Ischemic colitis in a child: a case report and narrative review.

INTRODUCTION: Ischemic colitis is extremely rare in preschool-aged children and is still susceptible to misdiagnosis due to atypical symptoms. Misdiagnosis can have serious consequences, including delayed treatment and disease progression, so raising clinical awareness of this condition is critical. This report aims to contribute by providing a relevant case and a narrative overview of ischemic colitis in children. CASE PRESENTATION: A 6-year-old boy presented with recurrent hematochezia for 6 days and mild abdominal pain. Laboratory tests showed elevated inflammatory markers. Colonoscopy rev

Ischemic Colitis in a Patient with Severe COVID-19 Pneumonia.

At the time of the current COVID-19 pandemic, on a daily basis, we encountered patients suffering from various manifestations of this infection. The most common are respiratory symptoms. Many of the patients require acute hospital care, and a smaller group of them are hospitalized in intensive care units. A subset of these critically ill patients demonstrates clinically remarkable hypercoagulability and thus a predisposition to venous and arterial thromboembolism, manifested by thrombotic events ranging from acute pulmonary embolism and splanchnic vascular ischemia to extremity ischemia. The a

A Case-control Study on the Risk Factors for Ischemic Colitis.

OBJECTIVE: Ischemic colitis (IC) is a relatively common acute inflammation disorder of the intestine. It was considered to be a disorder of elderly people with risk factors for arteriosclerosis; however, a considerable number of young people with IC have been reported recently. We performed a case-control study to determine the risk factors for IC and compare the risk factors between elderly and non-elderly people. METHODS: The study included 209 consecutive patients diagnosed with IC between December 2004 and March 2017 at Tokai University Hospital. The study also included 209 randomly selec

Seasonal variation in occurrence of ischemic colitis: a retrospective study.

Objectives We aimed to identify the clinical characteristics of ischemic colitis (IC) and to investigate the occurrence of seasonal variation. Methods From January 2008 to December 2014, 368 had IC as the reason for their admission. A total of 364 patients were enrolled in this study. We investigated patient characteristics and seasonal variations in incidence. Results The mean age (±standard deviation) of patients with IC at diagnosis was 66.8 (±16.9) years. Most patients had abdominal pain (341 cases), hematochezia (337 cases), and diarrhea (199 cases) as their chief complaints. The clinical

Passage of a sigmoid colon cast in a patient with ischemic colitis.

Colon cast passage, which is the spontaneous passage of a full-thickness, infarcted colonic segment per rectum, is a rare occurrence. The main cause is acute ischemic colitis resulting from a circulation compromise. Most of the colon cast cases reported were secondary to abdominal aortic aneurysm repairs or colorectal surgery. We report a case of an 80-year-old woman with ischemic colitis who excreted a 20-cm colon cast. In most cases that involve a colon cast containing a muscle layer component, invasive therapy is required owing to colonic obstruction or stenosis. However, in the present cas

Ischemic colitis of the ascending colon: a diagnostic and management conundrum.

Ischemic colitis is the most common form of intestinal ischemia. It most commonly involves the left side of the colon presenting with acute onset of abdominal pain followed by bloody diarrhea. Involvement of only the right or ascending colon is an infrequent occurrence. Because this problem is less recognized than its counterpart involving the left colon, the correct diagnosis and management may not be readily considered. We present a case of ischemic colitis presenting as a distal small bowel obstruction with emphasis on evaluation and management of this unusual clinical problem.

[Ischemic colitis as a complication after reconstruction of the aortic bifurcation].

Reconstructive surgery of the aortoiliac junction often requires interruption of the inferior mesenteric artery. Ischaemic colitis following this type of surgery occurs in 15-20% of cases, most often in the splenic flexure and the sigmoid colon. It is caused by general haemodynamic factors and/or atherosclerotic changes in the intestinal arteries. Clinical symptoms are often subtle or atypical and the diagnosis is difficult. However intestinal ischaemia causes considerable morbidity and mortality. Around 1000 aortoiliac reconstructive procedures were performed, with a 2.8% incidence of post-op

Comparison of transient ischemic colitis with that requiring surgical treatment.

