Prognostics of Hospitalization Length and Mortality in Patients with Traumatic Frontal Brain Contusions.

Traumatic brain injury has ripple effect on the physical, cognitive, behavioral, and emotional domains of quality of life and portends a long-term neurological disability in survivors. In this study we evaluated the prognostic role of demographic and clinico-radiological variables on the hospitalization length and mortality in 71 of patients with frontal brain contusions. The receiver operating characteristic (ROC) plots were performed, with area under the curve (AUC) values, for graphical comparison of variables that would predict mortality and hospitalization length. We found that the best p

Contusions bias cortical thickness estimates after traumatic brain injury: A TRACK-TBI study.

BACKGROUND: Traumatic brain injury (TBI) is characterized by both focal and diffuse pathology. Automated cortical thickness estimation is widely used to quantify structural brain changes following TBI; however, the impact of focal pathology such as contusions on cortical thickness estimates in TBI remains unknown. METHODS: We evaluated lesion-induced bias in cortical thickness under three lesion-handling strategies in 86 TRACK-TBI participants with MRI at 2 weeks and 6 months post-injury. Cortical thickness was estimated using the ANTsNetCT longitudinal pipeline with the default pipeline (No

Pulmonary Contusions in a Collegiate Football Player With Same-Game Return-to-Play.

Pulmonary contusions are relatively common lung parenchymal injuries associated with high-energy thoracic trauma but have rarely been reported in sports participation. The most common symptoms include dyspnea and hemoptysis, but severe cases may develop hypoxemia and acute respiratory distress syndrome. Diagnosis is confirmed with thoracic imaging and treatment is supportive care, with most pulmonary contusions resolving within a week. Limited information exists regarding return-to-play guidelines in athletes with pulmonary contusions. We present a case of a collegiate football player who sust

S100B and optic nerve sheath diameter correlation in head injury patients with contusions.

UNLABELLED: S100B is a biochemical marker of head injury and optic nerve sheath diameter (ONSD) is a non-invasive bedside technique to detect intracranial pressure. We aim to demonstrate whether ONSD correlates with S100B protein in head injury patients with contusions and also whether the grade of contusion correlates with S100B protein. This is a prospective study done on head injury patients aged between 18 and 75 years having isolated contusions admitted within 24 h of injury. Patients were assessed neurologically with Glasgow Coma Scale (GCS) and cranial computed tomography study on admis

Pulmonary contusions after blunt chest trauma: clinical significance and evaluation of patient management.

INTRODUCTION: A pulmonary contusion is an entity defined as alveolar haemorrhage and pulmonary parenchymal destruction after blunt chest trauma. According to the literature, most pulmonary contusions can only be seen on a chest CT. The aim of this study was to evaluate the patients with pulmonary contusions, as well as their management, considering diagnostic and therapeutic options related to their outcomes, since we assumed, based on everyday clinical practice, that an 'overdiagnosing' and 'overtreatment' attitude towards this injury could be present. PATIENTS AND METHODS: The research was

[Post-traumatic coxarthrosis. Medicolegal conditions of the likelihood of fractures, luxations or contusions].

Trauma is the cause of 5 to 10% of all cases of osteoarthritis of the hip. One third of patients with a dislocation or fracture-dislocation of the hip or an acetabular fracture develop osteoarthritis of the hip within one to 20 years. Symptoms may be minor or absent during the interval. In addition to severe injuries, simple contusions resulting for instance from a blow to the greater trochanter or knee or from forced abduction of the hip (e.g., splits) can also result in osteoarthritis of the hip. We developed a set of five imputability criteria, of which three are essential: 1) documentation

Rib fixation in patients with severe rib fractures and pulmonary contusions: Is it safe?

BACKGROUND: Pulmonary contusion has been considered a contraindication to surgical stabilization of rib fractures (SSRFs). This study aimed to evaluate the association between pulmonary contusion severity and outcomes after SSRF. We hypothesized that outcomes would be worse in patients who undergo SSRF compared with nonoperative management, in presence of varying severity of pulmonary contusions. METHODS: This retrospective cohort study included adults with three or more displaced rib fractures or flail segment. Patients were divided into those who underwent SSRF versus those managed nonopera

Posttraumatic subchondral bone contusions and fractures of the talotibial joint: occurrence of "kissing" lesions.

OBJECTIVE: The objective of this study was to determine the presence and location of subchondral bone contusions, fractures, and "kissing" lesions of the talotibial joint after a sprain of the ankle shown on MR imaging. MATERIALS AND METHODS: We retrospectively reviewed the images of all consecutive patients who underwent MR imaging of the ankle after acute or recurrent sprain occurring between January and December 1997. The number and location of subchondral contusions or fractures revealed on MR imaging were recorded, and a comparison was made with the radiographs obtained for each patient.

