A 54-year-old man was referred to our hospital due to calf pain and gait disturbance. Two years earlier, he had developed ptosis of the left eye. A routine health check conducted two years later revealed a thymoma with positive anti-acetylcholine receptor antibodies. He was diagnosed with myasthenia gravis and underwent thymectomy. Eight days after the surgery, he experienced involuntary calf muscle movements and pain, leading to worsening gait disturbance. Upon admission, he presented with cold toes, ptosis of the left eye, and continuous myokymia of both legs, which persisted during sleep. L

Antibodies directed against the voltage-gated potassium channel complex (anti-VGKCs) are implicated in several autoimmune conditions including limbic encephalitis and epilepsy. However, emerging evidence suggests that only specific subtypes of anti-VGKCs are pathogenic. We present the case of a 55-year-old man who initially presented with focal unaware seizures and behavioural changes mimicking anti-VGKC-seropositive encephalitis that further progressed to parkinsonism with evidence of frontotemporal dementia and pre-synaptic dopaminergic deficit. Aggressive treatment with immunotherapy was in

INTRODUCTION: We investigated the clinical, electrophysiological and neural autoantibody characteristics in cramp-fasciculation syndrome (CFS) patients.
METHODS: We reviewed Mayo Clinic records from 2000 to 2011 to identify clinically defined CFS patients who underwent neural autoantibody testing. Stored sera of patients who tested positive for antibodies to voltage-gated potassium channel complex (VGKC complex) were analyzed further for leucine-rich glioma-inactivated 1 (LGI1) or contactin-associated protein-2 immunoglobulin G (CASPR2-IgG) antibodies.
RESULTS: Thirty-seven patients were ide

The discovery of new target antigens in membranous nephropathy (MN) has revealed new disease phenotypes and, in some cases, has suggested mechanisms of disease shared by two concurrent autoimmune diseases. Subject of Review: Several recent reports and an accompanying editorial describe the association of anti-contactin-1 (CNTN1) autoantibodies of the IgG4 subclass with a novel subtype of MN that co-occurs with a form of chronic inflammatory demyelinating polyradiculoneuropathy caused by anti-CNTN1 antibodies. CNTN1, the cellular source of which is still undetermined, is identified as the targe

Neurofascin-155 (Nfasc155) is an essential glial cell adhesion molecule expressed in paranodal septate-like junctions of peripheral and central myelinated axons. The genetic deletion of Nfasc155 results in the loss of septate-like junctions and in conduction slowing. In humans, IgG4 antibodies against Nfasc155 are implicated in the pathogenesis of chronic inflammatory demyelinating polyneuropathy (CIDP). These antibodies are associated with an aggressive onset, a refractoriness to intravenous immunoglobulin, and tremor of possible cerebellar origin. Here, we examined the pathogenic effects of

The β-site amyloid precursor protein cleaving enzyme-1 (BACE1), an essential protease for the generation of amyloid-β (Aβ) peptide, is a major drug target for Alzheimer's disease (AD). However, there is a concern that inhibiting BACE1 could also affect several physiological functions. Here, we show that BACE1 is modified with bisecting N-acetylglucosamine (GlcNAc), a sugar modification highly expressed in brain, and demonstrate that AD patients have higher levels of bisecting GlcNAc on BACE1. Analysis of knockout mice lacking the biosynthetic enzyme for bisecting GlcNAc, GnT-III (Mgat3), revea

Contactin1a (Cntn1a) is a zebrafish homolog of contactin1 (F3/F11/contactin) in mammals, an immunoglobulin superfamily recognition molecule of neurons and oligodendrocytes. We describe conspicuous Cntn1a mRNA expression in oligodendrocytes in the developing optic pathway of zebrafish. In adults, this expression is only retained in glial cells in the intraretinal optic fiber layer, which contains 'loose' myelin. After optic nerve lesion, oligodendrocytes re-express Cntn1a mRNA independently of the presence of regenerating axons and retinal ganglion cells upregulate Cntn1a expression to levels t

Accurate ante-mortem diagnosis of non-infectious meningoencephalomyelitis (NIME) in dogs is challenging due to the similarity of clinical presentations, imaging findings, and cerebrospinal fluid (CSF) analysis results with other diseases. This study aimed to apply state-of-the-art quantitative proteomic technology to identify novel biomarkers for NIME. Serum and CSF samples from 11 dogs were included, with the control group consisting of patients presenting with intervertebral disc disease (IVDD, n = 6) and the study group consisting of dogs suffering from NIME (n = 5). Mass spectrometry-based

