Multidisciplinary team production of computer-based simulations to teach pediatrics.

A program of computer-assisted patient-management problems (PMPs) for teaching pediatrics at one medical school is described. The PMPs cover all clinical, laboratory, and outcome aspects, including the hazards and cost of inappropriate investigations and therapy. Some PMPs relate to individual conditions, while others (stem PMPs) relate to a symptom or sign, such as rashes or heart murmurs, in various conditions (branch PMPs). All of them require the students to make decisions to solve the problems. At each decision point, the computer provides choices or an opportunity for the student to ente

Online training for hearing implant surgery : A new approach to otological training.

OBJECTIVE: Education in microsurgery of the ear includes staged training to allow for mastering of the complex microsurgical procedures, particularly in the context of middle ear reconstruction and cochlear implantation. Traditional surgical training includes temporal bone preparations by cadaver dissection and supervised operating room practice. As these on-site trainings are limited, there is a need to broaden education facilities in an on-line format. Therefore, a first basic on-line training for otosurgery was developed. MATERIALS AND METHODS: The system consists of an artificial temporal

Algorithmic analysis of relational learning processes in instructional technology: Some implications for basic, translational, and applied research.

A few noteworthy exceptions notwithstanding, quantitative analyses of relational learning are most often simple descriptive measures of study outcomes. For example, studies of stimulus equivalence have made much progress using measures such as percentage consistent with equivalence relations, discrimination ratio, and response latency. Although procedures may have ad hoc variations, they remain fairly similar across studies. Comparison studies of training variables that lead to different outcomes are few. Yet to be developed are tools designed specifically for dynamic and/or parametric analyse

Factors influencing the process of learning mathematics among visually impaired and blind people.

Effective instruction and comprehension of mathematics are important for achieving academic and professional success but are especially difficult for visually impaired individuals because of the inherent difficulty in managing structural information included in math formulae. An evaluation of an alternative for computer-aided math instruction and comprehension among visually impaired students was developed, and the evaluation included seven detailed categories of factors: behavioral, emotional, cognitive, social, distracting, motivational, and modeling factors. Then, the proposed method was us

Monitoring and depth of strategy use in computer-based learning environments for science and history.

BACKGROUND: Self-regulated learning (SRL) models position metacognitive monitoring as central to SRL processing and predictive of student learning outcomes (Winne & Hadwin, 2008; Zimmerman, 2000). A body of research evidence also indicates that depth of strategy use, ranging from surface to deep processing, is predictive of learning performance. AIMS: In this study, we investigated the relationships among the frequency of metacognitive monitoring and the utilization of deep and surface-level strategies, and the connections between these SRL processes and learning outcomes across two academic

The effectiveness of a 2-year supplementary tutor-assisted computerized intervention on the reading development of beginning readers at risk for reading difficulties: a randomized controlled trial.

Children with low (pre-)literacy skills may benefit from individual tutoring during the early phases of learning to read. Dutch at-risk students from 13 schools received in first and second grade a computerized reading intervention, delivered by non-professional tutors at school. Digital logs indicated that treatment integrity was lower than intended; therefore, the intervention group was subdivided using a completion criterion. Third grade assessments revealed that the subgroup that had finished the program successfully (IF, N = 40) was able to read as fluent as the average reader, outperform

Spelling pronunciation and visual preview both facilitate learning to spell irregular words.

Spelling pronunciations are hypothesized to be helpful in building up relatively stable phonologically underpinned orthographic representations, particularly for learning words with irregular phoneme-grapheme correspondences. In a four-week computer-based training, the efficacy of spelling pronunciations and previewing the spelling patterns on learning to spell loan words in Dutch, originating from French and English, was examined in skilled and less skilled spellers with varying ages. Reading skills were taken into account. Overall, compared to normal pronunciation, spelling pronunciation fac

Auscultation of the heart: a trial of classroom teaching versus computer-based independent learning.

Declining skills in auscultation of the heart prompted an evaluation of teaching methods for medical students. A comparison of classroom teaching and computer-aided independent learning of auscultation was carried out with two groups of approximately 20 second-year medical students. Both groups used approximately 20 recorded normal and abnormal heart sounds and murmurs, chosen to illustrate learning issues. For the classroom group a cardiologist presented each case through multiple stethophones and led the discussion. The individual study group used a new CD-ROM collection of cases and recordi

Development and Use of an Interactive Computerized Dog Model to Evaluate Cranial Nerve Knowledge in Veterinary Students.

In veterinary medicine, the cognitive skills necessary to interpret neurological disorders from text-based case descriptions may not translate into the diagnostic capabilities required for clinical neurological patients. As live animals exhibiting certain specific neurological disorders are infrequent during a student's exposure to clinics, students may graduate without the experience necessary to make an accurate diagnosis in the field. To address this, we have developed a computerized simulated dog head that can exhibit cranial nerve dysfunctions and respond to specific testing procedures in

Multimedia instruction of cardiac auscultation.

The cardiac physical examination is in itself a multimedia experience. It is an amalgamation of visible, palpable, and audible sensations, preceded by the collection of an appropriate historical context in which to place these multiple sensations. It is unlikely that any electronic media could ever replace the real life experience of admitting, examining, diagnosing, and effectively treating a patient with mitral stenosis who has decompensated because of the onset of atrial fibrillation with rapid ventricular response, or a patient with sudden, severe aortic regurgitation due to endocarditis.

On-line delivery of continuing medical education over the World-Wide Web: an on-line needs assessment.

OBJECTIVE: On-line continuing medical education (CME) courses offer at least two potential advantages: They are extremely convenient and relatively inexpensive. Before expanding our department's 2-year-old on-line category 1 accredited CME program, we conducted a survey to assess the need for more courses of this type and to document those topics of most interest to our responders. MATERIALS AND METHODS: An on-line survey form was designed and linked to the home page of our departmental Web server. The 8-month survey asked whether responders were interested in earning category 1 CME hours, ho

A Surgical Simulator for Tympanostomy Tube Insertion Incorporating Capacitive Sensing Technology to Track Instrument Placement.

We describe a device engineered for realistic simulation of myringotomy and tympanostomy tube insertion that tracks instrument placement and objectively measures operator proficiency. A 3-dimensional computer model of the external ear and cartilaginous external auditory canal was created from a normal maxillofacial computed tomography scan, and models for the bony external auditory canal and tympanic cavity were created with computer-aided design software. Physical models were 3-dimensionally printed from the computer reconstructions. The external auditory canal and tympanic cavity surfaces we

Design and implementation of a simulation exercise for teaching confidentiality of patient information.

Allied health students, making the transition from a purely academic to a professional school setting, are suddenly faced with judgment decisions about disclosure of medical information. Obscure guidelines and new interpersonal relationships with other members of the health care team complicate this transition and pose a threat to confidentiality of patient information. This article describes the design and implementation of a simulation exercise to reinforce lecture guidelines specifying disclosure of medical information without risk to the patient or student. The simulation is comprised of 1