[Clinical-functional state of the retina and optic nerve in contusions of the eyeball].

The function of the retina and optic nerve was studied in 79 patients with contusions of the eyeball of various severity. A complex electrophysiological method was used, consisting of recording of visual evoked potentials and of the general and macular electroretinograms, assessment of the threshold electrical sensitivity and lability. Changes were detected in all the layers of the retina and optic nerve, the degree of these changes augmenting with the severity of injury. Changes in the functional parameters of the paired intact eyes, similar in the trend, were observed.

Focal ischemia due to traumatic contusions documented by stable xenon-CT and ultrastructural studies.

A traumatic cerebral contusion causes a zone of perifocal neuronal necrosis, the cause of which is not known; the surgical management of these lesions remains controversial. To determine the pathophysiological mechanisms responsible for brain damage after contusions, the authors performed cerebral blood flow (CBF) mapping studies and related these to change in local cerebral blood volume (CBV) and ultrastructure. In 11 severely head injured patients with contusion, CBF and CBV were measured in pericontusional areas using stable xenon-computerized tomography (CT). These studies demonstrated a p

[Chest CT in diagnosis of fire pericardial contusion].

The ability to diagnose gunshot pericardial contusion using chest CT is established. Pericardial contusion is diagnosed in 45.1% of mine blast chest wounds. Primary CT signs of contusion are pericardial edema and hemopericardium. Edema corresponds to pericardial leaflet thickening up to 2-3 mm. Pericardial thickness exceeding 3 mm indicates blood accumulation.

Lung Contusion in Polytrauma: An Analysis of the TraumaRegister DGU.

BACKGROUND: Thorax trauma frequently occurs in which injuries to the bony chest, lung contusions (LCs), and others are diagnosed. The significance of this violation is described very differently and is mostly based on monocentric data. METHODS: A retrospective analysis of the TraumaRegister DGU® (TR-DGU) dataset (Project 2014-062) was performed between 2009 and 2014 (injury severity score [ISS] ≥ 16, primary admission to a trauma center, no isolated traumatic brain injury). Patients with LC (Abbreviated Injury Scale [AIS] 3 + 4) were compared with the control group, and an analysis of differe

Desferrioxamine reduces oxidative stress in the lung contusion.

Our hypothesis in this study is that desferrioxamine (DFX) has therapeutic effects on experimental lung contusions in rats. The rats were divided into four groups (n = 8): control, control+DFX, contusion, and contusion+DFX. In the control+DFX and contusion+DFX groups, 100 mg/kg DFX was given intraperitoneally once a day just after the contusion and the day after the contusion. Contusions led to a meaningful rise in the malondialdehyde (MDA) level in lung tissue. MDA levels in the contusion+DFX group experienced a significant decline. Glutathione levels were significantly lower in the contusion

Persistent polyuria in a rat spinal contusion model.

Polyuria contributes to bladder overdistention, which confounds both lower and upper urinary tract management in individuals having a spinal cord injury (SCI). Bladder overdistention post-SCI is one of the most common triggers for autonomic dysreflexia, a potentially life-threatening condition. Post-SCI polyuria is thought to result from loss of vascular tone in the lower extremities, leading to edema and subsequent excess fluid, resulting in polyuria. Mild SCIs that have near complete recovery would therefore be expected to have little to no polyuria, while severe injuries resulting in flacci

Changes in anterior chamber depth due to contusion.

PURPOSE: We aimed to investigate changes in anterior chamber depth (ACD) prospectively in patients with recent unilateral contusion but no signs of angle recession, using a scanning peripheral anterior chamber depth analyzer (SPAC). METHODS: Among patients whose chief complaint was recent unilateral contusion and who showed no signs of angle recession by gonioscopy in 2006 and 2007, those who satisfied the following criteria were subjected to SPAC evaluation of ACD from the central to the peripheral region: no history of ocular diseases or ocular surgery including laser treatment, and similar

Cardiac contusion following blunt chest trauma.

Cardiac contusion following blunt chest trauma is not rare, and the works in the literature report incidence rates between 5 and 50%. Traffic accidents are the most frequent cause of cardiac contusion followed by violent fall impacts, aggressions and the practice of risky sports. The spectrum of post-traumatic cardiac lesions varies greatly, ranging from no symptoms to decrease in cardiac function. Cardiogenic shock is a rarely encountered manifestation of blunt cardiac contusion. We review our experience of cardiac contusion after blunt chest trauma, and we describe two very severe cases that