A 45-year-old woman was evaluated for right-sided hemicorporal scar-like skin lesions on her arm and thoracic and inguinal areas that appeared shortly after reduction mammoplasty. Five years later, she developed spontaneous cramps and involuntary abnormal, painful, twitching movements in the same areas. With time, the cramps worsened and disabled the patient. The use of her right arm triggered contractures of muscles and abnormal movements. A diagnosis of neuromyotonia (NMT) was established on the basis of clinical findings and on electromyographic findings of a burst of high-frequency motor u

The Cntn1 (Contactin/F3/F11) cell adhesion molecule is involved in axon growth and guidance, fasciculation, synapse formation, and myelination in birds and mammals. We identified Cntn1 genes in goldfish, zebrafish, and fugu, and provide evidence for a fish-specific duplication leading to Cntn1a and Cntn1b. Our analyses suggest a subfunctionalization for the Cntn1 paralogs in zebrafish compared to other vertebrates which have a single Cntn1 gene. Similar to Cntn1a, Cntn1b transcripts are found in subsets of sensory and motor neurons. However, Cntn1b is detected later and more restricted than Cn

Changes in neuronal and synaptic wiring contribute to neurological variations, including autism, schizophrenia, and major depressive disorder. Many developmental axon guidance cues, postsynaptic receptors, and other downstream molecular mechanisms govern axon and neurite extension required for circuit formation. Plasticity after development can include remodeling of neuron morphology and synapses to modify circuits and change behavior, but the signals and molecular mechanisms that regulate neurite remodeling after development are not well defined. We previously found that the synaptic adhesion

Voltage-gated sodium channels are glycoprotein complexes responsible for initiation and propagation of action potentials in excitable cells such as central and peripheral neurons, cardiac and skeletal muscle myocytes, and neuroendocrine cells. Mammalian sodium channels are heterotrimers, composed of a central, pore-forming alpha subunit and two auxiliary beta subunits. The alpha subunits form a gene family with at least 10 members. Mutations in alpha subunit genes have been linked to paroxysmal disorders such as epilepsy, long QT syndrome, and hyperkalemic periodic paralysis in humans, and mot

INTRODUCTION: Neurological syndromes associated with voltage-gated potassium channels (VGKC) affect the nerve and muscle physiology. Presence of antibodies to VGKC are associated with three main neurologic syndromes namely neuromyotonia (NMT), limbic encephalitis (LE) and Morvan's syndrome(MVS) LE is a variably treatable neurologic syndrome associated with high levels of antibodies to the voltage-gated potassium channel (VGKC) complex. These antibodies are directed against protein antigens that bind to the VGKC complex. These antigens are usually leucine-rich, glioma inactivated 1 (LGI1), and

CASPR2 (contactin-associated protein-like 2) is a cell adhesion molecule expressed in both the central and peripheral nervous systems. CASPR2 autoantibodies are associated with autoimmune encephalitis and epilepsy; however, the initial presentation and subsequent treatment can be challenging. We report a rare case of CASPR2-related new-onset seizures, culminating in a motor vehicle accident, as well as dysautonomia in a 50-year-old man with a prior history of vasovagal syncope. The patient experienced three types of seizures: (i) focal preserved consciousness seizures, (ii) focal preserved con

The field of autoimmune encephalitides associated with antibodies targeting cell-surface antigens is rapidly expanding and new antibodies are discovered frequently. Typical clinical presentations include cognitive deficits, psychiatric symptoms, movement disorders and seizures and the majority of patients respond well to immunotherapy. Pathophysiological mechanisms and clinical features are increasingly recognized and indicate hippocampal dysfunction in most of these syndromes. Here, we review the neuroimaging characteristics of autoimmune encephalitides, including N-methyl-d-aspartate (NMDA)

The chicken neural glycoprotein F11 is a cell recognition molecule implicated in neurohistogenesis, in particular in the context of neurite outgrowth and fasciculation. F11 is a glycosyl-phosphatidylinositol-linked member of the immunoglobulin superfamily that is also termed contactin or F3 in humans and rodents, respectively. In this study, we report the complete structure of the F11 gene. It is composed of 23 exons distributed over more than 100 kb of genomic DNA and each of the ten domains of the F11 protein is encoded by two exons. The sizes of the introns vary by two orders of magnitude r

Glaucoma is a leading cause of irreversible blindness, driven by progressive Retinal ganglion cell loss and optic nerve degeneration. The molecular mechanisms underlying primary open angle glaucoma (POAG) remain unclear, and current intraocular pressure-lowering therapies often do not halt disease progression. Moreover, limited molecular data from Asian patient cohorts restrict population-specific therapeutic development. This study describes comprehensive serum proteomic analysis of age-matched, clinically validated Indian POAG patients to identify molecular markers and pathways linked to gla
