Association between cervical conization and pregnancy outcomes: A nationwide population-based cohort study.

Cervical conization is the standard treatment for cervical intraepithelial neoplasia. However, its effects on pregnancy outcomes remain controversial. Using data from the Korean National Health Insurance System, this population-based retrospective cohort study evaluated the relationship between cervical conization and adverse pregnancy outcomes between 2006 and 2022. Altogether, 199,826 singleton primiparous women aged 19 years or older were included, of whom 18,602 had undergone conization prior to pregnancy. The adjusted odds ratios for obstetric complications were estimated using multivaria

Outcomes following conization and factors on HPV regression among young females in Wuxi.

PURPOSE: It is crucial to prioritize the detection of precancerous lesions in clinical practice, especially in young women who have not yet made decisions about family planning. Herein, we conducted a retrospective study to track HPV regression among young females who underwent conization in the past five years and identify predictors of persistent HPV infection. METHODS: We involved 400 women under the age of 35, who underwent colposcopy-guided biopsy after primary infection with high-risk HPV at the affiliated Hospital of Jiangnan University and were histologically confirmed with LSIL/HSIL

Prophylactic Cervical Cerclage in Cases Following Cervical Conization at a Japanese Perinatal Center.

The objective of this study was to re-examine the effect of cerclage on the possible factors associated with preterm delivery in women who had cervical conization. This was a retrospective cohort study comparing the obstetric outcomes of women with or without prophylactic cervical cerclage in pregnancy following a prior conization and managed at our institute between 2004 and 2023. In this study, there were 75% of pregnant women with a history of cervical conization. In 13 women of these (17%), prophylactic cervical cerclage was performed at 12-17 weeks' gestation. The incidence of preterm del

Pathologic risk factors for predicting residual disease in subsequent hysterectomy following LEEP conization.

OBJECTIVE: The loop electrosurgical excision procedure (LEEP) is widely used for cervical conization. It is unclear if margin status is the single most important factor for predicting residual or recurrent disease. We therefore sought to determine supplementary histologic factors that can be used to predict residual disease in subsequent hysterectomy specimens in order to select patients at high risk for residual and recurrent disease. METHODS: The study group comprised 172 women who had undergone LEEP conization and subsequent hysterectomy. Histological findings were assessed and compared in

Reevaluation of negative cervical conizations: Frequency, diagnostic errors, risk factors and management.

OBJECTIVE: Cervical conization is an effective treatment for precancerous lesions. However, in cases where no high-grade lesion is identified in the surgical specimen, managing these patients may be challenging due to the absence of established follow-up protocols for negative conizations. This study aimed to assess the negative conization rates at our institution by histopathological review, identify diagnostic errors, possible risk and recurrence factors and propose follow-up strategies for this group of patients. METHODS: A retrospective study from January-2010 to December-2020 analyzed pa

Are "additional cuts" effective for positive margins in cervical conization? It varies according to the doctor.

BACKGROUND: High-grade squamous intraepithelial lesion (HSIL) is a disease that is closely related to the development of cervical cancer. In clinical work, cold knife conization and a loop electrosurgical excision procedure (LEEP) are often selected for diagnosis and treatment. OBJECTIVE: In this paper, we aimed to discuss additional cuts, a common practice in cervical conization, and determine whether the doctor's choice to use additional cuts in conization can reduce the occurrence of a positive cone margin. METHODS: From January 2018 to October 2019, 965 patients underwent cervical coniza

Conization of the uterine cervix: does the level of gynecologist's training predict margin status?

The management of cervical intraepithelial neoplasia is becoming more individualized. The European Federation of Colposcopy has developed guidelines for quality assurance and also certifies specialists. The status of the resection margins of conization specimens is prognostically important and is a quality feature. We examined the rate of positive margins in conization specimens according to the training level of the gynecologic surgeon. We reviewed the hospital charts of 411 consecutive patients who underwent conization for cervical intraepithelial neoplasia or adenocarcinoma in situ between