The records of 27 patients with colonic ischemia were reviewed. Twelve patients had typical reversible or transient ischemic colitis. Thirteen patients had an ischemic stricture or gangrene of the colon that required operation. Two of the patients have asymptomatic strictures of the splenic flexure and are under observation. The sigmoid colon in our series of patients was the most frequent area of symptomatic stricture formation. Any patient with ischemic colitis, especially that involving the sigmoid colon, should be observed closely for the development of a stricture. Surgical intervention i

Gangrenous ischemic colitis localized to the cecum: a case report.

BACKGROUND: Ischemic colitis affects the left colon in elderly individuals and localization on the right side, especially in the cecum, is rare. We report a case of gangrenous ischemic colitis localized in the cecum of a patient undergoing hemodialysis. CASE PRESENTATION: A 73-year-old man had been undergoing hemodialysis for chronic renal failure caused by diabetic nephropathy. He experienced frequent vomiting, diarrhea, and abdominal pain. Contrast-enhanced computed tomography revealed thickening of the cecal wall, poor enhancement, dilation of the cecum, and intrahepatic portal emphysema.

Ischemic colitis as a cause of severe hematochezia: A mini review.

Ischemic colitis (IC) is a common cause of severe lower gastrointestinal bleeding (LGIB) in the elderly. There are very few studies of patients with IC as a cause of severe LGIB in the literature. This article aims to review diagnosis, colonoscopic findings, medical treatment, and outcomes of patients with IC as a cause of severe hematochezia. The majority of IC patients with severe hematochezia can be successfully managed with medical treatment. Colonoscopic hemostasis with hemoclips is safe and feasible in treating major stigmata of recent hemorrhage in focal ischemic ulcers. Colon surgery i

Ischemic colitis in an infant with constipation treated with stimulant laxative.

Ischemic colitis (IC), the most common form of intestinal ischemia, ranges from superficial mucosal and submucosal injury to full-thickness mural necrosis. As risk factors include cerebrovascular disease, hypertension, diabetes mellitus, prior abdominal surgery, irritable bowel syndrome, and constipation, IC typically occurs in elderly persons with multiple comorbidities rather than young children. A 1-year-old Japanese girl receiving a stimulant laxative for constipation since age 7 months was hospitalized for fever, vomiting, and hypovolemic shock. Her abdomen was swollen, and abdominal comp

[Toxic megacolon secondary to ischemic colitis. Report of a case].

We report a 67 years old male that consulted due to bloody diarrhea of several months of evolution and emaciation. According to endoscopic and radiological findings, the diagnosis of severe ulcerative colitis was made. Fifteen days after admission, the patient was subjected to an emergency total colectomy due to a toxic megacolon. The pathological study showed an ischemic colitis with extensive longitudinal ulcers in the antimesenteric border, presence of granulation tissue with inflammation and transmural fibrosis. Intestinal transit was reconstituted six months later and after 12 months of f

Ischemic colitis associated with oral semaglutide with recurrence upon rechallenge.

Ischemic colitis associated with glucagon-like peptide-1 (GLP-1) receptor agonists is an exceptionally rare condition, with only a few cases reported in the literature. We present the case of a 46-year-old woman treated with oral semaglutide (Rybelsus®) for weight loss who experienced three episodes of abdominal pain and hematochezia temporally associated with drug exposure. The third episode, which occurred after semaglutide was reintroduced, led to a confirmed diagnosis based on computed tomography, rectosigmoidoscopy, and histopathological findings consistent with ischemic colitis. Venous Congestive Ischemic Colitis After Sigmoid Colectomy: A Case Report.

Venous congestion is a possible cause of ischemic colitis following colorectal surgery. As such, congestive ischemic colitis should be considered in such cases where the mesenteric artery is preserved. Herein, we describe the case of a 73-year-old man who presented to the hospital with a two-week history of difficult defecation and frequent mucous stools and was subsequently diagnosed with refractory ischemic enterocolitis due to venous congestion. The patient had undergone resection of the sigmoid colon cancer with preservation of the inferior mesenteric artery 11 months before presentation.

Ischemic colitis after enema administration: Incidence, timing, and clinical features.

BACKGROUND: Enema administration is a common procedure in the emergency department (ED). However, several published case reports on enema-related ischemic colitis (IC) have raised the concerns regarding the safety of enema agents. Nevertheless, information on its true incidence and characteristics are still lacking. AIM: To investigate the incidence, timing, and risk factors of IC in patients receiving enema. METHODS: We consecutively collected the data of all adult patients receiving various enema administrations in the ED from January 2010 to December 2018 and identified patients confirmed
