Established in 1991 in New York City and now operating in at least seven other cities, Beacons are designed to rebuild communities of support for children and youth in urban neighborhoods. The Beacon framework is based on research findings and practitioner experience indicating that programs taking a youth development approach are more effective than those focused on "fixing" specific youth problems. Successful Beacon programs provide positive ways to meet young people's need for safety, a sense of belonging, and mastery; they also provide opportunities for decision making and contributing to

In 1996, the members of the St. Columba Collaboration conducted a grassroots neighborhood assessment of a Hispanic area in Newark, New Jersey. This assessment was undertaken during an era of significant political and policy change in New Jersey. As a community-driven and implemented effort, the processes, methods, and outcomes of this assessment provide lessons on community activism, political awareness, and strategic planning. Quantitative surveys were distributed to local residents as well as users of the various services provided through Collaboration programs. Ten qualitative focus groups

Alcohol and other drug residential rehabilitation is an abstinence-based modality for assisting people with long-standing concerns associated with their substance use. While ubiquitous, models of care in residential rehabilitation services vary widely and the impacts of the care delivered within them remain contentious. Critically, therapeutic processes in residential rehabilitation remain under-theorised with little attention given to the characteristics of "good care" within these settings. To examine this, an extended period of ethnographic fieldwork was conducted at one residential rehabil

OBJECTIVE: Over the past decade, a body of observational research has accrued about the effects of outreach-based human immunodeficiency virus (HIV) interventions for drug users. The authors reviewed the findings related to postintervention behavior changes and integrated findings across studies to provide the best estimate of program impact.
METHODS: The authors conducted a computerized literature search to locate published accounts of HIV intervention effects on drug users. Thirty-six publications covered outreach-based HIV risk reduction interventions for out-of-treatment injecting drug us

People experiencing homelessness have a high prevalence of substance abuse and mental and physical problems. Although they have very complex health needs, they face many barriers that reduce their access to health care and social services. Several research studies have shown the need to implement adapted nursing interventions to address these crucial access issues. In this article, we present the results of a critical ethnography of outreach nurses who work with homeless people (n = 12). Robert Castel's theoretical model, which focuses on the process of social disaffiliation, provided the conc

BACKGROUND: This paper describes a participatory methodology that supports investigation of the synergistic collaboration between communities affected by infectious disease outbreak events and relevant official institutions. The core principle underlying the methodology is the recognition that synergistic relationships, characterised by mutual trust and respect, between affected communities and official institutions provide the most effective means of addressing outbreak situations.
METHODS: The methodological approach and lessons learned were derived from four qualitative case studies includ

The challenge facing the authors was to develop a relationship with members of the Transgender community in a large urban city in order to develop research that would benefit this group. The situation is presented from the differing perspectives of the three women involved (one Transgender woman and two biological women); they provide their accounts of a meeting with key members of the Transgender community where the challenge became apparent. During the meeting, the issue of representation arose resulting in a discussion of whether the research agenda should be focused on a specific subgroup

Success of randomized controlled trials depends on effective recruitment and retention to ensure adequate power and generalizability. Historically underrepresented populations continue to face logistical, psychological, and socioeconomic barriers to participation despite movement toward pragmatic study design. This paper describes the multifaceted recruitment approaches used to identify and engage young pregnant women with low-income in a behavioral intervention study. Adaptive recruitment strategies employed in this study led to recruitment of 341 individuals in 49 months, with 84% of individ

BACKGROUND AND OVERVIEW: The National Institute of Dental and Craniofacial Research, or NIDCR, in 2001 sponsored the establishment of Centers for Research to Reduce Oral Health Disparities. The centers are based at Boston University; New York University; the University of Michigan; the University of Washington; and the University of California, San Francisco. Reflecting the importance of research to reduce disparities, the centers, along with related grants, represent one of the largest financial commitments ever made by the NIDCR. The centers are sponsored in part by the National Center on Mi

Vision loss among the elderly is a major health-care problem. Vision impairment is associated with a decreased ability to perform activities of daily living and an increased risk for depression. Cataract is a common cause of vision impairment among the elderly, but surgery is often effective in restoring vision. We describe the Sankara model of Gift of Vision, the doorstep-to-doorstep model of community eye care that offers high quantity-high-quality eye care. Our model demonstrates how a cost-effective one-time intervention such as a cataract surgery not only restores vision but also their qu

Hydrocephalus, a disease frequently associated with poverty, becomes even more challenging to treat in developing regions because of lack of neurosurgical manpower, inadequately equipped public health care facilities, meager resource allocations, high rates of neonatal infection, difficulty of accessibility to hospitals able to treat hydrocephalus, and high complication rates in patients who are able to access and receive shunting procedures. Definitive treatment of hydrocephalus that avoids shunting procedures and long-term shunt dependence is a safer option. In environments such as Sub-Sahar

The 9/11 terrorist attacks on New York City represented a new strain on already fractured communities with low collective efficacy. Like the majority of citizens in the greater metropolitan area, researchers at the Community Research Group of Columbia University Mailman School of Public Health wanted to "do" something to help in the aftermath of the attacks. The group proposed to promote collective recovery, that is, rebuilding social connections in the city as the foundation for individual and group recovery. After several months of organizing, New York City RECOVERS (NYCR)--a network of orga

This article examines the perceptions of villagers of Nepal towards effective modes of nutrition communication affecting their nutritional health. Based on the villagers' experience of the nutrition communication used in various vitamin A projects surveyed in three districts of Nepal, a majority of the villagers strongly believe that a nutrition project cannot be effective in achieving its goals unless the local villagers are consulted during the process (defined to include design, development, implementation, and evaluation). They also believed that, among other modes of communication, a grou

BACKGROUND: COVID-19 caused major outbreaks among immigrant and racialized workers in meat processing plants in North America, but trust barriers limited engagement with public health and research. This study investigates how community-based participatory research (CBPR) methods were employed to create a Community Scholar (CS) program to facilitate research and public health outreach among immigrant and racialized workers.
METHODS: We conducted a qualitative case study of a CS program to describe its co-creation, failures, successes and experiences working across multiple Canadian meat proces

To expand the tripartite mission to advance health equity, academic medical centers (AMCs) need to look outside their walls. In 2018, Stanford's primary care division launched the Community Partnership Program (CPP) between their AMC and local safety-net community health centers (CHCs) to support the goals of meaningful community engagement and addressing local clinic capacity needs. The CPP embeds Stanford physician faculty in local CHCs through service agreements. This study aimed to characterize the experiences of CHC leaders and faculty involved in the CPP to better understand barriers and

INTRODUCTION: People experiencing homelessness (PEH) have diverse and complex health and social needs and experience significant barriers to accessing support. As a result, PEH often use the emergency department (ED) for their unmet health and social needs. To address these challenges, the ED Outreach Programme at St. Michael's Hospital was developed with community partners within the Downtown East Toronto Ontario Health Team in Toronto, Canada. PEH presenting in the ED are connected to an outreach worker to identify immediate needs for a safe discharge and to access health and social supports

Mycobacterium tuberculosis, the bacterium that causes tuberculosis (TB), is spread from person to person by the airborne route. It can be transmitted extensively in congregate settings, making investigating exposures and treating infected contacts challenging. In December 2011, a student at a Colorado high school with 1,381 students and school personnel received a diagnosis of pulmonary TB disease. One of five household contacts had TB disease, and the other four had latent M. tuberculosis infection (LTBI). Screening of 1,249 school contacts (90%) found one person with pulmonary TB disease, wh
