be exacerbated by starvation and that may interfere with outcomes of treatment interventions. Cognitive Remediation Therapy (CRT), an adjunctive treatment focused on improving set-shifting, can target inefficiencies and may augment treatment effectiveness. The best way to add CRT to the standard of care (Family Based Treatment, FBT) for adolescents with anorexia remains understudied.
METHODS/DESIGN: This is a randomized controlled trial designed to determine if CRT is effective in increasing flexibility in

BACKGROUND: A fundamental challenge for many people with severe mental illness (SMI) is how to deal with cognitive impairments. Cognitive impairments are common in this population and limit daily functioning. Moreover, neural plasticity in people with SMI appears to be reduced, a factor that might hinder newly learned cognitive skills to sustain. The objective of this pilot trial is to investigate the effects of cognitive remediation (CR) on cognitive and daily functioning in people dependent on residential settings. In addition, transcranial direct current stimulation (tDCS) is used to promot

OBJECTIVE: Anorexia nervosa (AN) is associated with significant individual mental and physical suffering and public health burden and fewer than half of patients recover fully with current treatments. Comorbid exercise dependence (ExD) is common in AN and associated with significantly worse symptom severity and treatment outcomes. Research points to cognitive inflexibility as a prominent executive function inefficiency and transdiagnostic etiologic and maintaining mechanism linking AN and ExD. This study will evaluate the initial efficacy of adjunctive Cognitive Remediation Therapy (CRT), whic

INTRODUCTION: Previous studies suggest that cognitive remediation training (CRT) programs can help address neurocognitive impairment (NCI) and associated functional challenges in multiple populations. However, no such program has been developed for persons living with HIV in low- or middle-income countries (LMICs).
METHODS: In this qualitative study, we aimed to describe how South African people living with HIV perceived and experienced the local adaptations of two US-designed CRT programs. We recruited 34 people living with HIV (27 women; age 30-50 years) from community healthcare clinics an

Young adults with psychotic-spectrum disorder have lower odds of attaining educational goals, partly due to cognitive problems. Cognitive remediation (CR) could improve cognitive- and potentially academic functioning. The current study examined an adapted CR-intervention 'Mindset' aimed at academic functioning for people with a psychotic-spectrum disorder in secondary education. Pilot-feasibility study of Mindset including the experience of nine participants with psychotic-spectrum disorder who received Mindset and six CR trainers. Post-CR interviews with participants and trainers were subject

BACKGROUND: Cognitive impairment is a frequent consequence of bipolar disorder (BD) that is difficult to prevent and treat. In addition, the quality of the preliminary evidence on the treatment of BD through Cognitive Remediation (CR) with traditional methods is poor. This study aims to evaluate the feasibility of a CR intervention with fully immersive Virtual Reality (VR) as an additional treatment for BD and offers preliminary data on its efficacy.
METHODS: Feasibility randomized controlled cross-over clinical study, with experimental condition lasting three months, crossed between two grou

OBJECTIVE: The aim of this randomized controlled trial with a parallel design was to evaluate the effect of brief, cognitive remediation therapy (CRT) for anorexia nervosa (AN) on set-shifting.
METHOD: Two hundred seventy-five inpatient adults and adolescents with AN (mean age = 23.1; SD = 12.7) were randomly assigned (using simple randomization procedures) to either a CRT or control condition. All participants received treatment as usual; however, the CRT condition completed five CRT group sessions in lieu of other group therapies provided on the unit. Set-shifting abilities were evaluated b

BACKGROUND: Anorexia nervosa (AN) and Obsessive Compulsive Disorder (OCD) are among the most incapacitating and costly of mental disorders. Cognitive Behaviour Therapy (CBT), medication, and combination regimens, to which in AN personalised guidance on weight control is added, are moderately successful, leaving room for more effective treatment algorithms. An underlying deficit which the two disorders share is cognitive inflexibility, a trait that is likely to impede treatment engagement and reduce patients' ability to benefit from treatment. Cognitive remediation therapy (CRT) is an easy-to-u

BACKGROUND: Cognitive difficulties are common in people with multiple sclerosis (MS) and predict poorer quality of life, yet effective treatment is lacking. Cognitive remediation therapy (CRT) improves functioning in individuals with severe mental health conditions and could benefit people with MS if adequately adapted. Recognising patient and public involvement (PPI) benefits in developing acceptable interventions, this study engaged people with MS to develop a protocol to assess the feasibility of delivering a computerised, therapist-assisted CRT programme (CIRCuiTS™) to people with MS. BACKGROUND: Major depressive episode (MDE) is worldwide one of the most prevalent and disabling mental health conditions. In cases of persistent non-response to treatment, electroconvulsive therapy (ECT) is a safe and effective treatment strategy with high response rates. Unfortunately, longitudinal data show low sustained response rates with 6-month relapse rates as high as 50% using existing relapse prevention strategies. Cognitive side effects of ECT, even though transient, might trigger mechanisms that increase relapse in patients who initially responded to ECT. Among these side effects, r

Anorexia nervosa (AN) is an eating disorder without a recommended first-line treatment. Cognitive Remediation Therapy (CRT) is showing great promise in helping patients reduce cognitive inflexibility and excessive detail focus, thinking styles that could make engaging in psychological therapies difficult. CRT has shown to be effective, feasible and acceptable in both individual and group formats, and positive qualitative data has been gathered from both service users and clinicians. The aim of the current study was to assess the use of CRT as a self-help treatment for individuals with AN deliv
